Interaction Effects between Individual Socioeconomic Status and Regional Deprivation on Onset of Diabetes Complication and Diabetes-Related Hospitalization among Type 2 Diabetes Patients: National Health Insurance Cohort Sample Data from 2002 to 2013 (개인의 사회경제적 수준과 지역의 사회경제적 수준의 상호작용이 제2형 당뇨 환자에서 당뇨합병증 발생 및 당뇨와 관련된 입원에 미치는 영향: 2002-2013년 국민건강보험공단 표본 코호트 자료를 활용하여)

  • Background: In this study, we aimed to investigate the interaction effects of individual socioeconomic status and regional deprivation on the onset of diabetes complications and diabetes-related hospitalization among type 2 diabetes patients. Methods: Korean National Health Insurance Service National Sample Cohort data from 2002 to 2013 were used. A total of 50,954 patients newly diagnosed with type 2 diabetes from 2004 to 2012 and aged 30 years or above were included. We classified patients into six groups according to individual income level and neighborhood deprivation: 'high in advantaged,' 'high in disadvantaged,' 'middle in advantaged,' 'middle in disadvantaged,' 'low in advantaged,' and 'low in disadvantaged.' We calculated hazard ratios (HR) of onset of diabetes complication and diabetes-related hospitalization using the Cox proportional hazard model, with the reference group as diabetes patients with high income in advantaged regions. Results: In terms of the interaction effects of individual income level and regional socioeconomic level, even with the same low individual income level, the group with a high regional socioeconomic level (low in advantaged) showed low HRs for the onset of diabetes complication (HR, 1.04; 95% confidence interval [CI], 1.00-1.08) compared to the 'low in disadvantaged' group (HR, 1.10; 95% CI, 1.05-1.16). In addition, the 'high in advantaged' group showed slightly higher HRs for the onset of diabetes complication (HR, 1.06; 95% CI, 1.00-1.11) compared to the 'low in advantaged' and it appeared to be associated with slight mitigation of the risk of diabetes complication. For the low-income level, the patients in disadvantaged regions showed the highest HRs for diabetes-related hospitalization (HR, 1.29; 95% CI, 1.19-1.41) compared to the other groups. Conclusion: Although we need to perform further investigations to reveal the mechanisms that led to our results, interaction effects individual socioeconomic status and regional deprivation might be associated with on onset of diabetes complications and diabetes-related hospitalization among type 2 diabetes patients.

A Comparative Study of Predicted Environmental Concentrations from ECETOC TRA Based on Environmental Release Categories/Specific Environmental Release Categories and K-CHESAR Using Main/Industrial/Use Categories (환경배출범주/특수환경배출범주 기반 ECETOC TRA 및 주요/산업/용도 분류체계 이용의 K-CHESAR에 의한 환경예측농도 비교 연구)

  • Background: Environmental concentrations of substances can be estimated by K-CHESAR based on main, industrial, and use categories (MC/IC/UC) and ECETOC TRA based on environmental or specific environmental categories (ERC or spERC). Objectives: Three different systems for estimating environmental concentrations were compared to figure out their order with possible reasons along with relationship of regional predicted environmental concentrations (PECregional) and final PEClocal for various uses of a substance. Methods: Typical uses of the case substance and their corresponding ERCs were selected from the webpage of the European Chemical Agency. Proper MC/IC/UC and spERC were assigned to each ERC. Emission fractions were compared for each assessment code from the available database. PECs were calculated by three estimating systems: K-CHESAR using MC/IC/UC, ECETOC TRA using ERC, and ECETOC TRA using spERC with their default values for input parameters. Percentage of PECregional to PEClocal were manually calculated for each use. Results: Emission factors decreased in the order of ERC > MC/IC/UC > spERC. Values of the final PEClocal derived as sum of PECregional and Clocal decreased in the order of calculations using ECETOC TRA-ERC>KCHESAR with MC/IC/UC>ECETOC TRA-spERC for all environmental media. Percentages of PECregional,water to PEClocal,water ranged from 0 to 10.3% in industrial uses calculated with MC/IC/UC and ERC but 96.3 to 100% in wide dispersive uses of ERC and spERC where values of Clocal,water are estimated to be very low. Conclusions: ECETOC TRA generated the most refined PNEC values with spERC and the least with ERC, while K-CHESAR with MC/IC/UC generated values between the two results. The ratio of PECregional to PEClocal can be a good measure for performing suitable estimation of PNECs according to use.

