Quantitative sudomotor axon reflex test (QSART) as a diagnostic tool of small fiber neuropathy

  • Small fiber neuropathy is a painful neuropathy that cannot be assessed using nerve conduction studies. A skin biopsy and quantitative sensory testing (QST) are the gold standards for small fiber neuropathy diagnosis. However, a skin biopsy is invasive and commercially unavailable in Korea. QST is a method involving a thermal threshold, but its results can be affected by cognition as well as lesions of the central nervous system. Quantitative sudomotor axon reflex test (QSART) is a quantitative method of assessing sweat glands innervated by small fibers. In this review, we assessed the utility of QSART in evaluating small fiber neuropathy.

슬랙스 형태 적합성 평가의 정량적 평가 기준 설정 (Establishing Quantitative Evaluation Standards for the Shape Fitness test of Slacks)

  • This study presents quantitative evaluation standards for the shape fitness test, conducted in the process of the slacks fit test. This study aims to quantify appearance sensory test of slacks for the objectification of qualitative evaluation methods in existing shape fitness tests. The subjects were women of standard body type between the ages of 18 and 24. Slacks differing in their ease in waist girth, hip girth, crotch length and knee length were designed to test their shape fitness. The gap volume distribution between the body and slack, angle of the baseline by body part, and ratio of total thickness from the sideline using 3D human body scan data (which reflect the results of the appearance sensory test) were presented as quantitative evaluation standards. There were less wrinkles in the wrinkle/overstretch and ease categories of the appearance sensory test; in addition, ease was adequate and small enough to comfortably perform basic human activities in the standing upright posture. The gap volume distribution analysis between body and slacks showed that curves increased in the slacks surface along with an increase of ease in slacks and suggested that wrinkles also increased. In the baseline's horizontal categories in terms of a $0^{\circ}$ horizontal angle, the range of angles were evaluated as adequate by the clothing expert group. The total thickness ratio of 0.5:0.5 divides into the sideline from the baseline.

뇌졸중 환자의 상지부종에 대한 정량적 감각 평가 (Quantitative sensory Testing on Edema of Upper Extremity for Stroke Patients)

  • 본 연구는 뇌졸중 편마비 환자를 대상으로 비마비측과 상지부종이 있는 마비측의 정량적 감각 평가(QST)를 비교함으로서 부종 관리 및 치료에 대한 기초자료를 제공하고자 하였다. 연구의 목적에 맞는 아급성기 뇌졸중 환자I군(n=15)과 만성기 뇌졸중 환자II군(n=17)을 대상으로 비마비측과 마비측에 대한 상지용적의 차이, 기계적 동통 역치(MPT), 냉감역치(CST), 온감역치(WST), 냉통증역치(CPT), 온통증역치(WPT), 진동감각 역치(VST)에 대한 정량적 감각 평가(QST)를 실시하였다. 그 결과, 각 군내 비교 시 모든 측정 항목에서 유의한 차이가 나타났고(p<.001), I군과 II군의 군간 마비측 감각 평가 비교에서는 기계적 동통 역치(MPT)를 제외한 모든 측정 항목에서 유의한 차이가 나타났다(p<.05). 아급성기에는 만성기에 비해 상지부종으로 감각역치 증가로 인한 감각저하가 초래되었음을 확인할 수 있었다. 뇌졸중 환자의 부종에 따른 감각적 특성 차이를 정량적 평가를 통해 알아보고자 하였다.

Effect of Neuro-Feedback Training and Transcutaneous Electrical Nerve Stimulation (TENS) in Stress, Quantitative Sensory Threshold, Pain on Tension Type Headache

  • Purpose: The objective of this study is to evaluate the effect of neuro-feedback training and transcutaneous electrical nerve stimulation (TENS) on stress, quantitative sensory threshold and pain in patients suffering from tension type headache. Methods: 22 participants who passed the preliminary evaluation were enrolled in the study and 11 participants were randomly assigned to each group. The control group (n=11) was subject to the TENS treatment of which was composed of a 20-minute session for 5 times a week during 4 weeks, and the experimental group (n=11) was subject to both neuro feedback training and TENS treatment for 10 minutes a day and 5 days a week during 4 weeks. The Perceived Stress Scale (PSS) was used to measure a level of stress and the quantitative sensory testing (QST) was used for the measurement of cold pain threshold (CPT) and heat pain threshold (HPT); A degree of pain was evaluated through the headache impact test-6 (HIT-6). Results: In comparision of all dependent variables between the control and subject groups, there were significant differences in stress, quantitative sensory threshold and pain after the treatment (p<0.05), and the experimental group showed significant differences in stress, CPT, HPT and pain (p<0.05) and the control group showed only a significant difference in HPT (p<0.05). Conclusion: Findings of this study demonstrate that the concomitant administration of the TENS treatment and neuro feedback training is effective on alleviation of stress, quantitative sensory threshold and pain in patients with tension type headache.

