중소 종합병원 간호사의 직무배태성이 이직의도에 미치는 영향 (Influence of Job Embeddedness Factors on Turnover Intention of Nurses in Small and Medium Sized General Hospitals)

  • Purpose: This study was done to investigate the influence of organization and community job embeddedness on turnover intention of nurses in small and medium sized general hospitals. Methods: The participants, 333 nurses, were recruited from small and medium sized general hospitals in Korea. Data were collected by self-report questionnaires on job-embeddedness and turnover intention and were analyzed using descriptive statistics, Independent t-test, One-way ANOVA and $Scheff{\acute{e}}^{\prime}$, Pearson correlation coefficient, and multiple linear regression analysis with the SPSS 18.0 program. Results: The score for job embeddedness and it's 6 factors, and turnover intention were above 3 on a 5 point scale with the exception of organizational sacrifice. There was a significant difference in turnover intention according to age, marital status, salary, and position. There were significant negative correlations between the 6 factors of job embeddedness and turnover intention. Variables entered in multiple regression showed that organizational sacrifice, organizational fit and age were significant contributing factors to turnover intention. Conclusion: The findings suggest that there is a need for strategies to enhance job embeddedness, especially organization sacrifice and organization fit. These factors should be developed and used to decrease turnover intention of nurses in small and medium sized general hospitals.

중소병원 입원환자의 낙상발생 시 현황, 낙상위험요인 및 낙상발생 예측요인 (Circumstances, Risk Factors, and the Predictors of Falls among Patients in the Small and Medium-sized Hospitals)

  • Purpose: This study examined the circumstances, risk factors, and the predictors of fall incidents among patients in the small and medium-sized hospitals. Methods: Fifty patients with any fall experiences were matched by gender, age, and medical departments with 100 patients without fall incident at the same hospital. Data were collected from 5 small and medium-sized hospitals. Data were analyzed using descriptive statistics, a Chi-square test, a Fisher's exact test, and a logistic regression with the SPSS/WIN 21.0 program. Results: In the patients with falls, the largest number of falls occurred during the day shift, in the patients' rooms, and while they were walking. Further 74.0% of the patients had physical injuries, and 34.0% had to take further medical diagnostic tests. Significant differences were found between the patients with falls and the others on 14 variables (cardiovascular disease, anemia, sedative-hypnotics, vasodilators, narcotic analgesics, dizziness, general weakness, unstable gait, walking aids, anger, anxiety, depression, orientation, and fear of fall). Narcotic analgesic use, dizziness, walking aids, and cardiovascular disease were identified as the predictors of fall incidents. Conclusion: These findings are hoped to be used in developing a fall risk assessment tool and fall prevention nursing programs for small and medium-sized hospitals.

  • Objectives: The purpose of this study was to investigate the relationship between medical service quality, emotional attachment, customer satisfaction, and customer behavioral intention of small and medium hospitals. Methods: 228 patients who have used small and medium hospitals' medical services were selected. The analysis methods used were confirmatory factor analysis, validation of discrimination, path analysis, and bootstrapping using SPSS 23 and AMOS 23. Results: As a result of the hypothesis test, reliability, responsiveness, and assurance were found to have a positive (+) influence on emotional attachment and customer satisfaction, while empathy had a positive (+) influence on emotional attachment only. Emotional attachment had a positive (+) influence on customer satisfaction and behavioral intention, and customer satisfaction had a positive (+) influence on behavioral intention. Conclusions: Medical service providers in small and medium hospitals should recognize that increasing customer satisfaction based on emotional attachment is a pathway to customer acquisition and, ultimately, a way to promote effective management.