  • Background: Avian influenza (AI) is a contagious disease that causes illness and death in poultry and humans. High pathogenicity AI (HPAI) H5N6 outbreaks commonly occur in Quang Ninh province bordering China. In June 2021, the first HPAI H5N8 outbreak occurred at a Quang Ninh chicken farm. Objectives: This study examined the risk factors associated with HPAI H5N6 and H5N8 outbreaks in Quang Ninh. Methods: A retrospective case-control study was conducted in Quang Ninh from Nov 2021 to Jan 2022. The cases were households with susceptible poultry with two or more clinical signs and tested positive by real-time reverse transcription polymerase chain reaction. The controls were households in the same village as the cases but did not show clinical symptoms of the disease. Logistic regression models were constructed to assess the risk factors associated with HPAI outbreaks at the household level. Results: There were 38 cases with H5N6 clade 2.3.4.4h viruses (n = 35) and H5N8 clade 2.3.4.4b viruses (n = 3). Compared to the 112 controls, raising poultry in uncovered or partially covered ponds (odds ratio [OR], 7.52; 95% confidence interval [CI], 1.44-39.27), poultry traders visiting the farm (OR, 8.66; 95% CI, 2.7-27.69), farms with 50-2,000 birds (OR, 3.00; 95% CI, 1.06-8-51), and farms with ≥ 2,000 birds (OR, 11.35; 95% CI, 3.07-41.94) were significantly associated with HPAI outbreaks. Conclusions: Combining biosecurity measures, such as restricting visitor entry and vaccination in farms with more than 50 birds, can enhance the control and prevention of HPAI in Quang Ninh and its spread across borders.

Perforator Flaps after Excision of Large Epidermal Cysts in the Buttocks

  • Background Epidermal cysts are commonly occurring masses usually less than 5 cm in diameter, but in predisposed patients, epidermal cysts can grow relatively large due to chronic infection. Methods From June 2002 to July 2010, 17 patients received 19 regional perforator-based island flaps to cover defects due to the excision of large epidermal cysts (diameter >5 cm) in the buttocks. Eight patients had diabetes, and seven had rheumatoid arthritis. The pedicles were not fully isolated to prevent spasms or twisting. Results All the flaps survived completely, except for one case with partial necrosis of the flap, which necessitated another perforator-based island flap for coverage. There were two cases of wound dehiscence, which were reclosed after meticulous debridement. There were no recurrences of the masses during follow-up periods of 8.1 months (range, 6-12 months). Conclusions In patients with large epidermal cysts and underlying medical disorders, regional perforator-based island flaps can be the solution to coverage of the defects after excision.

  • Using careful background research and methodology, we proposed a revised sectoral index of vulnerability that is directly applied to two central labor examples: working hours and minimum wages. The valued calculated by each specific index represent the degree of weakness in corresponding conditions at the level of workplace units. Our proposed vulnerability index promises to be applicable to all business units under labor spot inspection. Also, it can be easily updated as the business environment evolves. The index is useful in selecting certain groups of business units and in evaluating the effectiveness of each regional government office.

A study on Monitoring for CFC-12 and CFC-11 in the atmosphere near Mt. Kwan-Ak. (관악산 대기 중의 CFC-12 및 CFC-11 모니터링에 관한 연구)

  • A monitoring system for atmospheric CFC-12 and CFC-11 has been established at Mt. Kwan-Ak, Seoul National University (SNU) since July, 1991. The concentrations showed quite a large variation ranging from 495 to 37600 pptv (pptv=part per trillion, 10$^{-12}$ , v/v) for CFC-12 and from 233 to 12100 pptv for CFC-11 due to many local sources. However, monthly medians show rather limited ranges ; 553~765 pptv for CFC-12 and 301~431 pptv for CFC-11. Furthermore minimum concentrations could be defined relatively well during the whole period of observation. The regional background concentrations in 1993 near SNU wer estimated as 533 pptv for CFC-12 and 293 pptv for CFC-11. These values are very comparable to global averages in Northern Hemisphere, 523 pptv for CFC-12 and 287 pptv for CFC-11, reflecting the fast atmospheric mixing processes within the hemisphere. Examinations with meteorological parameters such as wind speed and direction suggest the possible measurement-window at SNU, appropriate for regional monitoring. Studies for improving the monitoring capability of the SNU station such as automation of the analysis system along with correlation with other meteorological parameters, are in progress at the present time.

