Objectives: This study aimed to establish a linkage model involving regional responsible medical institutions after analyzing the existing conditions and deriving problems through qualitative analysis within the community care system. Methods: A total of 14 participants of this study were selected through the snowball sampling method, including 7 community care service providers and 7 service users. As for the research data, primary data were collected through interviews, and as a result of analyzing according to Aday&Anderson' model, a total of 5 catergories, 8 topics, and 22 sub theme were derived. Results: The problem derived from the interview is that division services are provided for each institution due to the absence of a key central institution of community care system, and users' commercial institutions is unclear. The second is the inconsistency between the needs and supply for community care, resulting in a possibility of delay in returning to the community after discharge. Based on these problems, it is necessary to unify it as an community care window of the Dong-community center. In addition, there is a need for public health centers to play an active role, and to establish a public-private joint system with the Health and Living Support Center to establish a model that can play a certain role. Conclusions: Therefore, based on the results of this study, it can be used as basic data when constructing community care model and applying it as an expanded model in the future.

Objectives: The purpose of this research was to explore Fall Risk Home Environment(FRHE) and to investigate the association between FRHE and fall experience among community-dwelling older adults. Methods: The data were collected from 299 older adults using FRHE through observation and interview at home of the participants and were analyzed with SPSS 22.0 applying descriptive statistics, χ2-test, t-test, and logistic regression analysis. Results: The prevalence of fall experience during the past year was 51.5%. 'No handles beside the toilet or bathtub'(73.2%) was most common FRHE factor, 'thresholds in your room or kitchen'(68.9%), 'wearing socks, outer socks, or slipper when you move in the house'(59.5%), and threshold on the gate (apartment entrance)(55.5%) were followed. The findings of logistic regression of FRHE on fall experiences showed darkness of house had the highest Odds Ratio (OR 9.83 95% CI 3.75-25.71), followed by furniture obstructs your walking in the house(OR 7.07, CI 2.88-17.36), dark kitchen (OR 5.13, CI 2.38-11.03). The group having fall experiences presented significantly higher score of FRHE than the group of non experiences of fall. Conclusion: The community dwelling older adults exposures to various FRHE factors and FRHE might increase the risk of falls. Assessing and modifying the home environment could be a good strategy to prevent fall among older adults.

The purpose of this study is to analyse the hazard risk by examining the magnitude and severity of each type of hazard in order to mitigate and prepare for disasters in medical facilities. Methods: The hazard risk analysis for hazard types was surveyed for team leaders of medical facilities. The questionnaire analyzed data from 27 facilities, which were returned from 41 Local Medical Centers. Results: When looking at the 'Risk' by category type of hazard, the influence of health safety and fire/energy safety comes first, followed by natural disaster, facility safety, and crime safety. On the other hand, as for 'Magnitude', facility safety and crime safety come first, followed by health safety, fire/energy safety, and natural disasters. Most of the top types of disaster judged to have high hazard in medical facilities are health types. The top five priorities of hazard in medical facilities, they are affected by the geographical and industrial conditions of the treatment area. In the case of cities, the hazard was found to be high in the order of infectious disease, patient surge, and wind and flood damage. On the other hand, in rural areas, livestock diseases and infectious diseases showed the highest hazard. In the case of forest areas, the hazard was high in the order of wildfire, fire accident, lightning, tide, earthquake, and landslide, whereas in coastal areas of industrial complexes, the hazard was high due to fire, landslide, water pollution, marine pollution, and chemical spill accident. Implications: Through the research, standards will be established for the design of hospitals with disaster preparedness, and will contribute to the preparation of preemptive measures in terms of maintenance.
The concept of "public health care (public health and medical services)" as discussed in South Korea is used in an unclear sense, with a meaning unlike the terminology used worldwide. The terms "public health care (public health and medical services)" and "health care (health and medical services)" have the same legal definition in Korea. Globally, "public health care (public health and medical services)" refers to medical services provided to the public that are operated as publicly funded resources, but in Korea, this term is confined to limited medical services prescribed by the government. The following considerations regarding "public health care (public health and medical services)" in Korea are proposed: All medical services performed by the state, regional governments, health care institutions, or health care workers to protect and promote the health of the people should be clearly established as "public health care (public health and medical services)" by definition. The financial burden borne by the state through national health insurance should be increased to an appropriate level to clarify the state's responsibility. Improving public health is an urgent priority in Korea, and this goal can be achieved by improving regional public health through systematic relationships between the state and regional governments, establishing a Ministry of Health, and efficiently allocating public health doctors who are important for providing regional medical care in rural and remote areas. It will be possible to actively deal with infectious diseases at the national level through establishment of a Ministry of Disease Control and Prevention.

