Analysis of Factors Affecting Profitability of General Hospital in Kyung-in Region (경인지역 종합병원의 수익성 관련요인 분석)

  • This study was attempted to identify the factors affecting profitability of general hospital in Kyung-In Region. Operating profit to gross revenues and net profit to gross revenues were used as a proxy indicator for profitability of hospitals. The unit of analysis was hospital, and the data were collected 5 years data from 20 hospitals. The major findings are as follows; (1) The average operating profit rate was 1.03% and the net profit rate was -5.00% in twenty hospitals in the Kyung-In Region for the last five years. In terms of maximum surplus, the operating profit rate was 14% and net profit rate was 3.40%. In terms of maximum loss revenue, the operating profit rate was -16.56% and the net profit rate was -22.83%. (2) Since the year 1993, which was the starting year of this study, the operating profits and the net profits consistently decreased. (3) Analyzing the difference in profits among various hospital groups, the tertiary hospital group and the 501-1000 beds group exhibited the highest in operating profit rate. Also, among the higher grade number of beds in hospital group, per 100 beds group, the 41-50 beds group exhibited the highest in operating profit rate. There is a statistically significant difference in those groups(p<0.05, p<0.01). (4) In the health care delivery system, the profit gain in the secondary hospital was 51.5% and in the tertiary hospital was 72.4%. Based on the number of beds in each hospital group, the highest profit gain was 75.0% in the over 1001 beds group, and 71.4% in the 501-1000 beds group. Also, among the higher grade number of beds in hospital group, per 100 beds group, the 41-50 beds group exhibited 88.6% surplus. (5) According to the surplus difference based on the analysis of health care utilization, a group with over 31 patients in bed turnover rate, a group with over 96% in bed occupancy rate and group with over 9% in emergency cases to outpatient visits exhibited the highest profit gains. In addition, a group with over 301 patients in daily outpatient visits per 100 beds and group with 11-12 days average length of stay exhibited the highest profit gains. These results are statistically significant(p<0.05, p<0.01). (6) According to a stepwise regression analysis, the variables measuring the bed turnover rate, number of licensed beds, and number of outpatient visits per specialist explain 34.1% of the variation in operating profits. In terms of net profits, the new outpatient visits, the bed turnover rates and the number of general bed variables explain 30.6%. These results are statistically significant(p<0.01).

Influence of Nursing Satisfaction, Self-Esteem and Depression on Adjustment of the Elderly in Long-term Care Hospital (요양병원 입원노인의 간호만족도, 자아존중감 및 우울이 적응에 미치는 영향)

  • This study was conducted to determine the influence of satisfaction, self-esteem, and depression on adjustment of elderly patients in long-term care hospitals. The subjects were 116 elderly patients in 15 long-term care hospitals in B and K city. Data were collected from February 14 to March 30, 2015, and analyzed by t-rest, ANOVA, Scheff's test, Pearson correlation coefficients, and stepwise multiple regression. According to the results, there were significant differences in adjustment by religion, marital status, economic status, and perceived level of difficult problems. Adjustment was positively related to satisfaction and self-esteem. In addition, a negative correlation was observed between adjustment and depression. Meaningful variables that influenced the adjustment of elderly patients were depression, satisfaction, self-esteem, and religion. These factors accounted for 53.5% of the total variance in adjustment of the elderly patients in long-term care hospitals, and depression was the most influential factor. In conclusion, development and implementation of effective programs and strategies to reduce depression and to increase satisfaction with care and self-esteem is needed in order to enhance the adjustment of elderly patients in long-term care hospitals.

Prognostic factors influencing clinical outcome of nonsurgical endodontic treatment (비외과적 근관치료의 임상적 성공에 영향을 미치는 예측 인자들의 평가)

  • Objectives: This study aimed to assess prospectively the clinical outcome of nonsurgical endodontic treatment and identify patient- and tooth-related factors, rather than treatment-related factors, that were the best predictors of this outcome. Materials and Methods: The inception cohort comprised 441 teeth (320 patients) and 175 teeth (123 patients) were followed up for 1-2 years. Age, gender, presence of medical disease, number of canals, previous endodontic treatment, presence of sensitivity and pain, pulp vitality, swelling or sinus tract of pulpal origin on the gingiva, periapical radiolucency and tendency of unilateral bite on the affected tooth were recorded at treatment start. Results: The outcome was classified on the basis of periapical radiolucency as healed or non healed. The overall healed rate in these cases, including nonsurgical retreatment, was 81.1%. Four tooth-related factors had a negative impact in the bivariate analysis: previous endodontic treatment, necrotic pulp, preoperative gingival swelling or sinus tract of pulpal origin, and preoperative periapical radiolucency. Stepwise logistic regression analysis including patient-, tooth-related factors and level of the root canal filling as a treatment-related factor showed that preoperative gingival lesion (odds ratio [OR]: 4.4; p = 0.005), preoperative periapical radiolucency (OR: 3.6; p = 0.011), and $\leq$ 1-2 mm under root filling length (OR: 9.6; p = 0.012) were significant predictors of failure. Conclusions: A preoperative gingival lesion of pulpal origin can influence the outcome of nonsurgical endodontic treatment in addition to preoperative periapical radiolucency.

