Clinical Analysis of Spontaneous Pneumomediastinum (자발성 종격동기종의 임상적 고찰)

  • Background: Spontaneous pneumomediastinum (SPM) is a relatively rare and benign condition that generally occurs in young adults without any precipitating factor or disease. The purpose of this study was to assess whether more uncomforting diagnostic procedures are necessary and to establish standards in the diagnosis and treatment of spontaneous pneumomediastinum. Material and Method: A retrospective study was done on 18 patients from the hospitals of Hanyang University Seoul Hospital and Hanyang University Guri Hospital between February, 1997 and June, 2004. All patients had presence of mediastinal air without a pneumothorax and no evidence of trauma or barotrauma. Result: Among the 18 patients, the majority were male patients with only two female patients. Their mean age was 20.95 years old with standard deviation of 14.3 years. The most common complaints were chest pain, dyspnea, and coughing. Evaluation included simple chest roentgenogram in all patients, 10 patients had a chest tomographic scan, 10 patients had an esophagoscopic exam, 6 patients had a bronchofiberoscopic exam, and 3 patients had an esophagogram done. The mean hospital stay was 10.9 days. All patients were treated conservatively and in a follow-up of 1 $\∼$ 8 years only one recurrence was found. Conclusion: SPM is caused by alveolar rupture in the pulmonary interstitium leading to dissection of air towards the hilum and mediastinum. Although SPM is a self-limiting condition, evaluation should include chest roentgenogram and chest tomographic scans to rule out any other secondary condition. More aggressive evaluation seems unnecessary.

Effects of Different Exercise Intensity on FDEIA and Related Mechanisms (운동 유발성 알레르기 질환(FDEIA)에 미치는 영향과 기전분석)

  • Food-dependent exercise-induced anaphylaxis (FDEIA) is a distinct form of food allergy induced by physical exercise. It is typified by the onset of anaphylaxis during exercise, which is preceded by the ingestion of causal food allergens. Diagnosis of FDEIA is heavily dependent on clinical history. To describe the physiopathological mechanism, etiologic factors, and clinical manifestations, we evaluated the spleen index, proliferation assay of lymphocyte, ROS, ASAS, and cytokines levels in sensitized and exercise-trained mice. One-hundred mice were bred in the animal lab at D and P university under controlled conditions [$22{\pm}2^{\circ}C$, RH 45-55%, and a 12-hour photoperiod]. Animals are 7-weeks-old at the time of study and were fed a standard commercial chow diet from 09:00 to 15:00 over the 8-week study period. The mice were allowed access to distilled deionized water ad libitum. Daily food intake and weekly body gains were routinely recorded throughout the experimental period using computing scale (CAS). Mice were divided into the control group (S; control sensitized, n=25), 30 min swim training group (S30, N=25), 50 min swim training group (S50, N=25), and 80 min swim training group (S80, N=25). The results were as follows: Spleen index showed the highest level in the S80 group compared to other groups; this level was exercise-dependent. In proliferation assay of Med and OVA, the S80 group showed the highest level compared to the other groups; this level also was exercise intensity- dependent. Peritoneal ROS and IL-4 showed a statistically significant difference compared to S; however, there was no significant differences in ROS among S30, 50, and 80. From the results, we concluded that FDEIA is correlated with exercise intensity based on the levels of peritoneal ROS and cytokine profiles.

Use of radiotherapy in patients with palliative double bypass for locally advanced pancreatic adenocarcinoma

