Purpose: The purpose of this study was to provide basic data for use in development of strategy for healthy aging preparation and successful transfer to old age by identifying factors influencing quality of life of the baby boom generation. Methods: The data were collected from a total of 205 members of the baby boom generation(aged 48 to 56), and analysis was performed using descriptive statistics, t-test, ANOVA, Pearson's correlation coefficient, and stepwise multiple regression using the SPSS(Version19.0) program. Results: Burden for taking care of children, aging anxiety, preparations for old age, and quality of life according to the general characteristics and health-related characteristics commonly showed a significant difference in education level and presence of stress. The greater the more burden for taking care of children and the deeper the aging anxiety, the lower the quality of life, and the better prepared for aging, the higher the quality of life. Factors affecting quality of life in the baby boom generation included preparations for old age, burden for taking care of children, monthly household income, and quality of sleep. Explanatory power was 32%. Conclusion: For successful transition to old age and for improvement of quality of life in the baby boom generation, a comprehensive approach in micro and macro dimensions will be sought.
Objectives: This study was performed to develop an assessment tool for middle aged women's health status based on dietary patterns, which will have practical applications in the working field of health and hygiene, aiming at improving the middle aged women's quality of life through their health improvement. Methods: As a first step, a literature review was conducted and the original data of '2008~2009 Korea Health and Nutrition Examination Survey' were reanalyzed. This analysis identified 65 preliminary questions that may be relevant to the study. After verifying the content validity by experts, the 65 questions were reduced into 51 questions. In order to secure higher validity of the candidate items, verification of their clinical validity was conducted among women aged between 45 and 60 years. Finally, an assessment tool was developed by applying weight and scoring. Results: Selected 51 questions were used to verify clinical validity and the results showed that 20 questions were relevant, nine questions ('regular meal time', 'regular amount of meal', 'intake frequency of dairy products', 'intake frequency of fruits', 'intake frequency of meat products', 'intake frequency of high cholesterol foods', 'intake frequency of salty foods', 'appetite', 'eat breakfast everyday') were related to dietary life. Eleven other questions ('self-rated health status', 'deep sleep', 'smoking', 'frequency of drinking', 'stress levels', 'health-related fitness levels', 'pounding of the heart', 'strange feelings on the skin', 'interfere with daily life', 'menopause will bring you a chance to see the life in a different perspective', and 'body mass index') were selected as valid questions. For the response scale for each question, 5 point Likert scale was used to make total 100 point score. Conclusions: This study is the first attempt to develop a health status assessment tool for middle aged women based on their dietary patterns. We conclude that this tool is expected to be a useful and practical tool in the field.
목적 : 본 연구는 요양원에서 휠체어를 타고 생활하고 있는 노인에게 ROM dance가 인지 기능과 신경정신 행동에 어떠한 영향을 미치는지 알아보는 것이다. 연구방법 : 본 연구는 대전 ${\bigcirc}{\bigcirc}$요양원에 거주하고 휠체어를 사용하는 15명을 대상으로 하였으며, 2010년 3월부터 2010년 8월까지 주 1회 총 18회 실시하였다. 평가도구는 인지기능을 알아보기 위해서 Mini-Mental State Examination-Korean(MMESE-K)를 사용하였고, 신경정신행동을 알아보기 위해서 Neuropsychiatric Inventory-Questionnaire(NPI-Q)를 사용하였다. 결과 : MMSE-K 검사 결과 초기평가에서는 14.6점에서 ROM dance를 한 후에 16.3점으로 향상되었으며, 특히 MMSE-K의 항목 중 시간에 대한 지남력에서 유의미한 차이가 있었다(p<.05) NPI-Q검사에서는 초기평가에서 88점에서 64점으로 감소되었음을 볼 수 있었으며, 수면/야간행동, 무감동/무관심 영역에서 유의미하게 차이가 있었다(p<.05). 결론 : 본 연구의 결과 휠체어를 이용하는 노인에게 ROM dance는 인지기능 향상과 신경 정신 행동의 감소에 긍정적인 영향을 미치는 것을 볼 수 있었다. 향후 휠체어를 사용하는 노인 뿐만 아니라 많은 노인에게 적용하여 그 효과가 검증된다면 임상에서 유용하게 활용 될 수 있을 것이다.
