The effectiveness of optical coherence tomography for evaluating peri-implant tissue: A pilot study

  • Purpose: Optical coherence tomography (OCT) has been investigated as a novel diagnostic imaging tool. The utilisation of this equipment has been evaluated through several studies in the field of dentistry. The aim of this preliminary study was to determine through basic experiments the effectiveness of OCT in implant dentistry. Materials and Methods: To assess detection ability, we captured OCT images of implants in each of the following situations: (1) implants covered with mucosae of various thicknesses that were harvested from the mandibles of pigs; (2) implants installed in the mandibles of pigs; and (3) implants with abutments and crowns fixed with temporary cement. The OCT images were captured before cementation, after cementation, and after removing the excess submucosal cement. Results: If the thickness of the mucosa covering the implant body was less than 1 mm, the images of the implants were clearly detected by OCT. In the implants were installed in pigs' mandibles, it was difficult to capture clear images of the implant and alveolar bone in most of the samples. Remnants of excess cement around the implants were visible in most samples that had a mucosa thickness of less than 3 mm. Conclusion: Currently, OCT imaging of implants is limited. Cement remnants at the submucosal area can be detected in some cases, which can be helpful in preventing peri-implant diseases. Still, though there are some restrictions to its application, OCT could have potential as an effective diagnostic instrument in the field of implant dentistry as well.

Use of adipose-derived stem cells in lymphatic tissue engineering and regeneration

  • The potential to differentiate into different cell lines, added to the easy and cost-effective method of extraction, makes adipose-derived stem cells (ADSCs) an object of interest in lymphedema treatment. Our study's goal was to conduct a comprehensive systematic review of the use of ADSCs in lymphatic tissue engineering and regeneration. On July 23, 2019, using PubMed/MEDLINE, Cochrane Clinical Answers, Cochrane Central Register of Controlled Trials, and Embase databases, we conducted a systematic review of published literature on the use of ADSCs in lymphatic tissue engineering and regeneration. There were no language or time frame limitations, and the following search strategy was applied: ((Adipose stem cell) OR Adipose-derived stem cell)) AND ((Lymphedema) OR Breast Cancer Lymphedema). Only original research manuscripts were included. Fourteen studies fulfilled the inclusion criteria. Eleven studies were experimental (in vitro or in vivo in animals), and only three were clinical. Publications on the topic demonstrated that ADSCs promote lymphangiogenesis, and its effect could be enhanced by modulation of vascular endothelial growth factor-C, interleukin-7, prospero homeobox protein 1, and transforming growth factor-β1. Pilot clinical studies included 11 patients with breast cancer-related lymphedema, and no significant side effects were present at 12-month follow-up. Literature on the use of ADSCs in lymphatic tissue engineering and regeneration demonstrated promising data. Clinical evidence is still in its infancy, but the scientific community agrees that ADSCs can be useful in regenerative lymphangiogenesis. Data collected in this review indicate that unprecedented advances in lymphedema treatment can be anticipated in the upcoming years.

혈소판유래 및 상피성장인자가 치주조직재생에 미치는 영향 (The Effects of Platelet-Derived Growth Factor and Epidermal Growth Factor on the Periodontal Tissue Regeneration)

  • 6 beagle dogs aged over one and half years and weighed 14 to 16 Kg were utilized in this study, Horizontal furcation defects were induced around 2nd, 3rd, and 4th premolars bilaterally, PDGF-BB in conjunction with EGF and PDGF-BB only were applied in the right and left premolars respectively. 2 animals were sacrificed at 4weeks, 8 weeks, and 12 weeks, after regenerative surgery respectively. Semi-thin sections using glass-knife were stained with toluidine blue for light microscopic study. The results were as follows: 1. At 4 weeks after regenerative surgery, bone formation in the PDGF-BB-applied site was thriving, but bone formation in the PDGF-BB-and-EGF-applied site was depressed. 2. Bony ankylosis was surely shown along the whole exposed root surface applied with PDGF-BB, but it was shown at the root surface near the base of the bone defect where was applied with PDGF-BB in conjunction with EGF. 3. Active bone formation was made from 8 weeks after regenerative surgery in the PDGF-BB- and-EGF-applied site. 4. Bone maturity as well as speed of bone formation in the PDGF-BB-applied site was superior to those in the PDGF-BB-and-EGF-applied site throughout the whole experimental period. Within the above results, PDGF-BB had the strong capability to form the new bone and EGF was not able to prevent the bony ankylosis thoroughly. However, EGF may have the possibility to prevent the bony ankylosis through the suppression of bone formation.

