쥐치아재식 모델에서 치주인대의 처리방법에 따른 치주조직의 치유와 치근흡수에 관한 연구 (THE EFFECT OF TWO KINDS OF PERIODONTAL LIGAMENT MANAGEMENT UPON PERIODONTAL HEALING AND ROOT RESORPTION AFTER REPLANTATION IN RATS)

  • The effect of bench drying or removal of the periodontal ligament with NaOCl upon periodontal healing and root resorption after replantation of molars was studied in rats. A total of 40 Sprague-Dawley female rats were used and fed a powdered purina rat chow diet containing 0.4% beta-aminoproprionitrile. The maxillary first molars were extracted and the periodontal ligaments were removed either by bench drying for 15 minutes or by immersion in 2.5% NaOCl solution. The rats were sacrificed at 5, 10, and 21 days by heart infusion. In order to observe the effect of 0.5% stannous fluoride, $10^{-4}M$, $10^{-2}M$, and 1M etidronate disodium on the early stage of periodontal healing, the periodontal ligament was removed with 2.5% NaOCl followed by immersion of the molar in the respective solutions for 5 minutes. The rats were sacrificed after 10 days and the following results were obtained. 1. The removal of the periodontal ligament with 2.5% NaOCl seemed to be more effective than bench drying, since the resorption area in the NaOCl treated group showed a gradual increase whereas a decline in resorption area from 5 days to 21 days was observed in the bench dried group. 2. The application of 0.5% stannous fluoride seemed to enhance the periodontal ligament attachment and active migration of fibroblasts could be observed. 3. The application of $10^{-4}M$, $10^{-2}M$, and 1M etidronate disodium led to a good periodontal ligament attachment. No evident areas of root resorption were found. 4. The use of ${\beta}$-APN made it possible to extract the maxillary first molar with all five roots intact.

하악 과두골절에 관한 임상적 연구: 효과적인 체외고정법 (A CLINICAL STUDY ON FRACTURES OF THE MANDIBULAR CONDYLES: EFFECTIVE EXTRACORPOREAL FIXATION TECHNIQUE)

  • Purpose: Fractures of the mandibular condyle accounts for 15% to 30% of all the mandibular fractures and lots of complications are reported. Among these complications, condylar resorption is the most important one to be taken into account. The purpose of this study is evaluating condylar resorption pattern in relation to extracorporeal fixation technique. Patients & methods: Thirty four patients with 42 mandibular condylar fractures were retrospectively examined with an average follow-up of 18.5 months (6-66 months). Male/female ratio is 27 males (7 bilateral cases) and 7 females (1 bilateral case). Applicated surgical techniques were extracorporeal fixation via submandibular approach and extracorporeal fixation via intraoral approach. We compared radiographic views of post-operative periods with that of most long term follow up in 34 operated sites respectively. Result: There were 2 generalized condylar resorption sites and 3 partial resorption sites with clinical implications. Others (29 joints) showed good healing or satisfactory adaptive state of temporomandibular joints clinically. Radiologically, signs of partial condylar resorption and/or osteoarthrosis were diagnosed in 11 patients. But, 8 out of the 11 patients didn't show significant clinical symptoms, which means successful adaptive remodeling of the surgically located condylar head. Conclusion: From the above results, intentional ramal osteotomy, temporary detachment of the fractured condyle and extracorporeal fixation technique seems to be effective and useful in those cases of condylar fractures in which reduction of the dislocated condyle is very difficult or virtually impossible.

