Perceptions of Barriers to Cardiovascular Risk Factors and Decision to Seek Treatment among Middle-aged Men with Acute Myocardial Infarction (중장년 남성 급성심근경색증 환자의 심혈관위험인자 인식 및 치료추구 장애요인)

  • Purpose: This study was designed to identify meaningful themes related to the recognition of lifestyle risk factors and barriers in seeking treatment following an acute event of first-time acute myocardial infarction. Methods: A methodological mixed method of thematic content analysis and a quantitative analysis was used. The sample consisted of 120 male patients < 65 years of age who agreed to be in the study were interviewed using a semistructured during 2008-2009. Data were analyzed according to the procedure of thematic content analysis and the meaningful themes were coded into SPSS data for quantitative analysis. Results: Pre-hospital delay greater than three hours reported by 58.3% (n=70) of the sample and similarly 63.3% had no recognition about their symptoms as cardiac in origin. The mean number of risk factors was $3.9{\pm}1.8$ out of 11 when lifestyle and psychosocial factors were included. From the interview data among the 70 patients delayed greater than three hours, thirty-five themes categorized into 12 main themes influenced the delayed decision which was identified according to personal-cognitive, socio-cultural, and contextual factors. Conclusion: Health care providers should consider these themes in designing individual interventions to make lifestyle changes and to facilitate more prompt decisions to seek care.

  • Blood pressure is one of the key physiological parameters for determining human health, and can prove whether human cardiovascular function is healthy or not. In general, what we call blood pressure refers to arterial blood pressure. Blood pressure fluctuates greatly and, due to the influence of various factors, even varies with each heartbeat. Therefore, achievement of continuous blood pressure measurement is particularly important for more accurate diagnosis. It is difficult to achieve long-term continuous blood pressure monitoring with traditional measurement methods due to the continuous wear of measuring instruments. On the other hand, radar technology is not easily affected by environmental factors and is capable of strong penetration. In this study, by using machine learning, tried to develop a linear blood pressure prediction model using data from a public database. The radar sensor evaluates the measured object, obtains the pulse waveform data, calculates the pulse transmission time, and obtains the blood pressure data through linear model regression analysis. Confirm its availability to facilitate follow-up research, such as integrating other sensors, collecting temperature, heartbeat, respiratory pulse and other data, and seeking medical treatment in time in case of abnormalities.

Experience of Alopecia in Adults: A Grounded Theory Approach (성인의 탈모경험: 근거 이론적 접근)

  • Purpose: The purpose of this study was to investigate the alopecia experience in adults and to explain the process of their experiences. Methods: Using a grounded theory methodology, 18 interviews were performed with fourteen men and four women, 34~57 years of age, suffering from alopecia. Data were analyzed using the constant comparative analysis method. Results: The core category emerged as "inescapable fetters". adults with alopecia engaged in three stages: embarrassment, seeking solution, and acceptance phase. Causal conditions were a vicious cycle of stress, biological factors and poor life style. Contextual conditions were recognition of irreversibleness, negative social awareness, and marriage. The central phenomenon of the adaptation process among the adults with alopecia was withdrawn life due to negative body image. Action/Interaction strategies included rely on medical treatment, efforts to take good care of hair, research for information treatment, efforts to cover up hair loss, and mind control. Intervening conditions were time cost, economic cost, support of surrounding people. Consequences was burden of unfinished lifetime homework. Conclusion: When caring for these adults, it is important to identify needs, allow patients to express what they want at that moment and support them in maintaining a daily life.

A Cross-Sectional Satisfaction Survey of 845 Traffic Accident Patients Treated with Korean Medicine

