Effects of Root of Liriope Spicata on LPS-induced Lung Injury (맥문동이 LPS로 유도된 폐손상에 미치는 영향)

  • This study was purposed to evaluate the effects of root of Liriope spicata (RLS) on LPS-induced COPD (chronic obstructive pulmonary disease) model. The extract of RLS was treated to A549 cells and LPS-induced COPD mice model. Then, various parameters such as cell-based cyto-protective activity and histopathological finding were analyzed. RLS showed a protective effect on LPS-induced cytotoxicity in A549 cells. This effect was correlated with analysis for caspase 3 levels, protein level of cyclin B1, Cdc2, and phospho-Erk1/2, and gene expression of TNF-${\alpha}$ and IL-$1{\beta}$ in A549 cells. RLS treatment also revealed the protective effect on LPS-induced lung injury in COPD mice model. This effect was evidenced via histopathological finding including immunofluence stains against caspase 3, and protein level of cyclin B1, Cdc2, and Erk1/2 in lung tissue. These data suggest that RLS has a pharmaceutical properties on lung injury. This study would provide an scientific evidence for the efficacy of RLS for clinical application to patients with COPD.

Goodness-of-Fits of the Spirometric Reference Values for Koreans and USA Caucasians to Spirometry Data from Residents of a Region within Chungbuk Province

  • Background: Korean regression models for spirometric reference values are different from those of Americans. Using spirometry results of Korean adults, goodness-of-fits of the Korean and the USA Caucasian regression models for forced vital capacity (FVC) and forced expiratory volume in one second ($FEV_1$) were compared. Methods: The number of study participants was 2,360 (1,124 males and 1,236 females). Spirometry was performed under the guidelines of the American Thoracic Society and the European Respiratory Society. After excluding unsuitable participants, spirometric data for 729 individuals (105 males and 624 females) was included in the statistical analysis. The estimated FVC and $FEV_1$ values were compared with those measured. Goodness-of-fits for Korean and USA Caucasian models were compared using an F-test. Results: In males, the expected values of FVC and $FEV_1$ using the Korean model were 12.5% and 5.7% greater than those measured, respectively. The corresponding values for the USA Caucasian model were 3.5% and 0.6%. In females, the difference in FVC and $FEV_1$ were 13.5% and 7.7% for the Korean model, and 6.3% and 0.4% for the USA model, respectively. Goodness-of-fit for the Korean model regarding FVC was not good to the study population, but the Korean regression model for $FEV_1$, and the USA Caucasian models for FVC and $FEV_1$ showed good fits to the measured data. Conclusion: These results suggest that the USA Caucasian model correlates better to the measured data than the Korean model. Using reference values derived from the Korean model can lead to an overestimation regarding the prevalence of abnormal lung function.

The Effects of Jeongcheon-tang and Cheongsangboha-tang on IL-4, IL-5, Il-6 and IL-10 in Asthma Model (정천탕(定喘湯)과 청상보하탕(淸上補下湯)이 asthma model 내(內)의 cytokine에 미치는 영향(影響))

  • Objectives: We aimed to identify. the effect of Jeongcheon-tang(定喘湯) and Cheongsangboha-tang(淸上補下湯) on the transcriptional activities of cytokine IL-4, IL-5, IL-6 and IL-10 involved in asthma model. Materials and Methods: RBL-2H3 cell lines were used. Cells were stimulated with calcium inophore($2{\mu}M$ : Sample group 1, $4{\mu}M$ : Sample group 2) for maximal gene expression. After 3rd treatment of samples and incubation(per 24hours), total cellular RNAs were collected using Trizol solution method. Then transcriptional activities of IL-4, IL-5, IL-6 and IL-10 were measured by RT-PCR with electrophoresis. Results: In IL-4 study, Jeongcheon-tang treated group showed 82.76%(Sample group 1) of transcriptional activities compared to the control group and Cheongsangboha-tang treated groups showed 85.77% (Sample group 1), 89.42% (Sample group 2) of transcriptional activities compared to the control groups. In IL-5 study, Jeongcheon-tang treated groups showed 88.24%(Sample group 1), 98.83%(Sample group 2) of transcriptional activities compared to the control groups and Cheongsangboha-tang treated group showed 73.66%(Sample group 2) of transcriptional activities compared to the control group. In IL-6 study, Jeongcheon-tang treated group showed 92.95%(Sample group 2) of transcriptional activities compared to the control group and Cheongsangboha-tang treated group showed n.40%(Sample group 2) of transcriptional activities compared to the control group. In IL-10 study, Jeongcheon-tang treated group showed 118.46% (Sample group 2) of transcriptional activities compared to the control group. Conclusions: This study shows that Jeongcheon-tang has the inhibitory effect on the transcription of IL-4, IL-5, IL-6 gene expression and the increasing effect on the transcription of IL-10 gene expression, and Cheongsangboha-tang has the inhibitory effect on the transcription of IL4, IL-5 and IL-6 gene expression in RBL-2H3 cell lines. Advanced studies are required to investigate the mechanisms of inhibition or increase by herbal medicine in asthma model.

