상대정맥 전장을 포함한 종격동 해면상 혈관종 - 1 례 보고 - (Mediastinal Cavernous Hemangioma Involving Whole SVC -A case report-)

  • 해면상 혈관종은 종격동에 발생한 혈관종중 가장 많은 형태로 주로 전종격동에 발생하고 그 빈도가 매우 드물다. 환자는 13세된 여아로 우연히 발견된 종격동 종괴로 수술적 절제를 하여 해면상 혈관종으로 판명되었다. 술후 추적한 x-ray 검사상 재발의 소견이 없으며 수술적치료시 너무 광범위한 무리한 박리와 절제는 필요하지 않을 것으로 보인다.

OK-432 경화요법으로 치유된 Plunging Ranula (Intralesional OK-432 Sclerotherapy for Plunging Ranula)

  • The incidence of plunging ranula has been reported as about 10% of all ranula cases. Treatment consists of wide surgical excision or marsupialization. However, it seldom provides a satisfactory result and various modalities of treatment have been proposed in addition to surgical extirpation. We reported our experience using intralesional OK-432 sclerotherapy for a case of plunging ranula in 20 year-old male patient. The OK-432 solution was prepared by dissolving 0.1 KE in 2ml of physiologic saline and was instilled in the same amount of the aspirated cystic fluid. Following two instillations, the lesion completely disappeared. The side effects were mild fever for 3 days and a mild local induration for several days. At present, 9 months after instillation, no recurrence was observed. OK-432 sclerotherapy appears to be simple, safe and effective method for the treatment of plunging ranula.

폐에 발생한 양성 투명세포종 -치험 1례 보고- (Benign Clear Cell Tumor of the Lung -Report of a case-)

  • We experienced a case of benign clear cell tumor, a very rare form of lung tumor. A forty-year-old male complained of non-specific right flank pain for 1 month prior to admission. Under the suspicion of a benign lesion, an exploratory thoracotomy was done. Since the first case was reported in 1963 by Leibow and Castleman, about 40 cases have been reported worldwide by 1993. Basically it was believed to be a benign lesion but in certain cases, it showed malignant behavior. Simple surgical excision is satisfactory to this benign lesion. But close long-term follow up is mandatory. We report this first case in Korea with the review of literature.

Minimally Invasive Excision of Epidermal Cysts through a Small Hole Made by a $CO_2$ Laser

  • To improve the cosmetic results of removing epidermal cysts, minimally invasive methods have been proposed. We proposed a new minimally invasive method that completely removes a cyst through a small hole made by a $CO_2$ laser. Twenty-five patients with epidermal cysts, which were 0.5 to 1.5 cm in diameter, non-inflamed, and freely movable, were treated. All of the patients were satisfied with the cosmetic results. This method is simple and results in minimal scarring and low recurrence rates without complications.

  • Cassia fistula commonly known as Sundali was selected to evaluate its wound healing potentials based on traditional use and literature references. Methanol extract of C. fistula leaves were examined for its wound healing property in the form of an ointment in two types of wound models in rats: i) Excision wound model and ii) Incision wound model. The ointment of the leaf extract of two different concentrations (5% and 10% w/w ointment of leaves extract in simple ointment base) responded significantly in both models of wounds tested. The results were also comparable to that of standard drug, nitrofurazone in terms of wound contraction ability, epithelisation period, tensile strength and regeneration of tissue at wound area.

Serially expanded flap use to treat large hairless scalp lesions

  • Hairless scalp areas can occur due to trauma, tumors, or congenital disease. This aesthetically unpleasing condition can lead to psychosocial distress, and thin skin flaps may be prone to scarring. Treating the hairless scalp by simple excision is challenging because of skin tension. Tissue expanders are a good option for hairless scalp resurfacing. However, a single expansion may be inadequate to cover the entire defect. This report describes good results obtained using a serial resurfacing method involving re-expansion of the flap with a tissue expander to treat two patients with large lesions: one due to aplasia cutis congenital and another who underwent dermatofibrosarcoma protuberance resection. The results suggest that scalp resurfacing by serial tissue expansion using a tissue expander can be used for extensive lesions.

Syringomyelia Associated with a Spinal Arachnoid Cyst

  • While syringomyelia is not a rare spinal disorder, syringomyelia associated with a spinal arachnoid cyst is very unusual. Here, we report a 62-year-old man who suffered from gait disturbance and numbness of bilateral lower extremities. Spinal magnetic resonance imaging (MRI) showed the presence of a spinal arachnoid cyst between the 7th cervical and 3rd thoracic vertebral segment and syringomyelia extending between the 6th cervical and 1st thoracic vertebral segment. The cyst had compressed the spinal cord anteriorly. Syringomyelia usually results from lesions that partially obstruct cerebrospinal fluid flow. Therefore, we concluded that the spinal arachnoid cyst was causing the syringomyelia. After simple excision of the arachnoid cyst, the symptoms were relieved. A follow-up MRI demonstrated that the syringomyelia had significantly decreased in size after removal of the arachnoid cyst. This report presents an unusual case of gait disturbance caused by syringomyelia associated with a spinal arachnoid cyst.

  • Pigmented villonodular synovitis (PVNS) is a rare proliferative disease affecting joint synovium, tendon sheaths, bursae. The usual treatment for PVNS is a surgical excision. If destructive joint lesions have occurred, complete resections must be performed followed by arthrodesis or arthroplasty. We report a case of a pigmented villonodular synovitis involving an ankle joint which was treated by total ankle replacement for recurrence after simple synovectomy.

  • Epidermal inclusion cyst is a common mass in life. It is covered with a stratified squamous epithelium, thus, there is a granular cell layer adjacent to the keratin-containing cyst lumen. It can be caused by mechanical force, trauma, or a spontaneous event. It can rupture spontaneously or be ruptured by external mechanical forces. Epidermal inclusion cysts that exhibit inflammation or recur should be removed by simple excision. In this case, the patient showed an epidermal inclusion cyst under an anterolateral thigh free flap, which can cause the palpable mass to go unnoticed. First we thought he had neuroma formation after a surgical procedure on his foot. However it was an epidermal inclusion cyst, which was diagnosed by a special pathologist. It is a curious and rare case.

설에 발생한 신경초종의 1증례 (A Case of the Nevrilemoma of the Tongue)

  • Neurilemoma (Schwannoma) is a solitary encapsulated benigh tumor and apparently derived from Schwann cells of the nerve sheath. This is a case of 17-year-old Korean female with the neurilemoma of the tongue. Chief complaint of the patient was painless palpable mass on left tongue body. Diagnosis was established by histologic examination after simple surgical excision of tumor mass. A brief review of related literatures was made.