Elevated extracellular calcium ions promote proliferation and migration of mesenchymal stem cells via increasing osteopontin expression

  • Supplementation of mesenchymal stem cells (MSCs) at sites of bone resorption is required for bone homeostasis because of the non-proliferation and short lifespan properties of the osteoblasts. Calcium ions ($Ca^{2+}$) are released from the bone surfaces during osteoclast-mediated bone resorption. However, how elevated extracellular $Ca^{2+}$ concentrations would alter MSCs behavior in the proximal sites of bone resorption is largely unknown. In this study, we investigated the effect of extracellular $Ca^{2+}$ on MSCs phenotype depending on $Ca^{2+}$ concentrations. We found that the elevated extracellular $Ca^{2+}$ promoted cell proliferation and matrix mineralization of MSCs. In addition, MSCs induced the expression and secretion of osteopontin (OPN), which enhanced MSCs migration under the elevated extracellular $Ca^{2+}$ conditions. We developed in vitro osteoclast-mediated bone resorption conditions using mouse calvaria bone slices and demonstrated $Ca^{2+}$ is released from bone resorption surfaces. We also showed that the MSCs phenotype, including cell proliferation and migration, changed when the cells were treated with a bone resorption-conditioned medium. These findings suggest that the dynamic changes in $Ca^{2+}$ concentrations in the microenvironments of bone remodeling surfaces modulate MSCs phenotype and thereby contribute to bone regeneration.

녹제초 추출물이 파골세포 분화 및 골 흡수에 미치는 영향 (Effects of Pyrola japonica Extracts on Osteoclast Differentiation and Bone Resorption)

  • Objectives This study was performed to evaluate the effect of Pyrola japonica extract (NJ) and its principal constituent, homoarbutin (HA) on osteoclast differentiation and gene expression and bone resorption. The osteoclastogenesis and gene expression were determined in receptor activator of nuclear factor kappa B ligand (RANKL)-stimulated RAW264.7 cell. Methods In order to evaluate the effect of HA extracted from NJ on bone resorption, osteoclasts were used to be differentiated and formed by stimulating RAW264.7 cells with RANKL. Tartarate-resistant acid phosphatase (TRAP) (+) polynuclear osteoclast formation ability was evaluated, and differentiation control genes including cathepsin K, matrix metalloproteinases-9 (MMP-9), and TRAP in osteoclast differentiation were analyzed by real-time polymerase chain reaction (PCR). Immunoblotting was performed to measure the effect of mitogen-activated protein kinase (MAPK) factors on bone resorption, and the effect of osteoclasts on osteoclast differentiation was measured. Results Both NJ and high concentration of HA blocked RANKL-stimulated differentiation from RAW264.7 cell to TRAP-positive multinucleated cells. NJ reduced RANKL-induced expression of TRAP, cathepsin K. Both NJ and high concentration of HA inhibited RANKL-mediated expression of MMP-9, nuclear factor of activated T-cells, cytoplasmic 1, and cellular Jun-fos. NJ suppressed RANKL-stimulated expression of cyclooxygenase-2 (COX-2), inducible nitric oxide synthase, tumor necrosis factor-alpha, and levels of interleukins. Both NJ and HA decreased bone resorption in osteoclast-induced bone pit formation model. Conclusions These results suggest that NJ and HA blocked bone resorption by decreasing RANKL-mediated osteoclastogenesis through down-regulation of genes for osteoclast differentiation.

계혈등 에탄올 추출물의 RANKL 처리 RAW264.7 세포의 분화와 염증성 골 손실에 미치는 영향 (Effect of Spatholobus Suberectus Extract (SSE) on RANKL-treated RAW264.7 and LPS-induced Bone Loss)

  • Purpose: We evaluated whether Spatholobus suberectus extract (SSE) can be used as a means of preventing and treating osteoporosis by measuring its effect on osteoclast differentiation, gene expression, and bone resorption. Methods: SSE was used to examine the effect on RAW 264.7 cells stimulated with RANKL to induce bone resorption. The inhibitory effect of TRAP formation and the expression of the bone resorption factors TRAP, cathepsin K, and MMP-9 during differentiation were measured. The effects on the differentiation-related factors NFATc and TRAIL and on the expression of OC-STAMP, DC-STAMP, ATP6v0d2, MITF, c-Fos, and inflammation-related factors were also evaluated. The effect on bone resorption was evaluated by culturing RANKL-treated osteoclasts on artificial bone fragments and observing the resulting resorption traces. The effect on bone damage in experimental animals was also measured. Results: SSE inhibited the differentiation of RANKL-stimulated osteoclasts into osteoclasts and suppressed the expression of cathepsin K, TRAP, MMP-9, NFATc1, TRAIL, MITF, OC-STAMP, DC-STAMP, ATP6v0d2, and c-Fos genes. Bone pore formation due to osteoclast action was also inhibited, and LPS-induced bone loss was suppressed in animal experiments. Conclusions: SSE could be useful for the prevention or treatment of osteoporosis by inhibiting osteoclast differentiation and bone resorption and suppressing bone loss induced in experimental animals. However, studies of larger populations are required.

