Sleep Quality and Associated Factors Among Firefighters in Bangkok, Thailand: A Cross-sectional Study

  • Objectives: Professional firefighters face various health hazards and are required to maintain both physical and mental fitness to effectively mitigate crises and respond to emergencies. Moreover, the long working hours and shift work of this profession impact sleep quality. This study investigated the quality of sleep and its associated factors among firefighters in Bangkok, Thailand. Methods: This was a cross-sectional study of firefighters affiliated with the Bangkok Fire and Rescue Department, Bangkok Metropolitan Administration, Thailand between January 2023 and March 2023. A cluster random sampling technique was utilized to distribute 600 questionnaires to firefighters in 15 fire stations. The questionnaire addressed demographic, work-related, and environmental factors. Sleep quality was assessed using the Thai version of the Pittsburgh Sleep Quality Index (PSQI). Data analysis involved both descriptive and inferential statistics. Bivariate and multiple logistic regressions were performed. Results: The response rate was 78.7% (n=472), and 44.1% of the firefighters reported poor sleep quality (PSQI score >5). Sleep quality was statistically associated with conflicting family relationships (odds ratio [OR], 1.8; 95% confidence interval [CI], 1.1 to 2.9), additional part-time jobs (daytime, OR, 2.4; 95% CI, 1.3 to 4.4; or nighttime, OR, 4.3; 95% CI, 1.1 to 16.7), noisy sleeping areas (OR, 1.7; 95% CI, 1.1 to 2.8), and the availability of adequate bedding (OR, 3.0; 95% CI, 1.8 to 4.9). Conclusions: Poor sleep quality among firefighters was associated with various personal, work-related, and environmental factors. Organizations should promote policies that improve sleep quality through good sleep hygiene practices and facilities.

수면의 질과 구강 안면 통증의 관계 (Relationship of Orofacial Pain and Sleep Quality)

  • 최근까지 수면과 stress 만성 동통에 관한 많은 석학들의 연구가 이루어져 왔다. 만성적인 구강안면 통증의 조절에 있어 단지 치료실내의 치료만으로는 반응하지 않는 치과적 원인만이 아닌 중요한 다른 원인이 존재하는 다양한 임상 증례를 접함에 있어 만성 통증을 유발 할수 있는 수면 장애에 대해 조사하여 구강안면 통증 환자의 진단과 치료에 있어 유용한 정보를 제공하고자 한다. 수면은 극히 개인적인 과정이며 같은 수면 양태를 지닌 사람은 거의 없다. 이렇듯 다양한 수면 양태를 Pittsburgh sleep quality index를 이용하여 주관적 수면의 질, 수면 잠복기, 수면 장애, 수면 효율, 수면제의 사용, 주간 기능 장애를 분석하여 총 수면 지수와 구강 안면 통증 양상을 연구하였다. 101명에게 PSQI 설문지와 behavioral pain scale을 이용하여 수면력과 두경부 통증과 통증의 확산, 두통과 수면, 통증에 대처 하는 방법등에 대해 조사하였다. 이에 본 연구에서 양호한 수면자와 불량 수면자에 따른 수면의 질 통증 환자군, 두통이 있는 군, 전신적 약 복용, 습관적으로 카페인 함유 음료를 마시는 대상에게서 수면 장애가 있음을 보여 주었고 불량 수면자에게서 두경부 통증과 통증의 확산, 운동시 통증 등의 빈도가 더 높았음을 보여주었다. 통증을 해소 하는 방법은 그냥 쉰다 등 으로 적극 적이지 못했으며, 추후 수면이 통증 해소에 도움이 될수 있는 지에 따른 연구가 필요하리라 생각된다.