Objectives: The purpose of this study was to evaluate the effects and develop the contents of a children's care program with traditional Korean medicine in a public health center. Methods: Data were collected from 28 parents who participated in a children care program with traditional Korean medicine. The program was started in June of 2011 in Gangjin-Gun public health center and finished in September of the same year. The questionnaires to measure effect and satisfaction with the program were composed of demographic items, family information, Psychological Well-Being Scale (PWBS), Parenting Stress Index (PSI), children health improvement checklist, etc. The questionnaires were executed before and after the program. Results: The PWBS scores increased (4.00, 3.66) and PSI scores decreased (-7.30. -5.95) significantly after the program. The satisfaction with the program was 100%, and 94% in the two groups, and treatment and lecture were mentioned as the best aspects of the program. Eating habits, sleep conditions, and activity also improved after the program. Conclusions: From these results, the children care program with traditional Korean medicine is worthy for improving children's health condition and reducing their parents' stress.
본 연구는 2015년 청소년 건강행태온라인 조사의 원시자료를 이용한 2차 분석연구로, 청소년의 건강행태 및 정신건강 특성과 흡연과의 관련성을 파악하여, 효과적인 흡연예방 교육 및 프로그램 개발을 위한 기초자료를 제공하기 위해 시행되었다. 연구결과, 일반적 특성에 따른 흡연은 성별, 학년, 학교유형, 학업성적, 평균용돈, 경제적 상태, 아버지 학력, 어머니 학력에서 유의한 차이가 있었고(p<0.001), 건강행태 특성에 따른 흡연은 평생 음주 경험, 가족흡연 여부, 친구흡연 여부, 신체활동 여부, 주관적 건강상태, BMI에서 유의한 차이가 있었다(p<0.001). 정신건강 특성에 따른 흡연은 스트레스 인지 여부, 주관적 수면 충족, 우울감 경험, 자살생각, 주관적 행복에서 유의한 차이가 있었다(p<0.001). 따라서 향후 흡연 관련 요인을 반영한 청소년의 효과적인 흡연 예방교육과 금연지도 프로그램의 개발 및 올바른 건강행태와 정신건강 증진을 흡연예방이나 금연교육과 통합하여 제공하는 방안이 필요하다.
뉴로피드백(neurofeedback)은 EEG biofeedback, neurotherapy 등의 이름으로 불리기도 하는 새로운 치료법으로, 뇌파를 이용하고 뇌(brain) 자체와 뇌 기능(brain function)을 대상으로 한다는 점에서는 일반적인 바이오피드백(biofeedback)과는 차이가 있다. 따라서 바이오피드백의 한 형태로 출발을 하였지만, 현재는 독자적이고 전문화된 방식의 새로운 치료법으로 자리 메김을 하고 있다. 뉴로피드백에 대한 이해를 도우기 위해 뉴로피드백의 역사와 이론적 배경을 우선 알아본 후, 실제 임상에서 어떻게 사용되고 있는 지를 알아보고자 한다. 뉴로피드백은 임상분야에서 간질(epilepsy)의 치료에 처음으로 도입된 이후 그 효과가 입증되었으며, 주의력결핍과잉행동장애(attention deficit hyperactivity disorder, ADHD), 불안 및 우울장애, 수면장애, 물질의존 등의 많은 정신과 질환에 효과적으로 사용되고 있다. 정신과 질환이외에도 두통, 만성통증, 만성피로증후군 등 많은 다양한 영역에 걸쳐 적용되고 있다. 최근에는 질환들 뿐 만이 아니라, 스포츠와 예술 활동에서의 수행능력(peak performance)의 향상, 인지기능의 향상 등의 분야로 활용범위가 확대되고 있어 뉴로피드백의 효용성과 그 가능성에 대한 기대가 더욱 높아지고 있다.
Neurocristopathy originates from aberrant development of the neural crest by genetic abnormality. Dysgenetic or neoplastic neurocristopathy mayor may not combine at one or more organs. Congenital central hypoventilation syndrome (Ondine's curse) is characterized by the respiratory depression during sleep, although showing normal ventilation while awake, because the baby does not responde to hypercapnea or hypoxia. One newborn girl, full-term, 3,020 g of birth weight with neurocristopathy is reported. It showed poor respiration at birth, and temporary Improvement with oxygen and respiratory stimulations. Abdomen was distended. Abdominal x-ray revealed small bowel obstruction and calcified opacity at the right lower quadrant. Because transitional zone was noticed at the distal jejunum during laparotomy, jejunostomy was performed. Several times trial of extubation have failed becaused of the repeated apneas. Brain sonography and echocardiogram were normal. The patient died of sepsis at 37 days of age. Para-aortic ganglioneuroblastoma was found at autopsy. In this case, congenital central hypoventilation syndrome, Hirschsprung's disease and congenital ganglioneuroblastoma are combined as a neurocristopathy.