Isolation of a Mutant with Thermotolerance and Ethanol Tolerance Using Proofreading-deficient DNA Polymerases in Saccharomyces cerevisiae (출아효모에서 proofreading-deficient DNA polymerase를 이용한 내열성 및 에탄올내성 변이 주의 분리)

  • In this study, we constructed a biological system that exhibited thermotolerance, ethanol tolerance, and increased ethanol productivity using a random mutagenesis method. We attempted to isolate a thermotolerant mutant using proofreading-deficient DNA polymerase ${\delta}$ and ${\varepsilon}$ encoded by the pol3 and pol2 genes, respectively, in Saccharomyces cerevisiae. To obtain mutants that could grow at high temperatures ($38^{\circ}C$ and $40^{\circ}C$), random mutagenesis of AMY410 (pol2-4) and AMY126 (pol3-01) strains was induced. The parental strains (AMY410 and AMY126) grew poorly at temperatures higher than $38^{\circ}C$. By stepwise elevation of the incubation temperature, AMY410-Ht (heat tolerance) and AMY126-Ht strains that proliferated at $40^{\circ}C$ were obtained. These strains were further incubated in medium containing 6% and 8% ethanol and then AMY410-HEt (heat and ethanol tolerance) and AMY126-HEt strain with ethanol tolerance at an 8% ethanol concentration was obtained. The AMY126-HEt strain grew even at an ethanol concentration of 10%. Furthermore, following the addition of high concentrations of glucose (5% and 10%), an AMY126-HEt3 strain with increased ethanol productivity was isolated. This strain produced 24.7 g/l of ethanol (95% theoretical conversion yield) from 50 g/l of glucose. The findings demonstrate that a new biological system (yeast strain) showing various phenotypes can be easily and efficiently bred by random mutagenesis of a proofreading- deficient mutant.

Effects of Self-directed Learning and Motivation to Transfer on Transfer of Learning for Nursing Students in Clinical Practice (간호대학생의 자기주도학습과 전이동기가 임상실습 중 학습전이에 미치는 영향)

  • The purpose of this study was to identify factors influencing the transfer of learning for nursing students in clinical practice. This study is a descriptive survey research conducted with 113 nursing students. Self-directed learning, motivation to transfer, and transfer of learning were measured. Data were analyzed by descriptive analysis, independent t-test, and ANOVA. The transfer of learning were significantly different according to the interpersonal relationship (t=10.43, p=.002), the satisfaction of nursing major (t=3.81, p=.006), satisfaction of nursing skills laboratory (t=4.61, p=.004). Transfer of learning had a correlation with self-directed learning, motivation (r=.46, p=<.001), and motivation to transfer (r=.60, p=<.001). In addition, motivation to transfer, the satisfaction of nursing skills laboratory, and learning evaluation were significant predictors of transfer of learning. Finally, in order to increase the transfer of learning for nursing students, nursing instructors need to encourage motivation to transfer, and to apply educational strategies that increase self-directed learning, as well as the satisfaction of the nursing skills laboratory.

The Influence of Clinical Violence Experience, Response to Violence Experience, Resilience on Academic Burnout among Undergraduate Nursing Students (간호대학생의 임상실습시 경험하는 폭력, 폭력경험 후 반응, 회복탄력성이 학업소진에 미치는 영향)

  • This study was a descriptive research to investigate clinical violence experience, response to violence experience, resilience and academic burnout and to identify the factors affecting the academic burnout among nursing students. The participants were 318 nursing students. who were recruited from the 2 Department of Nursing in B Metropolitan and 2 Department of Nursing in Gyeongsangnam-do. The structured questionnaire was self-administered from November 1 to 30, 2019. Data were analyzed through the SPSS/WIN 24.0 program using descriptive statistics, Independent t-test와 one-way ANOVA, Scheffe test, Pearson's correlation coefficient, and stepwise multiple regression. According to the results of this study, Mean score for academic burnout in nursing student was 2.58 out of 4 point. The factors of satisfaction with clinical practice(β=-.393, p<.001), satisfaction with nursing major(β=-.188, p<.001), responses to violence experience(β=.176, p<.001), perceived health status(β=-.156, p<.001) attacker(β=.135, p=.002), resilience(β=-.118, p=.016)were impact on the academic burnout level of nursing students. The total explanatory power accounted for 45.9%. In conclusion, since clinical practice satisfaction and major satisfaction are the factors that have the greatest influence on academic burnout, it is necessary to develop various educational strategies and a student counseling program to reduce negative reactions after experiencing violence and improve resilience. In addition, a qualitative study on the clinical violence experience, response to violence experience and academic burnout of nursing students is needed.

