The Odontogenic Keratocysts : A Consideration of the Clinical and Radiologic Features (치성각화낭 : 임상 및 방사선학적 고찰)

  • Purpose: To determine whether the significant relationship exists between radiographic appearance, whether it is unilocular or multilocular, and its corresponding clinical and histologic features by examining the odontogenic keratocyst clinically, radiologically and histologically. Materials and Methods: This study was conducted on 48 cases of odontogenic keratocyst from the files of Dental Hospital, Yonsei University for the years 1982 through 1995. Results and Conclusions : The mean age of patients was 30.5 years in the unilocular group and 35.5 years in the multilocular group. The male to female ratio was 1 : 1.06 in the unilocular group and 1 : 1.75 in the multilocular group. The chief complaint was swelling in both groups(unilocular 35.1 %, multilocular 54.5%). In the occurrence site, the border of the lesion, the displacement and external root resorption of the adjacent teeth, there were no statistically significant difference between the unilocular group and multilocular group, but in the border type, there was statistically significant difference (x²-test, p<0.05)wo recurred cases were observed among 11 cases of odontogenic keratocyst. One was unilocular case and the other was multilocular case.

Giant Cell Tumor of Tendon sheath in the Foot (족부에 발생한 건막 거대 세포종)

  • Purpose: We evaluate the clinical, radiologic and pathologic features of giant cell tumor of tendon sheath (GCTTS) in the foot Materials and Methods: Twelve cases of GCTTS excised from foot region, at our hospital from 1999 to 2002, were analyzed. The mean duration of follow up was 19 months. The age and sex of the patient, location and size of the lesion, symptom as well as radiologic findings were evaluated. Results: The most common symptom was painless mass in 7 patients. Three patients had pain sympton and 2 patients had tenderness. The mean duration from identification of the mass to excision was 14 months. Seven cases were located in the forefoot, most commonly in the big toe with 4 cases, 2 cases in the midfoot and 3 cases in the hindfoot. The average diameter along the long axis was 2.8cm. Conclusion: GCTTS in the foot was more common in the big toe and also plantar side same as in the hand. But bony erosion and pain were more frequent than in the hand.

  • The Tolosa-Hunt syndrome is one of the rare disease with facial, especially peri-orbital pain, and ophthalomoplegia associated mostly with granulomatous lesions in cavernous sinus or superior orbital fissure. In addition to ophthalmoplegia by multiple cranial nerve involvement, the sympathetic nervous system may also be involved leading to Horner's syndrome. A typical Tolosa-Hunt syndrome has a neuro-radiologic finding of an increased density in the involved region, and a laboratory finding of an elevated ESR, as well as a dramatic response to systemic corticosteroid therapy. An unusual case of the Tolosa-Hunt syndrome with normal radiologic and laboratory findings, unresponsive to systemic corticosteroid, and some response of pain relief to a stellate ganglion block, is presented.

Correlations between the Clinical Results and Radiologic Evaluation after Surgical Treatment of Calcaneal Fracture (종골 골절의 수술적 치료 후 방사선학적 평가와 임상적 결과의 상관 관계: 종골 골절의 술 후 방사선학적인 평가)

  • Purpose: The purpose of this study is to find out the correlation factors for prognosis of calcaneal fractures. Materials and Methods: 120 cases (feet) of calcaneal fractures, all of them followed up for more than 1 year after surgical procedures, consisted of 101 men (105 feet) and 13 women (15 feet) were reviewed retrospectively. The collected clinical data were as follows : injury mechanism, surgical procedures, time to procedure, time to work and the radiologic data: Bohler angle, heel width, displacement of posterior facet. AOFAS hindfoot score and VAS score were checked. With ANOVA test and multiple regression analysis, the data processed statistically. Results: According Sanders classification, type II was 37 cases (31%), type III 66 cases (55%), and type IV 17 cases (14%). On plane radiography, the Bohler angle improved to average 28.4 degree from 5.6 degree, and the displacement of posterior facet was corrected to average 1.2 mm. AOFAS hindfoot score was checked average 81.7 points postoperatively, and the meaningful difference existed between types of Sanders classification. The Bohler angle represented the outline of the calcaneus had the better correlation with the clinical outcome of calcaneal fractures rather than the anatomical reduction of the posterior facet did. And the width of calcaneus had good correlation with the clinical score. Conclusion: We should also concern about the outline of calcaneus, the width of calcaneus and the Bohler angle representing anatomical reduction, not only the acute reduction of the posterior facet.