PIV measurement of oscillatory flow in a micro-channel as a bronchiole model

  • The improvement of artificial respiration method has brought about the decrease in mortality of pulmonary diseases patients. Various respiratory curative methods, inclusive of HFOV (High Frequency Oscillatory Ventilation), have been developed for more effectual and less harmful management of acute respiratory failure. However, the mechanism of gas transfer and diffusion in a bronchiole has not yet been clarified in detail. As a first approach to the problem, we measured oscillatory flows in a Y-shaped micro-channels as bronchiole model by micro Particle Image Velocimetry(micro PIV). In order to establish the fundamental technique of PIV measurements on oscillatory air flow in a micro-channel, we used about 500-nm-diameter incense smoke particles, a diode laser, a high speed camera including an objective lens, and a HFOV, which is effective technique for medical care of pulmonary disease patients, especially, infants. The bronchiole model size is that parent tube is $500\{mu}m$ width and $500\{mu}m$ depth, and daughter tubes are $450\{mu}m$ width and $500\{mu}m$ depth. From this study made on the phenomenon of fluid in micro size bronchus branch of a lung, we succeeded to get time series velocity distribution in a micro scale bronchial mode. The experimental results of velocity distribution changing with time obtained by micro PIV can give fundamental knowledge on oscillatory airflow in micro-channel.

Antitussive, Expectorant, and Anti-inflammatory Effects of Mahwangyounpae-tang, a Polyherbal Formula in ICR Mice (ICR 마우스를 이용한 마황윤폐탕(麻黃潤肺湯)의 진해, 거담, 및 항염 효과 평가)

  • Objectives: The antitussive, expectorant, and anti-inflammatory effects of Mahwangyounpae-tang (MHYPT) aqueous extracts were observed in appropriate animal models of various respiratory disorders. Methods: MHYPT aqueous extracts were orally administered once a day for 11 days at dose levels of 400, 200, and 100 mg/kg. The effect of MHYPT was determined by comparing its antitussive effect with theobromine (TB), its expectorant effect with ambroxol (AM), and its anti-inflammatory effect with dexamethasone (DEXA). Results: MHYPT aqueous extracts (400 mg/kg) showed favorable antitussive effects comparable to those of TB (50 mg/kg) in the NH4OH-exposure coughing mouse model and expectorant effects comparable to those of AM (250 mg/kg) in the phenol red-secretion mouse model, but MHYPT (400 mg/kg) showed less anti-inflammatory activity compared to DEXA (1 mg/kg) in the xylene-induced acute inflammatory mouse ear model under the experimental conditions used. Conclusion: MHYPT aqueous extracts administered at dosage levels of 400, 200, and 100 mg/kg induced dose-dependent and favorable antitussive, expectorant, and anti-inflammatory activities that occurred by simultaneous modulation of the activity of mast cells and respiratory mucous production under the experimental conditions used in this study.

Association of the XRCC1 c.1178G>A Genetic Polymorphism with Lung Cancer Risk in Chinese

  • The X-ray repair cross-complementing group 1 protein (XRCC1) plays important roles in the DNA base excision repair pathway which may influence the development of lung cancer. This study aimed to evaluate the potential association of the XRCC1 c.1178G>A genetic polymorphism with lung cancer risk. The created restriction site-polymerase chain reaction (CRS-PCR) and DNA sequencing methods were utilized to evaluate the XRCC1 c.1178G>A genetic polymorphism among 376 lung cancer patients and 379 controls. Associations between the genetic polymorphism and lung cancer risk were determined with an unconditional logistic regression model. Our data suggested that the distribution of allele and genotype in lung cancer patients was significantly different from that of controls. The XRCC1 c.1178G>A genetic polymorphism was associated with an increased risk of lung cancer (AA vs GG: OR=2.91, 95%CI 1.70-4.98, p<0.001; A vs G: OR=1.52, 95%CI 1.22-1.90, p<0.001). The allele A and genotype AA may contribute to risk of lung cancer. These preliminary results suggested that the XRCC1 c.1178G>A genetic polymorphism is statistically associated with lung cancer risk in the Chinese population.

Association Between C-reactive Protein and Risk of Cancer: A Meta-analysis of Prospective Cohort Studies

