Dual Cytotoxic Responses Induced by Treatment of A549 Human Lung Cancer Cells with Sweet Bee Venom in a Dose-Dependent Manner

  • Objectives: Sweet bee venom (sBV) is purified from Apis mellifera, containing a high level of melittin-its main component. It has been used as a therapeutic agent for pain relief and anti-inflammation, as well as for treating neuronal abnormalities. Recently, there have been studies on the therapeutic application of sBV for anticancer treatment. In the present study, we investigated the pharmacological effect of sBV treatment in A549 human lung cancer cells. Methods: We used microscopic analysis to observe the morphological changes in A549 cells after sBV treatment. The MTT assay was used to examine the cytotoxic effect after dose-dependent sBV treatment. Molecular changes in sBV were evaluated by the expression of apoptosis marker proteins using western blot analysis. Results: Microscopic analysis suggested that the growth inhibitory effect occurred in a dose-dependent manner; however, cell lysis occurred at a concentration over 20 ㎍/mL of sBV. The MTT assay indicated that sBV treatment exhibited a growth inhibitory effect at a concentration over 5 ㎍/mL. On fluorescence activated cell sorting analysis, G0 dead cells were observed after G1 arrest at treatment concentrations up to 10 ㎍/mL. However, rapid cell rupture was observed at a concentration of 20 ㎍/mL. Western blot analysis demonstrated that sBV treatment modulated the expression of multiple cell death-related proteins, including cleaved-PARP, cleaved-caspase 9, p53, Bcl2, and Bax. Conclusion: sBV induced cell death in A549 human lung cancer cells at a pharmacological concentration, albeit causing hemolytic cell death at a high concentration.

Repetitive transcranial magnetic stimulation in central post-stroke pain: current status and future perspective

  • Central post-stroke pain (CPSP) is an incapacitating disorder that impacts a substantial proportion of stroke survivors and can diminish their quality of life. Conventional therapies for CPSP, including tricyclic antidepressants, anticonvulsants, and opioids, are frequently ineffective, necessitating the investigation of alternative therapeutic strategies. Repetitive transcranial magnetic stimulation (rTMS) is now recognized as a promising noninvasive pain management method for CPSP. rTMS modulates neural activity through the administration of magnetic pulses to specific cortical regions. Trials analyzing the effects of rTMS on CPSP have generated various outcomes, but the evidence suggests possible analgesic benefits. In CPSP and other neuropathic pain conditions, high-frequency rTMS targeting the primary motor cortex (M1) with figure-eight coils has demonstrated significant pain alleviation. Due to its associaton with analgesic benefits, M1 is the most frequently targeted area. The duration and frequency of rTMS sessions, as well as the stimulation intensity, have been studied in an effort to optimize treatment outcomes. The short-term pain relief effects of rTMS have been observed, but the long-term effects (> 3 months) require further investigation. Aspects such as stimulation frequency, location, and treatment period can influence the efficacy of rTMS and ought to be considered while planning the procedure. Standardized guidelines for using rTMS in CPSP would optimize therapy protocols and improve patient outcomes. This review article provides an up-to-date overview of the incidence, clinical characteristics, outcome of rTMS in CPSP patients, and future perspective in the field.

Long-Term Wildfire Reconstruction: In Need of Focused and Dedicated Pre-Planning Efforts

  • Wildfire disasters in the United States impact lives and livelihoods by destroying private homes, businesses, community facilities, and infrastructure. Disaster victims suffer from damaged houses, inadequate shelters, inoperable civil infrastructure, and homelessness coupled with long-term recovery and reconstruction processes. Cities and their neighboring communities require an enormous commitment for a full recovery for as long as disaster recovery processes last. State, county, and municipal governments inherently have the responsibility to establish and provide governance and public services for the benefit and well being of community members. Municipal governments' comprehensive and emergency response plans are the artifacts of planning efforts that guide accomplishing those duties. Typically these plans include preparation and response to natural disasters, including wildfires. The standard wildfire planning includes and outlines (1) a wildfire hazard assessment, (2) response approaches to prevent human injury and minimize damage to physical property, and (3) near- and long-term recovery and reconstruction efforts. There is often a high level of detail in the assessment section, but the level of detail and specificity significantly lessons to general approaches in the long-term recovery subsection. This paper aims to document the extent of wildfire preparedness at the county level in general, focusing on the long-term recovery subsections of municipal plans. Based on the identified challenges, the researchers provide recommendations for better longer-term recovery and reconstruction opportunities: 1) building permit requirements, 2) exploration of the use of modular construction, 3) address through relief from legislative requirements, and 4) early, simple, funding, and the aid application process.

