A Clinical Case Report of Soyangin Patient with Dyshidrosis (한포진으로 진단된 소양인 환자 치험 1례)

  • Introduction The aim of this study was to report significant improvement of Dyshidrosis(Pompholyx) patient who was diagnosed with 'Soyangin chest-bind symptomatic pattern' based on Sasang Constitutonal medicine. Methods The patient was treated with herbal medicine, acupuncture, as well as western medicine. To evaluate the results of this treatment, the patient assessed discomfort of dyshidrosis by using Numeral Rating Scale(NRS), percentage(%) and we assessed severity of dyshidrosis by using Dyshidrotic Eczema Area and Severity Index(DASI). Results At the time of admission, the degree of discomfort in the dyshidrosis symptoms was NRS 8 and the severity of dyshidrosis was DASI 16(moderate). After the treatment, her symptoms was improved(NRS 0, DASI 0). Sleep, digestion and defecation condition were also improved. Discussion The patient who had frequent recurrence showed a significant improvement of dyshidrosis and accessory symptoms without any side effects, who was treated with Hyeongbangdojeok-san, acupuncture and western medicine.

Modification and Test of Self-help Program for Patients having Fibromyalgia-Pilot Study- (섬유조직염환자용 자조관리과정의 수정 및 검증 -예비조사-)

  • The purposes of this study are 1) to modify a self-help program for patients having fibromyalgia, 2) to revise instruments to measure the self-efficacy and impact of fibromyalgia; and 3) to test the effects of the program in terms of self-efficacy, physical activities, and clinical symptoms. The subjects of this study were persons diagnosed as fibromyalgia at a university hospital. For the first two purposes, six patients were selected. Twenty three subjects for the third purpose were selected and assigned to experimental and control group on the base of their residence. Thirteen were assigned to the experimental group, while 10 to the control group. Instruments selected for revision were self-efficacy scale and fibromyalgia impact profile. On the basis of the text developed by Arthritis Foundation, a program for patients with fibromyalgia was tentatively developed. Instruments for measurement of self-efficacy and impact of fibromyalgia were revised to show high reliabilities. Results obtained by utilization of the program were as follows : 1) There were no increase of self-efficacy and physical activities. 2) There were no improvement of the number of tender points and pain. 3) There were improvement of depression, fatigue and sleep impairment. These findings indicate the needs for modification of the program with emphasis of exercise for relaxation of tender points, increases of enactive action and vicarious learning.

A Case Study of a Taeeumin Patient with Parkinson's Disease Diagnosed as Dry-heat Symptomatic Pattern (태음인 조열증으로 진단한 파킨슨병 환자의 치험 1례)

  • Objectives This case study is about a Taeeumin patient with Parkinson's disease identified as Dry-heat (Joyeol) pattern. In this study, we report significant improvement of motor and non-motor symptoms of this patient after Sasang constitutional medicine treatment. Methods The patient was identified as Taeeumin Dry-heat pattern and treated with Cheongsimyeonja-tang and acupuncture. The Unified Parkinson Disease Rating Scale(UPDRS) was used to assess the overall function of the patient. And the Global Assessment Scale (GAS) was used to assess the change of bradykinesia, tremor and dry mouth after the treatment. Results The UPDRS total score decreased from 145 points to 77 points after 5 weeks treatment. And symptoms of bradykinesia, tremor and dry mouth showed significant improvement in GAS after the treatment. Furthermore, constipation, sleep disturbance, dysuresia and anorexia were reported to be improved after treatment. Conclusions This study shows that Sasang constitutional medicine can be effective treatment for motor and non-motor symptoms caused by Parkinson's disease.

