폐경 후 비만 여성의 체중감량을 위한 가미태음조위탕의 임상적 활용: 후향적 차트 리뷰 (Gamitaeeumjowee-tang for Weight Loss in Post-Menopausal Obese Women: A Retrospective Chart Review)

  • Objectives: The purpose of this study is to evaluate weight change and analyze adverse events in post-menopausal obese women with Gamitaeeumjowee-tang for weight loss. Methods: A retrospective chart review was conducted for medical records of 115 post-menopausal obese women (body mass index, BMI≥25 kg/m2) who were administered with Gamitaeeumjowee-tang for 12 weeks for the purpose of weight loss. Weight, skeletal muscle ratio and BMI changes were compared before and after the program. Adverse events were evaluated by causality, severity and system-organ classes. Results: A total of 115 patients were included in this study. The average total weight loss in post-menopausal women was 5.72±2.04kg(p<0.001) and the average weight loss rate was 8.06±2.70%(p<0.001). After the 12-week program, the body fat rate was significantly decreased(3.76±2.20%)(p<0.001) and the skeletal muscle ratio was significantly increased(2.07±1.24%)(p<0.001). There were no significant differences in weight loss, skeletal muscle ratio change and body fat rate change depending on the number of hospital visits. Insomnia was frequently reported throughout the period, and no serious adverse events were reported. Conclusions: This study showed the potential that weight management treatment with Gamitaeeumjowee-tang could be a good way to lose weight of post-menopausal obese women without serious adverse events. Continuous well designed clinical studies are needed.

운동프로그램이 중년비만여성의 체구성, 체력 및 지질대사에 미치는 영향 (Effects of a Exercise Program on Body Composition, Physical Fitness and Lipid Metabolism for Middle-Aged Obese Women)

  • Purpose: This study was to examine the effects of an exercise program for middle-aged obese women. Method: The exercise program combined folk dance and resistance training. The subjects group consisted of 85 middle-aged obese women between 40 and 60 years of age. Three 8 week sessions consisted of a $55-80\%$ maximum heart rate (MHR) exercise for 60-90 minutes a day and 3 times a week from March to November, 2004. Data was collected through a pre- and post-exercise test before and after each session. Data was collected with Inbody, dynamometer and blood. This data was analyzed by descriptive statistics, and a paired t-test with an SPSS/PC(10.0 version) program. Results: There were significant positive changes in body weight, body fat mass, body mass index, percent body fat, muscle strength, muscle endurance, flexibility, and balance quality, but no significant positive changes in skeletal mass, fat free mass, percent abdominal fat(waist-hip ratio), visceral fat area, agility, total cholesterol, high density lipoprotein cholesterol, or triglycerides. Conclusion: This study showed that an exercise program has partially positive effects for middle-aged obese women. The results of this study show that exercise at community health centers should continue for middle-aged obese women's health.

8주 케톤체 투여가 마우스 지구성 운동수행능력과 골격근의 자가포식에 미치는 영향 (The Effects of 8-week Ketone Body Supplementation on Endurance Exercise Performance and Autophagy in the Skeletal Muscle of Mice)

  • 마우스 모델을 사용하여, 8주 케톤체(베타-하이드록시뷰티레이트, β-HB)가 지구성 운동 수행능력과 골격근의 단백질 합성 및 분해에 미치는 영향을 조사하였다. 48마리의 수컷 ICR 마우스(8주령)를 무작위로 4개 그룹으로 나누었다: 비운동 통제군(Sed+Con), 비운동+베타-하이드록시뷰티레이트(Sed+β-HB), 운동 통제군(Exe+Con), 운동+베타-하이드록시뷰티레이트(Exe+β-.HB). β-HB 투여를 위해 β-HB를 PBS (150 mg/mL)에 용해시켜 8주 동안 매일 피하 주사(250 mg/kg)하였다. 운동 훈련을 위해 마우스는 8주 동안 20분 트레드밀 달리기 운동 훈련을 주 5일 수행하였다. 훈련은 10° 경사도에서 10 m/min 속도에서 5 min 동안 실시하고 나서, 매 1 min 마다 1 m/min의 속도를 나머지 15 min 동안 증가시켰다. 8주간의 처치 후, 내장 지방량과 골격근량, 혈액 매개변수, 자가포식 및 단백질 합성 마커가 분석되었다. 데이터는 SPSS 21 프로그램을 사용하여 ANOVA (p<0.05)로 분석되었다. Exe+β-HB 그룹의 혈중 젖산 수치는 모든 3개 그룹(Sed+Con, Sed+β-HB 및 Exe+β-HB)보다 유의하게 낮았다(p<0.05). Sed+Con 및 Exe+Con 그룹에 비해, 8주 Exe+β-HB 처치는 최대 달리기 수행 시간(탈진 시간)을 유의하게 증가시켰다(p<0.05). 8주 β-HB 투여는 마우스의 골격근에서 자가포식 유동과 자가포식 관련 단백질을 유의하게 감소시켰다(p<0.05). 반대로, β-HB와 지구력 운동 훈련의 조합된 처치는 단백질 합성(mTOR 신호 및 번역 속도)을 증가시켰다(p<0.05). 8주간의 β-HB 처치와 지구력 운동 훈련은 마우스 골격근에서 지구력 수행능력 증가, 단백질 합성 증가, 단백질 분해 감소 효과를 보여주었다.

