  • In vitro fertilization and embryo transfer (IVF & ET) is widely used for the males with subnormal or abnormal semen quality, as this was recommended in view of the relatively small numbers of spermatozoa required for fertilization and subsequent pregnancies could be obtained. The aim of this study is to know how the various functional parameters of spermatozoa in semen analysis affect the outcome IVF. This study was carried out between 1988-1989, with male factor patients selected on the basis of the semen quality. The selection criteria was based upon the mean values of concentration,% motility and % normal morphology from at least two semen analysis. There is a significant decrease in the fertilization and embryo transfer rates in the study group compared with control group (35.9% vs. 68% and 48.6% vs. 85.5% respectively), however, there was no significant difference in the pregnancy or delivery rates (19.6% vs. 21.4% and 60.0% vs. 62.5% respectively) per embryo transfer cycles. Fertilization rate is variously affected by the type and degree of sperm defect. No pregnancy was occurred in triple defect group and asthenoteratospermia group. There is no significant increase in the abortion rate in the male factor group. Improvement have to be made with the fertilization rate, as the pregnancy rate per OPU cycle in male factor group is still lower than that of normal group (9.5% vs. 18.3%). In conclusion, IVF can be used as a treatment for male factor infertility and the preparation of the semen sample can be modified to improve sperm recovery and obtain fertilization from abnormal semen samples.

  • Bovine oocytes obtained from ovarian(2 to 5mm in diameter) of slaughtered cows were cultured in TCM 199 with 10~20% estrous-cow-serum(ECS) for 24~25hr at 39$^{\circ}C$ in 5% CO2-95% air. After culture, some oocytes were examined their maturation. The remainder were used to assess the fertilizability with frozen-thawed spermatozoa in a medium containing caffeine and heparin, and subsequent development in media with bovien cumulus cells(BCC) or bovine oviduct epithelial cells(BOEC). The results obtained were summarized as follows ; 1. The maturation rate of the oocytes in TCM199 with 15% ECS group(76.5%) was higher than that of 10% ECS(69.2%) or 20% ECS group(64.8%). 2. The proportions of the oocytes penetrated and the pronuclear oocytes in the presence of caffeine and heparin were 72.1%(62/86) and 93.5%(58/62), respectively. The rate of polyspermy in the fertilized oocytes was 8.1%. 3. When 73 oocytes recovered from fertilization drop were cultured in TC-199 medium with 10% fetal calf serum(FCS), 41 oocytes(56%) cleaved to 2-cell and further stages of embryos. Among these only one embryo developed upto morula stage. 4. The rate of the cleaved oocytes was higher in medium with BCC(80%:59/74) than BOEC(76%:58/76). However, the rate of developed morulae and blastocysts was higher in the medium with BOEC(40%;23/58) than with BCC(34;20/59).

  • Background: Dental fear is usually associated with hemodynamic changes. Fear of pain during the surgical removal of a lower impacted third molar might cause patients anxiety, thereby leading to avoidance of any future dental therapy. This study aimed to determine the effect of experiencing a surgical impacted-tooth removal on the pain perception-related anxiety and hemodynamic status. Method: Twenty-seven healthy patients aged 15-30 years (mean age, 24 years), for whom surgical removal of bilateral lower third molars was advised, were included. This prospective, randomized, controlled, split-mouth study involved operations on both sides of the mandibular arch, with a 1-month washout period in between. Blood pressure and heart rate were measured before the surgical procedure, during and after the injection, preoperatively, and postoperatively. Pain perception was evaluated using a 100-mm visual analog scale during the injection, preoperatively, and postoperatively after the numbness disappeared. Differences in the blood pressure, heart rate, and pain perception between the two appointments were analyzed using the paired t-test. For all statistical analyses, SPSS version 11.5 was used. Results: The mean pain perception values during the injection and preoperatively showed no significant differences between the two appointments (P > 0.05); however, significant differences in the blood pressure and heart rate were noted before the surgical procedure; preoperatively, the blood pressure alone showed a significant difference (P < 0.05). Conclusion: There was a significant decrease in the blood pressure and heart rate preoperatively; hence, experiencing a surgical impacted-tooth removal can reduce the subsequent preoperative anxiety in healthy patients.

  • The purpose of this study was to investigate the biodegradability and performance of organic removal and methane production rate when treating piggery wastewater using a pilot scale two-phase anaerobic system operated up to a volumetric rate of $10m^3/day$. The pilot scale two-phase anaerobic process is consisted of a continuous-flow stirred-tank reactor (CFSTR) for the acidification phase and an Upflow Anaerobic Sludge Blanket reactor (UASB) for the methanogenesis. The acidogenic reactor played key roles in reducing the periodically applied shock-loading and in the acidification of the influent organics. The acidogenic CFSTR was operated at organic loading rates (OLR) between 1.8 and $14.4kgCOD/m^3{\cdot}day$, and the UASB reactor was operated between 0.5 and $5.6kgCOD/m^3{\cdot}day$. A stable maximum biogas production rate was $81m^3/day$ and the methane conversion rate of the organic matter varied from 0.30 to $0.42L\;CH_4/g\;COD_{removed}$(0.40) at hydraulic retention time (HRT) above 3.5days. The methane contents ranged from 73 to 82% during the experimental period. It is known that most of the removed organic matter was converted to methane gas, and the produced biogas might be high quality for its subsequent use.