A Case of Rapidly Developed Obesity Hypoventilation Syndrome in a Patient with Kyphoscoliosis (척추후측만증 환자에서 급속히 진행된 비만성 저환기 증후군 1례)

  • Obesity hypoventilation syndrome (OHS) is characterized by severe obesity, excessive daytime sleepiness, hypoxemia and hypercapnea. Because OHS mimics pulmonary hypertension or cor pulmonale, clinicians should recognize and treat this syndrome appropriately. A 58-year-old female visited the emergency room because of dyspnea. She was obese and had kyphoscoliosis. The patient also experienced snoring, recurrent choking during sleep and daytime hypersomnolence which worsened after gaining weight in the recent year. The arterial blood gas analysis showed she experienced hypoxemia and hypercapnea not only during nighttime but also daytime. We suspected OHS and the patient underwent polysomnography to confirm whether obstructive sleep apnea was present. During the polysomnography test, sleep obstructive apnea was observed and apnea-hypopnea index was 9.2/hr. The patient was treated with bilevel positive airway pressure therapy (BiPAP). After BiPAP for 4 days, hypoxemia and hypercapnia were resolved and she is currently well without BiPAP. We report a case successfully treated with clinical improvement by presuming OHS early in a patient who had typical OHS symptoms, even while having other conditions which could cause hypoventilation.

Clinical Study on Effect of Auricular acupuncture theraphy and Auricular acupuncture theratphy with common acupuncture theraphy(Shinmum HT7) on Insomnia in stroke patients (뇌혈관장애환자의 불면증에 대한 이침요법과 신문혈자침 병행시술의 치료효과에 대한 비교연구)

  • Objective: This study is aimed to compare the change of clinical comfort and improvement after auricular acupuncture theraphy and auricular acupuncture theraphy with common acupuncture theraphy(Shinmun HT7) on insomnia in stroke patients. Methods : Clinical study was carried out on 40 stroke patients who was traeted insomnia by auricular acupuncture theraphy and auricular acupuncture theraphy with common acupuncture theraphy(Shinmun HT7) at Kwang-Dong Oriental Medical Hospital. Results: 1. Total sleeping time was increased in auricular acupuncture theraphy and auricular acupuncture theraphy with common acupuncture theraphy(Shinmun HT7). 2. Average delaying time before sleep and Average Awake Times was decreased in auricular acupuncture theraphy and auricular acupuncture theraphy with common acupuncture theraphy(Shinmun HT7). 3. Quality of sleep was improved in auricular acupuncture theraphy and auricular acupuncture theraphy with common acupuncture theraphy(Shinmun HT7). 4. Insomnia Score was increased in auricular acupuncture theraphy and auricular acupuncture theraphy with common acupuncture theraphy(Shinmun HT7). Conclusions : According to the results, both auricular acupuncture theraphy and auricular acupuncture theraphy with common acupuncture theraphy(Shinmun HT7) were effective on insomnia in stroke patient. but, auricular acupuncture theraphy was more effective than auricular acupuncture theraphy with common acupuncture theraphy(Shinmun HT7) in total sleeping time and insomnia score.

A study of the health related habits of dental technicians (치과기공사의 건강관련 생활습관에 관한 연구)

  • Purpose: This study aims to present practical data available for the effective health management and improvement for the dental technicians by exploring the interrelationships between general characteristics of dental technicians and seven health related habits like drinking, smoking, breakfast, and snacks among the others(alameda 7). Methods: Out of the random selection of the registered dental offices of Dental Technicians Association of Korea, 2,000 working dental technicians were asked to fill out the questionnaire from January 19 to April 30, 2009. We collected the 1,502 sheets of the questionnaire and took frequency analysis, crosstabulation, t-test, and analysis of variance using SPSS Win 12.0 program. Results: Among the seven health related habits, 54.9% of the subject answered 'never do exercise' in the question of exercise habits, which is the most problematic of all. Hours of sleep and BMI were found to be quite low. Of the general characteristics, sex proved statistically significant to all the seven habits(p<.001). Marital status, job title, and health check-up showed statistical significance in six items among the seven habits. Age, monthly income, job position showed statistical meanings in five items among the seven habits. The frequency of break showed statistical significance in four items among the seven habits. Working environment showed statistical significance only in the item of hours of sleep among the seven habits(p<.05). Conclusion: This study proved that it is vital to educate the dental technicians to learn the right habits for enhancing their health and quality of life and motivate them to practice those habits.