  • Background: We conducted a cross-sectional study to analyze the pain characteristics and satisfaction scores in traffic accident (TA) patients seeking Korean medicine treatment. Methods: A survey was conducted in 845 outpatients who visited 10 Korean medicine institutions due to TA-associated symptoms from February 25th, 2016 to March 9th, 2016. The questionnaire included information about patient sociodemographic characteristics, accident circumstance details, pain levels, treatment methods, satisfaction and pre-existing musculoskeletal disorders. All statistical analyses were performed using STATA 14.0. Results: A total 754 of the 845 respondents had experienced acute pain of onset within 1-2 days. Pain in the neck or low back was most common, and low back pain (LBP) was more prevalent in pedestrian-vehicle accident cases. Patients showed highest satisfaction with pharmacopuncture treatment, followed by acupuncture and Chuna manual therapy. Although treatments were not completed at the time of the survey, the majority of patients were satisfied with the Korean medicine treatments they received and indicated their intent to reuse this treatment in the future. The degree of pain reduction after treatment was a factor that influenced patient satisfaction with and intention to reuse Korean medicine. Conclusion: This study determined the characteristics and status of TA patients using Korean medicine and suggests the need for further more inclusive studies.

A Study on the Degradation Properties of MTBE in Solution using Ultrasound (초음파를 이용한 수용액 속의 MTBE 분해 특성 연구)

  • To supply safe drinking water to areas lacking in water supply and drainage system, such as rural area and military bases in proximity to Demilitarized Zone, effective method for treating organic contaminants such as MTBE is required. This study focuses on seeking optimal conditions for effective degradation of MTBE using a bath type ultrasound reactor. Effectiveness of MTBE degradation by ultrasound is dependent on the frequency, power, temperature, treatment volume, initial concentration, catalyst, etc. In this study the degradation rate of MTBE by ultrasound was proportional to power/unit volume ratio and removal is relatively more efficient for 0.1 mM than for 1 mM of MTBE solution. Efficiency of ultrasound treatment for 1 mM MTBE solution was enhanced under bath temperature of $30^{\circ}C$ compared to $4^{\circ}C$, but the temperature effect was negligible for 0.1 mM MTBE solution. Also for 0.1 mM MTBE solution, effect of catalyst such as $TiO_2$ and $Fe^0$ on treatment speed was negligible, and zeolite even increases the time taken for the degradation. Under these specific experimental conditions of this study, the most determinant factor for degradation rate of MTBE in solution was frequency and power of ultrasound. The results have shown that a continuous ultrasound reactor system can be used for small scale remediation of organically polluted groundwater, under optimal conditions.

A Qualitative Study of Physicians' Use of Clinical Information Resources and Barriers (임상의사의 진료목적 정보원 이용과 장애요인에 관한 질적 연구)

  • We analyzed the characteristics of the physicians preferred information sources and barriers through in-depth interviews. Information searches for patient treatment were subdivided into deciding patient treatment methods, understanding the latest treatment trends, and preparing presentation materials for conferences. The variables that affected the search behaviors were identified as being background knowledge on the topic, clinical experience, job title, search skills, user training, and familiarity with the library homepage. PubMed was the most preferred choice because of users' familiarity, reliability, and the vastness of information; Google was also used frequently for easy access and fast search result. The accuracy and the recentness of information were the most significant criteria. Easy interface and convenient access were also considered important due to physicians' time constraints. Searching obstacles were divided into difficulty of searching system, unfamiliar term, too vast resources, difficulty to get fulltext articles and complex advanced search features. The results of this study can be utilized as a basis for improving information service of library and curriculum development for physicians.

Effects of Low Grade Axial Loading on Discogenic Low Back Pain: A Case Report

  • Low back pain (LBP) is the most common reason for seeking physical therapy (PT) care. Recent studies suggest that axial loading can have a positive impact on the intervertebral disc by improving its tensile strength. Further, whole body vibration (WBV) appears to improve spinal muscle relaxation. Therefore, this case study describes the use of axial loading using a mini-trampoline in a female with chronic LBP. This case report is a single subject design. This patient is a 29-year-old female with a six-month history of low back pain following a motor vehicle accident. MRI found herniated discs at L4 and L5, clinical tests were positive for pain in the L4 and L5 dermatome and myotome the slump test was positive for neural tension, and LBP was constant at 4-6/10 over the past four months. She received axial loading exercises using a mini-trampoline and performed six sessions that were, scheduled twice a week for three weeks. Her Oswestry Disability Index (ODI) score improved from 40% at the time of her first visit to 22% at her final visit. Pain measure on the Numeric Pain Rating Scale (NPRS) after the first treatment was 7/10, and her pain after the final treatment was 0/10. These changes in the pain scores are clinically significant and exceed the minimal clinically important difference (MCID). This patient had a significant improvement in her pain using the NPRS and the ODI. This case study suggests that axial loading may be an effective treatment for some individuals with discogenic chronic low back pain.

