Change of Voice Handicap Index After Laryngeal Microsurgery for Benign Vocal Fold Lesions (양성 후두 질환 환자의 후두미세 수술 전후 음성 장애 지수의 변화)

  • Background and Objectives:The Voice Handicap Index (VHI) evaluates the patients perception of impact of voice disorder in term of functional, physical, emotional factors. The purpose of this study was to evaluate the change of patient's subjective voice handicap index before and after laryngeal microsurgery for benign vocal cord lesions. Materials and Methods:We analyzed 55 patients who received laryngeal microsurgery for benign vocal cord disease from January 2011 to February 2013 retrospectively. There were 50 vocal nodules, 3 vocal polyps, 2 vocal cysts. VHI were analyzed before surgery and 3 months after surgery. Results:The VHI scores showed statistically significant reductions postoperatively in functional and emotional VHI (p=0.01 and p=0.034). Also, Emotional VHI score after microsurgery was higher in female than male [adjusted odd ratio (AOR)=0.292 ; 95% confidence interval, CI=0.098-0.869, p=0.01]. Conclusion:In other words, males experience significant more emotional effects that are improved after microscopic surgery.

The Influence of Violence Experience on the Job Stress among Hospital Employees Working at Administration and Discharging Department (병원 원무행정근무자의 폭력경험이 직무스트레스에 미치는 영향)

  • Background: Workplace violence was recognized to be social problems that might impact the health status and the job satisfaction of employee in hospitals. This study investigated the current status of violence and job stress among hospital employees working at administration and discharging department. Methods: The study subjects were 213 administrative employees working at 20 general hospitals. Data were collected by self-administered questionnaire that included information such as demographics, job-related characteristics, experience of violence, and job stress. The violence was classified as verbal violence, physical threat, and physical violence occurred by patients and caregivers. Analysis of variance, t-tests, correlation analysis, and multiple linear regression analysis were performed to examine the associated factors with job stress. Results: The levels of verbal violence, physical threat, and physical violence were $1.64{\pm}1.08$, $0.54{\pm}0.67$, and $0.04{\pm}0.17$, respectively. The score of job stress was $2.74{\pm}0.50$ and it was associated with age, existence of spouse, drinking frequency, subjective health status, disease history, night-time treatment, and public health administration career in simple analysis. In multiple linear regression analysis, the level of verbal violence experience was significantly associated with job stress (B=0.09, p=0.001). Also physical threats (B=0.18, p<0.001) and physical violence (B=0.48, p=0.008) showed positive association with job stress. Conclusion: This study attempted to examine the association between experience of violence and job stress in administrative employees at medical institutions. Levels of violence showed positive correlation with the job stress. Environment improvement to protect employee from violence and management of employees who experienced workplace violence are needed to reduce the job stress.

The impact of health belief model in the middle and high school students on oral health behaviors (중·고등학생의 구강건강신념이 구강건강관리행동에 미치는 영향: 건강신념모형을 적용하여)

  • Objectives: The aim of the study is to investigate the health belief model affecting the oral health behavior in middle and high school students. Methods: The subjects were 296 middle and high school students in Seoul, Gyeonggi and Incheon from February 15 to March 21, 2014. The students filled out the self-reported questionnaires after receiving informed consents. The instrument was adopted and revised from those of Kim & Hwang, and Choi & Joo. The questionnaire consisted of 4 questions of general characteristics, 9 questions of oral health status including subjective oral health status, frequency of tooth brushing, duration of tooth brushing, method of tooth brushing, use of oral health devices, dental clinic visit, scaling services, snack intake, and smoking. The oral health belief consisted of 25 questions including susceptibility, seriousness, barriers, benefit, and self-efficacy using Likert 5 scale. The reliability of Cronbach's alpha in the study was 0.725. Data were analyzed using SPSS ver 18.0 for frequency analysis, t-test, ANOVA, ${\chi}^2$-test, and Pearson's correlation coefficient, simple regression, and binary logistic regression. Results: Oral health beliefs of middle and high school students affected the oral health behaviors. Susceptibility, barriers and self-efficacy also influenced on the oral health behaviors. In order to provide the best oral health education, susceptibility and self-efficacy are the primary factors to increase motivation because the motivation endows the students with correction of oral health behaviors that improve the knowledge, attitudes, and decrease barriers in oral hygiene. Conclusions: It is important to correct oral health behaviors in the middle and high school students by providing the continuing and systematic oral health education.