1985년 9월 30일부터 1990년 12월 31일 사이에 서울대병원 소아정신과에 입원하여 행동장애로 진단받은 어린이 및 청소년 총 48명을 대상으로 병록고찰을 통해 간호문제를 분석해보았다. 그 결과 미성숙한 자아발달, 낮은 자아정체감, 역기능적 부모-자녀관계, 가족내 상황적 위기, 정신지체, 간질 등 원인적 요인에 따르는 개인의 비효율적 대처기전, 사회적 상호작용장애, 자기개념장애, 폭행잠재성, 부모역할장애, 성장 ${\cdot}$ 발달장애 같은 문제가 입원초기부터 중기이후까지 지속되었고 자기간호결핍, 수면장애, 영양장애, 불안, 고체온증 등 입원중기 이후에는 해결된 일시적 문제들을 보였다. 따라서 행동장애 입원환아들을 위한 간호접근은 보다 근본적이고 지속적인 문제에 따른 장기적 계획을 통해 환아들의 자아기능 증진, 자아정체감 확립, 대인관계개선을 도와야 하고 환아 뿐 아니라 가족을 위한 지지적, 교육적 접근에 중점을 두어야 함을 보았다.

Anti-N-methyl D-aspartate receptor (anti-NMDAR) encephalitis, recently recognized as a form of paraneoplastic encephalitis, is characterized by a prodromal phase of unspecific illness with fever that resembles a viral disease. The prodromal phase is followed by seizures, disturbed consciousness, psychiatric features, prominent abnormal movements, and autonomic imbalance. Here, we report a case of anti-NMDAR encephalitis with initial symptoms of epilepsia partialis continua in the absence of tumor. Briefly, a 3-year-old girl was admitted to the hospital due to right-sided, complex partial seizures without preceding febrile illness. The seizures evolved into epilepsia partialis continua and were accompanied by epileptiform discharges from the left frontal area. Three weeks after admission, the patient's seizures were reduced with antiepileptic drugs; however, she developed sleep disturbances, cognitive decline, noticeable oro-lingual-facial dyskinesia, and choreoathetoid movements. Anti-NMDAR encephalitis was confirmed by positive detection of NMDAR antibodies in the patient's serum and cerebrospinal fluid, and her condition slowly improved with immunoglobulin, methylprednisolone, and rituximab. At present, the patient is no longer taking multiple antiepileptic or antihypertensive drugs. Moreover, the patient showed gradual improvement of motor and cognitive function. This case serves as an example that a diagnosis of anti-NMDAR encephalitis should be considered when children with uncontrolled seizures develop dyskinesias without evidence of malignant tumor. In these cases, aggressive immunotherapies are needed to improve the outcome of anti-NMDAR encephalitis.
Objectives: The present study was intended to assess the mental health of nurses working for university hospitals and to establish which factors determine their mental health. Methods: Self-administered questionnaires were given to 1,486 nurses employed in six participating hospitals located in Daejeon City and Chungnam Province between July 1 st and August 31st, 2006. The questionnaire items included sociodemographic, job-related, and psychosocial factors, with job stress factors (JCQ) as independent variables and indices of mental health status (PWI, SDS and MFS) as dependent variables. For statistical analysis, the Chi-square test was used for categorical variables, with hierarchical multiple regression used for determining the factors effecting mental health. The influence of psychosocial and job-related factors on mental health status was assessed by covariance structure analysis. The statistical significance was set at p<0.05. Results: The factors influencing mental health status among subject nurses included sociodemographic characteristics such as age, number of hours of sleep, number of hours of leisure, and subjective health status; job-related characteristics such as status, job satisfaction, job suitability, stresses such as demands of the job, autonomy, and coworker support; and psychosocial factors such as self-esteem, locus of control and type A behavior patterns. Psychosocial factors had the greatest impact on mental health. Covariance structure analysis determined that psychosocial factors affected job stress levels and mental health status, and that the lower job stress levels were associated with better mental health. Conclusions: Based on the study results, improvement of mental health status among nurses requires the development and application of programs to manage job stress factors and/or psychosocial factors as well as sociodemographic and job-related characteristics.