Discharge Nursing Needs of Patients having Surgery with Gastric Cancer (위암 수술 환자의 퇴원 간호요구)

  • Purpose: The purpose of this descriptive study was to identify the discharge nursing needs of patients who received gastrectomy. Method: The subjects of this study consisted of 100 patients who had gastric surgery in 4 university hospitals in chungnam province. The instrument for this study was developed by the researcher based on literature through in-depth interviews. Data was collected from October 4 to October 30, 2000. Result: The results were as follows: Discharge nursing needs were classified in 8 main categories. The 8 categories were 'emotional and spiritual support', 'nutrition and diet control', 'medication', 'treatment planning and follow up care', 'activity level', 'symptom management' 'health seeking behavior' and 'social support'. The total mean score for nursing needs was 3.89. 'Nutrition and diet control' was the highest degree of nursing needs and 'activity level' was the lowest degree of nursing needs. There were significant differences in degrees of nursing needs between different ages(F=2.74, p=.048), level of education(F=2.91, p=.038) and period since diagnosis(F=4.46, p=.037). Conclusion: This study looked at 8 categories of discharge nursing needs. Patients identified 'nutrition and diet control' as the highest need and 'activity level' as the lowest need. Further research needs to be done to identify discharge educational needs for various age groups, educational level and for patients with varing lengths of diagnosis time to surgery.

The Effects of Hospital Home Nursing Interventions based on the Nursing Diagnosis (간호진단중심의 퇴원계획과 가정간호적용의 효과 -만성질환자를 중심으로-)

  • Home nursing interventions based on nursing diagnosis were implemented to the patient who are discharged from one hospital often the treatment for chronic neuromuscular system problem, and its effects were studied. The purpose of this study was to find out the effectiveness of hospital bouned home nursing provided by hospital nurses and to categorize home nursing diagnosis and its interventions. Data from experimental group patients were collected at three different time ; at the time of discharge, two weeks after discharge and our weeks after discharge. Data from controll group patients were collected twice ; the first one at the time of discharge, and the other one four weeks after discharge. For this study nursing assessment and intervention booklet developed by the research team. There were no significant decrease of the number of nursing problems and life satis-faction. But daily activity level of patients showed the signs of significant improvement at the time of four weeks after discharge. Results of this study indicates that home nursing intervention based on nursing diagnosis provided the patients with noticeable difference in health maintanance, impairment of physical mobility, potential for infection, impaired home marntenance management, health seeking behavior, chronic pain, disuse syndrome, impaired skin integrity.

Detection of methicillin-resistant S. pseudintermedius (MRSP) isolated from dogs and cats in Kyungpook Area (경북지역 개와 고양이에서 methicillin-resistant Staphylococcus. pseudintermedius (MRSP) 검출률 조사)

  • Identification of antibiotic resistant bacteria isolated from animals is necessary for seeking a proper treatment and for preventing the spread of the bacteria among animals. Methicillin-resistant Staphylococcus pseudintermedius (MRSP) is of worldwide concern in veterinary medicine. This study was conducted to investigate the antimicrobial susceptibility of Staphylococcus pseudintermedius (S. pseudintermedius) isolated from dogs and cats. Out of the total number of 150 specimens, 35 isolates (23.3%) were identified as S. pseudintermedius when tested by MALDI-TOF MS (Matrix Assisted Laser Desorption Ionization-Time of Flight Mass Spectroscopy). Among them, 5 isolates (14.3%) were the case of MRSP. In the results of the antimicrobial susceptibility test, the isolates of S. pseudintermedius were susceptible to amikacin (100%), cephalothin (100%), vancomycin (100%), amoxicillin (85.7%), oxacillin (85.7%), enrofloxacin (82.8%), chloramphenicol (80%). On the other hand, they were resistant to penicillin (71.4%), tetracycline (48.6%), ampicillin (37.1%), kanamycin (31.5%), erythromycin (31.4%), respectively.
