  • During the past decade, the use of osseointegrated implants as a foundation for prosthetic replacement of missing teeth has become highly predictable and successful. SPT(Supportive Periodontal Therapy) identified as regular visits to the therapist for periodontal control and maintenance in a well-organized scheme, the number of appointments per year following a pre-designed subject-tooth/implant-site risk assessment method. Peri-implant disease was a frequent finding in subjects having natural healthy dentition and in subjects without periodontitis. Supportive periodontal program were found to be strongly related to implant survival. This study demonstrates that regular maintenance reduces the risk for peri-implant inflammation significantly as compared with irregular maintenance. This underlines the value of the SPT in enhancing the long-term outcomes of implant therapy, particularly in subjects affected by periodontitis, in order to control reinfection and limit biological complications. It is highly recommended to maintain implant patients under a strict supportive periodontal treatment protocol that might contribute to implant survival, and regular maintenance reduces the risk for periimplant inflammation significantly as compared with irregular maintenance. Ideally, patients may be informed on the beneficial effect of a regular patient-related post-therapy care before implant insertion.

  • Statement of problem: Little is known about the effect of abutment preparation procedure on do-torque values in different implant platform and the relationship of final do-torque values with different implant platform size. Purpose: This study evaluated the effect of abutment preparation procedure on do-torque values in different implant platform and the relationship of final do-torque values with different implant platform size. Material and method: Six ITI implants (2 narrow-neck implants, 2 regular-neck implants, 2 wide-neck implants) and six Branemark implants (2 narrow platforms, 2 regular platforms, 2 wide platforms) were embedded in each acrylic resin block with epoxy resin. Eighteen $synOcta^(R)$ abutments (6 narrow-neck implant-abutments, 6 regular-neck implant-abutments, 6 wide-neck implant-abutments) and eighteen esthetic abutments (6 narrow platform-abutments, 6 regular platform-abutments, 6 wide platform-abutments) were tightened to each implant with digital torque gauge. Initial do-torque values were measured using digital torque gauge. After preparation of abutments, Final do-torque values were measured with digital torque gauge. Results and conclusion: 1. Screws loosening or abutments motion were not detected in all experimental group, but some scratches of implant-abutment joints were detected in all group 2. Reduction ratios of final do-torque values were greater than initial do-torque values in all measured group, except in narrow-neck implant-abutment group (p<0.05). 3. Reduction ratios of final do-torque values in wide-neck implant-abutment group were greater than regular-neck implant-abutment group (p<0.01). 4. The greatest standard deviation value was detected in wide platform group in both implant systems.

  • Statement of problem. One of the common problems of dental implant prosthesis is the loosening of the screw that connects each component, and this problem is more common in single implant-supported prostheses with external connection, and in molars. Purpose. The purposes of this study were: (1) to compare the initial abutment screw detorque values of the six different implant-abutment interface designs, (2) to compare the detorque values of the six different implant-abutment interface designs after cyclic loading, (3) to compare the detorque values of regular and wide diameter implants and (4) to compare the initial detorque values with the detorque values after cyclic loading. Material and methods. Six different implant-abutment connection systems were used. The cement retained abutment and titanium screw of each system were assembled and tightened to 32Ncm with digital torque gauge. After 10 minutes, initial detorque values were measured. The custom titanium crown were cemented temporarily and a cyclic sine curve load(20 to 320N, 14Hz) was applied. The detorque values were measured after cyclic loading of one million times by loading machine. One-way ANOVA test, scheffe’s test and Mann-Whitney U test were used. Results. The results were as follows : 1. The initial detorque values of six different implant-abutment connections were not significantly different(p>0.05). 2. The detorque values after one million dynamic cyclic loading were significantly different (p<0.05). 3. The SS-II regular and wide implant both recorded the higher detorque values than other groups after cyclic loading(p<0.05). 4. Of the wide implants, the initial detorque values of Avana Self Tapping Implant, MIS and Tapered Screw Vent, and the detorque values of MIS implant after cyclic loading were higher than their regular counterparts(p<0.05). 5. After cyclic loading, SS-II regular and wide implants showed higher detorque values than before(p<0.05).