치아이동시 치근 흡수에 따른 치근항체의 역가 변화 (Changes in the Titer of Tooth Root Antibodies Accompanying Root Resorption Associated with Orthodontic Tooth Movement)

  • 치아이동시 발생되는 치근 흡수에 관련된 특이면역반응의 역할을 연구하기 위해 치근 흡수에 따른 치근항체의 역가 변화를 관찰하였다. 생후 2년된 성견 다섯마리를 실험동물로 사용하였다. closed coil spring을 사용하여 각 실험동물의 상악 6전치에 200-250gm의 함입력을 가하였으며, 하악 6전치를 발거하여 동종항원으로 사용하였다. 발거된 하악 전치의 상아질 부위를 분리하여 6M Guanidine-HCl-10% EDTA(pH 5.0)에서 해리시킨 다음 부유물 만을 투석하여 항원을 준비하였다. 치아를 함입시키기 전 1회 혈청을 채취하고 함입시키면서 매주 간격으로 11회 형철을 채취하였다. 실험 9주째 치근 흡수가 일어나고 있는 상악 6전치를 모두 발거하여 항원의 근원을 제거하였다. 혈청 내 치근항체의 역가는 ELISA(enzyme-linked immunosorbent assay)로 측정하였다. 항체 특이성에 대한 대조군으로 혈청과 치근 항원 그리고 혈청과 관련없는 박테리아 항원(Porphyro monas gingivalis 33277)을 섭씨 25도에서 3시간 반응시켜 모든 과정에서 항원-항체 반응을 측정하였다. 치근 흡수를 관찰하기 위하여 실험 전과 실험 후 한 달 간격으로 방사선 교합사진을 촬영하였고 9주째 발거한 치아의 치근단 부위를 입체현미경으로 관찰하였다. 치근항원에 대한 자가항원의 역가 변화를 측정하여 다음과 같은 결과를 얻었다. 자가항체의 역가는 치아가 함입되면서 즉시 감소하였다가 1주후 다시 증가하였으며 4주째 최고수준에 도달한 다음 다시 지속적으로 감소하였다. 흡수 중인 치근을 발거한 직후 급격히 증가하는 양상을 보였다. 치근 항원과 반응시킨 혈청내에서는 항원-항체 반응이 거의 나타나지 않았으며 관련없는 박테리아 항원과 반응을 시킨 혈청에서는 활성도가 동종 치근 항원에 대한 자가항체의 활성도와 비슷하게 나타났다. 방사선 교합사진 상에서는 육안으로 구별할 수 있는 차이가 거의 없었으나 입체현미경하에서는 치근단 부위의 다양한 흡수 양상을 관찰할 수 있었다.