Background: Canine herpesvirus type 1 (CaHV-1) infects dogs and is associated with neonatal deaths and reproductive, ocular, neurological, and respiratory problems. In Brazil, reports of CaHV-1 have been restricted to the southeast and south regions, particularly in municipalities in the state of Rio Grande do Sul. Objectives: To assess the presence and variability of CaHV-1 in canine populations in the state of Pará, North Brazil. Methods: Biological samples from 159 dogs from 4 municipalities in the State of Pará were evaluated using polymerase chain reaction and phylogenetic analyses, with the target being the viral enzyme, thymidine kinase. Results: CaHV-1 was detected in 13 dogs (8.2%), with 2 animals being from the municipality of Santa Bárbara do Pará, 8 from Algodoal Island, 2 from Salinópolis, and one from Capanema. The study sequences revealed 100% identity among themselves and 64% to 100% identity with the other nucleotide sequences from Australia, Brazil, United Kingdom, and United States, including 100% identity with the 2002 isolate from Australia. The 1996 isolate from France was grouped in a branch that was different from the sequence of this study. Conclusions: This study presents the first molecular detection of CaHV-1 in dogs from the Amazon region in northern Brazil. The nucleotide identity between the strains and cytosine insertion in the sequences isolated in this study suggests at least 2 strains of CaHV-1 circulating in Brazil (Pará and BTU-1).

목적: 2017년 지하수를 식수로 사용하고 있는 한 장기요양 정신병원(H병원)에서 A형간염 환자가 집단 발병하여 이에 대한 역학조사를 실시하고 조치 결과를 기술하고자 하였다. 방법: 노출기간 동안 H병원의 근로자 및 재원 환자 234명을 대상으로 사례군 조사 디자인으로 역학조사를 실시하였고, IgM, IgG 혈청검사 및 A형간염 바이러스(HAV)에 대한 PCR검사를 시행하였다. 또한 오염원으로 의심되는 지하수, 병원에서 제공되는 식품 및 인근 저수지의 물에서 HAV 검사를 실시하였고, 검출된 HAV는 유전형 검사를 진행하였다. 결과: H병원 환자 및 직원 234명 중 IgG 양성인 168명을 제외한 66명 중 19명이 최종적으로HAV 감염자로 확인되어 감수성자 중 발병률은 28.8%로 나타났다. 환자, 지하수, 식품(석박지) 및 저수지에서 동일 유전형의 HAV가 검출되어 지하수 오염에 의한 집단발병으로 결론 내렸으나, 최초 오염원은 확인하지 못하였다. 유행 종결 선언 이후 지하수에 대한 관리로 염소소독과 UV 조사를 하였음에도 불구하고 6개월 동안 지속적으로 HAV가 검출되어 새로운 관정을 개발하여 상황을 종결하였다. 결론: 본 연구에서 지하수를 식수로 사용하는 장기요양 정신병원에서 지하수 오염에 의한 19명의 HAV 집단발병을 조사하였다. HAV 항체가 없는 대상자 중에서 HAV의 높은 발병률을 확인하였다. 지하수 수질검사에서 바이러스 검사는 포함되어 있지 않기 때문에 지하수가 HAV에 오염시 HAV 집단발병 가능성이 높고 상당기간 지속적으로 검출되기 때문에 지하수에 대한 관리지침에 바이러스 검출을 위한 방안을 추가하고 관련 법을 정비할 필요가 있다.