Clinicopathological Characteristics of Hepatocellular Carcinoma in Turkey

  • Background: Hepatocellular carcinoma (HCC), the main malignant tumor of the liver, is very common and highly lethal. The aim of this study was to determine its clinicopathologic characteristics and risk factors in Turkey. Materials and methods: In this study, patients who were diagnosed as suffering from HCC in the period between August 2004 and December 2011 were evaluated retrospectively. Results: A total of 98 patients were included, with a median age 61 (range: 16 to 82). Seventy nine (80.6%) were male 59 (60.2%) were infected with hepatitis B virus (HBV) and 15 (15.3%) with HCV, another 15 (15.3%) being alcohol abusers. Seventy two (73.5%) were at advanced stage and 54 (55.1%) had elevated serum alpha-fetoprotein (AFP). Surgery, chemoembolization, systemic chemotherapy and application of the tyrosine kinase inhibitor sorafenib were the major treatment options. Conclusions: According to our findings HCC is mostly diagnosed in advanced stage and age, being five times more common in males than females. Main risk factors of HCC are HBV infection, HCV infection and alcohol abuse. Elevation in AFP may facilitate early diagnosis of HCC in high risk groups.

  • Land Use Information is the key of the rational decision in urban and regional planning. Although the local and central governments have data related to land use, urban and regional planners can gather the land use information in difficulty. For use the data which the governments have made, they should formally show their needs and applications, On this background, this study try to show the demands of such information for urban planning using questionnaire. Analyzing the result, it can present the hardness of gathering land use information, the high demands, and the needs of the each data field.

  • The government established Special Accounts for Balanced National Development in 2005, and the Metropolitan Council has enacted and executed ordinance relevant to special accounts to relieve regional unbalanced growth and to promote regional balanced growth. This study derived a political implication through the comparison analysis of the background of establishment and its purpose, the selection criteria of support target, finance scale and tax revenue finance, appropriation target and finance distribution, etc. on the basis of Chungcheongnam-do Special Accounts for Balanced Development and Jeollabuk-do Eastern part Special Accounts among the special accounts implemented in metropolitan council. The major conclusions of this study are as follows. First, the transition from the concept of balanced development to specialized development is necessary for the development of distressed region. Second, the pure wastefulness ratio of the local government's tax revenue finance of special accounts designed concentrating upon special accounts for Wide-area Autonomous Communities development should be gradually expanded. Third, Special Accounting budget should be supported to promote specialized development through the selection and concentration centering around comparatively advantageous resource within the region. Fourth, the strategic special accounts budget application system of cities and counties to utilize the projects of the government and the province to achieve the goal of cities and counties should be prepared.

  • The need for designing and implementing integrated policy was further emphasized in tandem with the increase in interest concerning policy coordination and interactions. An active discussion is taking place in the field of innovation policy concerning "integrated innovation policy," which considers innovation along with financial, regional development, social, and environmental policies together in a holistic manner. In Korea since the beginning of the 2000s, there were many attempts at implementing integrated innovation policy through the restructuring of the overall S&T administration system. For the purposes of taking an integrated approach to S&T policies as well as to S&T-related human resources, industrial, and regional development policies, the Roh Administration (February 2003~February 2008) elevated the S&T Minister to the level of Deputy Prime Minister as well as launching the Office of Science, Technology, and Innovation (OSTI) (October 2004 ~ February 2008) under the Ministry of Science and Technology. This study investigates the policy coordination activities of the OSTI from the perspective of policy integration. It deals with the background of the OSTI, its roles and responsibilities, the coordination process, and its achievements and limitations while discussing the important implications for developing effective policy measures with the hope of contributing to the development of theories of integrated innovation policy.