Original Article 1 - 즉시 혹은 조기하중 임프란트에 관한 후향적 연구 (Retrospective Clinical Study of Immediate or Early Loading of Implants)

  • Introduction. In this study, we applied the immediate loading to implant using temporary prosthesis, and compared the treatment results by each placement site, amongst pre-loading period. The study was intended to search for occurrence of complications and the amount of marginal bone resorption. Materials and Methods. We retrospectively reviewed the medical records of 99 cases of implants from 29 patients who were treated at Seoul National University Bundang Hospital from September 2003 to September 2008. We grouped all the cases by placement arches and pre-loading periods, and subdivided each group by placement site(anterior/posterior), type of prosthesis(full-mouth cross splinting/Partial multiple splinting, Single). Results. In the cases of anterior maxilla, the average amounts of marginal bone resorption showed $1.71{\pm}0.71mm$ in group 1, and $1.44{\pm}0.69mm$ in group 2, which means they revealed no statistically significant difference between group 1 and group 2(p=0.646). In the cases of posterior maxilla, the average amounts of marginal bone resorption showed $1.25{\pm}0.72mm$ in group 1, and $1.14{\pm}1.15mm$ in group 2, which means they were not statistically significant. In the cases of anterior mandible, there was no cases classified as group 4, but the average amounts of marginal bone resorption in group 3 showed $1.38{\pm}0.79mm$. In the cases of posterior mandible, Group 3 showed $1.39{\pm}0.64mm$, and group 4 showed $1.84{\pm}1.19mm$ as amounts of marginal bone resorption, which means they revealed no statistically significant difference between group 3 and group 4(P=0.210). The survival rate of group1 was 97.14%, 92.1% of group3, and 100% of group 2 and group4. According to each type of prosthesis, each average amount of marginal bone resorption revealed no statistically significant difference in maxi11a(p=0.575) in mandible(p=0.206). Conclusion. It is concluded that the marginal bone resorption and the rate of complications might not be affected by placement sites and pre-loading periods. The marginal bone resorption and the rate of complications might vary as different bone quality of placement site and implant system, diameter, length, etc. It is suggested that the proper placement of immediate loading implants decreases the whole treatment period and any inconvenience occurred to patients.

즉시 하중 임플란트에서 변연골 흡수량에 관한 후향적 연구 (Retrospective study on marginal bone resorption around immediately loaded implants)

  • 목적: 원광대학교 치과대학병원 임플란트 센터에서 임플란트를 식립한 후 1주일 내에 임시보철물을 수복한 다음, 즉시 하중을 가한 환자들을 대상으로 골 흡수량을 측정하고자 하였다. 이러한 후향적 분석을 통하여 임플란트에 대한 즉시 하중의 임상적 근거를 보강하고 주의해야 할 사항 등을 파악하고자 하였다. 대상 및 방법: 2005년 1월 1일부터 2016년 6월 31일 사이에 원광대학교 치과대학병원 임플란트 센터에서 임플란트를 식립한 후 1주일 내에 임시보철물을 수복한 환자들을 대상으로 하여 진료기록부와 방사선 사진을 조사하였다. 총 임플란트의 수는 70개였으며, 임플란트의 제조회사, 길이, 직경, 체결 방식에 따른 변연골의 흡수량을 측정하였다. 변연골의 흡수량을 측정하기 위해 임플란트 식립 직후와 6개월 이후의 치근단 방사선 사진을 비교 분석하였다. 통계 프로그램인 SPSS 22.0을 이용하여 Mann-whitney U test와 Kruskal-wallis test를 시행하였다. 결과: 즉시 하중 임플란트 군에서 임플란트와 지대주의 연결 구조에 따른 변연골 흡수량을 비교한 결과 내부 육각 연결 구조 임플란트에서의 변연골 흡수량은 평균 $1.24{\pm}0.72mm$, 외부 육각 연결 구조 임플란트에서 $1.73{\pm}1.27mm$로 나타났으며 통계적으로 유의한 차이를 보였다. 임플란트 제조회사, 길이, 직경에 따른 변연골 흡수량의 차이는 통계적으로 유의한 차이를 보이지 않았다. 결론: 즉시 하중 임플란트에서 내부 육각 연결 구조 임플란트에서의 변연골 흡수량은 외부 육각 연결 구조 임플란트에서의 변연골 흡수량보다 적게 나타났으며, 임플란트의 다른 조건들에서는 변연골 흡수량에 차이를 보이지 않았다.