  • Purpose: Pancreatic cancer (PC) has not changed overall survival in recent years despite therapeutic efforts. Surgery with curative intent has shown the best long-term oncological results. However, 80%-85% of patients with these tumors are unresectable at the time of diagnosis. In those patients, first therapeutic attempts are minimally invasive or surgical procedures to alleviate symptoms. The addition of radiotherapy (RT) to standard chemotherapy, ergo chemoradiation, in patients with locally advanced pancreatic cancer (LAPC) is still controversial. The study aims to compare outcomes in patients with a double bypass surgery due to LAPC treated or not with RT. Materials and Methods: A retrospective cohort study of patients with double bypass for LAPC were registered and divided into two groups: treated or not with postoperative RT. Baseline characteristics, postoperative complications, those related to RT and their relation to the main event (mortality) were compared. Results: Seventy-four patients were included. Surgical complications between the groups did not offer significant differences. Complications related to RT were mostly mild, and 86% of patients completed the treatment. Overall survival at 1 and 2 years for patients in the exposed group was 64% and 35% vs. 50% and 28% in the non-exposed group, respectively (p = 0.11; power 72%; hazard ratio = 0.53; 95% confidence interval, 0.24-1.18). Conclusion: We observed a tendency for survival improvement in patients with postoperative RT. However, we've not had enough power to demonstrate this difference, possibly due to the small sample size. It is indispensable to develop randomized and prospective trials to guide more specific treatment lines in this patients.

A Study on Middle and High School Girl Students' Menstruation Characteristics and their Menstruation Dysmenorrhea (중.고등학교 여학생의 월경 특성과 월경곤란증에 관한 연구)

  • Middle and high school girl students' menstruation characteristics, appearance of menstruation dysmenorrhea, and coping with the menstruation dysmenorrhea were examined in order to give information in instructing girl students about menstruation dysmenorrhea, in delivering health education in the school, and establishing health policy. 970 girl students in middle and high schools in Cheju-do were selected and surveyed using the questionnaire the author developed based on the previous studies. The findings are as follows. First, the age of first menstruation was average $12.9{\pm}1.22$ years. The younger they were, the earlier their first menstruation came. The most common menstrual cycle was such one that continuously irregular from the beginning or that regular and irregular cycle run together. The first irregular menstrual cycle changed regular one within 1 or 2 years. Most of the respondents had normal menstrual cycle of $21{\sim}35$ days and menstruation period of $3{\sim}8$ days. Second, symptom of menstruation dysmenorrhea was orderly; hurt in the underbelly, tired, hurt in the waist, and headache. It was higher with high school students than middle school students. For the most part, time of heavy menstruation dysmenorrhea was the 2nd day after menstruation starts and period of menstruation dysmenorrhea was 2 days. More than the half of the respondents felt that the present menstruation dysmenorrhea was similar with the first one and they felt not comport in daily life due to menstruation dysmenorrhea, and 'hard to live a normal life' was higher with the high school girl students than with the middle school students. Third, how to cope with menstruation dysmenorrhea was enough sleep, relax and rest, taking a hot water shower, listening to music or watching a movie, taking medicine, and doing excercise. Few of the respondents have ever visited a doctor due to menstruation dysmenorrhea, and the reason was mostly irregular menstruation and heavy dysmenorrhea. Forth, when they took medicine due to menstruation dysmenorrhea, they gathered information on the medicine from family members and friends, while extremely low from a pharmacist or a doctor. The most of them have ever took medicine during menstruation once and took standard dose. While excess dosage was higher with high school students than with middle school students. From these findings, a good many girls are experiencing menstruation dysmenorrhea since their first menstruation. To discover early the severely abnormal findings and to treat, more interest from home and school, counseling and sexual education on time, and expert's diagnosis are necessary. Rather than unconditionally taking pain-relievers which can plainly solve the pain, continuous health education at home and in the school should be carried on for the girls to practice proper measures one think effective or an expert recommends.

The Examination of Load Carrying Capacity Based on Existing Data for Improved Safety Assessment Method of Expressway Bridges (고속도로 교량의 개선된 안전성 평가방안을 위한 실측자료에 기초한 공용 내하력 검토)

  • The safety of expressway bridges was estimated by checking the external condition rank based on the nondestructive inspection and material test and by measuring load carrying capacity based on the result of load test. Although the load carrying capacity of the bridges was clearly low compared to the design standard, it was examined that many of the bridges have good external condition rank relatively. Also, it can be assured that load carrying capacity shows a considerable difference according to various condition even though the bridges have similar construction year and a structural type. Therefore, this study showed various problems of the current safety measurement of expressway bridges by considering the status of the expressway bridges, external condition rank, and method of safety diagnosis and repair, rehabilitation for maintenance. Based on the existing data of over 400 expressway bridges, the load carrying capacity was analyzed quantitatively considering bridge type, serviced life, design live load, external condition rank and traffic count as variables. The result of this study will be expected to provide the basic information for a reasonable safety assessment of expressway bridge.