중소병원간호사의 재직의도에 미치는 영향 요인: 회복탄력성과 팔로워십 중심으로 (Affecting Factors of Small and Medium-sized Hospital Nurses' Intention on Retention: Focused on Resilience and Followership)

  • Purpose: The purpose of this study was to identify the relationships between variables and factors affecting the intention on retention of nurses in small and medium-sized hospitals. Methods: The participants of this study were 203 nurses working in six small and medium-sized hospitals, and data was collected from August 15 to August 30, 2019 using a structured questionnaire. The collected data were analyzed by t-test, ANOVA, Pearson's correlation coefficients, and multiple regression using IBM SPSS/WIN 24.0. Results: As a result of the study, intention on retention was positively correlated with resilience (r=.40, p<.05) and followership (r=.38, p<.05). Affecting factors of the intention on retention were marital state (married) (β=.21, p=.001), resilience (β=.22, p=.016), and active engagement as a subcartegory of followership (β=.19, p=.034), and the variables explained 20.8% of the nurses' intention on retention. Conclusion: In order to increase the intention on retention of small and medium-sized hospitals nurses, it is necessary to provide an environment in which small and medium-sized hospital nurses are able to have active engagement and independent thinking and to develop a program to promote their resilience.

중소병원 간호사 이직의도 영향 요인 (Factors Influencing Turnover Intention of Nurses in Small-medium sized Hospitals)

  • Purpose: This study was done to examine the relationships among practice environment, nursing professionalism, career commitment, and turnover intention in nurses working in small-medium sized hospitals. Method: This study utilized a descriptive correlational design. Data were collected using structured questionnaires from 362 nurses in 7 small-medium sized hospitals and analyzed with SPSS 18.0. Result: The mean scores for practice environment, nursing professionalism, career commitment, and turnover intention were 2.29 on a 4-point scale and 3.13, 2.78, 3.51 on a 5-point scale, respectively. The practice environment showed significantly positive correlations with nursing professionalism and career commitment. Nursing professionalism showed a significantly positive correlation with career commitment. The highest significant negative correlation was between practice environment and turnover intention. Nursing professionalism and career commitment had negative correlations with turnover intention. Factors having significant influence on turnover intention included age, assigned ward, average wage, practice environment, and career commitment. These factors explained 43.0% of variance in turnover intention. Practice environment was identified as the most important variable in explaining turnover intention. Conclusions: The results of this study demonstrate the necessity of improving the practice environment and increasing nurses' career commitment if turnover intention in small-medium sized hospitals is to be lowered.

중소병원 간호사의 셀프리더십, 조직몰입, 간호근무환경이 임상수행능력에 미치는 영향 (Effect of Self-Leadership, Organizational Commitment, Nurse Practice Environment on Clinical Competency of Nurses in Small and Medium sized Hospitals)

  • 본 연구는 중소병원 간호사의 셀프리더십, 조직몰입 및 간호근무환경과 임상수행능력 간의 관계를 확인하고, 이들 요인이 임상수행능력에 미치는 영향을 규명하고자 시도되었다. 연구대상자는 대전 및 충청북도 지역의 300병상 미만의 중소병원 3곳에서 교대 근무를 하는 간호사 123명이며, 자료수집은 2022년 4월동안 시행되었다. 자료분석은 기술통계, t-test, ANOVA, Pearson's correlation coefficients, multiple regression analysis를 이용하여 시행되었다. 연구결과 대상자의 임상수행능력에 영향을 미치는 주요 요인은 조직몰입과 셀프리더십으로 나타났으며, 모형의 전체 설명력은 15.4%로 나타났다. 따라서 중소병원 간호사의 임상수행능력을 높이기 위해서 조직몰입과 셀프리더십을 향상시킬 수 있는 프로그램 개발 및 적용이 필요하다.

AHP를 이용한 중·소 병원 입지선택요인 분석 : 척추·관절 병원중심으로 (An Analysis of the Locational Selection Factors of the Small- and Medium-sized Hospitals Using the AHP : Centered on the Spine and Joint Hospitals)