광용적맥파와 정량적 감각검사의 비교를 통한 진단적 유용성 (Diagnostic Usefulness of Quantitative Sensory Test Comparison with Photoplethysmography)

  • 본 논문의 목적은 당뇨병 및 신경병증의 병력이 없는 17명의 일반 성인을 대상으로 한 정량적 감각검사(QST)와 광용적맥파(PPG)와의 비교를 통해 정량적 감각검사의 임상적 유용성을 파악하는 것이며, 광용적맥파 검사를 이용한 혈당수치에 따른 자율신경계의 부교감신경의 변화를 살펴보는 것이다. 일반인을 대상으로 혈당검사와 광용적맥파 검사를 실시하여 상관관계를 분석한 결과 혈당수치에 따라 자율신경계 부교감신경이 느려지는 상관관계를 보였다. 이상의 결과에서 정량적 감각 검사가 주관적 평가에 의한 검사이기는 하나 검사의 민감도를 신뢰할 수 있다고 보이며 따라서 말초 신경의 평가도구로서 정량적 감각검사가 당뇨병성 신경병증 등의 진단의 민감도를 높이는데 폭넓게 이용될 수 있을 것으로 보이며, 광용적맥파 검사가 심박변화율에 의한 자율신경계를 평가하는 후속연구에 기초자료연구로 이용될 수 있을 것으로 사료된다.

Orofacial Thermal Quantitative Sensory Testing (QST): A Study of Healthy Korean Women and Sex Difference

  • Purpose: Thermal sensory test as an essential part of quantitative sensory testing (QST) has been recognized as a useful tool in the evaluation of the trigeminal nerve function. Normative data in the orofacial region have been reported but the data on differences in the test site, sex and ethnicity are still insufficient. Thus, this study aimed to investigate the normal range of orofacial thermal QST data in the healthy Korean women, and assess sex difference of thermal perception in the orofacial regions. Methods: Thermal QST was conducted on 20 healthy women participants (mean age, 26.4 years; range, 21 to 34 years). The thermal thresholds (cold detection threshold, CDT; warm detection threshold, WDT; cold pain threshold, CPT; and heat pain threshold, HPT) were measured bilaterally at the 5 trigeminal sites (the forehead, cheek, mentum, lower lip and tongue tip). The normative thermal thresholds of women in the orofacial region were evaluated using one-way ANOVA and compared with the previously reported data from age- and site-matched 30 healthy men (mean age, 26.1 years; range, 23 to 32 years) using two-way ANOVA. One experienced operator performed the tests of both sexes and all tests were done in the same condition except the time variability. Results: Women showed significant site differences for the CDT (p<0.001), WDT (p<0.001), and HPT (p=0.047) in the orofacial region. The CDT (p<0.001) and the CPT (p=0.007) presented significant sex difference unlike the WDT and the HPT. Conclusions: The thermal sensory evaluation in the orofacial region should be considered in the context of site and sex and the normative data in this study could be useful for assessment of the sensory abnormalities in the clinical setting.

체성감각유발전위검사에 대한 고찰 (Review of Somato Sensory Evoke Potential Test)

  • The SSEP(SomatoSensory Evoke Potentials) test is a valid and repeatable technique which correlates with clinically assessed joint position & vibration sense, skin touch & pressure sense. Also SSEP study is a simple and quantitative test, and has been used to evaluate the sensoty system along the somatosensory pathway from peripheral sensory receptor to the cortex. The ascending pathway of SSEP has been know to be posterior column-lemniscal pathway, but not without controversy. There are two kind of test mathods : one of test is median nerve SSEP and other test is posterior tibial nerve SSEP. Recently, SSEP used to performed to evaluate the usefulness of dermatomal SSEP(D-SSEP) and segmental SSEP(5-SSEP) for the diagnosis of lumbasacral radiculopathy, and it can be measure of ingual ahd palatine evoked potentials & indicator of medullary function useful for the diagnosis of brain death.