A case of interdigitating dendritic cell sarcoma studied by whole-exome sequencing

  • Interdigitating dendritic cell sarcoma (IDCS) is an aggressive neoplasm and is an extremely rare disease, with a challenging diagnosis. Etiology of IDCS is also unknown and most studies with only case reports. In our case, immunohistochemistry showed that the tumor cells were positive for S100, CD45, and CD68, but negative for CD1a and CD21. This study aimed to investigate the causative factors of IDCS by sequencing the protein-coding regions of IDCS. We performed whole-exome sequencing with genomic DNA from blood and sarcoma tissue of the IDCS patient using the Illumina Hiseq 2500 platform. After that, we conducted Sanger sequencing for validation of sarcoma-specific variants and gene ontology analysis using DAVID bioinformatics resources. Through comparing sequencing data of sarcoma with normal blood, we obtained 15 nonsynonymous single nucleotide polymorphisms (SNPs) as sarcoma-specific variants. Although the 15 SNPs were not validated by Sanger sequencing due to tumor heterogeneity and low sensitivity of Sanger sequencing, we examined the function of the genes in which each SNP is located. Based on previous studies and gene ontology database, we found that POLQ encoding DNA polymerase theta enzyme and FNIP1 encoding tumor suppressor folliculin-interacting protein might have contributed to the IDCS. Our study provides potential causative genetic factors of IDCS and plays a role in advancing the understanding of IDCS pathogenesis.

치주치료를 받은(1981-1995) 환자들의 연령과 치료술식에 대한 조사 연구 (Investigation of Age and Treatment Modalities in the Periodontally Treated(1981-1995) Patients)

  • The aim of this study was to recommend the optimal age for prevention of periodontal disease and to investigate the trend of treatment modality according to different period. From the chart recordings of the patients who had been treated periodontally from Jan. 1981 to Dec. 1995 in the dept .of periodontics, Chosun University Dental Hospital, those of the periodontally treated patients on more than 4 sixtants were selected for the present study. The distribution of the patients was counted according to the age group and the gender. And they were divided into 3 groups(group 1: 1987-1958, group 2: 1985-1990, group 3: 1991-1995) by 5 year interval according to the treated year. The periodontal treatment modalities were classified into non-surgical therapy, pocket elimination surgery, regenerative periodontal surgery, mucoginigival surgery, clinical crown lengthening, and others. The results were as follows; 1.In the distribution of the periodontally treated patients according to the age group, 40's age group was the highest, and 30's, 40's, and 50's age groups occupied more than two thirds(73%). 2.The sexual distribution of the periodontally treated patients showed that males(53.4%) were a little more than females(46.6%). Within 20's group female was higher, but within 40's male was higher. 3.Regardless of the age group and the gender, pocket elimination surgery was the most frequent treatment modality. 4.In group 1 and 2(1987-1990) the main treatment modality was pocket elimination surgery and non-surgical therapy, but in group 3(1991-1995) it was regenerative periodontal surgery and pocket elimination surgery. The above results suggest that the prevention of periodontal disease should be initiated from early twenties, and the most frequent treatment modality may be closely related with development of new material and method.

  • The advent of osseointegration and advances in biomaterials and techniques have contributed to increased application of dental implants in the restoration of maxillary partial edentulous patients. Often, in these patients, soft and hard tissue defects result from a variety of causes, such as infection, trauma, and tooth loss. These create an anatomically less favorable foundation for ideal implant placement. Reconstruction of the atrophic maxillary alveolar bone through a variety of regenerative surgical procedures has become predictable; it may be necessary prior to implant placement or simultaneously at the time of implant surgery to provide a restoration with a good long-term prognosis. Regenerative procedures are used for horizontal and vertical ridge augmentation. Many different techniques exist for effective bone augmentation. The approach is largely dependent on the extent of the defect and specific procedures to be performed for the implant reconstruction. It is most appropriate to use an evidenced-based approach when a treatment plan is being developed for bone augmentation cases. The cases presented in this article clinically demonstrate the efficacy of using a autogenous block graft, guided bone regeneration, ridge split, immediated implant placement technique on the atrophic maxillary area.

  • 척수 손상은 중추 신경계의 손상으로 신경학적 회복이 어려운 것으로 알려져 있었으나 최근 그 병태 생리에 대한 이해의 발전과 함께 다양한 신경 보호적 시도 및 신경 재생에 대한 시도가 보고되어 이에 대한 새로운 치료적 접근법이 제시되고 있다. 이에 본 종설에서는 제시되고 있는 병태 생리와 함께 신경학적 재생을 위한 실험적 연구의 내용을 정리하며, 임상적 시도에 대하여 현재까지의 결과와 함께 향후 객관적이며 안전한 임상적 적용을 위한 선행 조건 등에 대하여 정리하여 보고자 한다.

