A Case Study on the Human Error Analysis of Forklift Operations in a Small Enterprise (소규모 사업장의 지게차 작업에 관한 휴먼에러 분석 사례 연구)

  • A forklift is an industrial vehicle with a power-operated fork for lifting and moving heavy loads over short distances. A significant number of accidents are caused by forklifts every year. Most of them are known to be caused by the unsafe acts of workers. However, only a few studies have focused on the risks of forklift work from the perspective of human error. In addition, various methods have been developed to analyze the risk of human error, while it is hard to find studies that directly compare the effectiveness or strengths/weaknesses of those methods. This study aims to analyze risk factors related to unsafe behavior in forklift operations using two representative human error analysis techniques, i.e., .SHERPA and HE-HAZOP, and compare their advantages and disadvantages. The analysis was performed on three main forklift operations ('unloading from the truck', 'moving and loading into the storage', and 'loading on the truck'). As a result, 118 errors and 34 remedial measures were derived by SHERPA. Through HAZOP, 139 errors and 54 measures were derived. The two techniques were compared in terms of the number of results and the method of deriving errors and remedial measures, cause analysis, and risk assessment. This study might be used to reduce human error related disasters in workplaces using forklifts. In order to provide a guide for choosing an appropriate analysis method, more comparative studies on different techniques involving wide range of tasks are needed in the future.

  • This paper aims to provide pragmatic application guidelines of the interior finish materials for apartment houses with newly-established evaluation methodology of the sustainable degree of interior products. With reference to the standards and criteria of domestic eco-labeling accreditation schemes for sustainable products in the area of architecture which focus on the sustainable elements classified as the health, recyclability, durability, and energy efficiency, in this study, a systematic evaluation method has been established for interior finish products with quantifiable indicators for sustainable performance. Base on the evaluation system introduced here, most interior finish products can be classified into a database and applied effectively to the realities from the perspective of the sustainability. There are the necessities of enforcement issues with the idea of revising or taking remedial measures of the current performance criteria of domestic eco-labeling accreditation to bolster their reliability. As well as already-commercialized products, hereafter, continued efforts are needed to control the whole process of manufacturing new interior finish products from their designing, constructing, consuming, recycling and to dismantling in terms of sustainability, which promises more pragmatic follow-up measures for the detail embodiment of the environment-friendly spaces.

  • As rapid industrialization continues in these days, construction in the down town areas increases. Since constructions are performed around old and existing structures, the need to provide reinforcements to protect the existing structures from collapse and damage arises. Furthermore, if the construction is to take place in the down town area, difficult work space and damage caused by noise, vibration and collapse of structure can't be ignored. Among the remedial measures available today, micropile reinforcement is considered the best method to remedy these problems. But up to the present the characteristics of micropiles and ground behaviour has not been proven and no standard design is not yet available. Therefore, most design are performed based on previous experiences. In this study, the difference in the bearing capacity with changing reinforcement angle, space and sphere around foundation was monitored. These results were induced to broaden heighten the limits of micropile application.

  • Major cause of defects in steam generator tubes at nuclear power plant is the accumulation of magnetite and other byproducts of corrosion in the crevice gap between support plates and tubes. Since damaged tubes result in contamination of the secondary coolant by the radioactive primary coolant, they represent a safety hazard. Early detection of magnetite buildup is, therefore, imperative in order to take remedial measures such as chemical flushing. Although the eddy current testing is being used for the inspection of steam generator tubes, the interpretation of resulting signals is generally a difficult task. This paper uses the phase of sensor coil emf as the test signal to find a way of easier signal interpretation. Numerical study using FEM shows that the shape of resulting signal is good for identifying the relative position of the probe to the support plate, and for discreminating the different shapes and degrees of magnetite buildup in the crevice gap region.

  • Advancements in materialistic life styles and increasing awareness about adverse climatic changes and its negative effects on human life have been the driving force of finding new and clean sources of energy. Wind power has become technologically mature and commercially acceptable on global scale. However, fossil fuels have been the major sources of energy in most countries, renewable energy (particularly wind) is now booming worldwide. To cope with this wind energy technology, various related aspects have to be understood by the scientific, engineering, utility, and contracting communities. This study is an effort towards the understanding of the (i) wind turbine blade and tower structural stability issues, (ii) turbine blade and tower failures and remedial measures, (iii) weather and seismic effects on turbine blade and tower failures, (iv) gear box failures, and (v) turbine blade and tower failure analysis tools.

