A Measurement Method for Cervical Neural Foraminal Stenosis Ratio using 3-dimensional CT (3차원 컴퓨터단층촬영상을 이용한 신경공 협착률 측정방법)

  • Cervical neural foraminal stenosis is a very common spinal disease that affects a relatively large number of people of all ages. However, since imaging methods that quantitatively provide neural foraminal stenosis are lacking, this study attempts to present quantitative measurement results by reconstructing 3D computed tomography images. Using a 3D reconstruction software, the surrounding bones were removed, including the spinous process, transverse process, and lamina of the cervical spine so that the neural foramen were well observed. Using Image J, a region of interest including the neural foramen area of the 3D image was set, and the number of pixels of the neural foramen area was measured. The neural foramen area was calculated by multiplying the number of measured pixels by the pixel size. In order to measure the widest area of the neural foramen, it was measured between 40-50 degrees in the opposite direction and 15-20 degrees toward the head. The measured cervical neural foramen area showed consistent measurement values. The largest measured area of the right neural foramen C5-6 was 12.21 ㎟, and after 2 years, the area was measured to be 9.95 ㎟, indicating that 18% stenosis had progressed. Since 3D reconstruction using axial CT scan images, no additional radiation exposure is required, and the area of stenosis can be objectively presented. In addition, it is good to explain to patients with neural stenosis while viewing 3D images, and it is considered a good method to be used in the evaluation of the progression of stenosis and post-operative evaluation.

  • Diverse official policies in community are available for caregivers to take care of the elderly in the US and the UK. This study aims to examine the recent changes in government supports based on The Recognize, Assist, Include Support and Engage (RAISE) in the US, and those by Carer Act 2014 in the UK, to take any good lessons for Korean policy. Caregivers will play a new role to develop innovative treatment for patient-centered care. The UK strengthens to provide various efforts for working carers while assuring economic efficiency in labor market with empirical evidence. The major four ways to support carers were developed agreed with the acknowledge of caregiving value and their human right; financial support, direct services for carers, flexible work time, and advocacy. Korean supports policies for carers in long-term care and social welfare will be more effective in community care system if more evidence based policies are prepared.

A Study on Consumer perception changes of online education before and after COVID-19 using text mining (텍스트 마이닝을 활용한 온라인 교육에 대한 소비자 인식 변화 분석: COVID-19 전후를 중심으로)

  • Coinciding with the advent of COVID-19, online education is on the rise both domestically and globally, and has become an absolutely necessary and irreplaceable form of education. It is a very curious question what the perception of people about the suddenly growing form of education is, and how it has changed. This study investigated changes in consumers' perception of online education using big data. To this end, we divided the time into four stages: before COVID-19 (November to December 2019), after the triggering of COVID-19 (January to February 2020), right after the online classes started (March to April 2020), after experiencing some online education (May to June 2020). Then we conducted text mining, namely, keyword frequency analysis, network analysis, word cloud analysis, and sentiment analysis were performed. The implications derived as a result of the analysis can help education policy makers and educators working in the field to improve online education quality and establish its future directions.

A Comparison for Cervical Neural Foraminal Area by 3-dimensional CT in Normal Adults (3차원 컴퓨터단층촬영상을 이용한 정상 성인의 경추 신경공 면적 비교)

  • Cervical foraminal stenosis is a disease in which the nerves that pass from the spinal canal to the limbs are narrowed and the nerves are compressed or damaged. Due to the lack of an imaging method that provides quantitatively stenosis, this study attempted to evaluate the area of the cervical vertebrae by reconstructing a three-dimensional computed tomography image, and to determine the area of the neural foramen in normal adults to calculate the stenosis rate. Using a three-dimensional image processing program, the surrounding bones including the posterior spinous process, lateral process, and lamellar bones of the cervical vertebra were removed so that the neural foramen could be observed well. A region of interest including the neural foraminal area of the three-dimensional image was set using ImageJ, and the number of pixels in the neural foraminal area was measured. The neural foraminal area was calculated by multiplying the number of measured pixels by the pixel size. To measure the largest neural foraminal area, it was measured between 40~50 degrees in the opposite direction and 15~20 degrees toward the head. The average area of the right C2-3 foramen was 44.32 mm2, C3-4 area was 34.69 mm2, C4-5 area was 36.41 mm2, C5-6 area was 35.22 mm2, C6-7 area was 36.03 mm2. The average area of the left C2-3 foramen was 42.71 mm2, C3-4 area was 32.23 mm2, C5-6 area was 34.56 mm2, and C6-7 area was 31.89 mm2. By creating a reference table based on the neural foramen area of normal adults, the stenosis rate of patients with neural foraminal stenosis could be quantitatively calculated. It is expected that this method can be used as basic data for the diagnosis of cervical vertebral foraminal stenosis.