Pain management in periodontal therapy using local anesthetics and other drugs: an integrative review

  • Background: Surgical and non-surgical periodontal procedures often lead to postoperative pain. Clinicians use pharmacological methods such as anesthetics, anti-inflammatory drugs, and analgesics for relief. However, the multitude of options makes it challenging to select the best approach for routine dental care. Objective: This review aimed to describe previous studies regarding the pharmacological management used for pain control during periodontal procedures as well as factors that may interfere with patients' perception of pain. Methods: We included studies (period of 2000-2023, whose approach corresponded to the pharmacological protocols used for preoperative, trans-operative, and postoperative pain control in adult patients undergoing surgical and non-surgical periodontal therapy. Results: A total of 32 studies were included in the analysis, of which 17 (53%) were related to anesthetic methods and 15 (47%) were related to therapeutic protocols (anti-inflammatory/analgesic agents). These studies predominantly involved nonsurgical periodontal procedures. Studies have reported that factors related to age, type of procedure, and anxiety can influence pain perception; however, only seven of these studies evaluated anxiety. Conclusions: Numerous methods for pain control can be applied in periodontal therapy, which are accomplished through anesthetic methods and/or therapeutic protocols. Factors such as anxiety, age, and type of procedure are related to pain perception in patients. Thus, it is the responsibility of dentists to evaluate each clinical situation and define the best protocol to follow based on the literature.

Surgical Treatment for Primary Lymphedema: A Systematic Review of the Literature

  • This is a retrospective review of surgical management for primary lymphedema. Data were extracted from 55 articles from PubMed MEDLINE, Web of Science, SCOPUS, and Cochrane Central Register of Controlled Trials between the database inception and December 2022 to evaluate the outcomes of lymphaticovenous anastomosis (LVA) and vascularized lymph node transfer (VLNT), and outcomes of soft tissue extirpative procedures such as suction-assisted lipectomy (SAL) and extensive soft tissue excision. Data from 485 patients were compiled; these were treated with LVA (n = 177), VLNT (n = 82), SAL (n = 102), and excisional procedures (n = 124). Improvement of the lower extremity lymphedema index, the quality of life (QoL), and lymphedema symptoms were reported in most studies. LVA and VLNT led to symptomatic relief and improved QoL, reaching up to 90 and 61% average circumference reduction, respectively. Cellulitis reduction was reported in 25 and 40% of LVA and VLNT papers, respectively. The extirpative procedures, used mainly in patients with advanced disease, also led to clinical improvement from the volume reduction, as well as reduced incidence of cellulitis, although with poor cosmetic results; 87.5% of these reports recommended postoperative compression garments. The overall complication rates were 1% for LVA, 13% for VLNT, 11% for SAL, and 46% for extirpative procedures. Altogether, only one paper lacked some kind of improvement. Primary lymphedema is amenable to surgical treatment; the currently performed procedures have effectively improved symptoms and QoL in this population. Complication rates are related to the invasiveness of the chosen procedure.

학교급식효과(學校給食效果)와 제도개선(制度改善)에 관(關)한 연구(硏究) (A Study on Improvement of School Lunch Programs and Its Health and Education Effect in Korea)