Use of Artificial Intelligence for Reducing Unnecessary Recalls at Screening Mammography: A Simulation Study

  • Objective: To conduct a simulation study to determine whether artificial intelligence (AI)-aided mammography reading can reduce unnecessary recalls while maintaining cancer detection ability in women recalled after mammography screening. Materials and Methods: A retrospective reader study was performed by screening mammographies of 793 women (mean age ± standard deviation, 50 ± 9 years) recalled to obtain supplemental mammographic views regarding screening mammography-detected abnormalities between January 2016 and December 2019 at two screening centers. Initial screening mammography examinations were interpreted by three dedicated breast radiologists sequentially, case by case, with and without AI aid, in a single session. The area under the receiver operating characteristic curve (AUC), sensitivity, specificity, and recall rate for breast cancer diagnosis were obtained and compared between the two reading modes. Results: Fifty-four mammograms with cancer (35 invasive cancers and 19 ductal carcinomas in situ) and 739 mammograms with benign or negative findings were included. The reader-averaged AUC improved after AI aid, from 0.79 (95% confidence interval [CI], 0.74-0.85) to 0.89 (95% CI, 0.85-0.94) (p < 0.001). The reader-averaged specificities before and after AI aid were 41.9% (95% CI, 39.3%-44.5%) and 53.9% (95% CI, 50.9%-56.9%), respectively (p < 0.001). The reader-averaged sensitivity was not statistically different between AI-unaided and AI-aided readings: 89.5% (95% CI, 83.1%-95.9%) vs. 92.6% (95% CI, 86.2%-99.0%) (p = 0.053), although the sensitivities of the least experienced radiologists before and after AI aid were 79.6% (43 of 54 [95% CI, 66.5%-89.4%]) and 90.7% (49 of 54 [95% CI, 79.7%-96.9%]), respectively (p = 0.031). With AI aid, the reader-averaged recall rate decreased by from 60.4% (95% CI, 57.8%-62.9%) to 49.5% (95% CI, 46.5%-52.4%) (p < 0.001). Conclusion: AI-aided reading reduced the number of recalls and improved the diagnostic performance in our simulation using women initially recalled for supplemental mammographic views after mammography screening.

Patterns of Mercury Concentrations in Blood and Urine After High Mercury Exposure (고농도 수은 노출자의 혈 중 및 뇨 중 수은 농도 변화에 관한 연구)

  • Blood and urine mercury level of three workers were monitored during 60~80 days after high exposure to mercury at the silver refining plant. Mercury was used to form silver-mercury amalgam from plating sludge. Workers were exposed to mercury about 70 days at the several processes, such as hand held weaving, vibration table, and heating from the furnace. mercury was analysed by atomic absorption spectroscopy-vapor generation technique. Recovery from the biological sample was 95.51% and pooled standard deviation was 0.033. At the time of study, there was no work at the workplace. So, airborne mercury concentration was measured with area sampling 5 days after the work, ranged from 0.1459 to 1.2351 mg/㎥(Arithmatic mean 0.4711 mg/㎥, Geometric mean 0.3566 mg/㎥) at the inside of the plant, that is far above the ACGIH's TLV(0.025 mg/㎥) and ranged from 0.0073 to 0.0330 mg/㎥ at the outdoor. Blood mercury levels at the beginning of the monitoring were 4~14 times greater than the American Conference of Governmental Industrial Hygienists Biological Exposure Index(ACGIH BEI, 15 ug/L). Blood mercury levels were decreased logarithmically, that is, rapidly at the high level and slowly at the low level but sustained above the level of the ACGIH BEI 60~80 days after the work. Urine mercury levels at the beginning of the monitoring were 8~16 times greater than the ACGIH BEI(35 ug/g creatinine). Urine mercury levels were decreased logarithmically, but correlation between urine level and off-days were lower than those of blood. Decreasing pattern of blood mercury levels were little affected than that of urine levels when the chelating agent, D-penicillamine, was administered. There was correlation between blood mercury level and urine mercury level(0.81~0.83) but it didn\`t mean that the highest blood mercury level corresponded the highest urine mercury level. In our study, Case 1 always shows the highest level in urine but case 3 always shows the highest level in blood. Creatinine correction represented better correlations between urine mercury levels and blood levels, and between urine levels and off-days rather than by urine volume. Spot urine sampling had a wide variation than that of whole day urine sampling. So, We recommend spot urine sampling for screening and whole day urine sampling for exact diagnosis.