Effect of adipose-derived stem cells on bone healing on titanium implant in tibia of diabetes mellitus induced rats (당뇨 유도 백서 경골에 매식한 티타늄 임플란트 주위에 지방조직 유래 줄기세포 적용 시 골치유에 미치는 영향)

  • Introduction: Diabetes mellitus, as a major health problem for the elderly has been shown to alter the properties of the bone and impair bone healing around a titanium implant in both humans and animals. The aim of this study was to examine the effect of adipose-derived stem cells on the healing process around a titanium implant in streptozotocin-induced diabetic rats. Materials and Methods: Thirteen rats were divided into two groups: adipose-derived stem cells injected group and a control group. A titanium screw implant (diameter: 2.0 mm, length: 3.5 mm) was placed into both tibia of 13 rats: 13 right tibia as the control group and 13 left tibia as the experimental group. The rats were sacrificed at different intervals (1, 2, and 4 weeks) after implantation for histopathology observations and immunohistochemistric analysis. Results: The histopathological findings revealed earlier new formed bone in the experimental group than the control group. In particular, at 1 week after implantation, the experimental group showed more newly formed bone and collagen around the implant than the control group. In immunohistochemistric analysis, osteoprotegerin (OPG) expression in the experimental group increased early compared to that of the control group until 2 weeks after implantation. However, after 2 weeks, OPG expression in the experimental group was similar to OPG expression in the control group. The receptor activator of nuclear factor ${\kappa}B$ ligand (RANKL) expression in the experimental group increased early compared to that of the control group, and then decreased at 2 weeks. After 2 weeks, the level of RANKL expression was similar in both groups. Conclusion: These results suggest that adipose-derived stem cells in implantation can promote bone healing around titanium, particularly in diabetes mellitus induced animals.

The effects of action observation and motor imagery of serial reaction time task(SRTT) in mirror neuron activation (연속 반응 시간 과제 수행의 행위 관찰과 운동 상상이 거울신경활성에 미치는 영향)

  • Purpose : The object of this study was to examine the effect of motor learning on brain activation depending on the method of motor learning. Methods : The brain activation was measured in 9 men by fMRI. The subjects were divided into the following groups depending on the method of motor learning: actually practice (AP, n=3) group, action observation (AO, n=3) group and motor imagery (MI, n=3) group. In order to examine the effect of motor learning depending on the method of motor learning, the brain activation data were measured during learning. For the investigation of brain activation, fMRI was conducted. Results : The results of brain activation measured before and during learning were as follows; (1) During learning, the AP group showed the activation in the following areas: primary motor area located in precentral gyrus, somatosensory area located in postcentral gyrus, supplemental motor area and prefrontal association area located in precentral gyrus, middle frontal gyrus and superior frontal gyrus, speech area located in superior temporal gyrus and middle temporal gyrus, Broca's area located in inferior parietal lobe and somatosensory association area of precuneus; (2) During learning, the AD groups showed the activation in the following areas: primary motor area located in precentral gyrus, prefrontal association area located in middle frontal gyrus and superior frontal gyrus, speech area and supplemental motor area located in superior temporal gyrus and middle temporal gyrus, Broca's area located in inferior parietal lobe, somatosensory area and primary motor area located in precentral gyrus of right cerebrum and left cerebrum, and somatosensory association area located in precuneus; and (3) During learning, the MI group showed activation in the following areas: speech area located in superior temporal gyrus, supplemental area, and somatosensory association area located in precuneus. Conclusion : Given the results above, in this study, the action observation was suggested as an alternative to motor learning through actual practice in serial reaction time task of motor learning. It showed the similar results to the actual practice in brain activation which were obtained using activation of mirror neuron. This result suggests that the brain activation occurred by the activation of mirror neuron, which was observed during action observation. The mirror neurons are located in primary motor area, somatosensory area, premotor area, supplemental motor area and somatosensory association area. In sum, when we plan a training program through physiotherapy to increase the effect during reeducation of movement, the action observation as well as best resting is necessary in increasing the effect of motor learning with the patients who cannot be engaged in actual practice.

The Effects of Parents' Energy Conservation Awareness and Behaviors on Young Children's Environment-Friendly Attitudes and Environmental Preservation Knowledge (학부모의 에너지절약 인식 및 행동이 유아의 친환경태도와 환경보전지식에 미치는 영향)

