Open rhinoplasty in secondary cleft nose deformity with suture techniques

  • Background: Correction of secondary cleft nose deformity is one of the most important portions in the management of cleft lip patients. Various techniques have been introduced to achieve adequate shape, balance, and symmetry of anatomical landmarks. None of these methods can claim to universally solve all aspects of the problems encountered in secondary cleft deformity surgery. Some authors overlook the aspect of functional rehabilitation with regard to nasal respiratory pathway problems, which is present in over 90% of the patients. This study aimed to evaluate the aesthetic and functional improvements of the authors' non-destructive technique. Methods: With over 15 years of experience, open rhinoplasty was performed, which included total remodeling of the deformed lower lateral cartilage using several suture fixation techniques without any graft or implantation with septo-turbinoplasty. A total of 150 questionnaires were sent by e-mail, but 55 completed questionnaires were returned. Surgical outcomes were evaluated using questionnaire responses, and outcomes were divided into five categories each for esthetic and functional analyses. Results: The satisfaction rate ranged from 75 % to 98%, which means "more or less," "very much," and "absolutely yes" in the esthetic and functional viewpoints. Conclusion: The results of this study strongly recommend performing the suture fixation technique and functional rehabilitation simultaneously for cleft lip/nose correction.

Evaluation of the Non-use Values of Taishan Mountain Cultural Heritage

  • The contingent valuation method (CVM) is one of the most commonly used and effective methods to evaluate non-use value of resources. Reasonable application of CVM to value the cultural heritage is the key process of evaluation. CVM was used to evaluate the non-use value of cultural heritage of Taishan Mountain combined with questionnaire survey and field research in this study. The results indicated that the importance of the degree of the three components of non-use value was heritage value ranked highest (40.22%) > followed by existence value (38.58%) >then option value (21.20%). In addition, the rate of willingness to pay was 54.52%, the average and median values of per person were 40.17 CNY·a-1 and 20.00 CNY·a-1 and the non-use values of Taishan Mountain cultural heritage was 33 million CNY·a-1. The median value of WTP was consistent with Asian countries but was lower than European and American countries. Factors influencing WTP showed that monthly income and satisfaction with Taishan Mountain were correlated to WTP, and family location and willingness to revist were correlated remarkably with WTP. In addition, monthly income was correlated remarkably with WTP value, however other factors were not. The results showed the importance and necessity of protective development of Taishan Mountain cultural heritage, which would be used as an important reference for decision makers.

Predictive Factors of Nurses' Turnover Intention during the COVID-19 Pandemic (COVID-19 유행 동안 간호사의 이직의도 예측요인)

  • Purpose: Nurses are exposed to stressful circumstances during the COVID-19 outbreak, which increases their turnover intention. This study aimed to identify predictive factors of turnover intention among nurses during the COVID-19 outbreak. Methods: This study was a cross-sectional design using self-report questionnaires. A total of 164 nurses working at three hospitals participated voluntarily in the study. Questionnaires included nursing professionalism, job satisfaction, control belief and COVID-19 stress. Data were collected from February 1, 2021, to March 15, 2021. Data were analyzed with SPSS WIN 23.0 program using descriptive statistics, x2-test, independent t-test and logistic regression. Results: The turnover intention rate of nurses was 33.5%. Two significant predictors of nurses' turnover intention were identified, including risk acceptance in caring for patients with COVID-19 (odds ratio [OR] 4.24, 95% confidence interval [CI]=1.76~10.49) and nursing professionalism (OR=1.06, 95% CI=1.02~1.10). Conclusion: Hospital administrations should provide an improved work environment, such as individual protective equipment, compensation and protective system to nurses to avoid turnover intention in COVID-19 outbreak.