  • The purpose of school lunch service programs (SLPs) is to provide school children with nutritious meals each day, to assist them in improving their mental and physical development, to make them understand the basic knowledge needed for healthy daily living throughout their lives with sound nutritional habits, and further, to enhance the national health status as well as the individual. The major purpose of this study is to describe the history and current status and promotional plans in Korea's SLSPs to improve the nation's health status. The major findings of this study are as follow: 1) The progression of the SLSPs can be devided into four major periods: Relief (1953-1972), Self-supportive (1973-1977), System-establishment (1973-1983), System-development(1984- ). 2) As result of the implementation of SLSPs in Korea, many positive effects have been found in school education, notably in the field of physical growth, social development and personality cultivation. 3) This study deals with a more detailed assessment with regard to the effects of the program on school education activities and the improvement of nutritional habits in the family. Although it is limited and not yet complete, the program implementation has contributed to the improvement in children's food-intake habits, understanding of food, and manners, and increased sanitary consciousness. It has also affected of parent's concerns and attitudes towards the improvement of daily meals as well as national eating patterns, and eventually provided motivation for increased farm production of nutritious food stuffs and reasonable food consumption. 4) The SLSPs does have its stare of problem such as an unsatisfactory budget, lack of understanding of the program by school personnel and parents. However, there is no doubt that the SLSPs should be carried out effectively for better health, and especially for the growth and development of the children. Finally, the problems and promotive Plans of the SLSPs can be approached by aspects of the administrative, financial and operational systems. These system have to coordinate with relevant authorities to expand participation in the program for the successful implementation of the SLSPs. The eventual objective this study is to propose ways and means for mobilizing every available element in the effort to attain the goals of educating and training healthy and competent personnel manpower, stabilizing the demand and supply of food and the ensuring adequate coordination of national food production and consumption.

  • The international sale contract is the central contracts in export-import transactions. A good sale contract or set of general conditions of sale will cover all the principal elements of the transaction, so that uncertainties are avoided. The parties' respective duties as concern the payment mechanism, transport contract and insurance responsibilities, inter alia, will all be clearly detailed in the contract. Under the American commercial code (UCC) the standard for this relief is one of commercial impracticability. In contrast, many civil law jurisdictions apply the term force majeure to this problem. Under CISG, the standard is based on the concept of impediments to performance. Because of the differences between these standards, parties might be well advised to draft their own force majeure, hardship, or excusable delays clause. The ICC publication, "Force Majeure and Hardship" provides a sample force majeure clause which can be incorporated by reference, as well as a hardship clause which must be expressly integrated in the contract. In addition, the ICC Model provides a similar, somewhat more concise formulation of a force majeure clause. When the seller wishes to devise his own excusable delays clause, he will seek to anticipate in its provision such potential difficulties as those related to obtaining government authorisations, changes in customs duties or regulations, drastic fluctuations in labour, materials, energy, or transportation prices, etc.

방사선치료 조사영역 내에 발생한 설암 환자에서 입체조형방사선치료 경험 : 증례보고 (Conformal Radiotherapy in a Patient with Cancer at the base of the Tongue in a Previously Irradiated Area)

  • Objectives: We report an interim result of conformal radiotherapy in a patient with early stage cancer at the base of the tongue, which developed in a previously irradiated area. Materials and Methods: A 64-year-old male patient was diagnosed with T4N0M0 supraglottic cancer. He received 72Gy of radiation therapy from 21 November 1988 to 24 February 1989. He had local failure and underwent a salvage total laryngectomy on 28 August 1989. Subsequently, he did well. In early 1999, he suffered from throat pain. He had a 2.5cm ulcerative mass at the base of his tongue, in the area that had been irradiated previously. Biopsy showed squamous cell carcinoma. After workup, he was diagnosed with base of tongue cancer with T2N0M0. Surgery was not feasible because the morbidity was not acceptable. Since it was difficult to re-irradiate the area with a curable dose using conventional 2D radiation therapy with an acceptable morbidity, we decided to try conformal radiotherapy. We used 7 static beam ports with field sizes from $7x6.4\;to\;8x8cm^2$, using 6 and 10MV photons. The fractionation regimen was 1.8Gy, 5 times per week. He received 64.8Gy in 36 fractions from 9 April 1999 to 1 June 1999. Results: In the 21 months since radiotherapy, the patient has not experienced any acute or chronic complications, such as xerostomia. He experienced relief of pain shortly after the start of radiotherapy, showed a complete response, and is still doing well. Conclusion: Conformal radiotherapy can be used to treat cancer that develops within a previously irradiated field, with curative intent.