담낭용종의 잘 알려진 위험요소는 대사증후군, 연령, 이상지질혈증 등이다. 제주도는 한반도에서 가장 큰 섬으로 한국의 본토에서 약 80km 떨어져 있으며 행정구역상 제주시와 서귀포시로 나뉘어 있지만 기업들과 편의시설들, 학교들 때문에 자연스럽게 시내와 시외로 더 나누어진다. 많은 이주민들과 젊은 성인들은 상업 활동이 활발한 시내 지역에 사는 경향이 있다. 고령 성인들은 어업이나 농업에 종사하기 때문에 시외 지역에 사는 경향이 있다. 고령의 성인들은 전통적인 식이를 유지하고 있고, 그들 중 다수는 힘든 노동으로 인해서 고위험 알코올 음주자의 비율이 높다. 이러한 요인들로 인해 두 지역간의 담낭용종 유병률에도 차이가 있을 것이다. 따라서 이 연구의 목적은 제주도 두 지역간에 담낭용종의 유병률을 알아보고 그 유병률과 관련된 다양한 요인들을 비교해 보고자 하는 것이다. 이 연구는 2009년 1월부터 2019년 12월까지 제주대학교병원 단일 건강검진센터에서 건강검진을 시행한 21,734명을 대상으로 하였다. 시내 지역과 시외 지역 거주민들의 담낭용종 유병률은 각각 10.1%와 8,7% (P = 0.006)로 의미있게 차이가 있었다. 평균 총콜레스테롤 수치가 시내 지역 거주민들에서 시외 지역 거주민들보다 높았고, 음주와 관련된 평균 알카라인 인산분해효소와 감마-글루타밀전이효소 수치들과 평균연령은 시외 지역 거주민들이 시내 지역 거주민들보다 더 높았다. 나이, 알코올 섭취와 평균 콜레스테롤 수치가 이러한 두 지역 간에 담낭용종 유병률의 차이를 만들었을 것이다.
Purpose: The study aim was to examine the home healthcare system and relevant education, as well as the special certification examination, and propose a plan to increase the supply of home health nurses. Methods: A narrative literature review was conducted using data from research articles, Korean and U.S. educational institutes and organizations, Korean national statistical data, government press releases, and related medical legislation. Results: Between 2005 and 2006, 763 home health nurses were certified through the special qualification examination; however, in the 16 years from 2007 to 2023 (after the graduate-level program was established), a total of 555 home health nurses were certified, with an average of approximately 35 per year. Currently, 790 home health nurses are working at 194 medical institutions nationwide. Relatively few institutions exist in rural areas, and the supply of home health nurses is low. Only seven educational institutions offer home health nurse certification programs, with a total of 77 designated students. In contrast to Korea, post-master's certification courses are offered in the U.S.. Conclusion: To expand the supply of home health nurses, we recommend revising the rules for the special qualification examination and introducing a post-master's certification program for home health nurses. Future studies should provide additional education for applicants from other specialties in post-master's certification programs.

= ABSTRACT =Objectives : We evaluated the effect of the aircraft noise on the behavior and affect status of the local elementary school students.
Methods : The daytime noise levels of the exposed and control areas were compared and the self answered questionnaires from 272 students and their parents of the exposed area and 91 of the control area were collected. The questionnaires are regarding the noise sensitivity and the psychosocial status of the students, noise sensitivity of the parents and the parents' behavioral evaluation for their children.
Results : The mean noise level of the exposed area (67.6 dB(A)) showed higher level than the control area (57.1 dB(A), 58.2 dB(A), 60.2 dB(A)) and it was statistically significant. The mean score for noise sensitivity and the mean score for brief mental exam. in the exposed students were lower than the mean scores in the control group but they were not statistically significant (p=0.056 & 0.165). The mean score for
behavioral evaluation of students by their parents in the exposed area(15.5) was higher than the mean score in the control group(13.2) and it was statistically significant(p=0.043). The mean score for noise sensitivity in the exposed parent(27.5) was higher than the mean score in the control group(24.1) and it was statistically significant(p=0.045). Conclusions : This study revealed that the noise sensitivity of the parents affected the behavioral evaluation of the students than noise-exposure itself. The further study is needed to evaluate relationship between aircraft noise and it's effect on children's health.
The purpose of this study is to establish the basic data for the management of the elderly patients aged more than 60 suffering from musculoskeletal disorders in geriatric hospitals. From January, 1 2012 to December 31, 2012, 2,500 patients who were taken the x-ray inspection and were analyzed x-ray order 5,042 cases above spines, upper lower extremities and joints. The elderly age was divided into 3 groups according to Brody elderly statistics. The majority of elderly patients who visited the hospital are exposed to spinal disorders and joint diseases. Shoulder, knee, L-spine examination cases had accounted for more than half the number of total. By age group, the number of the first quarter was most dominant commonly. Second and third quarter was maintaining slightly decrease. The fourth quarter had reduced compared to the first quarter. If the visiting purposes and the disease causes analysis and preventive measures are taken in a study, it is expected to utilize as basic data to make a plan of the education for preventing diseases and elderly behavior planning for geriatric hospitals and to redistribute health care resources of the rural areas efficiently.
