Development of ELISA for cortisol and it's application to clinical use (ELISA를 이용한 cortisol 측정법의 정립 및 임상적 응용)

  • ELISA kit for cortisol was developed and then evaluated. Polyclonal antihydrocortisone-3-(o-carboxymethyl)oxime BSA rabbit serum was used to coat the 96-well microplates. The minimum detection limit of the kit was 250pg of cortisol per milliliter. The within-run variation and the day to day variation of the ELISA system were 2.0 and 5.9 at maximum, respectively. The kit was used to determine whether salivary cortisol concentration could replace blood cortisol concentration in dexamathasone suppression test of dogs. Changes of cortisol concentration were measured in serum or saliva after intravenous administration of 0.01mg of dexamethasone per kilogram of body weight. Blood alone, saliva alone or both were collected at 0, 30, 60, 120, 240, and 360 minutes after injection of dexamethasone. The change in blood cortisol concentration was found to be suitable in dexamathasone suppression test of dogs, but the change in salivary cortisol concentration was not. The kit was also used to determine whether salivary cortisol concentration could be a stress index as well as blood cortisol concentration in dogs. Two types of trial were performed to estimate the stress either by blood or salivary cortisol concentration. The first trial was stress experiment by intravenous injection of 0.2IU of PZI-insulin per kilogram body weight. Either blood alone or saliva alone was collected at 0, 30, 60, and 90 minutes after insulin administration. Both blood and salivary cortisol concentration were found to be suitable index in estimating stress from hypoglycemia by injection of insulin. The second trial was stress experiment by electrical irritation. The dogs were irritated with anti-bark device for 10 seconds. Blood was collected before and at 2 and 5 minutes after electrical irritation. Saliva was collected before and at 3 and 6 minutes after electrical irritation. The blood cortisol concentration, but not the salivary cortisol concentration was found to be suitable index in estimating stress from electrical irritation. Cushing syndrome in a dog was also successfully diagnosed with this kit.

Significance of Corticosteroids and Their Relationship with Other Parameters in Patients with Sepsis (패혈증에서 혈중 스테로이드의 의의 및 기타 혈중 지표와의 연관성)

  • Background: Corticosteroids are known to be significant prognostic parameters in sepsis. Recently, an absolute and relative insufficiency of the corticosteroids system has often been reported to often develop particularly in severe sepsis. Degree of such an adrenal insufficiency not only has prognostic implications but also can be used to guide corticosteroids replacement therapy. The 24-hour urinary cortisol levels as well as serum cortisol concentrations were measured to assess the clinical significance and their relationship with the other parameters of sepsis, and also evaluated the clinical implications of the relative adrenal insufficiency. Methods: 26 consecutive patients with sepsis were enrolled. The basal random serum cortisol, ACTH, ADH, lactate levels and 24-hour urinary free cortisol amount were measured. The rapid ACTH ($250{\mu}g$) stimulation test was also performed. Results: Basal serum cortisol levels were higher in the non-survivors than in the survivors. The 24-hour urinary free cortisol levels were higher in the patients with severe sepsis than in those without. The serum cortisol levels strongly correlated with the serum ADH and lactate levels. The 24-hour urinary free cortisol levels strongly correlated with the serum cortisol and lactate levels. The fractional changes in the cortisol levels after the rapid ACTH stimulation tests correlated with the serum cortisol, ADH, and lactate levels. Conclusion: Both the serum cortisol and 24-hour urinary cortisol were found to be significant prognostic factors in sepsis, and showed a strong correlation with the other parameters. The relative adrenal insufficiency might also be an important clinical parameter.

  • Liquid chromatography/mass spectrometry using isotope dilution method has been established as a primary method for the measurement of cortisol in human serum. Verification of this method was accomplished by the participation in Consultative Committee for Amount of Substance-Metrology in Chemistry (CCQM) pilot study. Two levels of cortisol certified reference materials were prepared and certified by the established method. They were used as sample materials for the proficiency testing. The expanded uncertainty in the measurement of cortisol in human serum was approximately 1.2% at 95% confidence level. The results of the proficiency testing showed a good precision among the participants, but some bias to the certified values. This means that commercial field laboratories should keep traceability chain to SI unit through available reference measurement procedures and/or available reference materials.