  • 도침치료와 냉각치료, 한약치료를 병행한 복합한방치료를 24명을 대상으로 4주간 시행한 결과, 허벅지둘레, 체중, BMI, 체지방률 등 비만지표가 모두 통계적으로 유효하게 감소하였다. 또한 중등도의 부작용 발생률은 1.04%로 나타났다.

The Association Between the Number of Natural Remaining Teeth and Appendicular Skeletal Muscle Mass in Korean Older Adults

  • Background: The aim of this study was to examine the correlation between the number of remaining natural teeth (NRT) and the appendicular skeletal muscle mass index (SMI) in older adults. Methods: This study was based on data from the 2008-2010 Korea National Health and Nutritional Examination Surveys. The participants were 2,378 older participants (984 men and 1,394 women) aged over 65 years. Survey dentists conducted oral health examinations, and ASM was measured using dual-energy X-ray absorptiometry. Results: The participants with $NRT{\geq}20$ had more ASM and SMI than those with NRT<20 in both sexes. SMI was correlated with NRT in men (r=0.018, p<0.001) and in women (r=-0.007, p<0.001). The positive correlation between the NRT and SMI remained significant in men even after adjusting for age, marital status, income, smoking, drinking, physical activity, protein intake, energy intake, calcium intake, body mass index, fasting blood glucose level, medications, and prostheses (${\beta}=0.011$, p=0.001). In women, the correlation disappeared after adjustment for smoking, alcohol, physical activity, protein intake, energy intake, calcium intake, marital status, income, fasting basal glucose, medication administration, and prostheses. Conclusion: This study showed a correlation between NRT and SMI in those ${\geq}65$ years of age in Korea. The relationship persisted in men, but not in women, even after adjusting for confounders.

산삼복합약침과 한약을 병행한 식이요법이 체성분 변화에 미치는 영향: 후향적 연구 (Effects of Dietary Therapy with Korean Herbal Medicine and Cultivated Wild Ginseng Pharmacopuncture on Change of Body Composition: A Retrospective Study)

  • 26명의 환자에게 8~12주동안 산삼복합약침과 한약을 병행한 식이요법을 시행하여 체성분 변화에 미치는 영향을 알아보았다. 체성분 검사 결과 체중, BMI, 골격근량, 체지방량, 복부지방률, 내장지방단면적, 체지방률이 유의하게 감소하였고, 체중이 감소하면서 골격근량이 감소하긴 하였으나 체지방량 위주의 감소로 골격근의 비율은 오히려 유의하게 증가하였다. 태음조위탕을 증례에 맞게 가감하고, 하루 한 시간 혹은 만 보 걷기의 운동지도를 통해 하루 400~800 kal의 초저열량 식이로 인해 야기될 수 있는 부작용 중 일시적인 현기증과 무기력감 외에 탈모, 부종, 설사, 월경 이상 등은 관찰되지 않았다. 향후 체성분 변화뿐만 아니라 생화학적 검사나 골밀도 검사를 통한 장기적인 탐색이 필요할 것으로 생각한다.

