Association Between Unplanned and Planned Readmissions in an University Hospital (비예정과 예정된 재입원 환자들간의 관련 요인 분석)

  • This study describes associated factors of readmission of 213 inpatients from an university hospital in Seoul. This retrospective study reviewed medical records of patients who discharged from a hospital stay for general diseases between 1 August 1995 and 31 October 1995, Cases were 68 discharge patients with an unplanned readmission within 30 days of discharge from an index stay. And the other cases are 145 patients who had more than two discharges and didn't have an unplanned readmission within 30 days. Logistic regression model was analyzed and the results were as follows; 1. duration of readmission, rate of unpayed, room, path, and risk of disease were more likely to be readmitted unexpectedly than the expected readmission patients. 2. early readmission, low risk condition group, and inadquateness of discharge plann for patients had unplanned radmissions rather than planned readmissions. Therefore, discharge planning education to health care provider is required and assessement of discharge planning should be evaluated. Readmissions are usually for related problems that arose during the original hopitaliztion and caused cost problems. Especially the unplanned readmissions are frequently preventable. Ultimately, models for readmissions can serve as a valuable clinical tool for target high-risk patients and older patients and with this kind of tools we can reduce hospital readmissions and maintain high-quality of inpatient care.

A Comparative Study of Maternal Satisfaction with the Mode of Newborn Care Education - Videotaped vs. Verbal - (신생아실의 퇴원교육 방법에 따른 산모의 만족도 비교 연구)

  • Background : This study was conducted to compare the level of satisfaction between mothers who were educated on the care of newborns by video-taped method and those by verbal one before discharge. Methods : Fifty-two mother (verbal method group) from a free standing OB & GY hospital in Wonju City and forty-seven mothers (videotaped method group) from the OB & GY section of Wonju Christian Hospital(WCH) were selected for a study sample. The knowledge and understanding of newborn care and the degree of maternal satisfaction were assessed after the education by self-administered questionnaires. The opinions of the 11 nurses of WCH who were involved in the education were also collected by a questionnaire. Results : With respect of the level of understanding and satisfaction with the discharge education on newborn care, there is no significant difference between the videotaped educated group and verbal educated one. The correlation analysis of the video education group revealed a strong correlation between the score of understanding and satisfaction with the discharge preparation(r=0.409, p=0.01), and between the satisfaction scores for discharge preparation and discharge education(r=0.844, p=0.001). As for the verbal education group, the score of understanding also correlated significantly with both the score of satisfaction with the discharge education(r=0.272, p=0.001) and the discharge preparation(r=.680, p=0.001). Conclusions : This study showed that the discharge education to mothers regarding newborn care by a videotaped method does not improve the understanding or satisfaction of the mothers. However, the response to the mode of discharge education varies by the characteristics of the mothers. In conclusion, the characteristics of the mother should be considered in order to improve the understanding and knowledge of newborn care when the mode discharge education is chosen.

Influence of stress on the oral health and quality of life of university students (일부 대학생의 스트레스가 구강건강과 삶의 질에 미치는 영향)

  • Objectives: The purpose of this study was to analyze Influence of stress on oral health and quality of life. Methods: The participants were 172 students from 4 universities located in DaeJeon, South Korea. A questionnaire was administered, assessing the impacts of the general characteristics of the participants, temporomandibular joint (TMJ) symptoms, xerostomia, and halitosis on stress. And oral impacts on daily performance were also investigated. Data were analyzed using the Statistical Package for the Social Sciences 18.0. Results: The level of stress was higher in female, and the causes of stress was reported scholastic achievement, relations with professors, get a job. The students who were higher stress were more conscious of TMJ symptoms, xerostomia and halitosis(p<0.05). Stress was significantly positively correlated with TMJ symptoms (r=0.376, p<0.001), dry mouth (r=0.360, p<0.001), and bad breath (r=0.343, p<0.001). Oral Impacts on Daily Performance (OIDP) scores were significantly positively correlated with dry mouth, bad breath, TMJ symptoms, and stress. Regression analysis showed that 54.5% of the independent variables were related to OIDP scores. Stress and TMJ symptoms exerted a significant effect on OIDP scores. Conclusions: Stress among university students affects oral health and quality of life. Stress coping mechanisms and oral health care education programs should be developed and applied at universities.