자기공명영상을 이용한 찜질 자극에 의한 통증 완화 효과에 대한 혈류 영상학적 고찰 (The Study of Vascular Dynamics for the Effect of a Compress Pack on Pain Relief using Magnetic Resonance Angiography)

  • 본 연구는 찜질 자극이 국소 통증을 완화시킬 뿐만 아니라 편두통과 같은 만성 질환의 통증 완화에 효과적으로 애용되고 있는 이유에 대한 근거를 혈류 역학적인 측면에서 고찰하고자 한다. 이를 위해 찜질 자극에 의한 자극 부위에서의 국소 혈류 변화가 일어나는 것을 관측하고, 이를 통해 두개 내 외부의 혈액 순환에 어떠한 변화를 야기하는지를 정상인 피험자 (n=8, 평균나이: $32.13{\pm}4.61$) 를 대상으로 자기공명영상 시스템을 이용하여 관측하였다. 국소 혈류변화는 찜질 자극에 의한 피부 색깔 및 온도 변화를 통하여 관측하고자 하였고, 두개 내 외부 혈관의 자극에 따른 변화를 자기공명혈관조영술을 통해 관측하고 영상을 분석하여 비교하고자 하였다. 찜질 자극 전 $36.4^{\circ}C$에서 자극 15분 후 $36.7^{\circ}C$, 그리고 자극 30분 후 $37.1^{\circ}C$ 로 피부온도가 상승한 것으로 나타났으며, 찜질 자극이 시작되는 시점과 자극 30분 후의 혈관 영상 분석 결과를 살펴보면 두개 내부 혈관에서는 유의한 변화를 보이지 않은 반면, 두개 외부 혈관에서는 찜질 자극 후 상당한 혈류 신호의 변화를 보여주었다(+38.8%). 결과에서 보듯이 찜질 자극 부위의 이러한 국소 온도의 변화와 두개 외의 혈류의 변화를 통하여 찜질 자극이 국소적으로는 근육 통증 및 편두통과 같은 만성질환의 통증을 완화시키는 효과를 발휘하고 더 나아가 혈액 순환을 촉진시키는 작용을 통하여 전신에 미치는 결림 현상 및 통증의 완화에 실질적으로 효과를 보일 수 있게 되는 것으로 사료된다.

Modeling and analysis of selected organization for economic cooperation and development PKL-3 station blackout experiments using TRACE

  • A series of tests dedicated to station blackout (SBO) accident scenarios have been recently performed at the $Prim{\ddot{a}}rkreislauf-Versuchsanlage$ (primary coolant loop test facility; PKL) facility in the framework of the OECD/NEA PKL-3 project. These investigations address current safety issues related to beyond design basis accident transients with significant core heat up. This work presents a detailed analysis using the best estimate thermal-hydraulic code TRACE (v5.0 Patch4) of different SBO scenarios conducted at the PKL facility; failures of high- and low-pressure safety injection systems together with steam generator (SG) feedwater supply are considered, thus calling for adequate accident management actions and timely implementation of alternative emergency cooling procedures to prevent core meltdown. The presented analysis evaluates the capability of the applied TRACE model of the PKL facility to correctly capture the sequences of events in the different SBO scenarios, namely the SBO tests H2.1, H2.2 run 1 and H2.2 run 2, including symmetric or asymmetric secondary side depressurization, primary side depressurization, accumulator (ACC) injection in the cold legs and secondary side feeding with mobile pump and/or primary side emergency core coolant injection from the fuel pool cooling pump. This study is focused specifically on the prediction of the core exit temperature, which drives the execution of the most relevant accident management actions. This work presents, in particular, the key improvements made to the TRACE model that helped to improve the code predictions, including the modeling of dynamical heat losses, the nodalization of SGs' heat exchanger tubes and the ACCs. Another relevant aspect of this work is to evaluate how well the model simulations of the three different scenarios qualitatively and quantitatively capture the trends and results exhibited by the actual experiments. For instance, how the number of SGs considered for secondary side depressurization affects the heat transfer from primary side; how the discharge capacity of the pressurizer relief valve affects the dynamics of the transient; how ACC initial pressure and nitrogen release affect the grace time between ACC injection and subsequent core heat up; and how well the alternative feeding modes of the secondary and/or primary side with mobile injection pumps affect core quenching and ensure stable long-term core cooling under controlled boiling conditions.
