The purpose of this paper is to develop the Plastic Cortex Stimulator(PCS) for stroke patients using ZigBee technology. The PCS consists of an implantable neuro-stimulator and a user controller, The neuro-stimulator has the stimulus circuit which is the H-bridge circuit to generate a bipolar pulse. The bipolar pulse is known to be effective for stroke recovery. The user controller sends several wave-shape parameters (amplitude, pulse width, cycle, etc.) to the neuro-stimulator for variable stimulation using ZigBee technology. The CC2420 and atmega128L was used to implement ZigBee protocol stack. The wireless control of PCS based on ZigBee can help the tele-rehabilitation of the stroke patients. The most effective pulse shape parameters are being investigated through animal experiments. The bio-compatibility and user-friendly interface are supposed to be handled in further study.
Purpose: This study was to identify the significant acute physiological predictors of mortality and of functional and cognitive recovery in hemorrhagic stroke patients. Methods: The subjects were 108 hemorrhagic stroke patients admitted to Neurological Intensive Care Unit of a university hospital. Results: The significant physiological predictors of mortality and of functional and cognitive recovery were quite different upon admission Glasgow Coma Scale scores: respiratory rate, hematocrit, serum pH, osmolality, and $PaCO_2$ were the predictors in the subjects with a high Glasgow Coma Scale scores while blood pressure, $PaO_2$, respiratory rate, and hematocrit in the subjects with a low Glasgow coma scale scores. Conclusion: The physiological derangements induced by acute stroke are undoubtedly influence clinical outcome. More study is required to determine their diverse impacts on clinical outcomes.
Aphasia is frequent in stroke patients and most patients with aphasia exhibit spontaneous progressive improvement in language abilities over time, but few recover completely. Neurological variables, especially initial severity of aphasia and time post-onset, appear to have influence on improvement. Effect of speech therapy and pharmacotherapy has been studied and some drugs, like amphetamine are proved to be benefit for recovery of aphasia following stroke. But there has been few evidence to facilitate recovery from aphasia by acupuncture or herbal medication therapy. So we report two cases of aphasic stroke patients who treated by combination therapy of acupuncture, herbal medication(Cheongsinhaeo-tang) and speech therapy over 6 months and improved in language abilities. Further clinical studies will be needed to explore the effects of acupuncture and herbal medication therapy for aphasia. Researchers should examine the long term effect of these treatment, and whether it is more effective than speech therapy and western pharmacotherapy or not..
Purpose: The purpose of this study was to propose a new therapy algorithm that combines motor imagery and physiotherapy as a physiotherapeutic clinical intervention technique that can stimulate the recovery of damaged physical function for patients with stroke. Methods: A variety of scientific research results related to motor imagery were reviewed and analyzed to investigate their applicability to physiotherapy in clinics. Results: As a new therapy algorithm for the therapeutic approach of motor imagery in stroke rehabilitation, a therapy algorithm that combines motor imagery with physiotherapy is proposed, which consists of three stages or steps: STEP 1 motor imagery familiarization, STEP 2 explicit learning stage, and STEP 3 implicit learning. Conclusion: The new therapy algorithm proposed in this study is expected to be a very useful clinical therapeutic approach for stimulating the recovery of damaged physical function in patients with stroke. It is believed that it will be necessary to confirm and standardize the effects of the therapeutic algorithm proposed in this study in the future by conducting diverse clinical studies.
Purpose: The purpose of this study was to determine the effect of a 2-week somatosensory stimulation program on cognitive function and ADL of patients with brain damage. Methods: The sample consisted of two groups of patients with stroke: 10 patients with a mean age of 59.0 years who were treated with somatosensory stimulation, and 9 patients with a mean age of 51.78 years, who were not treated with somatosensory intervention. A nonequivalent control group non-synchronized design was used to assess the functional recovery after stroke. Instruments used in this study were MMSE-K for cognitive function and FIM for ADL. Results: The hypothesis 1 that "Patients with stroke who were treated with the somatosensory stimulation program will show higher MMSE-K score than that of the non-treatment group" was supported(Z = -2.390, p = .017). The hypothesis 2 that "Patients with stroke who were treated with the somatosensory stimulation program will show higher FIM score than that of the non-treatment group", however, was partially supported(social cognition: Z = -2.204, p = .045). Conclusion: Somatosensory stimulation was effective to patients with stroke in improving their cognitive function. These findings suggest that somatosensory input can be adopted to nursing intervention for functional recovery after stroke.
In this study, the three type hybrid interrupters according to the arrangement of the thermal expansion chamber and the puffer cylinder(they are called 'serial type', 'parallel/exchanged type', and 'parallel/separated type' respectively in this work) were designed and manufactured. This paper presents the tested results of the thermal recovery characteristics on the interrupters using a simplified synthetic test facility. The 'serial type' hybrid interrupter which is to obtain more easily the pressure rise for the thermal recovery compared with the others has the best capability in the thermal recovery characteristics. In order to investigate the stress on the operating mechanism, the distortion of the stroke wave in on-load test was examined to the stroke curve in no-load test. The biggest distortion was occurred in the 'parallel/exchanged type' hybrid interrupter. Finally, the small interruption capability on the three type interrupters was estimated by a theoretical form and the 'parallel/separated type' hybrid interrupter has the advantage of the others in the view of structure.