  • Background: Associations between elevated C-reactive protein (CRP) and cancer risk have been reported for many years, but the results from prospective cohort studies remains controversial. A meta-analysis of prospective cohort studies was therefore conducted to address this issue. Methods: Eligible studies were identified by searching the PubMed and EMBASE up to October 2012. Pooled hazard ratios (HR) was calculated by using random effects model. Results: Eleven prospective cohort studies involving a total of 194,796 participants and 11,459 cancer cases were included in this meta-analysis. The pooled HR per natural log unit change in CRP was 1.105 (95% confidence interval (CI): 1.033-1.178) for all-cancer, 1.308 (95% CI: 1.097-1.519) for lung cancer, 1.040 (95% CI: 0.910-1.170) for breast cancer, 1.063 (95% CI: 0.965-1.161) for prostate cancer, and 1.055 (95% CI: 0.925-1.184) for colorectal cancer. Dose-response analysis showed that the exponentiated linear trend for a change of one natural log unit in CRP was 1.012 (95% CI: 1.006-1.018) for all-cancer. No evidence of publication bias was observed. Conclusions: The results of this meta-analysis showed that the elevated levels of CRP are associated with an increased risk of all-cancer, lung cancer, and possibly breast, prostate and colorectal cancer. The result supports a role of chronic inflammation in carcinogenesis. Further research effort should be performed to identify whether CRP, as a marker of inflammation, has a direct role in carcinogenesis.

MRI Artifact Correction due to Unknown Respiratory Motion (미지 호흡운동에 의한 MRI 아티팩트의 수정)

  • In this study, an improved post-processing technique for correcting MRI artifact due to the unknown respiratory motion in the imaging plane is presented. Respiratory motion is modeled by a two-Dimensional linear expending-shrinking movement. Assuming that the body tissues are incompressible fluid like materials, the proton density per unit volume of the imaging object is kept constant. According to the introduced model, respiratory motion imposes phase error, non-uniform sampling and amplitude modulation distortions on the acquired MRI data. When the motion parameters are known or can be estimatead a reconstruction algorithm based on biliner superposition method was used to correct the MRI artifact. In the case of motion parameters are unknown, first, the spectrum shift method is applied to find the respiratory fluctuation function, x directional expansion coefficient and x directional expansion center. Next, y directional expansion coefficient and y directional expansion center are estimated by using the minimum energy method. Finally, the validity of this proposed method is shown to be effective by using the simulated motion images.

The Effects of Kamihaepyoijin-tang on the Respiratory Patterns and Tracheal Tissues in Allergic Asthma (가미해표이진탕이 알레르기 천식 백서의 호흡 양상과 기관조직에 미치는 영향)

  • The present study was made with the aim of finding out the Kamihaepyoijin-tang's effect on the allergic asthma using the Egg-White Implant(EWI) method model recommended by de Siqueira et al. Kamihaepyoijin-tang has widely been prescribed and used in Oriental Medicine for controlling and curing of cough, asthma, nasal obstruction. Previous studies have demonstrated that Kamihaepyoijin-tang had analgesic, anticonvulsive effects. But it is uncertain whether Kamihaepyoijin-tang could inhibit the allergic reaction of rats with experimentally induced allergic asthma using EWI method. With such aim. the respiratory patterns and eosinophil infiltration in the tracheal mucosa of the rats were mainly observed. The results were summerized as follows ; 1. The frequency of malformed respiratory patterns in Kamihaepyoijin-tang treated groups were decreased as compared with the control group. 2. The Ratio of malformed respiratory patterns in Kamihaepyoijin-tang treated groups were significantly decreased as compared with the control group(p<0.02). 3. The eosinophil infiltration in Kamihaepyoijin-tang treated groups were decreased as compared with the control group. 4. The eosinophil infiltration that inside of trachea tissue in Kamihaepyoijin-tang treated groups were significantly decreased as compared with the control group(p<0.05). Depending upon above results, it is considered that Kamihaepyoijin-tang has the inhibitory effects on the allergic asthma of rats and suggested that it could be used in relieving patients of the symptoms which are caused by allergic asthma.

Chest Radiography of Tuberculosis: Determination of Activity Using Deep Learning Algorithm

  • Background: Inactive or old, healed tuberculosis (TB) on chest radiograph (CR) is often found in high TB incidence countries, and to avoid unnecessary evaluation and medication, differentiation from active TB is important. This study develops a deep learning (DL) model to estimate activity in a single chest radiographic analysis. Methods: A total of 3,824 active TB CRs from 511 individuals and 2,277 inactive TB CRs from 558 individuals were retrospectively collected. A pretrained convolutional neural network was fine-tuned to classify active and inactive TB. The model was pretrained with 8,964 pneumonia and 8,525 normal cases from the National Institute of Health (NIH) dataset. During the pretraining phase, the DL model learns the following tasks: pneumonia vs. normal, pneumonia vs. active TB, and active TB vs. normal. The performance of the DL model was validated using three external datasets. Receiver operating characteristic analyses were performed to evaluate the diagnostic performance to determine active TB by DL model and radiologists. Sensitivities and specificities for determining active TB were evaluated for both the DL model and radiologists. Results: The performance of the DL model showed area under the curve (AUC) values of 0.980 in internal validation, and 0.815 and 0.887 in external validation. The AUC values for the DL model, thoracic radiologist, and general radiologist, evaluated using one of the external validation datasets, were 0.815, 0.871, and 0.811, respectively. Conclusion: This DL-based algorithm showed potential as an effective diagnostic tool to identify TB activity, and could be useful for the follow-up of patients with inactive TB in high TB burden countries.