노인의 휴식.활동 일주기 생체리듬과 수면양상 (Rest-Activity Rhythm and Sleep pattern in the Elderly)

  • Purpose: The study compared the rest-activity rhythm and sleep pattern of elderly with young group. Methods: The subjects were 22 over than 65 years old and 23 under 65 years old. An actigraph, sleep diary, Pittsburgh Sleep Quality Index and Insomnia Severity Index scale were used as measurement tools for this study. The data were analyzed with $x^2$, Lamda test, t-test and correlation with SPSS 15.0 program. Results: The elderly had lower curve than the young group in rest-activity rhythm on each time zone. In particular, the elderly group had lower rest-activity rhythm curve of 8, 9, 14, 18, 19, 20, 21, 22 and 23 time zone than those of young group. Sleep pattern had statistical difference in the total sleep time, PSQI and insomnia. Total sleep time of elderly had lower score and PSQI and Insomnia had higher score than young group. Age had correlation with rest-activity rhythm, sleep efficiency, PSQI and insomnia. Conclusion: Rest-activity rhythm of the elderly showed an increase in activity in the early morning because of earlier get up than the young group and an decrease in activity in the afternoon because of taking a nap at this time. Elderly sleep was that total sleep time increased but sleep efficiency decreased and insomnia intensified. This sleep pattern was related to age and rest-activity rhythm.

The Effect of Cognitive Rehabilitation Program Combined with Physical Exercise on Cognitive Function, Depression, and Sleep in Chronic Stroke Patients

  • Objective: This study was conducted to verify the effectiveness of a cognitive rehabilitation program consisting of physical exercises and mental activities for patients with chronic stroke with mild cognitive impairment (MCI). We aimed to investigate how this cognitive rehabilitation program affects patients' cognitive ability, depression, and sleep quality. Design: One group pretest-posttest design Methods: The study was conductedon 12 patients who participated in thecognitive rehabilitation complex exercise program for 16 weeks. The Korean version of the Montreal Cognitive Assessment (MoCA-K), Hamilton Depression Rating Scale (K-HDRS), and Pittsburgh Sleep Quality Index (PSQI) were used to evaluate the measured variables before and after study participation. The cognitive rehabilitation complex exercise program included 30 minutes of cognitive exercise and 30 minutes of Brill Exercise. The Wilcoxon signed-rank test was used to compare the variables before and after program participation. Cronbach's ɑ was used to assess the reliability of the test variables. Results: The post-program assessment showed a statistically significant increase in the MoCA-K score, which measures cognitive function (Z=-2.628, P=0.009). For depression ratings, there was a statistically significant decrease in the K-HDRS score (Z=-2.041, P=0.041). For sleep quality, although there was a numerical increase in the PSQI score, the difference was not statistically significant (Z=-0.702, P=0.483). The reliability test confirmed that all the individual test variables exhibited high reliability (cognitive function, 0.859; depression, 0.872; sleep, 0.822). Conclusions: We found that cognitive rehabilitation program used in this study had a positive effect on the cognitive function and depression in patients with chronic stroke with MCI.

일개 시 노인대학 노인들의 지각된 건강상태, 수면, 우울이 삶의 질에 미치는 영향 (Effect of Perceived Health Status, Sleep and Depression on Quality of Life of Elderly School Participants)

  • Purpose: This study was to examine the effect of perceived health status, sleep and depression on quality of life of older people in the elderly school. Methods: A cross-sectional descriptive study was conducted with a survey of 155 older persons in person 2 elderly school at J City. The data were analyzed with t-test, ANOVA, Pearson's correlation coefficients, and multiple regression using SPSS WIN 18.0 program. Results: The mean score of the quality of life was 8.10 (SD=8.79). Quality of life was significantly different by educational levels (F=4.711, p=.001). The result of the regression indicated that perceived health status, sleep, and depression explained 18.5% of variance in quality of life. Conclusion: Perceived health status, sleep and depression were associated with quality of life in the elderly. It is necessary to develop nursing strategies for the improvement of quality of life, considering health status, sleep, and depression in the elderly.