  • 본 연구는 급격한 사회 경제적 여건의 변화에 따라 중소병원의 설립에 대한 선택요인 및 입지 선정 요인의 중요성이 점점 커짐에 따라 의료서비스 시설의 선택요인 및 입지선정 요인에 대해 실증 분석하였다. 본 연구는 중 소 병원 선택요인 및 입지선정요인에 관한 연구를 통하여 각 평가항목 요인들 간의 중요도에 따른 우선순위를 분석하고, 부동산 입지적 요인과 비입지적 요인을 구분하여 기존 중소병원의 경쟁력에 미치는 중요 요인이 무엇인지 도출하여, 신규 중 소 병원 개원을 준비하고 있는 의료 공급자에게 중소병원 선택요인의 중요 요인에 따른 입지특성을 고려한 중소병원의 개원전략의 기초 자료로 할용 하는데 목적이 있다. 기존의 선행연구와 사례연구 결과를 바탕으로 의료수준, 의료서비스, 병원의 접근성, 병원의 편리성, 물리적 환경 측면에서 28개의 평가요인을 도출 하였으며, 수집된 28개의 세부평가요인은 관련 전문가들과의 인터뷰를 통하여 의료수준, 의료서비스, 병원의 전문성, 병원의 편리성, 물리적환경 등 5개 요인을 상위 계층 평가요인으로 선정하였고 각 상위 계층에 따라 총 28개의 하부 평가 요인을 선정하였다. 선정된 최적 정평가요인들은 의료전문가 200명을 대상으로 AHP 설문조사를 실시하여 최적 입지요인을 선정하였다. AHP 분석결과는 선행연구의 사례와 유사하게 의료수준, 의료서비스, 병원의 접근성, 물리적 환경, 편리성의 순으로 중요도가 나타났으며, 병원의 시설과 관련된 요인은 낮게 나타났다. 본 연구의 결과는 향후 중 소병 입지선정 의사 결정을 위한 기초를 제공하는데 응용할 수 있을 것이다.

중소 병원 간호사들의 업무상 손상경험에 영향을 미치는 요인파악 (Factors Associated with Work-Related Injuries of Nurses in Small and Medium Sized Hospitals)

  • Purpose: This study was conducted to examine the factors associated with work related injuries of nurses in small and medium sized hospitals. Method: A cross-sectional survey was conducted with nurses in eight hospitals from October 2007 to January 2008. A questionnaire was designed to collect information on nurses' work related injuries, and individual and job related characteristics. The response rate was 65.1%. Data from 294 nurses were analyzed. Multiple logistic regression analysis was performed to determine factors associated with work related injuries. Result: Of the 294 nurses, 19.1% (n=56) responded as having at least one injury during their job performance. The logistic regression analysis revealed that the significant factors influencing work related injuries were job satisfaction, stress recognition, and hospital's location. Nurses with a higher job satisfaction were less likely to experience work related injuries (OR=0.58). Nurses with a higher stress recognition (OR=2.57) and those working at hospitals in metropolitan cities (OR=3.28) were more likely to experience work related injuries. Conclusions: The result of this study indicated that a substantial proportion of nurses in small and medium sized hospitals had experienced injuries related to nursing job. Interventions to prevent work related injuries among nurses should take into account the job satisfaction, stress recognition, and hospital characteristics.

A Study on the Medical tourism customized service platform for clinic

  • Through diversification of 4th industries and transportation and globalization, tourism was developed for medical purposes, not limited to touring tourist attractions. The trend of medical tourism in Korea is shifting from large and medium-sized hospitals to professional medical services in one area of small hospitals rather than professional medical services. However, small hospitals lack medical coordinators and interpretation services, and tourists who visit Korea for treatment lack information on small hospitals. Therefore, a platform is needed to match small hospitals with medical tourists. In this study, using Platform as a Service (Paas) in cloud computing, clinic administrators can access the app to introduce information about their hospitals with simple manipulations. Tourists also want to propose a customized platform to select the right area for them to check information about small-scale hospitals and choose the treatment and tourist attractions they wants.   • Purpose: The purpose of this study was to investigate the factors influencing nurse turnover intention in small and medium sized hospitals in the metropolitan area. Methods: A cross-sectional survey design was used. A questionnaire was distributed to the nurses in small and medium sized hospitals. The data of 209 nurses was analyzed using t-test, ANOVA, Pearson correlation coefficient and multiple regression. Results: Organizational commitment and age were identified as factors influencing turnover intention. These factors explained 56.8% of variance of turnover intention. Conclusion: Results suggest that establishment of organizational commitment needs to be discussed and continuously developed. Further study is also necessary to identify the key mechanism in nurse turnover intention.