  • Objective : The main concern during transfemoral carotid artery stenting (CAS) is preventing cerebral embolus dislodgement. We compared clinical outcomes and intraprocedural embolization rates of CAS using a distal filter protection device or proximal balloon occlusion device. Methods : From January 2011 to March 2015, a series of 58 patients with symptomatic or asymptomatic internal carotid artery stenosis ${\geq}70%$ were treated with CAS with embolic protection device in single center. All patients underwent post-CAS diffusion-weighted magnetic resonance imaging (DW-MRI) to detect new ischemic lesions. We compared clinical outcomes and postprocedural embolization rates. Results : CAS was performed in all 61 patients. Distal filter protection success rate was 96.6% (28/29), whose mean age was 70.9 years, and mean stenosis was 81%. Their preprocedural infarction rate was 39% (11/28). Subsequent DW-MRI revealed 96 new ischemic lesions in 71% (20/28) patients. In contrast, the proximal balloon occlusion device success rate was 93.8% (30/32), whose mean age was 68.8 years and mean stenosis was 86%. Preprocedure infarction rate was 47% (14/30). DW-MRI revealed 45 new ischemic lesions in 57% (17/30) patients. Compared with distal filter protection device, proximal balloon occlusion device resulted in fewer ischemic lesions per patient (p=0.028). In each group, type of stent during CAS had no significant effect on number of periprocedural embolisms. Only 2 neurologic events occurred in the successfully treated patients (one from each group). Conclusion : Transfemoral CAS with proximal balloon occlusion device achieves good results. Compared with distal filter protection, proximal balloon occlusion might be more effective in reducing cerebral embolism during CAS.

  • This study was carried out to determine the effects of ionomycin and 6-dimethylaminopurine (6-DMAP) treatments on parthenogenesis of rabbit oocytes. The oocytes were randomly assigned to the activation treatments with either ionomycin plus 6-DMAP or electric stimulation. The oocytes were colected from the oviducts of superovulated rabbits at 13~14 hours and 19~20 hours post hCG injection and were activated with 5$\mu$M ionomycin for 5 min and 2 hours incubation in 2mM 6-DMAP. The other oocytes were stimulated by three pulses of 3.6kV/cm for 60 $\mu$sec each 30 min apart, starting 19 hours post in hCG in 0.28M mannitol solution with 100$\mu$M Ca2+ and Mg2+. The results obtained were summarized as follows: 1. Following treatment of the oocytes with ionomycin plus 6-DMAP, the cleavage rate and in vitro developmental rate to blastocyst were significantly(P<0.01) higher in the oocytes collected bet ween 19~20 hours than between 13~14 hours after hCG injection. 2. When the oocytes were treated with ionomycin plus 6-DMAP, 85(98.8%) of 86 treated oocytes extruded the second polar bodies, with the entire chromatin complements outside ooplasm. However when the oocytes were restored during subsequent incubation in the drug-free medium, the cytoplasts regain their full capacity for parthenogenetic activation and nuclear remodelling. 3. The cleavage rate and the in vitro developmental rate to blastocyst were not significantly different in the oocytes activated by ionomycin plus 6-DMAP treatment(91.2 and 45.6%) or electrical stimulation(89.6 and 34.3%).

  • The aim of this study was to evaluate the changes in dentinal permeability after application of dentin desensitizer on exposed dentin immediately after ultrasonic scaling to teeth with non-carious cervical lesions. Thirty caries-free extracted molars were fixed to slide glasses after horizontally being sectioned at 5 mm below the cemento- enamel junction (CEJ). The prepared specimen was connected to a fluid flow measuring device (nano-Flow), and a V-shaped cavity was formed at the CEJ to imitate the non-carious cervical lesion. After no fluid leakage was confirmed in the connected system with specimen, tooth surface was treated ultrasonic cleaning with piezoelectric ultrasonic scalers until dentinal tubules were exposed. And 6 different desensitizers were applied on exposed dentin. Real-time measurements of dentinal fluid flow were performed during ultrasonic scaling and application of dentin desensitizer. To evaluate the occlusion of exposed dentinal tubules, tooth surface was examined by SEM. Following results were observed. After ultrasonic scaling, more dentinal tubules were exposed on the tooth with non-carious cervical lesions compared to tooth without lesions. The rate of fluid flow measured with nano-Flow system had correlation with the degree of dentin occlusion observed with SEM after application of desensitizers on exposed dentin. Desensitizers with glutaraldehyde and HEMA did not decrease the rate of fluid flow and did not show dentin occlusion. Desensitizers with oxalate showed the limited effects on the rate of fluid flow and dentinal tubule occlusion. Desensitizer with resin monomer showed the significant effect on the rate of fluid flow and dentin occlusion.

  • Objective: To compare our in-house method of embryo freezing with Cryotop vitrification in terms of immediate survival, subsequent cleavage and blastocyst formation, and cell numbers in blastocysts. Methods: Two-cell mouse embryos were randomly allocated into three groups: a non-frozen control group (group 1, n = 300), a group that underwent Cryotop vitrification (group 2, n = 300), and a group that underwent our in-house freezing method (group 3, n = 300). Results: There were no significant differences between groups 2 and 3 in the immediate survival rate (96.3% vs. 98.6%, respectively; p= 0.085), the further cleavage rate (91.7% vs. 95.0%, respectively; p= 0.099), or the blastocyst formation rate (80.7% vs. 78.6%, respectively; p= 0.437). The cell numbers in the blastocysts from groups 1, 2, and 3 were comparable ($88.99{\pm}10.44$, $88.29{\pm}14.79$, and $86.42{\pm}15.23$, respectively; p= 0.228). However, the percentage of good-quality blastocysts in the Cryotop vitrification group was significantly higher than in the group in which our in-house method was performed, but was lower than in the control group (58.0%, 37.0%, and 82.7%, respectively; p< 0.001). Conclusion: At present, our method is inferior to the commercial Cryotop vitrification system. However, with further improvements, it has the potential to be useful in routine practice, as it is easier to perform than the current vitrification system.