Effects of marine oligomeric polyphenols on body composition and physical ability of elderly individuals with sarcopenia: a pilot study

  • [Purpose] We aimed to identify the effects of marine oligomeric polyphenol (MOP) intake in elderly individuals with sarcopenia. [Methods] Older adults (aged 65 years or older) were recruited based on the diagnostic criterion for sarcopenia and were randomly assigned to the MOP intake group (n=10) or the placebo (PBO) intake group (n=10). To determine the effect of MOP intake received for four weeks, the pre- and post-intake body composition (weight, skeletal muscle mass, and bone density) and senior fitness tests were assessed. [Results] Our results showed there were significant differences in the skeletal muscle mass (p=0.039), bone density (p=0.020), fat-free mass index (p=0.026), and 2.4 m up and go test (p=0.001) between pretest and post-test. There was a significant difference between the pre-test and post-test and an interaction effect for the one-leg stand test (p=0.010 and p=0.049, respectively). However, there were no significant differences in body fat percentage, calf circumference, grip strength, or the chair rise test. [Conclusion] Some variables exhibited significant differences in the pre- and post-assessments, and there was an interaction effect for the one-leg stand. However, this was insufficient to prove the effectiveness of MOP intake in improving sarcopenia. Therefore, additional studies are essential to examine the effects of MOP intake and exercise intervention on the body composition and fitness of patients over a longer period.

American Paint Horse에서의 다당류저장성근질환 (Equine Polysaccharide Storage Myopathy in an American Paint Horse)

  • 노책성 횡문근융해증의 재발을 보이는 거세된 4세령 Paint horse가 다당류저장성근질환으로 진단되었다. 말이 폐사하기전 보인 임상증상으로는 근약화, 근손실, 운동불내성, 장제시 사지를 들어올리는데 어려움이 있었고, grooming 시 민감한 반응, 이동 혹은 기립시 장애였다. 골격근 샘플의 조직검사를 통해서 말의 다당류저장성근질환으로 확진되었다.

Effect of exercise interventions on sarcopenic obesity in middle-aged and older adults: a comprehensive review

  • Purpose: This study examined the definitions, diagnostic criteria, and measurements of sarcopenic obesity and identified effective exercise interventions that improve cardiometabolic outcomes in middle-aged and older adults, in whom the prevalence of sarcopenic obesity is increasing. Methods: This comprehensive review followed the principles of literature search, data extraction, and review, as described in the PRISMA 2009 guidelines. Results: The 11 articles included in this study presented different concepts of sarcopenic obesity. Exercise interventions for sarcopenic obesity varied in their effects. Resistance exercise improved muscle mass and physical function, while aerobic exercise primarily impacted obesity and cardiometabolic indicators. Combined exercise had mixed results across indicators. Conclusion: This study addressed a pressing public health concern in the context of an aging population, acknowledged the unique challenges of sarcopenic obesity, and attempted to clarify definitions and assessment methods, while identifying effective exercise interventions to reduce cardiometabolic risk. Sarcopenic obesity is a multifaceted condition with varying definitions and diagnostic criteria. Its association with cardiometabolic risk underscores the need for comprehensive assessments considering both muscle and obesity indicators. While exercise interventions hold promise for managing sarcopenic obesity, further research is required to establish effective strategies.

Ankle Sprain Affects Lower Leg Muscle Activation on Vertical Landing, Half Point, and Gait in Female Ballet Students

  • Purpose: This study aimed to investigate effect of ankle instability on peripheral muscle activation among female ballet dancers to provide information on the development of prevention programs for ankle injury rehabilitation. Methods: 32 female ballet dancers were randomly divided into two groups: experience ankle sprain group (n=16, age, $20.7{\pm}0.8years$, BMI $18.6{\pm}1.2kg/m2$) and non-experience ankle sprain group (n=16, $age=21.0{\pm}0.8years$, BMI $19.6{\pm}2.0kg/m2$). Activation of the peroneus longus, tibialis anterior muscle, and gastrocnemius during vertical landing, half pointe, and gait between the two groups were measured. Body composition analyzer was used to examine skeletal muscle mass and body fat mass. Results: A total of 32 patients were included. In the experience ankle sprain group (n=16: left sprain 14, right sprain 2), average ankle sprain injury occurred 7.5 months before the study. The average age of the dancers in the experience ankle sprain group and non-experience ankle sprain group was $20.7{\pm}0.8$ and $21.0{\pm}0.8years$, major period was $64.5{\pm}23.8$ and $71.6{\pm}25.8months$, BMI was $18.6{\pm}1.2$ and $19.5{\pm}2.0kg/m2$, respectively. No significant differences were found on body composition between the two groups (p>0.05). The experience ankle sprain group showed significantly lower tibialis anterior and peroneus longus muscle activation (p<0.5), while gastrocnemius muscle activation appeared to be significantly higher (p<0.05) during landing, half pointe, and normal gait. Conclusion: Ankle sprain can cause a decline in peripheral muscle activation and coordination, which increased the risk for repetitive ankle sprain in the future. Moreover, ankle peripheral muscle selective strength training, coordination program development, and application need to be considered to prevent ankle sprain.