  • This study investigated the effects of Oligonol intake on cortisol, interleukin (IL)-$1{\beta}$, and IL-6 concentrations in the serum at rest and after physical exercise loading. Nineteen healthy sedentary male volunteers participated in this study. The physical characteristics of the subjects were: a mean height of $174.2{\pm}2.7$ cm, a mean weight of $74.8{\pm}3.6$ kg and a mean age of $22.8{\pm}1.3$ years. Each subject received 0.5 L water with Oligonol (100 mg/day) (n = 10) or a placebo (n = 9) daily for four weeks. The body composition, the white blood cell (WBC) and differential counts as well as the serum cortisol, IL-$1{\beta}$, and IL-6 concentrations were measured before and after Oligonol intake. The cortisol concentration and serum levels of IL-$1{\beta}$ and IL-6 after Oligonol intake were significantly decreased compared to before treatment (P < 0.01, respectively). In addition, the rate of increase of these factors after exercise was decreased compared to the placebo group. There was no change in the WBC and differential cell counts. These results suggest that oral Oligonol intake for four weeks had a significant effect on inhibition of inflammatory markers in healthy young men.

Serum Levels of IL-$1\beta$, IL-2 and IL-6 in Functional Gastrointestinal Disorders in Relation to Depression and Stressful Life Events (기능성 위장관 장애에서 혈청 IL-$1\beta$, IL-2, IL-6의 변화 : 우울증 및 스트레스와의 상관성)

  • Objectives : Stress induces alteration of gastrointestinal function in which interleukin (IL) and hypothalamo-pituitary-axis are involved. Depression again is associated with functional gastrointestinal disorders (FGID) and interleukins. The author attempted to look into the role of interleukins in the FGID also the association of depression and stress in this context. Methods : Entered were 20 patients with FGID, diagnosed by Rome IT criteria and 20 healthy controls. Depression was measured by Hamilton Rating Scale for Depression (HAM-D), and stress of the last one year by Schedule of Recent Experience (SRE). Serum levels of IL-$1\beta$, IL-2, IL-6 were measured by Enzyme-linked Immunosorbent Assay and serum cortisol by Fluorescence Polarization Immunoassay. The results were as follows : Results: 1) Serum levels of IL-$1\beta$, IL-2 were significantly lower in the patients with FGID than those in the controls, but level of IL-6 did not differ between two groups. 2) The patient group showed significantly higher level of serum cortisol as well as higher degrees of depression and stress. 3) Positive correlation was noted between depression and serum cortisol, and between depression and IL-2. A trend of negative correlation was seen between depression IL-$1\beta$. Positive correlation was noted between SRE and IL-6 and IL-6 and serum cortisol. Conclusions : It was concluded that FGID might be related to depression and stress. Changes of the interleukins might be involved with elevated cortisol level.

The Effect of Imagery on the Stress of Clinical Nurses (심상요법이 간호사의 스트레스에 미치는 효과)

  • All clinical nurses are not only faced with the physical and emotional needs of patients but also exposed to the heavy workload. In clinical settings, the nurses' stress becomes more and more critical. Understanding the stress experienced by nurses is essential for planning programs to enhance patient care and to promote nurses health. Many methods to reduce the stress were practiced for patients. In this study, the investigator examined how the imagery influence the reducing the stress of clinical nurses. For this study, the recorded tape for imagery developed in the Society of Rheumatics was applied. In order to evaluate the effect of imagery on the stress of clinical nurse, stress score, serum cortisol, blood pressure, and pulse were measured on 20 clinical nurses for 20 days. The results were as follows: 1. Stress score decreased significantly in the experimental group. And the rate of decrease in stress score was significantly higher in experimental group than in the control group. 2. The level of serum cortisol decreased significantly after the application of imagery. 3. Systolic blood pressure decreased significantly after the application of imagery. These results indicate that after applying the method of imagery on the nurses, stress score, serum cortisol level, and systolic blood pressure were significantly decreased. Therefore, this study proves that imagery is one of the methods to reduce the stress of clinical nurses.

A Study on the Effect of Acupuncture on Anesthesia and the Mode of Action (The Second Report) (자침(刺鍼)이 마취(痲醉)에 미치는 작용기전(作用機轉) 연구(硏究) (제(第) 2 보(報)))