전기자극수준이 자외선에 의한 홍반의 정량적 감각검사에 미치는 효과 (Effect of Electrical Stimulation Level on Quantitative Sensory Test Induced Erythema by UV Radiation)

  • Purpose : This study is to investigate the modulatory effects to the ultraviolet induced erythema of pain processing system. Methods : Thirty six healthy volunteers were divided into none treatment group (n=6), indomethacine group (n=6), subsensory level electrical stimulation group (n=6), sensory level electrical stimulation group (n=6), motor level electrical stimulation group (n=6), noxious level electrical stimulation group (n=6). Subjects were induced erythema for three times minimal erythema dose (MED) at upper arm of dermatome C6 level. Each experimental group had mechanical pain threshold (MPT), electrical pain threshold (EPT), thermal pain threshold (TPT). Results : This study revealed that we observed that pain thresholds were significantly correlated with each other in pain processing system. The effect of electrical stimulation levels evaluates were shown to be significant differences pain control effect in electrical stimulation group (sensory, motor level electrical stimulation groups) more than indomethacine group, subsensory level and control group. Conclusion : In this study, it was found that the effect of ultraviolet induced erythema of pain control by modulatory electrical stimulation.

묘사분석 및 소비자 조사에 의한 참외의 관능특성 (Sensory properties of oriental melon (Cucumis melo var. makuwa) byquantitative descriptive analysis and consumer test)

  • 본 연구는 친환경 참외의 품질 등급 설정을 위한 기초자료 제공을 위하여 성주지역에서 재배되는 참외의 묘사적 관능 특성 개발과 재배 방법별 품질 지표의 품질 특성을 조사하였다. 성주산 참외의 묘사분석 결과 총 40개가 도출되었는데, 외과피에서 도출된 특성 6종과 향 특성 2종 및 중과피의 외관 특성 3종, 내과피 (endocarp) 외관 특성 4종, 또 중과피의 향 특성 5종과 내과피의 씨부분의 특성 3종이 도출되었다. 이외에 중과피의 맛 7종 및 조직감 특성 6종이었다. 한편 유기농 재배와 관행 재배된 참외의 pH, 총산도, 고형분 함량 등이 전반적으로 재배 방법과 재배 농가에 따라 유의적인 차이를 보였다. 또 이들 시료를 대상으로 묘사 분석 및 일반 소비자 대상으로 관능검사를 실시한 결과 전반적으로 관행재배 시료와 유기농 재배 시료 간에 차이가 있었고, 재배 농가별로 차이가 있다고 분석되었다. 묘사분석에 의한 외관특성과 참외 중과피와 내과피의 품질 특성들의 주성분 분석결과 첫 번째 주성분(F1)과 두 번째 주성분(F2)에 의한 총설명력은 각각 58.36%, 46.18%였다. 전반적으로 과피가 포함된 참외 시료의 경우는 외관에 의해 재배 방법과 재배 농가별로 비교적 차이가 뚜렷하였으나, 과피가 제거된 중과피와 내과피의 관능특성 항목에서는 관행재배(CC)를 제외하고는 농가별 시료는 확실하게 구분되지 않았다. 한편 소비자 조사 후 관능적 품질 특성의 주성분 분석 결과가 묘사 분석 결과와 유사하였고, 전체적인 기호도와 관련이 있는 관능적 품질 특성은 단맛, 단향, 참외 고유의 향과 맛, 참외 껍질의 노란색, 아삭함, 다즙성, 경도가 중요한 영향 요인이었다.

묘사분석을 이용한 쌀 과자의 관능적 특성 연구 (Sensory Characteristics of Rice Confections by Descriptive Analysis)

  • The objective of this study was to determine sensory profiles of rice confections. The samples used in this study obtained from Korea (traditional Korea rice snack and local specialty rice snack) and three countries (USA, Japan, and China) were evaluated and compared. The sensory characteristics of five kinds of rice confections were evaluated using a sensory test and were analyzed via quantitative description analysis (QDA), principal component analysis (PCA), and hierarchical cluster analysis (HCA). In the descriptive analysis, 10 trained panelists evaluated sensory characteristics consisting of 19 attributes, and there were significant differences (p<0.05) among the 16 characteristics. For the descriptive data, multivariate analysis of variance was carried out and identified differences among the samples. The PCA of rice confections for the first two principal components could explain 85.66% of the variations. The Korean, Japanese, and Chinese rice confections were savory, gritty, and particle-sized, the other Korean local specialty rice confections were fruity, sweet, honey-flavored, compact, and crispy, and those from the USA were glossy, grainy, bright, adhesive, cohesive, crispy, and sweet.