The aim of this study was to evaluate the motor recovery in 4 chronic hemiparetic patients with Fugl-Meyer (FM) and EMG characteristics before and after the training program. The training was performed at 1hr/day, 5days/week during 6 weeks in 4 chronic stroke patients. Electromyographic activities of the affected hand were recorded during isometric wrist flexion/ extension movements. In all patients, FM was significantly improved after the 6-week training. Onset/offset delay of muscle contraction significantly decreased in the affected wrist after the training. The co-contraction ratio of flexor/extensor muscles decreased significantly. Also, onset/offset delay of muscle contraction and co-contraction ratio correlates significantly with upper limb motor impairment and motor recovery. This EMG technique allows an objective evaluation of changes in muscle activity in post-stroke patients, providing easily measurable, quantitative indices of muscle characteristics.
The aim of this study was performed to determine the inter-rater reliability of the Stroke Rehabilitation Assessment of Movement (STREAM) translated in Korean. This was a new clinical measurement tool for evaluating the recovery of voluntary movement and basic mobility following stroke. A direct-observation reliability study was conducted on 20 patients who had strokes and were in a rehabilitation setting. Subjects were assessed by two physical therapists. The reliability of the STREAM scores was demonstrated by weighted kappa statistics for inter-rater agreement on scores for individual items ranged from .83 to 1.0, intraclass correlation coefficients for total score was .99, and for subscale scores was ranged from .96 to .99. The internal consistency of the STREAM scores was demonstrated by Cronbach alphas of greater than .99 on the subscales and overall. These high levels of reliability support the use of the STREAM translated in Korean instrument for the measurement of motor recovery following stroke.
Background: Neuromodulation therapy has been used to an adjunctive treatment promoting motor recovery in stroke patients. The objective of the study was to determine the effect of repetitive transcranial magnetic stimulation (rTMS) on neurobehavioral recovery and evoked potentials in rats with middle cerebral artery occlusion. Methods: Seventy Sprague-Daley rats were induced permanent middle cerebral artery occlusion (MCAO) stroke model and successful stroke rats (n=56) assigned to the rTMS (n=28) and sham (n=28) group. The 10 Hz, high frequency rTMS gave on ipsilesional forepaw motor cortex during 2 weeks in rTMS group. The somatosensory evoked potential (SSEP) and motor evoked potential (MEP) were used to evaluate the electrophysiological changes. Behavioral function of the stroke rat was evaluated by the Rota rod and Garcia test. Results: Forty rats ($N_{rTMS}=20;\;N_{sham}=20$) completed all experimental course. The rTMS group showed better performance than sham group in Rota rod test and Garcia test at day 11 (p<0.05) but not day 18 (p>0.05). The amplitude of MEP and SSEP in rTMS group was larger than sham group at day 18 (p<0.05). Conclusions: These data confirm that the high frequency rTMS on ipsilesional cerebral motor cortex can help the early recovery of motor performance in permanent middle cerebral artery stroke model and it may simultaneously associate with changes in neurophysiological activity in brain.
Background: Stroke recovery is a long and complex process. Successful stroke recovery seems to be strongly associated with patients' high motivation and committed participation. Patients' motivation is a key determinant of successful rehabilitation outcomes, but it is difficult in defining and measuring. Patients' participation is defined as the degree or extent to which subjects take part in rehabilitation activities and can be measured by observable behavior. Objects: The purpose of this study was to investigate the impact of patients' level of participation in rehabilitation on functional outcomes in patients with stroke. Methods: Forty post-stroke inpatients participated in this study. The level of rehabilitation participation was measured by the Hopkins Rehabilitation Engagement Rating Scale (HRERS). Other measures used for the evaluation were the Rivermead Mobility Index (RMI) and Korean version of the Modified Barthel Index (K-MBI). Overall measurements were made at early intervention and late intervention. Spearman correlation and multiple regression were used to measure the relationships between HRERS, RMI, and K-MBI. Results: The correlation found between HRERS total scores at early intervention and RMI total scores of late intervention was above moderate (r = 0.607, p < 0.01). RMI total scores at early intervention (p < 0.000), HRERS total scores at early intervention (p < 0.001), and disease duration (p < 0.003) were significant predictors of RMI total scores at late intervention. Conclusion: The level of participation at early intervention was associated with improvement in mobility. The level of mobility at early intervention, disease duration, and patients' participation at early intervention were important determinants of functional outcome. These findings suggest that patients' participation should be encouraged in order to achieve successful stroke recovery.