A STUDY ON THE RELATIONSHIP BETWEEN RADIOLOGIC CLASSIFICATION AND GLYCOSAMINOGLYCAN ANALYSIS OF CYSTIC FLUIDS IN ORAL REGION (구강영역에서 발생된 낭의 방사선학적 분류에 따른 낭액내 glycosaminoglycan 성분의 비교 연구)

  • This study was designed to evaluate the correlationship between radiologic classifications of cysts in oral region and glycosaminoglycan analysis of cystic fluids using cellulose acetate electrophoresis. The materials for this study consisted of 37 cases-8 periapical cysts, 10 dentigerous cysts, 10 primordial cysts, 2 residual cysts, 3 incisive canal cysts, 2 post-operative maxillary cysts, 1 mucocele on maxillary sinus, & 1 unicystic ameloblastoma-diagnosed as cystic lesions radiologically. The obtained results were as follows: 1. At the stepwise discriminant analysis, four variables-low mobility material, heparin, hyaluronic acid, & dermatan sulfate-were used to define diagnostic model for the odotogenic cyst. The model produced a sensitivity of 100% and a specificity of 85%. 2. The intensities of heparin and chondroitin-4-sulfate were greater in dentigerous cyst than periapical cyst(p<0.05). The intensity of chondroitin-4-sulfate was greater in primordial cyst than in periapical cyst(p<0.05). 3. It showed no statistically significant difference in glycosaminoglycan of the cystic fluids between dentigerous cyst and primordial cyst(p>0.05). 4. On the fluids of the cysts originated from maxillary sinus, there were especially high intensities of heparin and dermatan sulfate, and low intensity of chondroitin-4-sulfate. 5. On the fluids of unicystic ameloblastoma, there were high intensity of dermatan sulfate and low intenity of chondroitin-4-sulfate.

Clinical Significance of Thoracoscopy on Spontaneous Pneumothorax (자연기흉에 대한 Thoracoscopy 의 임상적의의)

  • The thoracoscopic study was reported on 21 cases of spontaneous pneumothorax requiring surgical management, and clinical values of thoracoscopic examination on spontaneous pneumothorax were also discussed. patients were treated in the Department of .Thoracic Surgery, Hanyang University Hospital for the period of two Years from May 1972 to April 1974. For exact detection of etiologic factors on spontaneous pneumothorax, the thoracoscopic examination in the intrapleural space was performed in parallel with X-ray study. this study, the difference of diagnostic and therapeutic significance between radiological and thoracoscopic findings were observed and compared simultaneously. The results are summerized as follows: Patients age was distributed between 3 and 70 years old with highest incidence in the age group of sixty decade [33. 3%], and sex ratio of male to female was 5:2. The tuberculous processes which developed superficial subpleural layer in the lung parenchyme, on the pulmonary surface could be observed by thoracoscopic examination in a characteristic picture. detection ratio of pulmonary tuberculosis by the radiologic study to that by thoracoscopy was 8:2. The adhesion between the visceral and the parietal pleura which could possibly make a rupture of the alveola and the visceral pleura was found to be localized in a small area of the lung surface. The other part of the lung surface was free of the adhesion and, therefore, the movement of the lung took place completely without any difficulty. The ruptured orifice of the pleura and pathological changes surrounding the orifice can be detected by thoracoscopy, but not by other means such as radiologic examination. A single tuberculous bleb and multiple emphysematous blebs were found on 6 cases out of 21 cases of spontaneous pneumothorax. Among these cases, radiologic Study revealed the bleb only in one patient. On the other hand, the blebs were found in all the six patients by means of thoracoscopic examination. It gives the detection ratio of bleb by radiologic study to that by thoracoscopy was 1:6. By thoracoscopy, the rupture on the lung surface were visualized on the 10 patients out of a total of 21 patients [10 patients of visual rupture]. However, the rupture of the pleura was not observed on the rest of 11 patients even by thoracoscopic examination [11 patients of non visual rupture]. Five patients [50%] out of ten who had the visual rupture on the lung surface was required a surgical operation to remove pneumothorax. For the patients who were detected to have the visual rupture of the pleura by thoracoscopy, be considered in the early stage of closed thoracostomy. of 21 patients, 16 patients [11 patients of non visual rupture of the pleura and 5 patients of visual rupture of the pleura] who received no surgical management, were treated with closed thoracostomy with continuous suction, and the` pneumothorax was healed completely up in each cases. Therapeutic measures for the remaining 5 patients of visual rupture of the pleura who were subjected to surgical approach for radical treatment of spontaneous pneumothorax were accordingly complicated, and the following different procedures were properly indicated case by case, that is, rib resection thoracostomy, simple closure of ruptured visceral pleura, wedged resection of the lung, and lobectomy.