  • This study was carried to identify whether acupuncture at several meridian points can affect the human anesthesia or not through the analysis of serum intermediates, such as, melatonin, serotonin, TSH, cortisol in human serum. We investigated the effect of accupuncture on the change of serum intermediates which have frequently been made a subject of discussion owing to its hyponic and sedative properties in practice. The result obtained were as follow ; 1. Serum melatonin levels was hardly affected at 40 minutes after general acupuncture on Hab-Gog, Nae-Gwan, and the other meridian points as compared with that($5.64{\pm}1.02pg/ml$) of resting control group. But on day 1 after acupuncture, these tend to increase significantly ($7.95{\pm}2.05pg/ml$ to $8.21{\pm}1.57pg/ml$, p <0.05). On the other hand, control group under sleep showed the serum level of $7.39{\pm}1.03pg/ml$. Additional acupuncture at In-Dang induced the increased melatonin level, especially at 40minutes after acupuncture(p <0.05). 2. Serotonin level in resting control serum recorded $51.14{\pm}8.17{\mu}g/ml$. Acupuncture at determined meridian point intend to increase average level of serotonin, but not significant. However observation on day 1 after acupuncture at Hab-Gwan meridian point and under sleeping showed the significantly increased serotonin level, $74.05{\pm}35.83{\mu}g/ml$, $70.44{\pm}13.08{\mu}g/ml$, respectively. 3. TSH level in intact human serum recorded $0.85{\pm}0.24{\mu}IU/ml$. and the serum exposed to acupuncture showed the irregular pattern of TSH level in a mean range of $0.76{\pm}0.44{\mu}IU/ml$ to $1.06{\pm}0.38{\mu}IU/ml$, regardless of meridian point or time after acupuncture. 4. The values of serum cortisol in control group were $11.76{\pm}2.06{\mu}g/dl$ in resting, $7.51{\pm}2.85{\mu}/dl$ under sleep, respectively. The level of serum cortisol in 40minutes after acupuncture was markedly (p <0.05) reduced to the serum levels of $5.65{\pm}2.58{\mu}g/dl$ (Hab-Gog, Nae-Gwan), $7.58{\pm}3.21{\mu}g/dl$ (Gyo-Gam, Sin-Mun besides Hab-Gog, Nae-Gwan), $6.43{\pm}3.54{\mu}g/dl$ (In-Dang besides to Hab-Gog, Nae-Gwan), respectively, as compared with control, intending to increase a little on day 1 after acupuncture. From the above results, the analysis of serum intermediates suggest that acupunctuation at meridian points applied to at this study act upon the phase of light anaesthesia or hypnosis, at the same time affect pituitary-adrenal axis rather than hypothalamus-pituitary axis in the secretary system of hormone and also don't affect dorsal-raphe nucleus according to the observation of change transition in serum intermediates, such as TSH, serotonin, and cortisol.

Effects of Addition of Electrolyte and Ascorbic Acid in Feed during Heat Stress in Buffaloes

  • The ameliorative effect of salts and ascorbic acid polyphosphate supplementation on heat stress was studied in buffaloes. Adult buffaloes of either sex were randomly divided into 2 groups of 4 animals each. Group I served as control and Group II was supplemented with sodium bicarbonate, potassium carbonate and ascorbic acid polyphosphate. All the animals were exposed to two conditions of temperature and humidity: hot-dry and hot-humid in a psychrometric chamber for 4 h daily for 10 days. Blood was collected on day 1, 5 and 10 of treatment. The activities of catalase and superoxide dismutase (SOD), concentrations of serum glutathione (GSH), cortisol, sodium, potassium, and chloride and lipid peroxidation were estimated in serum. Lymphocyte proliferation was assessed in blood. The activities of catalase and SOD, serum concentration of GSH, sodium, potassium and chloride decreased while lipid peroxidation and serum cortisol increased in both groups when subjected to heat stress. Dietary supplementation resulted in further decreasing of the enzyme activities but increasing of the serum concentrations of GSH, sodium, potassium and chloride. Lipid peroxidation and serum cortisol increased in the supplemented group in both types of stress. Dietary supplementation caused an increase in lymphoproliferative response to con A. Thus, supplementation of ascorbate in addition to electrolytes relieves the animals of oxidative stress and boosts cell mediated immunity.

The Experimental study on the Anti-Stress effect of Yangsintang(YST) (양신탕(養神湯)의 항(抗)스트레스 효과(效果)에 대(對)한 실험적(實驗的) 연구(硏究))

  • Objective: The study aimed to evaluate the anti-stress effect of YST on the cold-stressed Mice. Method: The experimental animals were stressed in cold room ($4{\pm}0.5^{\circ}C$) for 1 hour in a day during 2weeks, and administerd 115mg/100g YST extract for 2weeks before stress. For 2weeks, we conducted a research about the change of weight and content of norepinephrine, epinephrine, dopamine, cortisol and GOT, GPT in the mice serum. Results: YST significantly inhibited the decrease of body weight induced by stress, compared with the control group. YST had no significant decreasing effect on the change of content of the serum GOT But decreased the content of the serum GPT. YST decreased the content of the serum cortisol. YST decreased the content of the serum dopamine a little and decreased the content of the serum epinephrine significantly. Conclusion: According to the above results, it is concluded that YST will be useful as a remedy against stress disease.