류마티스 관절염 환자의 수면 및 관련 요인에 관한 연구 (A Study on Sleep in Rheumatoid Arthritis Patients)

  • This study is to investigate sleep patterns of rheumatoid arthritis patients through a survey research. The subjects for this study were 97 patients registered in Hanyang University Hospital Rheumatoid Arthritis Center, and the period of data collection was from July 15, 1998 to August 30, 1998. The research instruments used in this study were the measures of sleep, pain, and fatigue, and SPSSWIN 8.0 Program was used for data analysis. The research results are as follows ; The patients went to bed between 11 and 12 p.m., but many of them found difficulty in falling asleep within 5 minutes. They woke up quite early at around 4 to 6 o'clock in the morning and remained in bed about 1 hour. Only 39 percent of the subjects reported satisfaction with their sleep. Fifty six percent of the subjects took a nap, generally did in the afternoon and 22.7 percent of them napped for half an hour. They suffered sleep disturbance, but their sleep environments were calm and comfortable, and they turn off the light when they went to sleep. As for the quality of sleep, over 50 percent of the subjects reported not being able to sleep deeply, 30 percent of the subjects woke up frequently during sleep, 60 percent experienced frequent arousal after sleep onset. Over 90 subjects slept for 6 to 8 hours. This shows that even though they had rheumatoid arthritis, the patients remained in bed for a sufficient period of time. They also reported waking up or turning frequently during sleep. The sense of fatigue from sleep disturbance scored a relatively high 35.84 points on average against the possible score of 64 points. Behavior for sleep promotion was very active. Sleep disturbance occurred in proportion to the sense of fatigue and pain, and was negatively correlated with quality of sleep. The pain had positive correlations with the illness duration, sleep disturbance and had a negative correlation with the quality of sleep.

  • 목 적 : 본 연구는 폐쇄성 수면무호흡증후군 환자에서 수면무호흡지수, 수면관련 척도, 기분관련 척도, 그리고 수면중 신체활동도 간의 연관성을 보고자 하였다. 방 법 : 폐쇄성 수면무호흡증후군을 보이는 122명(평균연령 $43{\pm}11$세)의 남자환자를 선정하였다. 폐쇄성 수면무호흡증후군 진단은 수면무호흡지수>5인 경우로 하였고, 수면무호흡지수의 평균은 $39.6{\pm}26.0$이였다. 수면관련척도로 Stan-ford Sleepiness Scale(SSS), Epworth Sleepiness Scale(E-SS), Pittsburg Sleep Quality Index(PSQI), Morningness-Ev-eningness Scale(MES)이 사용되었고, 기분관련척도로 Beck Depression Inventory(BDI), Beck Anxiety Inventory(BAI), State-Trait Anxiety Inventory(STAI) I, II, Profile of Mood States(POMS)가 사용되었다. 수면중 활동도를 측정하기 위해 양측 손목에 손목 액티그라프(wrist actigraphy, WATG)를 착용한 채 1일밤 야간수면다원검사(nocturnal polysom-nography, NPSG)를 시행하였다. WATG에서 산출된 지수는 총활동도(total activity score), 평균활동도(mean activity score) 및 분절지수(fragmentation index)였다. 수면무호흡지수, 수면 및 기분관련척도, 수면중 활동도 간의 상관관계를 분석하였다. 결 과 : ESS는 PSQI, BDI, BAI, STAI I, II와 각각 유의한 양의 상관관계를 보였다(p<0.01). SSS는 PSQI, BAI와 유의한 양의 상관관계를 보였다(p<0.05, p<0.01). BAI는 총활동도, 평균활동도 및 분절지수(fragmentation index)와 유의한 양의 상관관계를 보였다(p<0.05, p<0.01, p<0.05). 수면중 총활동도는 ESS, BAI와 각각 유의한 양의 상관관계를 보였다(p<0.05). 수면 분절지수는 ESS, PSQI, BAI와 유의한 양의 상관관계를 보였다(p<0.05, p<0.01, p<0.05). 수면무호흡지수는 수면 및 기분관련 척도와 유의한 상관관계를 보이지 않았다. 결 론 : 주간 졸리움을 호소하는 정도가 수면무호흡증의 심한 정도와 연관이 있기 보다는 오히려 야간수면 만족도, 우울 및 불안의 정도, 수면중 활동도와 더 연관성이 있었다.