  • Infectious and inflammatory bone diseases include a wide range of disease process, depending on the patient's age, location of infection, various causative organisms, duration from symtom onset, accompanied fracture or prior surgery, prosthesis insertion, and underlying systemic disease such as diabetes, etc. Bone infection may induce massive destruction of bones and joints, results in functional reduction and disability. The key to successful management is early diagnosis and proper treatment. Various radionuclide imaging methods including three phase bone scan, Ga-67 scan, WBC scan, and combined imaging techniques such as bone/Ga-67 scan, WBC/bone marrow scan add complementary role to the radiologic imaging modalities including plain radiography, CT and MRI. F-18 FDG PET imaging also has recently been introduced in diagnosis of infected prosthesis and chronic active osteomyelitis. Selection of proper nuclear medicine imaging method will improve the diagnostic accuracy of infections and inflammatory bone diseases, based on understading of pathogenesis and radiologic imaging findings.

Suggestion for Improper Radiologic Examination Using Ionizing Radiation (전리방사선을 이용한 영상 검사의 부적절한 이용에 대한 제언)

  • The radiation emitted during radiological examinations can be harmful to the human body, but the technique is still used because it is helpful to the patient. Therefore, radiologic examinations must follow radiation protection principles such as justification and optimization, and only minimal required re-examinations should be performed. Proper use of radiation in the medical field can be achieved by proper education of individuals from multiple medical fields.

Diagnosis, Clinical Course and Treatment of Hypersensitivity Pneumonitis (과민성폐렴의 진단, 임상 경과 및 치료)

  • Hypersensitivity pneumonitis (HP) is an inflammatory and fibrosing interstitial lung disease caused by inhaled antigens in susceptible individuals. Various environmental materials can serve as inciting agents of HP. Recent guidelines have defined typical radiologic and histopathologic findings of HP, classifying it into nonfibrotic and fibrotic HP categories, as this classification better correlates with clinical outcomes. Diagnosing HP poses challenges, necessitating multidisciplinary discussions based on clinical assessments and radiologic or histopathological features. The cornerstone of treatment lies in avoiding the causative antigen. Oral glucocorticoids serve as the initial treatment for symptomatic HP. Immunosuppressants may be employed in patients who do not respond to initial treatment, and antifibrotics could be considered for patients displaying progressive fibrosis. This review outlines the clinical features, diagnostic evaluation, and treatment of HP.

A Case Report on the Traumatic Pneumothorax associated with Acupuncture (자침과 관련된 외상성 기흉 1례)

  • Objective : There was no report on traumatic pneumothorax associated with acupuncture in korea yet, although many people guess at its possibility. In order to make known that acupuncture can cause pneumothorax in reality and to prevent more acupuncture-related adverse effect cases in future, this report was made. Methods : Close observation was done on clinical symptoms & radiologic examination of acupuncture-related traumatic pneumothorax patient. Results : During acupuncture therapy on neck stiffness & shoulder pain, a acupuncture needle was inserted too deeply into upper part of thorax unintentionally, patient showed dyspnea, chest pain & discomfort, cough, sweating and mild rupture of right lung in chest radiologic findings, and recovered after 2 weeks of conservative treatment. Conclusion : In order to prevent traumatic pneumothorax during acupuncture therapy, needling into thorax sometimes requires tilted acupuncture needle inserting under consideration of needle width & length. At least 0.40mm width of needle is generally considered appropriate in needling into taut band of thoracic muscles, because too thin and long needle sometimes can be bent and inserted into wrong way, which can cause adverse effects like pneumothorax.