A study on the cervical abrasion in some industrial workers (일부 산업장 근로자의 치경부마모증에 관한 연구)

  • Objectives : This study was carried out to investigate stepwise regression analysis on cervical abrasion & general characteristics, subjective oral health perception, habit related to oral health, and oral symptom. Methods : The study subjects were 2,158 workers in 23 industrial work places located in Gumi-si, Gyeongsangbuk-do Province from June 1 to July 1, 2012. As a result, the following conclusions were obtained. Results : 1. The cervical abrasion rate was high in men and older age group. 2. The cervical abrasion rate was high in a case of having not received scaling for the past one year. 3. The cervical abrasion rate was high in frequent tooth brushing. 4. The cervical abrasion rate was high in group having no bleeding in tooth and the gum. 5. The cervical abrasion rate was high in group having the cold symptom in teeth. 6. Excluding gum pain and bleeding, factors affecting tooth abrasion were gender, age, scaling over the past year, frequency of brushing a day and tooth sensitivity when cold food was ingested. 7. Gender and tooth sensitivity turned out to have negative(-) effect. age, scaling over the past year and frequency of brushing a day turned out to have positive(+) effect. Conclusions : Through this study, the best prevention method of cervical abrason is effective tooth brushing education and regular dental check up.

Emerging Currents in Health and Medicine - A Socio-Cultural Critique of Their Discourses and Practices - (건강과 의학의 새로운 흐름 - 담론과 실천 방식에 관한 사회문화적 비평 -)

  • We have witnessed several kinds of new discourses and practices in health and medicine since the 1970s, such as popular concerns with alternative or complementary medicine, inordinate attention to the promotion of 'healthy' living, rapid resurrection of traditional medicine and ecological management of health. Four structural and situational factors are discussed to underlie these new trends:(i) as 'crisis' in health care of the 1970s was translated into health care reform of the 1980s backed up by neo-liberal political philosophy, the state responsibility for nation's health is being transferred to the individual ;(ii) it resulted from the limits of biomedical paradigm in dealing with chronic diseases;(iii) medico-scientific knowledge of disease is transformed into the subjective discourses and technologies of health in postmodern society ; and (iv) it is deeply associated with the considerable increase in environmental risk perception of health and disease. There are some inherent countervailing forces in these new discourses and practices. First, while they derive from lifestyle-oriented behavioral change, medicalization of life and death is still consolidated in the new trends. Second, inasmuch as new tides are reliant upon science, they. are likely to be remote from techne that means not the practical application of theoretical knowing but a special form of practical knowing. Third, as new discourses and activities accomplished'in the name of health'increasingly occupy important strategies in forming the self-identity, they serve as moral apparatus which involves prescriptions about how we should live our lives and conduct our bodies, both individually and collectively. Therefore, two points are suggested to consider seriously whether these streams will succeed in improving the‘healthy’living of all the people. Instead of limiting tile perspective to medicine, healing and health care, a new matrix that interweave welfare, ecology and labor along with them is timely needed for enhancing the health for all. In addition, as the World Health Report fm strongly shows, inequality in health heavily depends upon socio-economic development of a society, and it is not the richest countries that have the best health status, but those that have the smallest income differences between rich and poor.

A comparative study of switching intention of Smartphone users: Korean and Chinese (스마트폰 이용자의 전환의도에 관한 비교연구: 한국과 중국)

  • This study investigates factors, which affect subjective judgment of Smartphone users when they switch to a new product or other mobile carrier, and examine a relationship between switching intention and switching expected performance through users of in Korea and China. The results of the study showed that switching intention is positively associated with switching expected performance in both Korean and Chinese users. Brand reliability, functional quality, and general climate for Smartphone business have a significant impact on switching intention in both countries. In addition, personal innovation has a significant impact on switching intention of Korean users but Chinese. However, content quality has the effect on switching intention of Chinese user but Korean. This study provides useful strategies for switching behavior of Smartphone users of two different countries in the mobile industry.