Presentation on Health Impact Assessment of Transportation Noise (교통소음 건강영향평가 소개)

  • Because many people suffer from physical and mental damage caused by the noise created by transportation infrastructure, including road traffic, rail, and aircraft, developed countries have conducted research on predicting and solving the impact to human health from being exposed to transportation noise. Therefore, this study suggests a fundamental plan to assess the health impact of transportation noise on the basis of domestic and foreign prediction results regarding the health impact of transportation noise. The domestic and foreign exposure-response expressions, including the noise index and the health impact indicator of annoyance and sleep disturbance, are compared, and it is found that domestic individuals show a more sensitive response to transportation noise. Based on domestic and foreign research, and a case study regarding the health impact of transportation noise, a fundamental plan to assess the health impact of transportation noise comprises the preparation of objective assessment standards through the improvement of exposure-response models, and the establishment of reduction measures which can improve the quality of the transportation noise environment.

  • This study is carried out to critically explore the direction of the improvement of the protection clauses of minor artists which is the core of Popular Culture Art Industrial Development Act enforced from July 29, 2014. The analysis shows that the law accepted the social issues such as the prevention of procuring minor prostitution, slave contracts(unfair long-term contracts), and third party's profit-extortion. However, the law missing or not specifically stated about age-based differential enforcement of work hours, dangerous acting scenes shouldn't be forced to minors, right to sleep, health, and study, as well as the penalty regulations in violation of each clause. Consequently, the Act's revision and supplementation is necessary to fully meet above insufficiencies referring to foreign practices.

A Clinical Report of Insomnia due to deficiency of Yang (양허(陽虛) 불면증(不眠症)환자 치험 1례)

  • The Common Symptoms of insomnia are to be hard to sleep, often wake up at night, be easy not to fall asleep again after waking up. In serious conditions, an insomniac can never get to sleep overnight. This is the case that we diagnosed a 49-year-old patient who demonstrates insomnia, palpitation, anxiety, dizziness, distress in the stormach as Deficiency of Yang(陽虛) and treated her. At 1st stage we diagnosed the patient as Insufficiency of both the heart and the spleen(心脾兩虛) and prescribed Gwibi-tang(歸脾湯), and next stage we dispensed bogol-dan(補骨丹) to cure Deficiency of Kidney Yang. As the result of treatment, the patient made a good improvement. We considered that this case report to be a good prcedent of Insomia due to deficiency of Yang.

A Study on the Classification of Nursing Diagnoses by Student Nurses (간호학생이 내린 간호진단 분석에 관한 연구)

  • This research was done to promote improvement of practical application of nursing diagnoses and to improve the quality of nursing. The subjects of this research were 156 second year students of C junior nursing college who were giving adult patient care. The nursing diagnoses of 312 reports were analyzed using NANDA. In these case reports only nursing diagnoses were considered, of which there were a total of 982. In the data analysis the 9H of the nursing students' nursing diagnoses matched with 105 NANDA nursing diagnoses, Of these, the most frequent diagnoses were pain(165, 17.48%), anxiety(101, 10.70%), alteration in nutrition(83, 8.79%) , sleep disturbance (67, 7.10%), in activity intolerance (67, 7.10%), ineffective breathing pattern(51,5.40%). The etiology for the students' nursing diagnoses were compared with NANDA's nursing diagnoses by frequency. The most frequent etiology for the nursing diagnoses of pain was a biological etiology(50, 31%), for anxiety, situation crisis(58, 57.43%), for alteration in nutrition, indigesion(23, 27.71%), for sleep disturbance, external etiology(25, 37.32%), for activity intolerance, immobile position(22, 32.84%), for ineffective breathing pattern, pain(35, 68.63%), and for ,impaired physical mobility, pain(31, 65.96%). The most frequent etiology for constipation was inadquate digestion of water and cellulose (16, 34.78%), for fluid volume felicity, loss of body fluid (21, 52.50%), for impaired skin integrity, external etilogy(16, 43.24%), for impaired physical mobility, pain(22, 62.86%) , for knowledge deficits, cognition disturbance(9, 27.27%), for ineffective air way clearance, secretion obstruction(14, 48.27%) , for impaired gas exchange, loss of transport ability of blood oxygen(9, 37.50%) , and for powerlessness, therapy environment (5, 22.73%). The number of nursing diagnoses by pattern was exchange(16), moving(6), feeling(4), choosing(4), relating(3), communication(1), perceiving(1), knowing(1), valuing(1).
