Development of Easily Chewable and Swallowable Korean Barbecue Beef for the Aged (저작·연하 용이 소불고기 노인식 개발)

  • This study was conducted to develop for the elderly food that is safe, well-shaped, and easy to chew and swallow, using gelification. Gelatin, which has low adhesiveness, thereby lowering the possibility of being swallowed wrongly into the respiratory track, was used as a viscosity agent for the gelification. Water and gelatin amounts were adjusted to facilitate breaking the food with the tongue. Various foods for the elderly with chewing and swallowing difficulties were used for sensory assessment. Sensory panel consisted of 10 dietitians (10 women) in nursing care facilities. The sensory optimal composite recipes were determined by central composite design (CCD). The sensory measurements were significantly different in appearance (p<0.05), saltiness (p<0.05), sweetness (p<0.01), and overall quality (p<0.05). The optimum formulation of the Korean barbecue beef calculated by numerical and graphical method was 3.71 g of sugar and 19.53 g of soy sauce. Moisture content, hardness and adhesiveness of the Korean barbecue beef were 78.85%, 2.40, -1.87, respectively. The result shows that food for the elderly, which is easy to chew and swallow, using gelification will have sufficient competitiveness in terms of safety, taste, and preference. The development of food for the elderly that take into account the difficulties in chewing and swallowing among the elderly, reflects their preference, and has sufficient nutrients, is important to enable the elderly to enjoy their meals. Also, as the Korean society is getting older, it remains one of the biggest social challenges.

A Study on the Convergence of Sleep Patterns and Sleep Disturbance Factors of Inpatients Children Recognized by Care Providers (돌봄 제공자가 인식하는 입원아동의 수면양상 및 수면방해 요인에 대한 융합연구)

  • This was a basic study to ascertain the changes in sleep patterns and the factors disrupting sleep in hospitalized children. The participants were children aged 6 years or younger who had been hospitalized in an advanced general hospital in a particular region. A total of 81 participants were included, and for data analysis using SPSS/WIN 21.0. The results showed that children's sleep duration decreased significantly after hospitalization compared to before (p<.001), and there were significant differences in sleep onset latency (p<.001) and frequency of sleep deprivation (p<.001). Factors perceived by the children's mothers to disrupt sleep included the child's health state, noise in the ward, nurses' treatments, and lighting in the ward. This study verified that children's sleep quality and duration decreases during hospitalization, demonstrating the need to prepare measures to improve sleep in hospitalized children based on an understanding of changes in sleep patterns and disrupting factors.

Factors Related to Depression among Older People Living in Homes for the Aged of the Western Part of Thailand

  • Introduction: Geriatric depression is one of the significant mental health issues for older people. This descriptive study aimed to investigate the correlation between selected factors and depression among older people, living in homes for the aged in the western part of Thailand. Methods: 107 older people, living in two homes for the aged of the western part of Thailand were recruited to the study, using cluster sampling. Three questionnaires, including the Barthel Index Activity of Daily living (BADL), the Thai Geriatric Depression Scale (TGDS), and personal characteristics, perceived health status, chronic diseases, family relationship, and recent grief and loss experience were used. The quality of all questionnaires, especially the reliability of BADL, TGDS, and selected factors (e.g., health factors) were 0.95, 0.93, and 0.72 respectively. After finishing data collection, percentages, medians, and Spearman's rank correlation and Mann-Whitney U were used to analyze the data. Results and discussion: The results revealed that nearly half (41.1%) of older people suffered from depression. Furthermore, income, perceived health status, and Barthel ADL index were significantly negatively associated with depression at p<0.01; whereas, the number of chronic diseases was significantly positively associated with depression at p<0.05. Conclusion: results on the correlation may guide the practice in the future. For example, the officers or health care providers in homes for the aged should encourage older people to be independent, improve their perceived health status, increase health promoting behaviors to prevent or delay chronic illnesses, or increase older people incomes by promoting the appropriate occupation.

Review of Rehabilitation Programs for Cancer survivors (암생존자의 재활을 위한 국내 프로그램 고찰)

  • With advances in health care, the cancer survival rate has significantly increased. Even though cancer treatment has finished, cancer survivors may continuously suffer from physical and psychosocial problems. In order to investigate the problems cancer survivors have experienced, a comprehensive rehabilitation program must be developed. This study sought to determine the content and structure of rehabilitation programs for cancer survivors, analyze the effectiveness of such programs, and develop a basis for future comprehensive rehabilitation program development. Through this review, thirteen programs were selected and classified according to exercise, psychosocial, and integrated or comprehensive aspects. Exercise programs improve physical function, fatigue, and hormone-related cancer effects. Psychosocial programs improve quality of life, mood changes such as depression, anxiety, and post traumatic factors. Comprehensive programs improve both physical and psychosocial aspects.

An Importance-Performance Analysis of the Healthcare Reform on a Doctor-Designation System - Focused on the Perception of Medical Providers' at Tertiary Hospital in Seoul - (선택진료제도 개편에 대한 중요도-성취도 분석(IPA) - 서울시내 상급종합병원 의료종사자 인식을 중심으로 -)

  • Purpose: The purpose is to understand the perception of medical providers at tertiary hospitals in Seoul on the importance-performance of the reform on a doctor-designation system, and to provide the policy suggestion of the perception on such a system. Methodology: To achieve the purpose, this study utilized structured survey tools to conduct a questionnaire survey of nursing, administrative and medical technology professionals at six tertiary hospitals in Seoul. An importance-performance analysis was utilized for an analysis that showed the perception of the reform on a doctor-designation system. Findings: First, it was identified that those medical providers had the highest perception of the importance about the performance of the reform on a doctor-designation system. Second, according to the result of action grid of an importance-performance analysis (IPA), it can be identified that in the 2nd stage of 'Concentrate here' included are the items as to design and effect of medical care quality and subsidies & patient infection and safety and control fees, effect of income security of the method for compensating for loss established by the government and offering sufficient information on general medical services and doctor-designation medical services. In the 1st stage of 'Keep up the Good Work', and the 2nd stage of 'Concentrate here' included is the item as to the performance of patients of the reform on a doctor designation system. Practical Implications: There could be identified the effect of the reform bill on the perception of medical providers. It is expected that a better environment would be provided for patients to use a hospital and for medical providers to offer medical services, if the goverment makes efforts to improve methods for compensating for loss and continuous monitoring of the performance of patients.