탈구치의 alendronate 도포가 재식 후 염증반응에 미치는 효과 (EFFECT OF TOPICAL ALENDRONATE APPLICATION ON INFLAMMATION OF REPLANTED RAT MOLAR)

  • 본 연구는 탈구치에 대한 alendronate(ALN)도포가 지연재식모델에 있어 염증반응의 억제에 미치는 효과를 관찰하고자 태생 6주된 54마리 SD Rat의 상악 제 1 대구치를 재식하였다. 실험군은 15분 자연건조 후에 즉시 재식군(I군), 60분 자연건조 후 HBSS 도포군(II군) 그리고 60분 자연건조 후 ALN 도포군(III군)으로 분류하였으며 각 군 간의 치근의 염증성 흡수상태, 치근의 대치성 흡수정도, 염증의 정도, 그리고 치주인대의 치유정도를 관찰하여 다음과 같은 결론을 얻었다. 1. I군은 II, III군에 비하여 염증의 정도나 치근의 염증성 흡수정도 등 전반적인 염증반응이 적었고, 치주조직의 재부착 및 재생의 정토가 가장 높았다. 2. II군에서는 전반적으로 염증이 가장 심하였으며, 치근흡수 또한 광범위하였다. 3. III군은 II군에 비하여 치근면에 국소적인 염증반응이 나타났으며 치근의 염증성 흡수 또한 국소적으로 발생하였다(p<0.001). I군에 비하여 치주조직의 염증반응은 증가하였으나(p<0.05) 염증성흡수와 대치성흡수에 있어서는 유의성이 없었다. 4. 각 군에서 대치성흡수의 양상은 크게 차이가 나지 않았으며, 시간의 경과에 따라 약간의 증가가 있었으나 유의성은 없었다. 이상의 결과로부터 탈구치가 구강 외에서 장시간 건조되었을 경우 ALN의 사용은 치근흡수를 억제하고 치주조직의 재생에 유용하다는 것을 알 수 있었다. ALN은 치아의 탈구시에 사용되는 여러 약재들의 대체품으로 사용할 수 있으며, 향후 추가적인 연구를 통하여 탈구치의 재식시 ALN 사용을 위한 적절한 용량과 도포용제재의 개발을 통하여 치아의 재식시 쉽게 사용할 수 있도록 연구 개발이 필요할 것으로 생각된다.

요추 추간판 탈출증 환자의 한의치료 후 디스크 흡수율 비교 및 흡수에 영향을 주는 요인 분석 연구 (Comparative Analysis on Disc Resorption Rate of Lumbar Disc Herniation Patients after Korean Medicine Treatment and Predictive Factors Associated with Disc Resorption)

  • Objectives The purpose of this study is to analyze the effects of Korean Medicine treatment on lumbar disc herniation (LDH) and predictive factors associated with disc resorption by magnetic resonance imaging (MRI). Methods A retrospective study was carried out in 79 lumbar disc herniation patients who had visited Haeundae Jaseng Hospital of Korean Medicine. Patients' diagnosis was based on MRI. MRI was performed on two or more occasions and patients were received Korean Medicine treatment within the period. The volume of each herniated disc was measured three-dimensionally and patient characteristics, interval between MRIs, herniated disc level, disc herniation type, disc migration, intactness of posterior longitudinal ligament (PLL), initial volume of herniated disc, modic change, disc resorption rate were statistically analyzed. Results The mean volumes of herniated discs before Korean Medicine treatment and after Korean Medicine treatment were $1,547.81{\pm}598.15mm^3$ and $947.06{\pm}335.28mm^3$, respectively. The mean resorption rate was $35.7{\pm}16.3%$. Disc extent, intactness of PLL and initial volume of herniated discs were significantly correlated with resorption rate (p=0.003, p=0.001 and p=0.024, respectively). Conclusions Korean Medicine treatment is an effective conservative treatment for lumbar disc herniation. Factors such as disc migration, intactness of PLL, initial volume of herniated disc have a significant association with disc resorption rate.