  • 차세대식품으로 주목받고 있는 스피룰리나의 항균성 유산균(Probiotics: Lactobacillus Pentosus) 발효를 통해 얻어지는 항균성 발효스피룰리나의 국민건강증진 방편으로 이를 이용, 소비자 및 수요자의 접근을 용의하기위하며, 주 소비층인 환자, 노약자, 임산부, 어린이층의 Target content의 개발과 발효스피룰리나에 대한 시험결과 즉, 2010년 3월 락토바실러스 펜토수스 K34를 이용해 발효한 결과 발효공정을 통해 락트산; 아세트산; 구연산; 숙신산등의 유기산이 적어도 3종 이상 생성되었으며, 단백질 함량은 10% 이하로 변화를 보이지 않았고, 탄수화물의 거의 분해되어 락트산 및 프로피온산 등의 유기산으로 변화됨에 따라, 개발된 새로운 식품원료의 다양한 식품개발은 건강증진을 위해 소비수요층의 용의한 접근의 한 방편으로, 만화나 일러스트레이션을 이용한 접근을 통한 홍보 전략이 필요하다.

  • Expert systems represent the application of artificial intelligence research that is at least 10-15 years old. It may well take another 5-10 years before the present technology is exploited by industry. The objective of this paper is to show the applicability of Combined Hypertext-Expert System Techniques to the professionals working in the construction industry. Expert systems alone don't give the user any control over the system. Hypertext systems that focus only on displaying messages have the opposite problem. While they allow the user to control the system., the only control the system designer has is in setting up the hypertext links. Therefore the combination of these two techniques, offered by KnowledgePro, may bring us closer to real user-expert communication. The system developed in this work offers information on ACI Manual concerning various problem areas in concrete practice, consultation about mix design of concrete and guidance to identify types of cracks in concrete. Once the type of crack is identified through the rule-based knowledge, the system provides the user with remedial measures through the control of the user.

  • Braced excavation for a new building was carried out at a very close proximity of an existing tall building of T.hick columns are supported by indict.ideal spread footings on sand layer The excavation was planned to reach far below the footing level of the existing building. To assess the foundation performance and stability of the existing building, the behavior of 9round subjected to loss of confinement from excavation was analytically studied using finite element method. Field instrumentation was also conducted to monitor the actual ground responses during excavation. Based on these studies, various remedial measures weere taken to minimize the adverse effects to the building, and excavation was successfully completed. This paper presents the results from the analytical studies and field monitoring, and measured and measured responses at different stages of excavation.

Effect of Inadequate Design on Cost and Time Overrun of Road Construction Projects in Tanzania

  • Completing road construction projects within the budget and time has been a problem for the Tanzania National Roads Agency (TANROADS); and the major problem highlighted in almost all projects being inadequate design. However, the extent to which inadequate design contributes to both time and cost overruns and its causes remained under-studied. The objective of this study is therefore to determine the extent of the effect of inadequate design, its causes and remedial measures. The methodology used in this study includes reviewing recent projects completion reports, holding roundtable discussions with consultants and TANROADS officials and analyzing the information. Cost and time overruns for the reviewed projects averaged 44% and 26% respectively. Similarly, the extents to which inadequate design contributes to cost and time overruns were on average found to be 61% and 85% respectively. The overruns are predominantly related to problems that occurred during the design process. To alleviate the problem, TANROADS is advised to improve the management of design projects, enhance the process of reviewing design reports, improve the design process including introducing Road Safety Audit and geometric design manuals, and increase staff to match the work-load.

  • The objective of this study is to suggest the process and method of local accident investigation for local authorities with a view to efficient and effective managing traffic accidents. With a project city selected accident-type maps, accident lists, accident diagrams, priority of black-spots/-lengths, site visits, remedial measures, opportunity costs, monitoring etc. are taken into consideration, by using accident data in the last 3 years. Analyzed are accident savings to be expected when applying technical, organizational, and administrative processes attached to local accident investigation.