골형태형성단백질과 흡수성차폐막이 치주조직 재생에 미치는 영향 (The Effects of Bone Morphogenetic Protein-4 and Resorbable Membrane on the Regeneration of Periodontal Tissues)

  • The aim of our study is to achieve complete periodontal tissue regeneration by the application of BMP and resorbable membrane. Three beagle dogs aged over one and half years and weighed 14 to 16 kg were used in this study. Mandibular 1st, 2nd premolars were extracted bilaterally. Horizontal furcation defects were induced around 3rd, 4th premolars bilaterally. BMP-4 were applied in the right side with resorbable membranes and only resorbable membranes were applied in the left side respectively. Each animal was sacrificed at 2, 4, and 8weeks, after regenerative surgery. Specimens were prepared with Hematoxylin-Eosin stain and Goldner's modified Masson Trichrome stain for light microscopic evaluation. The results were as follows: 1. At 2 weeks after regenerative surgery, downgrowth of junctional epithelium was observed both in the membraneapplied site and BMP-4-and-membrane-applied site. 2. At 4 weeks after regenerative surgery, resorbable membranes were completely resolved, therefore would not prevent downgrowth of junctional epithelium. New bone formation, new cementum formation and Sharpey's fiber were observed in BMP-4-andmembrane-applied site. 3. At 8 weeks after regenerative surgery, downgrowth of junctional epithelium was observed in the membrane-applied site. But, new cementum formation was observed in the same site. The extensive regeneration of new bone, new cementum and remarkable formation of Shapey's fiber were showed in BMP-4-and-membrane-applied site. 4. Resorbable membranes were resolved via the cell-mediated processes. 5. Periodontal tissue regeneration were better achieved in the BMP-4-andmembrane-applied site than in the membrane-applied site. Within the above results, BMP-4 may have the strong capability to form the new bone and resorbable membrane may be able to prevent the bony ankylosis. However, resolution rate of resorbable membrane may not be enough to protect rapid epithelial downgrowth for ideal periodontal regeneration. Use of platelet-rich plasma and modified nanofat grafting in infected ulcers: Technical refinements to improve regenerative and antimicrobial potential

  • Background Surgical reconstruction of chronic wounds is often infeasible due to infection, comorbidities, or poor viability of local tissues. The aim of this study was to describe the authors' technique for improving the regenerative and antimicrobial potential of a combination of modified nanofat and platelet-rich plasma (PRP) in nonhealing infected wounds. Methods Fourteen patients met the inclusion criteria. Fat tissue was harvested from the lower abdomen following infiltration of a solution of 1,000 mL of NaCl solution, 225 mg of ropivacaine, and 1 mg of epinephrine. Aspiration was performed using a 3-mm cannula with 1-mm holes. The obtained solution was decanted and mechanically emulsified, but was not filtered. Non-activated leukocyte-rich PRP (naLR-PRP) was added to the solution before injection. Patients underwent three sessions of injection of 8-mL naLR-PRP performed at 2-week intervals. Results Thirteen of 14 patients completed the follow-up. Complete healing was achieved in seven patients (53.8%). Four patients (30.8%) showed improvement, with a mean ulcer width reduction of 57.5%±13.8%. Clinical improvements in perilesional skin quality were reported in all patients, with reduced erythema, increased thickness, and increased pliability. An overall wound depth reduction of 76.6%±40.8% was found. Pain was fully alleviated in all patients who underwent re-epithelization. A mean pain reduction of 42%±33.3% (as indicated by visual analog scale score) was found in non-re-epithelized patients at a 3-month follow-up. Conclusions The discussed technique facilitated improvement of both the regenerative and the antimicrobial potential of fat grafting. It proved effective in surgically-untreatable infected chronic wounds unresponsive to conventional therapies.

  • Upper extremity reconstruction may pose clinical challenges for surgeons due to the often-critical, complex functional demands of the damaged and/or missing structures. The advent of vascularized bone grafts (VBGs) has aided in reconstruction of upper extremity (UE) defects due to their superior regenerative properties compared with nonvascularized bone grafts, ability to reconstruct large bony defects, and multiple donor site options. VBGs may be pedicled or free transfers and have the potential for composite tissue transfers when bone and soft tissue are needed. This article provides a comprehensive up-to-date review of VBGs, the commonly reported donor sites, and their indications for the treatment of specific UE defects.