  • The purpose of this study was to figure out the effects of parents' energy conservation awareness and behaviors on young children's environment-friendly attitudes and environmental preservation knowledge. To achieve this, a survey was carried out to 160 kindergarten children and their parents in the farming and fishing communities located in N city, Chungnam through June 10 to June 30, 2013. The results of the analysis were as follows. First, the effect of parents' energy conservation awareness on young children's environment-friendly attitudes showed that parents' environmental preservation awareness and energy conservation awareness had an effect on young children's environment-friendly attitudes. Second, the effect of parents' energy conservation behaviors on young children's environment-friendly attitudes showed that parents' general behaviors and selective behaviors had an effect on young children's environment-friendly attitudes. Third, the effect of parents' energy conservation awareness on young children's environmental preservation knowledge showed that parents' environmental preservation knowledge and energy conservation awareness had an effect on young children's environmental preservation knowledge. Fourth, the effect of parents' energy conservation behaviors on young children's environmental preservation knowledge showed that parents' general behaviors and selective behaviors had an effect on young children's environmental preservation knowledge. As for findings stated above, parents' energy conservation awareness and behaviors had a positive effect on children's environment-friendly attitudes and environmental preservation knowledge. This implies that parents' right energy conservation awareness and behaviors are important to enhance children's positive environment-friendly attitudes and environmental preservation knowledge.

  • The purpose of this study is to analysis of the difference between dementia policy perception, dementia knowledge, dementia attitude and dementia prevention behavior of middle-aged. The subjects were 217 middle-aged in their 40 to 64 years old who lived in D and G cities. The mean sore of were dementia policy perception 75.42±8.52, dementia knowledge 8.71±1.90, dementia attitude 28.55±5.10, dementia prevention behavior 27.04±3.35. Dementia knowledge were significant differences in gender, number of diseases, source of dementia information. Dementia attitudes were significant differences in gender, age, education, dementia in family, dementia living together, job status, early examination, source of dementia information. Dementia prevention behavior were significant difference in dementia living together and early examination. The most category of dementia prevention behavior was 'cigarette smoking(Inverse question)', and blood pressure and diabetes management' were the lowest. Therefore, development of a professional and systematic dementia education program to raise the awareness of dementia policy among middle-aged adults and form the right dementia knowledge and positive dementia attitude. Development of customized dementia prevention behavior programs to maintain cognitive activities, social activities, proper eating habits, and health care is needed.

Effect of Smoking on Gingival Blood Flow (흡연이 치은혈류량에 미치는 영향)

  • Recent studies have demonstrated that smoking may be one of the most significant risk factors in the development and progression of periodontal disease. Reports have indicated that smoking causes gingival blood flow to be decreased. However, studies on the effects of smoking on gingival blood flow have yielded contradictory results. The purpose of the present study was to determine the effect of smoking on gingival blood flow. One hundred volunteers(fifty non-smokers and fifty smokers) with good general and periodontal health, aged twenties(non-smoker : 22-29 years, mean=25.36, smoker : 23-29 years, mean=26.64) were selected. Laser Doppler flowmetry (floLAB, Moor Instruments Ltd., England) was applied to measure the gingival blood flow of interdental papilla, marginal gingiva, attached gingiva and alveolar mucosa of left and right upper lateral incisors. In smokers, following an overnight abstinence from smoking, gingival blood flow was measured before smoking, immediately after smoking, 1-, 2-, 3-, 4-, 5- and 6- hour after smoking from 9 a.m. to 3:30 p.m. The difference of blood flow in each tissue of non-smokers and that of each measuring time and each tissue of smokers were statistically analyzed by one way ANOVA and Tukey test. And the difference of blood flow between smokers and nonsmokers in each tissue was statistically analyzed by t-test. The results were as follows : 1. Mean blood flow was highest in alveolar mucosa, followed by interdental papilla, attached gingiva and marginal gingiva in both smokers and nonsmokers. There was a statistically significant difference in each tissue(p<0.05) . 2. There was no consistent result between mean blood flow before smoking in smokers and that of nonsmokers in each tissue. 3. There was a statistically significant difference between gingival blood flow at measuring time point and gingival blood flow of smokers in each tissue(p<0.05). The present study suggested that smoking could alter the gingival blood flow, thus might be partly contributed to periodontal destruction.

Knowledges, consciousnesses, and attitudes of some university students on the use of radiations (일부 대학생의 방사선이용에 대한 지식, 의식도 및 태도)

  • This study attempts to extract certain directions for the education program to provide right information by investigating knowledges, consciousnesses, and attitudes of some university students. They have a high level of knowledges among average persons on the use of radiations in order to recognize the radiation safety in daily lives including the change in national recognitions for radiations safety that is consistent with the situation in these days. The survey was performed for 528 university students from Oct. 10 to Oct. 15, 2005. The results of this survey can be summarized as follows; students who didn't get educations for the use of radiations were recorded as 415 students (80.7%) in which the reason that they didn't get such educations was there were no chances to receive the education' as 265 students (77.9%). The basic knowledge on the use of radiations was presented as a low level of $5.96{\pm}2.90$ points based on the sum of 15 points, and the consciousness on the use of radiations was investigated as $3.65{\pm}0.50$ points based on the sum of 5 points. In addition, the attitude on the use of radiations was investigated as $3.23{\pm}0.46$ points based on the sum of 5 points.