제조업 근로자의 건강 관련 삶의 질 영향 요인: 울산 지역 자동차 하청업체 제조업 근로자를 중심으로 (The Factors Affecting the Health-related Quality of Life of Manufacturing Industry Workers: Focusing on Sub-contractual Workers of Ulsan's Vehicle Industry)

  • Purpose: This study aimed to investigate variables affecting health-related quality of life of manufacturing industry workers. Methods: We used data from 200 survey questionnaires collected from a vehicle manufacture factory located in Ulsan metropolitan area. The participants were manufacturing workers. The data were analyzed using t-test, one-way analysis of variance, and $Scheff{\acute{e}}^{\prime}s$ test to identify differences among variables. Pearson's correlation coefficient and stepwise multiple regression were used to identify variables that affected health-related quality of life. Results: The findings revealed that occupational stress and quality of sleep were statistically significant variables of the physical component score with an explanatory power of 42.2%. Occupational stress, depression, and religion were statistically significant variables of the mental component score with an explanatory power of 43.3%. Conclusion: Continuous monitoring and appropriate intervention for occupational stress, quality of sleep, and depression are needed to improve manufacturing workers' health-related quality of life.

재가 여성노인의 수면에 영향을 미치는 요인 (Factors Influencing Sleep of Elderly Women)

  • Purpose: The study was done to identify factors influencing the sleep of elderly women. Methods: Data were collected by questionnaires from 203 elderly women in Seoul and Gyeong-gi province. Measures were physical health state, family support, life satisfaction, sleep, depression, and quality of life. Data were analyzed using Pearson correlation coefficients, t-test, ANOVA, and stepwise multiple regression with SPSS/WIN 13.0 version. Results: The mean score for sleep for elderly women was 39.04. Sleep for elderly women according to religion, education level, spouses, and monthly income showed significant differences. Sleep for elderly women showed significantly positive correlations to physical health state, family support, life satisfaction, and quality of life. The significant factors influencing sleep of elderly women were physical health state, family support, depression, and quality of life, which explained about 70.4% of the variance. The strongest factors influencing the sleep was physical health state. Conclusion: The results suggest that in developing nursing interventions and practice for sleep pattern of elderly women spouses and family support should be considered.

대학생의 수면양상과 주간졸음증에 영향을 미치는 요인 (Sleep Patterns and Factors Associated with Excessive Daytime Sleepiness in University Students)

  • Purpose: The purpose of this study was to investigate sleep patterns and predictors of excessive daytime sleepiness (EDS) in university students. Methods: Participants were 120 university students who were attending two universities in S-city and C-city. Data were collected from May 20 to June 15, 2012 using self-report questionnaires which included Johns' Epworth Sleepiness Scale, Yi's Sleep Quality Scale, and Beck Depression Inventory. Data were digitalized and analyzed using frequency, percentages, means and standard deviations, Wilcoxon rank sum test, Chi-square test, Fisher's exact test, and multiple logistic regression with SAS 9.0. Results: Mean total sleep time was 6.6 hours on weekdays, 8.1 hours on weekends. Mean sleep latency was 19.1 minutes and the score for mean sleep quality was 22.6. Prevalence of EDS was 12.5%. Depression was significantly different between EDS and Non-EDS students (t=2.17, p=.030). Multiple logistic regression showed that the only factor associated with EDS was depression (adjusted odds ratio of depression=5.33, 95% Confidence Interval=1.49-19.04). Conclusion: Results of this study indicate that university students experience short sleep time, low sleep quality, and common EDS with depression, suggesting that students with complaints of EDS should be completely assessed for depression as well as sleep problems.