Assessment of Potential Distribution Possibility of the Warm-Temperate Woody Plants of East Asia in Korea (한국에서 동아시아 난대 목본식물의 잠재분포 가능성 평가)

  • The prediction of changes regarding the distribution of vegetation and plant species according to climate changes is important for ecosystem management. In this study, we attempted to develop an assessment method to evaluate the possibility of the potential distribution of warm-temperate woody plant species of East Asia in Korea. To begin with, a list of warm-temperate woody plants distributed in China and Japan, but not in Korea, was prepared, and a database consisting their global distribution and bioclimatic variables was constructed. In addition, the warm-temperate vegetation zone in Korea was delineated using the coldness index and relevant bioclimatic data were collected. After the exclusion of multicollinearity among bioclimatic variables using correlation analysis, mean temperature of the coldest quarter, mean temperature diurnal range, and annual precipitation were selected as the major variables that influence the distribution of warm-temperate plants. A multivariate environment similarity surfaces (MESS) analysis was conducted to calculate the similarity scores between the distribution of these three bioclimatic variables in the global distribution sites of the East Asian warm-temperate woody plants and the Korean warm-temperate vegetation zone. Finally, using stepwise variable-selection regression, the mean temperature of the coldest quarter and annual precipitation were selected as the main bioclimatic variables that affect the MESS similarity index. The mean temperature of the coldest quarter accounted for 88% of the total variance. For a total of 319 East Asian warm-temperate woody plant species, the possibility of their potential distribution in Korea was evaluated by applying the constructed multivariate regression model that calculates the MESS similarity index.

Factors Affecting Subjective Well-being and Depression of the Elderly with Chronic Disease (만성질환을 가진 노인의 주관적 행복감과 우울에 영향을 미치는 요인)

  • This study was conducted to investigate the effect on subjective well-being and depression of the elderly with chronic diseases. Using data from the 15th Korea Welfare Panel in 2020, 3,910 people who responded that they had chronic diseases over the age of 65 were analyzed with stepwise regressions. As a result, the factors affecting the subjective well-being of the elderly with chronic diseases were in the order of satisfaction with leisure life, subjective health status, satisfaction with children relationship, satisfaction with family income, satisfaction with spouse. The explanatory power of this model was 32.0% (F=351.44 p<.001). And also, factors affecting depression were in order of subjective health status, satisfaction with spouse, satisfaction with social relationship, satisfaction with children relationship, satisfaction with leisure life, and satisfaction with family income. The explanatory power of this model was 21.4% (F=178.59, p<.001). Therefore, policy systems to support leisure life of the elderly with chronic disease and measures to induce participation in programs using community resources are needed. And health management programs and institutional support to improve subjective health status are also needed. In addition, it is expected that economic support at the national policy level and various program execution strategies and support personnel will be secured at the community level.

Exploring the moderating effect of learning motivation on the effect of cognitive presence on cognitive control among the components of self-regulation in blended classes (블랜디드 수업에서 자기조절의 구성 요인 중 인지조절에 대한 인지적 실재감의 영향에 대한 학습동기의 조절효과 탐색)

  • Purpose of study: The purpose of this study is to explore the moderating effect of learning motivation in the effect of cognitive presence on cognitive regulation. Research content and method: The subjects of this study were 160 preservice teachers who are enrolled in the Faculty of Education of a university. The survey measured cognitive control, human presence, and learning motivation. Among the collected data, learning motivation was averaged. In addition, a group with low learning motivation and a group with high motivation were distinguished. Next, this study compared the averages of cognitive control by classifying the learning motivation levels. The collected data was analyzed using a multiple regression (Stepwise) method to explore the effect of cognitive presence on cognitive control. First, multiple regression analysis was performed on the entire group. Second, multiple regression analysis was performed by dividing the group with low learning motivation from the group with high motivation. Conclusions and Suggestions: As a result of the analysis, the group with high learning motivation showed a higher level of cognitive control than the group with low motivation. In the group with high learning motivation, only content understanding among the sub-factors of cognitive presence had an effect on cognitive control. In the group with low learning motivation, all factors of cognitive presence were affected.

The Clustering and Variables in Discriminating the Groups by the Level of the Loss Experience and Ego-integrity in the Elderly (노인의 상실경험과 자아통합감 수준에 따른 적응군집화 및 집단판별에 기여하는 변인)

  • The purpose of this study was to search the clustering and variables in discriminating the groups by the level of the loss experience and ego-integrity among the elderly. In addition, the study aimed to confirm the moderating effects of variables related to resilience in the relationship between a loss experience and ego-integrity. The subjects were 245 elderly aged 60 or above, most of whom were socially active with comparatively high education levels. Six individual variables(physical self-efficacy, general self-efficacy, social self-efficacy, existential spirituality, religious spirituality, optimism) and five environmental variables(emotional support, instrumental support, informational support, appraisal support, social activity) were used in the study. The results of the study are as follows. To data analysis, ANOVA, Stepwise Discriminant analysis, Cluster analysis, Regression analysis and Two-way analysis of variance was employed. First, the subjects were 4 cluster into overcome group, crisis group, stable group and vulnerable group according to the level of loss experience and ego-integrity. Second, optimism, physical self-efficacy, existential spirituality and emotional support were seen to be significant variables in discriminating the groups. Third, the ego-integrity became higher as the variables related to resilience were increased.
