A novel photonumeric hand grading scale for hand rejuvenation

  • Background Few scales are currently available to evaluate changes in hand volume. We aimed to develop a hand grading scale for quantitative assessments of dorsal hand volume with additional consideration of changes in skin texture; to validate and prove the precision and reproducibility of the new scale; and to demonstrate the presence of clinically significant differences between grades on the scale. Methods Five experienced plastic surgeons developed the Hand Volume Rating Scale (HVRS) and rated 91 images. Another five plastic surgeons validated the scale using 50 randomly selected images. Intra- and inter-rater agreement was calculated using the weighted kappa statistic and intraclass correlation coefficients (ICCs). Paired images were also evaluated to verify whether the scale reflected clinical differences. Results The intra-rater agreement was 0.95 (95% confidence interval, 0.922-0.974). The interrater ICCs were excellent (first rating, 0.94; second rating, 0.94). Image pairs that differed by 1, 2, and 3 grades were considered to contain clinically relevant differences in 80%, 100%, and 100% of cases, respectively, while 84% of image pairs of the same grade were found not to show clinically relevant differences. This confirmed that the scale of the HVRS corresponded to clinically relevant distinctions. Conclusions The scale was proven to be precise, reproducible, and reflective of clinical differences.

  • Subjective voice evaluation is necessary and important to assess the voice disorders in addition to objective voice evaluation. Subjective voice evaluation is divided into examiner and examinee subjective voice assessment. The examiner assessment represents perceptual judgment to the patient's voice such as GRBAS scale, Buffalo voice profile, consensus auditory perceptual evaluation of voice (CAPE- V) and so on. The examinee assessment consists of indirect method including voice handicap index (VHI), voice outcome survey (VOS), voice symptom scale (VoiSS), voice related quality of life (V-ROQL) and direct method which is called patient's self-subjective voice rating. This review article describes a general rule, advantages and pitfalls about GRBAS scale, VHI and patient's self-subjective voice rating which are presently most representative voice assessment tools.

Effect of Different Air Hole Diameters of the Inspiratory Muscle Trainer on the Rating of Perceived Exertion and Inspiratory Muscle Activity during Breathing Exercise

  • Purpose : This study aims to investigate the rating of perceived exertion (RPE) and muscle activity of the inspiratory primary and accessory muscle during breathing exercise with different air hole diameters of the inspiratory muscle trainer (IMT). Methods : The Borg's scale and surface electromyography (EMG) was used to collect data of the RPE and muscle activity of the inspiratory primary the external intercostal (EI) and diaphragm (DIA) and accessory muscles anterior scalene (AS), sternocleidomastoid (SCM), pectoralis major (PM), and upper trapezius (UT) muscles during breathing exercise with different air hole diameters (6 mm, 4 mm, and 2 mm) of the IMT in healthy young male subjects. Results : The RPE and muscle activities of the AS, SCM, and UT are increased significantly in accordance to the decreasing diameter of air hole of air tip in IMT. However, there are no differences in the muscle activities of the PM, EI, and DIA based on differences of diameters of air hole of air tip in IMT. Conclusion : The smaller the diametr of IMT air-hole, RPE and muscle activities of AS, SCM and UT were increased. Therefore, further study would be necessary to investigate the proper intensity and relaxation posture for the exercise protocol to strengthen the inspiratory primary muscles.

Effects of Active Movement with Skin Mobilization on Range of Motion, Pain, RPE on Patients with Axillary Web Syndrome: A Case Study

  • Objective: This study was conducted to investigate the effect of active movement with skin mobilization on range of motion, pain, and rating of perceived exertion in patients diagnosed with axillary web syndrome after axillary lymph node dissection. Design: A Case report Methods: It was performed on 7 patients diagnosed with axillary web syndrome after lymph node dissection. The subjects experienced a decrease in the range of joint motion and pain in movement when raising their arms in their daily lives, and complained of discomfort. The active range of motion, numeric rating scale, and modified Borg scale of shoulder joint flexion were measured, and the differences after active movement with skin mobilization were compared. Results: All subjects increased by 24.9 degree on average in active range of motion after active movement with skin mobilization intervention. There was no pain in the maximum range of joint motion measured before intervention, and rating of perceived exertion was significantly reduced. Conclusions: Active movement with skin mobilization can be a very useful way to help improve and treat axillary web syndrome, and it is recommended for improving the function and quality of life of axillary web syndrome patients. and It is also believed that it can be used steadily at home through the education of patients and families.

  • In this paper, change of load carrying capacity of bridges with time was analyzed referring to the safety inspect data stored in the KISTEC. From this study, it was known that the capacity is rated differently by various parameters such as inspector's subjective. Accordingly, an improved method, in this paper, is suggested which can correct deviation due to several uncertainties involved in rating process. The suggested method can be utilized for efficient maintenance of bridges such as cycles and determination of priority of retrofit, and estimation of service life etc.

  • Currently, wind power has become a major research field in the area of sustainable development. As one important component of a wind turbine transmission system, most instances of downtime due to a gearbox failure are caused by bearing failures. Gearboxes for wind turbines must have the highest levels of reliability over a period of approximately 20 years, withstanding high dynamic loads. At the same time, a lightweight design and cost minimization efforts are required. These demands can only be met with a well-thought-out design, high-quality materials, a high production quality and proper maintenance. In order to design a reliable and lightweight gearbox, it is necessary to analyze methods pertaining to the bearing rating lifetimes of the standard and of different companies, also including calculation methods for modification factors. This can determine the influence of the bearing lifetime.