A Study on the Application of Standard Korean Medicine Clinical Pathway for Dysmenorrhea (월경통 한의표준임상경로 적용 가능성 및 유효성 평가 연구 프로토콜)

  • Objectives: The purpose of this study protocol is firstly to investigate the status of Korean medical treatment of dysmenorrhea, and secondly to investigate effectiveness of the Clinical Pathway (CP) of dysmenorrhea and review the applicability. Methods: This is a multi-center observational study. The data will be prospectively collected from 14 Korean medicine clinics. 45 patients for each of the CP-applied and non-applied groups will be recruited to compare the degree of improvement in menstrual pain. All the diagnosis records, treatment methods, treatment results, adverse events, and medical expenses of patients with dysmenorrhea will be collected. We will investigate the proportion of patients who could be applied with CP, and the actual number of CP applications, and CP completion rate to evaluate the applicability. Additionally, the satisfaction survey will be used to collect feedback from medical staff members and patients. Results: The results of this study will be disseminated through peer-reviewed publications and scientific presentations. Conclusions: This study is expected to provide valuable data for application of standard Korean medicine clinical pathway for dysmenorrhea.

Improving the Infection and Isolation Alarm Program: a Case Study (감염 및 격리 알람 프로그램 개선: 사례 연구)

  • Purpose:The infection and isolation program used at a university hospital in A city was assessed and improved to provide medical staff with easy-to-understand information on isolation precautions and infectious diseases. Methods: Based on the results of the root cause analysis, the infection and isolation alarm computer program was improved. Subsequently, a survey was conducted with infection control leaders and unit managers (n=98) within the department to evaluate the degree of improvement. Results: The isolation registration and release procedures were simplified and unified to prevent confusion among the relevant departments. Additionally, the screen composition was improved so that various information related to infection can be easily accessed. After improvement in the program, the rate of isolation registration (53.0% to 100.0%, p<.001) and user satisfaction (67.6% to 92.2%) improved. Conclusion: This study will help improve the program so that other medical institutions can comply with the isolation precautions in accordance with the type of infections.

  • Pusan National University School of Medicine (PNUSOM) began analyzing the cohort of pre-medical students admitted in 2015 and has been conducting purposeful analyses for the past 3 years. The aim of this paper is to introduce the process of cohort establishment, cohort composition, and the utilization of cohort analysis results. PNUSOM did not initially form a cohort with a purpose or through a systematic process, but was able to collect longitudinal data on students through the establishment of a Medical Education Information System and an organization that supports medical education. Cohort construction at our university is different in terms of a clear orientation toward research questions, flexibility in cohort composition, and subsequent guideline supplementation. We investigated the relevance of admission factors, performance improvements, satisfaction with the educational environment, and promotion and failure rate in undergraduate students, as well as performance levels and career paths in graduates. The results were presented to the Admissions Committee, Curriculum Committee, Learning Outcomes Committee, and Student Guidance Committee to be used as a basis for innovations and improvements in education. Since cohort studies require long-term efforts, it is necessary to ensure the efficiency of data collection for graduate cohorts, as well as the validity and ethics of the study.

The Changes of Living Arrangement in Elderly and Reasons for Their Choice (노년기 가구형태의 변화와 선택 이유)

  • This study aims to analyze the trend of changes in the elderly living arrangement over the past 20 years and the reasons why the elderly choose these changes. The subjects of the survey were households aged 65 years or older in Korea. Data from the survey of Ministry of Health and Welfare conducted in 2004, 2014, 2020 were used for the analysis. The result of analysis was as follows; First, since 2000, the number of elderly households living with their children has been rapidly decreasing, while the number of elderly couple households and single elderly households is gradually increasing. Second, elderly living alone appeared more in rural areas, women, older people, and elderly with low education. They are in a vulnerable position that needs help. Therefore, welfare policies for the elderly should be focused on elderly single households. Finally the reason for choosing elderly single household in 2020 was that the voluntary choice by the individual accounted for a much higher rate than the unintentional choice by the children. In addition, the life satisfaction of the elderly who arbitrarily selected the living arrangement was higher than that of the elderly who deliberately chose the living arrangement.