Study on the herbology test items in Korean medicine education using Item Response Theory (문항반응이론을 활용한 한의학 교육에서 본초학 시험문항에 대한 연구)

  • Objectives : The evaluation of academic achievement is pivotal for establishing accurate direction and adequate level of medical education. The purpose of this study was to firstly establish innovative item analysis technique of Item Response Theory (IRT) for analyzing multiple-choice test of herbology in the traditional Korean medicine education which has not been available for the difficulty of test theory and statistical calculation. Methods : The answers of 390 students (2012-2018) to the 14 item herbology test in college of Korean medicine were used for the item analysis. As for the multidimensional analysis of item characteristics, difficulty, discrimination, and guessing parameters along with item-total correlation and percentage of correct answer were calculated using Classical Test Theory (CTT) and IRT. Results : The validity parameters of strong and weak items were illustrated in multiple perspectives. There were 4 items with six acceptable index scores, and 5 items with only one acceptable index score. The item discrimination of IRT was found to have no significant correlation with difficulty and discrimination indices of CTT emphasizing attention of professionals of medical education as for the test credibility. Conclusion : The critical suggestions for the development, utilization and revision of test items in the e-learning and evidence-based Teaching era were made based on the results of item analysis using IRT. The current study would firstly provide foundation for upgrading the quality of Korean medicine education using test theory.

A review of the qualification criteria for the national examinations for emergency medical technicians (응급구조사 국가시험 응시 자격기준에 관한 고찰)

  • Purpose: Emergency medical technicians (EMTs) have so far been trained as professionals under the same conditions, with no change in the 1995 Act. We aimed to find ways for them to secure expertise in accordance with social needs by strengthening the quality of the EMT education. Methods: This is a descriptive study comparing the operation status of the national emergency medical examination conducted by the Korea Health Personnel Licensing Examination Institute, and the national examinations of EMT paramedic and EMT basic. The scope of the national examinations for EMT was compared by subject and area. Results: The national written exam for EMT paramedic consists of five areas. EMT basic does not include basic medicine in three related subjects, 11 areas, and 18 detail areas. Paramedic care does not include advanced pediatric life support. In addition, nine areas and 20 detail areas are not included. Conclusion: The study suggests the need for institutional supplementation so that those who have completed EMT basic and the subjects prescribed by the ordinance of the Ministry of Health and Welfare at universities, etc., in the Higher Education Act can take the EMT paramedic national exam.

Physicians' behavior and attitude toward Review system of National Health Insurance claim in Korea (진료비 심사제도에 대한 개원의 들의 태도 및 만족도)

  • The purposes of this study are to understand the doctors' attitude and satisfaction about the review system of national health insurance claim in Korea and to suggest the way to improve this system This study conducted a survey of the doctors registered in the medical association in Seoul city. The survey was performed as a form of self-administered questionnaire from January 2004 to February 2004. The contents of questionnaire dealt with doctors' attitude and satisfaction about the review system of medical service claim. Totally, 1,037 members replied to our survey and we analysed 981 doctors' data, excluding incomplete responses. As a result, 89.7% of repliers showed a negative attitude about the influences of the review system on improvement of medical service quality, 98.0% of repliers have had experiences that they have given distorted insufficient medical services in order to evade the curtailment of service claim. Also, 91.6% of repliers stated that they have had experiences of intentional modification or alteration of diagnostic code to shun the curtailment. Most of the doctors showed negative attitude to the curtailment procedure and the review system of service claim originally intended to be one of the quality control methods of medical service in Korea also, the development of both scientific and reasonable parameters and criteria for claim is needed. 'Through the improvement of review system for appropriate medical service, there is a need of a way to increase the satisfaction of medical service providers, and to encourage the motivation for quality control. Also, education is strongly needed to provide doctors with sufficient information about review criteria and curtailment cases.

  • Recently several hospitals have established a new nursing position so called, 'the nurse in charge of education (NCE).' The job description of NCE is to give a detail explanation on examination or test introduced to patients or a guide for those who are not familiar to hospital facilities at the out-patients level. The motivation of NCE position is the quality improvement in patient education on the sophisticated procedures or the follow up care for medical purposes to improve their compliances, as well in delivering services for outpatients or visitors in more informative and efficient way. The application of NCEA has been turned out positive and effective in patient satisfaction and unit management. However, special attention is brought to the scope and depth of the contents of education delivered by NCE which might overlap with the duty of physicians by the Medical Act. It is needed to clarify the role and job description of NCE in the context of Medical Act. The engagement of NCE to the Advanced Nursing Practitioner (ANP) is one of the possible solutions for a duty charge on patient education since ANP is a legislatively official position with higher license of RN at master level. Further discussion is needed to elaborate and arrange the details on the scope and content of patent education among health science professionals including RN, ANP and physicians.