  • In order to assess the safety diagnosis and grouting reinforcement effect of old reservoir facility, local governments and public offices mainly use electrical resistivity survey. However, electrical resistivity survey is a qualitative evaluation that varies the resistivity value by various exploration conditions. It is also difficult to grasp the stiffness change directly related to the stability of reservoir, thus an electrical resistivity survey is not applicable to continuous stability monitoring after grouting. The purpose of this study is to investigate and validate the quantitative evaluation of reinforcement effect of reservoir with cement grouting through shear velocity (Vs), which is closely related to the stiffness (${\mu}$) of the ground. This study was carried out on two reservoir facilities. The reinforcement effect was evaluated by comparing the permeability test, standard penetration test, down-hole test and MASW(Multi-channel Analysis of Surface wave) survey before and after cement grouting. Shear wave velocity changes before and after grouting were analyzed by phase velocity difference and inversion analysis, respectively, and the reliability of the analytical results was evaluated by comparing with field test results. Shear wave velocity increases to 5~10% in case of the D levee, and 10~20% in the levee of H reservoir. These results are showed similar pattern to the field test results.

Monitoring of the mortalities in the aquaculture farms of South Korea (한국 주요 양식종의 수산동물 폐사피해 모니터링)

  • The practical monitoring was performed to survey the mortalities in aquaculture farms of olive flounder, rockfish, rainbow trout, Japanese eel, white shrimp and abalone in South Korea from May to November, 2011. The aquatic organism disease inspectors, who have the national licenses for the diagnosis and prevention of aquatic disease and have close relationship with the farms, investigated the rates and causes of mortalities according to the standard manual. In 70 flounder farms, the cumulative mortalities rate was 27.9%, and the mortalities were caused by scuticociliatosis, streptococcosis, VHS, non-infectious loss, vibriosis and gliding bacterial disease. The moralities rate of 30 rockfish farms was 13.6%, and those were mainly contributed by gill flukes and streptococcosis. Most of mortalities of rainbow trout were caused by non-infectious loss and protozoan white spot disease. The mortalities rate of Japanese eel was 0.6% by edwardsiellosis, protozoan white spot disease and gill flukes. The loss rate of white shrimp was 71.2%, and most of them was related with non-infectious ones, such as carnivalization, transportation loss, and the rest was caused by viral white spot disease. The mortalities rate in the abalone farms was 10.7% and all of them were related with non-infectious loss.

  • Domestically, technical condition evaluation for breakwaters has been conducted through safety checks and safety diagnosis. If necessary, maintenance for the facility is conducted. However, in recent years, the need for infrastructure management has been increasing from a life-cycle-cost perspective. For these reasons, the "Sustainable Infrastructure Management Basic Act" was enacted. Previously, only the technical part of the breakwaters was evaluated. However, based on the act, comprehensive management of breakwaters will be possible through performance improvement by adding economic and political evaluations. In this study, evaluation factors and evaluation methods were developed for a breakwater rehabilitation project. The purpose of this study is to present a development direction, items, and methods for the evaluation of breakwater performance improvement to be applied in future practice. This study reviews the concept of maintenance and performance improvement, the literature related to performance improvement, and the project type for the common standard of performance improvement. Evaluation items and evaluation methods for breakwater rehabilitation are suggested to be reviewed by experts. The methodology suggested in this study could be used for preventive maintenance and to reduce accidents.