  • Osseointegrated implants have been established as the standard treatment modality for full/partial edentulous patients since the 1960's, and the long term results for full edentulous patients have proven to be successful. Based on these results osseointegrated implants are now widely used for partial edentulous patients. There has been an increased interest towards the efficacy of wide implants, despite many reports mentioning the lower success rate of wide implants compared to regular implants. Recently, mandibular molar area defects are commonly restored using 2 wide implants, but it is not determined whether which treatment modality-3 regular implants or 2 wide implants-shows superior success rate. In this study, 2 wide implants and 3 regular implants used for the restoration of mandibular molar area are used to compare the survival rate of 1-4 years, and to analyze and compare the failure factors. The following conclusions could be drawn from this study. 1. Wide implants and regular implants showed 94.5% and 97,6% of survival rate respectively. After prosthodontic work, the survival rate was 100% and 98.1% for wide implants and regular implants respectively. 2. 5 failed implants have been removed. 2 wide implants and 1 regular implant have been removed due to failure of osseointegration. 1 wide implant was removed due to abscess formation caused by over-heating, and 1 regular implant was removed due to mechanical failure caused by over-loading within the first year of function. 3. No statistically significant difference was observed with respect to the amount of marginal bone loss of wide and regular implants.(P>0.05) In conclusion, restoration of the mandibular molar area using 3 regular implants was found to be a good treatment modality, and 2 wide implants could he considered a good treatment modality when success factors are taken into account.

  • This study aims to describe the clinical experience of single and bridge crowns fabricated using a cementless screw-retained implant prosthesis system. In the case of single crown (#37), regular link (HDL) was used, and bridge crowns (#15~#24), (#26~#27), (#17~#14) (#24~#26) were fabricated by selecting regular link and short link considering the vertical height. One abutment was hex shaped to ensure that it could be mounted while preventing insertion and prosthesis rotation. The advantages of cementless implant prosthesis include shorter chair time and periodic care, strong retention with LINK abutment, safety from inflammation, bacterial infection, and complications due to peri-implant cement, and high patient satisfaction. Dentists should double-check the position of the implant fixture and dental technicians should continuously manage the fit of the link and prosthesis with digital equipment to reduce screw loosening and fractures.

  • The aim of this study was to determine the condition of supportive periodontal therapy (SPT) in implant patients and the effect of SPT on implant long-term survival. Implants placed at the Dept. of Periodontology, Chonnam National University Dental Hospital over a 5-year period, were traced for up to 8 years. Patients who had visited the hospital at least once a year were defined as regular SPT, and patients who were treated by active periodontal therapy were defined as patients with periodontitis. Kaplan-Meier survival analysis was performed based on the observation periods, and the effect of SPT and history of periodontitis on implant survival assessed by chisquare test. A total of 183 patients (age: 21-91, 98 males and 85 females), and 508 implants were used for this study. Three hundred eight implants were under SPT and 87 implants was under regular SPT. For the patients with periodontitis 136 implants were placed. The 5-year survival rate was 94.8%. The overall survival rate in patients who received SPT was 97.1% and 91.0% for those who did not (p=0.004). The survival rate in patients who received regular SPT was 97.7%, and 96.8% for patients received irregular SPT. The survival rate was 93.4% in patients with periodontitis and 95.2% in patients without periodontitis. Among patients with periodontitis, the survival rate was 100.0% in patients who received regular SPT and 89.2% for irregular SPT (p=0.012). These results suggest that regular SPT improves implant survival rate and is more effective in patients with periodontitis.

  • Peri-implantitis refers to the development of inflammation around the implant. Peri-implantitis refers to inflammation accompanied by uptake of bone in the submucosa. Perhaps the most serious of the symptoms of peri-implantitis is the mobility of the implant. Peri-implantitis may occur more frequently in patients with periodontitis than in patients without periodontitis. Therefore, regular checkups and plaque control are more important and important when implants are placed in patients involved with gingivitis or periodontitis. There are many factors such as plaque, calculus and smoking, which can cause peri-implantitis. However, regular plaque management can prevent and reduce the incidence of peri-implantitis. In conclusion, it is important to successfully surgery implants in patients, but preventing peri-implantitis is the success of implant treatment.