General Anesthesia and Endoscopic Upper Gastrointestinal Tumor Resection (전신 마취와 내시경적 상부위장관 종양절제술)

  • Appropriate sedation and analgesia are crucial for successful endoscopic procedures, patient safety, and satisfaction. Endoscopic resection for upper gastrointestinal tumors requires a deep sedation level because the procedure is lengthy and induces moderate to severe pain. Continuous patient consciousness assessment and vigilant vital signs monitoring are required for deep sedation. General anesthesia may unintentionally occur even during deep sedation for endoscopic tumor resection, which may cause unexpected complications, especially in high-risk patients. Previous studies have revealed that general anesthesia increases the en bloc resection rate and decreases the procedure time. Complications, such as perforation, aspiration pneumonia, and cardiopulmonary instability, including hypoxemia, hypotension, and arrhythmia, occurred more frequently in patients with sedation compared to those with general anesthesia. Therefore, general anesthesia demonstrated potential benefits in endoscopic treatment results and patient safety. General anesthesia should be considered a useful alternative for sedation in patients undergoing endoscopic gastrointestinal tumor resection. However, more high-quality prospective studies are required to determine the safety and effectiveness of general anesthesia in endoscopic upper gastrointestinal tumor resection because most studies comparing general anesthesia and sedation in these procedures have been retrospectively conducted and the results were inconsistent.

Silicone radial head prostheses revisited: do they have a role in today's practice? A systematic review of literature on clinical outcomes

  • Background: Silicone radial head prostheses (SRHP) are considered obsolete due to reports of frequent failure and destructive silicone-induced synovitis. Considering the good outcomes of modern non-radial silicone joint implants, the extent of scientific evidence for this negative view is unclear. The aim of this research was to systematically analyze the clinical evidence on complications and outcomes of SRHP and how SRHP compare to both non-SRHP and silicone prostheses of other joints. Methods: A systematic literature review was conducted through the Cochrane, PubMed, and Embase databases. Results: Eight cohort studies were included, consisting of 142 patients and follow-up periods ranging from 23 months to 8 years. Average patient satisfaction was 86%, range of 71%-100%, and 58 complications were seen, but no cases of synovitis. These outcomes were in line with non-SRHP. Four case series with 11 cases of synovitis were found, all due to implant fractures years to decades after implantation. Six systematic reviews of currently used non-radial silicone joint implants showed excellent outcomes with low complication rates. Conclusions: Since SRHP have satisfactory clinical results and an acceptable complication rate when selecting a patient group in suitable condition for surgical indications, it is considered that SRHP can still be chosen as a potential surgical treatment method in current clinical practice.

Evaluation of Structural Safety for Hydrogen Tube Trailer Considering Dynamic Property (동적 특성을 고려한 수소 튜브 트레일러의 구조 안전성 평가)

  • Recently, hydrogen energy has been widely used because of strict regulations on greenhouse gas emissions. For using the hydrogen energy, it is required to supply hydrogen through a tube trailer. However hydrogen tube trailer can have excessive load problems during transportation due to reasons such as road shape and driving method, which may lead a risk of hydrogen leakage. So it is necessary to secure a high level of safety. The purpose of this study is to evaluate structural safety for the conservative design of hydrogen tube trailer. First, finite element(FE) modeling of the designed hydrogen tube trailer was performed. After that, safety evaluation method was established through static structural simulation based on the standard GC207 conditions. In addition, effectiveness of the designed model was confirmed through the results of the structural safety evaluation. Finally, driving simulation was used to derive acceleration graph according to time, which was considered as a dynamic property for the evaluation of conservative tube trailer safety evaluation. And dynamic structural simulation was conducted as a condition for actual transportation of tube trailer by applying dynamic properties. As a results, conservative safety was evaluated through dynamic structural simulation and the safety of hydrogen tube trailer was confirmed through satisfaction of the safety rate.