Status of non-face-to-face learning at selected regional universities for paramedicine since the coronavirus infectious disease 2019 outbreak: a cross-sectional survey on undergraduates (코로나19 발생 이후 일부 지역대학 응급구조학과의 비대면 교육 현황)

  • Purpose: This study aimed to investigate the current status of non-face-to-face education at universities for paramedicine and measure students' education satisfaction after the coronavirus infectious disease 2019 (COVID-19) outbreak. Methods: A cross-sectional survey was conducted for paramedic students at the Chungcheong and Honam areas using Google Forms. Convenience sampling was used. A structured questionnaire was created and modified according to Park and Choi's test tool developed to review online lectures and practical courses. Results: A total of 202 students responded to the survey. The satisfaction level of online lectures was 3.06±1.12 (n=202) out of 5. Students experiencing online lectures responded that it was difficult to focus on the class, and the overall quality and lecture delivery should be improved. They also experienced technical difficulties. The satisfaction level of practical course lectures was 3.24±1.04 (n=133) out of 5. It was higher than those of other types of classes because it was conducted by the more familiar face-to-face lecture. Conclusion: This study has shown that the universities and instructors have examined a variety of methods in paramedic education after the COVID-19 pandemic. However, further research and consideration are required to improve paramedic education during the COVID-19 pandemic.

Predictors of Total Quality Management in Health-Care Organizations (의료기관에서의 종합적 품질경영과 관련된 변인분석)

  • The study was to examine the relationships among those variables related to total quality management and to reveal those variables affecting total quality management in Health-Care Organizations. To study the relationship, a questionaire was designed and sent to 220 hospitals of which the number of beds is over 200 beds. Out of 660 questionaire, 263 questionaire were collected. Data analysis were conducted by using t-test, ANOVA, Pearson correlation coefficients and stepwise multiple regression. The result were as follows: 1. TQM was significantly different according to activities of QI, department of QI, age, and position. 2. Leadership was significantly different according to activities of QI, department of QI, age, and position. 3. Significant differences in the level of satisfaction of employee were found according to activities of QI, department of QI, age, position, and duration of employ. 4. Significant differences in the level of participation of employee were found according to activities of QI, department of QI, and position. 5. Education trainning was significantly different according to amount of bed, activities of QI, department of QI, level of education, kind of job, and position. 6. Medical technique was significantly different according to activities of QI, department of QI, and position. 7. Quality of work was significantly different according to activities of QI, department of QI, age, and position. 8. Significant differences in the level of investigation's system in satisfaction of client were found according to activities of QI, department of QI, kind of job, and position. 9. Positive correlations were observed between TQM and those variables related to TQM. 10. TQM was significantly predicted by the level of satisfaction of employee(40.9%), participation of employee(7.0%), investigation's system in satisfaction of client(2.7%), leadership(1.4%), and medical technique(1.1%), respectively.

The Effects of Nutrition Education and Regular Exercise on Nutritional Status, Quality of Life and Fatigue in Hemodialysis Patients (영양교육과 규칙적인 운동이 혈액투석환자의 영양상태와 삶의 질 및 피로도에 미치는 효과)

  • The purpose of this study was to investigate the effects of a nutrition education and exercise on nutritional status in hemodialysis patients, with regard to quality of life (QoL), and fatigue. The subjects were divided into two groups : an educated (E) group (11 men and 9 women) and a non-educated (NE) group (7 men and 22 women). The educated group received a nutrition education and a regular exercise program for 3 months. Data on anthropometric indices and biochemical parameters were obtained from medical records. Dietary habits and nutrient intake were assessed through questionnaires and three-day food records. QoL (through the SF-36 questionnaire) and fatigue (measured by a visual analogue scale questionnaire) were assessed before and after the interventions. Dietary habit scores regarding food variety and fluid intake control significantly increased (P<0.05) after the nutrition education. Furthermore, the intake of total energy, carbohydrates, Fe, and vitamin A significantly increased (P<0.05). Vitamin C intake also significantly increased (P<0.01). Hb levels decreased whereas TG levels increased in the NE group. BUN levels decreased in the E group. After the nutrition education, QoL aspects, such as feelings of pain and general health, and the total score significantly increased (P<0.05) and the level of fatigue score decreased (P<0.01). The results of this study indicate that nutrition education and exercise for hemodialysis patients provides changes in dietary habit, daily nutrient intake, biochemical parameters, QoL, and the level of fatigue. Therefore, nutrition education and exercise help improve nutritional status and QoL.