Study on Family Strength and Happiness of the Pre-Elderly and the Elderly (노인과 예비노인의 가정건강성과 행복에 관한 연구)

  • The purpose of this study is to examine the relative influence of various characteristics that affect the happiness and family strength of pre-elderly and the elderly. This study also shows the differences between the two age groups. The National Survey of Korean Families has been done by the Ministry of Equality and Family on 2010. Respondents selected from national survey were 1016 pre-elderly and 629 elderly. The results of this study are as follows; first, 66% of the respondents chose health as the condition is being the important elements of their happiness following money, work (job), children, spouse and family life. Second, the group of people considering work and health as important elements of their happiness is happiest, but the group considering money and health as the important elements of their happiness feels a lower level of happiness. Third, the pre-elderly chose health as the most important condition, but the elderly chose subjective economic condition, among the factors affecting family strength. Fourth, family strength affects most on in regard to the happiness of pre-elderly and the elderly. Next, the more they are financially stable, physically healthier and living in rural areas, they feel the happier they feel. Based on the results of this study, further studies are needed to determine components of family strength that both pre-elderly and the elderly consider as important elements of their happiness. Support programs for the elderly without a spouse will increase the well-being of the elderly.

Construction of a Structural Model Incorporating Nurse Image, Image Determinants, and Self-esteem for Evaluation of Cambodian Nursing Students (캄보디아 간호대학생들의 간호사 이미지, 이미지 결정요인, 자아존중감 사이의 관계 모형 구축)

  • Purpose: This study, on the basis of a structural model that includes nurse image, image determinants, and self-esteem of Cambodian nursing students influenced by the Korean nursing education system, demonstrates distinctions between senior and junior according to experience in clinical practice. Methods: Data were collected via a questionnaire from 194 nursing students in Cambodia and subsequently analyzed using the STATA IC 12 program. Results: First, image determinants and their sub-factors-subjective, institutional, and media-effect both the nurse image and self-esteem of nursing students. Second, the study confirms that nurse image has no significant effect on self-esteem. Also, the individual factor, a sub-factor of nurse image, possesses a weak relationship with nurse image. Third, the structural model mediating between senior and junior reveals differences resulting from experiences derived from clinical practice. Conclusion: The study has significance in that Cambodian nursing students, who are rarely studied in terms of nurse image, have been systemically analyzed via a structural model incorporating image determinants and self-esteem.

A new transform coding for contours in object-based image compression (객체지향 영상압축에 있어서 윤곽선에 대한 새로운 변환 부호화)

  • In the content-based image coding, where each object in the scene is encoded independently, the shape, texture and motion information are very important factors. Though the contours representing the shape of an object occupy a great amount of data in proportion to the whole information, they strongly affect the subjective image quaility. Therefore, the distortion of contour coding has to be minimized as much as possible. In this paper, we propose a new method for the contour coding in which the contours are approximated to polygon and the eorror signal occurring from polygonal approximation are transformed with new basis functions. Considering the facts that confour segments occurring from polygonal approximation are smooth curves and error signals have two zero-ending points, we design new basis functions based on the Legendre polynomial and then transform the error signals with them. When applied to synthetic images such as circles, ellipses and etc., the proposed method provides, in overall, outstanding results in respect to the transform coding gain compared with DCT and DST. And in the case when applied to natural images, the proposed method gives better image quality over DCT and comparable results with DST.

A "GAP-Model" based Framework for Online VVoIP QoE Measurement

  • Increased access to broadband networks has led to a fast-growing demand for voice and video over IP(VVoIP) applications such as Internet telephony(VoIP), videoconferencing, and IP television(IPTV). For pro-active troubleshooting of VVoIP performance bottlenecks that manifest to end-users as performance impairments such as video frame freezing and voice dropouts, network operators cannot rely on actual end-users to report their subjective quality of experience(QoE). Hence, automated and objective techniques that provide real-time or online VVoIP QoE estimates are vital. Objective techniques developed to-date estimate VVoIP QoE by performing frame-to-frame peak-signal-to-noise ratio(PSNR) comparisons of the original video sequence and the reconstructed video sequence obtained from the sender-side and receiver-side, respectively. Since processing such video sequences is time consuming and computationally intensive, existing objective techniques cannot provide online VVoIP QoE. In this paper, we present a novel framework that can provide online estimates of VVoIP QoE on network paths without end-user involvement and without requiring any video sequences. The framework features the "GAP-model", which is an offline model of QoE expressed as a function of measurable network factors such as bandwidth, delay, jitter, and loss. Using the GAP-model, our online framework can produce VVoIP QoE estimates in terms of "Good", "Acceptable", or "Poor"(GAP) grades of perceptual quality solely from the online measured network conditions.