Internal Consistency and Concurrent Validity of Korean Language Version of WHODAS 2.0: 12 Item-Self Administered

  • Purpose: The aim of this study was to validate the Korean version of World Health Organization Disability Assessment Schedule 2.0 (KWHODAS 2.0) in 12 item-self administered version (12-self). Methods: The KWHODAS 2.0 and Korean Functional Rating index (KFRI) were tested for internal consistency, ceiling and floor effects, and concurrent validity in 111 patients with low back pain and/or neck pain. Results: A very high level of internal consistency was shown for both instruments; ${\alpha}$=0.96 with KWHODAS 2.0; 12-self and ${\alpha}$=0.97 for KFRI. No ceiling and/or floor effects were found in both the instruments. The KWHODAS 2.0 and KFRI were highly correlated (r=0.77), and the relationship of each item between KWHODAS 2.0 and KFRI was ranging from r=0.09 to 0.72. Conclusion: We conclude that the KWHODAS 2.0: 12-self and KFRI are reliable and are valid instruments for the measurement of disability in Korean speaking patients with low back and/or neck pain. Both instruments, the KWHODAS 2.0; 12-Self and KFRI are now suitable for use in clinical practice and research applications.

10 Cases of Patients with Lumbar Disc Herniation Improved after Oriental Intensive Therapy (한방 집중 치료 후 호전된 요추 추간판 탈출증 환자 10례)

  • Patients with lumbar disc herniation are faced with lowered quality of life. For this reason, we investigated the effects of Oriental Intensive Therapy (OIT) including acupuncture, pharmacopuncture, manual therapy and herbal medicine on patients with lumbar disc herniation. Ten patients, diagnosed with lumbar disc herniation by MRI in hospital, were treated with OIT for 12 weeks. Symptoms of the patients were assessed using Verbal Numerical Rating Scale (VNRS) and Oswestry Disability Index (ODI). All patients were investigated every a week. In our results, intensity of pain was reduced after 4 weeks treatment significantly by VNRS, Quality of life has also improved after 4 weeks treatment by ODI. In conclusion, this study suggests that OIT may be effective to treat patients with lumbar disc herniation.

Development of the Brown Planthopper, Nilaparvata lungens Sta$\{aa}$l, Biotypes and Their Hybrid Progenies Fed on Different Cultivars of Rice with Various Resistance Gene and Damage of Different Rice Cultivars (벼멸구 생태형의 교잡종별 발육 및 수도 품종별 피해)

  • Some attempts were made to investigate the gentic characteristics of the brown planthopper(BPH), Nilaparvata lugens Sta${\aa}$l, biotypes. In terms of egg and nymphal periods, egg hatcgability, emergence ratio, and population change of BPH and plant damage of rice cultivars by the infestation of BPH biotypes and their hybrid progenies in field conditions. The egg and nymphal periods of the three of BPH biotypes and their hybrid progenies on Dongjinbyeo with no resistance gene were shorter than on rice cultivars with resistance gene. But the periods of biotype-2(♀) X 2(♂) (E) on Samgangbyeo with Bph-1 gene and biotype-3(♀) X 3(♂)(I) on Mirying 63 with bph-2 gene were as short as on Dongjinbyeo. The egg hatchability and emergence ratio of the three Bph biotypes and their hybrid progenies on Dongjinbyeo were higher than on the other rice cultivars, and those of E on Samgangbyeo and I on Miryang 63 were as high as on Dongjinbyeo. In rice paddy field, biotype-1(♀) X 1(♂)(A)$\sim$I infested on Dongjinbyeo, E onSamgangbyeo, and I on Miryang 63 were increased their population densities remarkably in next generation. Dongjinbyeo infested by A$\sim$I, and Myryang 63 infested by I were hopperburned at 60 days after infestation and had a damage rating of 0.9. But Samgangbyeo infested by E was no hopperburn(damage rating: 7.3) though it showed high BPH population at 60 days after infestation.

A Case Report of a Migraine Patient Treated with Six-Meridian Pattern Identification (六經辨證) (육경변증(六經辨證)을 이용하여 치료한 편두통 환자 치험 1례)

  • The purpose of this case report is to describe the effect of Korean medicine therapy on patient with migraine. We treated migraine for 2 months using a Korean medical treatment and six-meridian pattern identification, including herbal medicine (Soshiho-tang), acupuncture, cupping, chuna, and pharmacopuncture. The measurements included a numerical rating scale (NRS), migraine disability assessment (MIDAS), headache impact test-6 (HIT-6), and migraine specific quality of life questionnaire (MSQoL) to confirm the effectiveness of the herbal medicine treatment. After treatment, the patient's complaints decreased, as shown by the results of the NRS, MIDAS, HIT-6, and MSQoL. For migraines, Korean medical treatment and six-meridian pattern identification, such as herbal medicine (Soshiho-tang), has beneficial effects for the control of migraine and improvement in the quality of life.