An ethnographic research study on experience of identity in Korean multigravidas (경임부의 정체감 경험)

  • The childbearing process is not only a biological phenomenon of a woman who gives birth to a child but also a sociocultural phenomenon which is reflected on her value, belief in the sociocultural context according to social change and acculturation. The familial relation and sociocultural context in the multigravidas are more complex and intermingled than in the primigravidas. The purpose of this ethnographic research study was to explore the experience of identity from the first trimester of pregnancy to the third trimester of pregnancy in the Korean multigravidas and to understand deeply the perspectives of pregnant women reflected on Korean sociocultural values, beliefs, norms and familial culture. The participants of 10 pregnant women in Seoul, Korea were observed for 10 months from January to October 2000 and interviewed in their homes and comfortable place. Data analysis was accomplished 'line by line method' and significant concepts were classified according to themes, categories, and domains. The results of this study were as follows : The participants experienced 4 categorized subjects : understanding the oneself - mother to be, performing the dual role, drifting the emotion, and living disheartened during pregnancy. The participants were showed universality and diversity pattern in the self understanding process. The universal pattern were 'mother to be' showing maturation, life along family and priority on motherhood between being a mother and a woman. The diverse pattern were taking the dual role in working mothers having the higher self actualized value and personal identity rather than maternal identity, drifting emotion in resigned mothers, and living disheartened in mothers who have two daughters and no son. In conclusion, the Korean multigravidas experienced womanhood as well as motherhood through the self understanding process with familial connections during pregnancy. Therefore it is suggested that if the harmony and the balance between a mother and a woman is accomplished, the woman will lead a healthy and high quality of life. Also, this study sought to confirm the sociocultural factors affecting during pregnancy in the perspectives of the women with children. Therefore, the health care providers have to divert their attention from biomedical perspectives to biocultural perspectives integrating bio-psycho-sociocultural aspects of pregnant women in a clinical setting.

Clinical Usefulness of Critical Patient Severity Classification System(CPSCS) and Glasgow coma scale(GCS) for Neurological Patients in Intensive care units(ICU) (Glasgow coma scale의 임상적 유용성 평가 - 중환자 중증도 분류도구 -)

  • The tools that classify the severity of patients based on the prediction of mortality include APACHE, SAPS, and MPM. Theses tools rely crucially on the evaluation of patients' general clinical status on the first date of their admission to ICU. Nursing activities are one of the most crucial factors influencing on the quality of treatment that patients receive and one of the contributing factors for their prognosis and safety. The purpose of this study was to identify the goodness-of-fit of CPSCS of critical patient severity classification system(CPSCS) and Glasgow coma scale(GCS) and the clinical usefulness of its death rate prediction. Data were collected from the medical records of 187 neurological patients who were admitted to the ICU of C University Hospital. The data were analyzed through $x^2$ test, t-test, Mann-Whitney, Kruskal-Wallis, goodness-of-fit test, and ROC curve. In accordance with patients' general and clinical characteristics, patient mortality turned out to be statistically different depending on ICU stay, endotracheal intubation, central venous catheter, and severity by CPSCS. Homer-Lemeshow goodness-of-fit tests were CPSCS and GCS and the results of the discrimination test using the ROC curve were $CPSCS_0$,.734, $GCS_0$,.583, $CPSCS_{24}$,.734, $GCS_{24}$,.612, $CPSCS_{48}$,.591, $GCS_{48}$,.646, $CPSCS_{72}$,.622, and $GCS_{72}$,.623. Logistic regression analysis showed that each point on the CPSCS score signifies1.034 higher likelihood of dying. Applied to neurologically ill patients, early CPSCS scores can be regarded as a useful tool.

Expressional Analysis of Two Genes (Scd1 and Idi1) Down-regulated by Starvation Stress (영양고갈-스트레스에 의해서 하강발현하는 유전자(Scd1과 Idi1)의 분석)

  • Diet exerts a major stress on the body and may affect gene expression and physiological functions. Understanding of cellular responses during starvation is necessary in developing strategies to reduce damage caused by diet. In this study, we isolated 10 genes (Comt, RGN, Scd1, Temt, Idi1, Fabp5, Car3, Cyp2c70, Pinx1, and Poldip3) that are down-regulated in starvation and are closely related to liver metabolism. Water supply during starvation had no effect on the induction of apoptosis, autophagy, and ERQC. The genes down-regulated by starvation were associated with many related pathways rather than limited to the liver homeostasis pathway. Water supply during starvation is important. However, maintaining NaCl homeostasis is more important. The results are thought to be closely related to gender-specific metabolism in starvation and NaCl.

Factors Associated with the Long-Stay Admissions in Geriatric Hospitals - Focused on Dementia's Inpatients - (요양병원 치매노인의 장기입원 관련 요인)

  • Purposes: The purpose of this study was to identify the factors related to the long-stay hospitalization of dementia patients aged 65 years or older who had received inpatient care at geriatric hospitals according to the minute facility characteristics and patient features. Methodology: This study was conducted on 317,353 cases of 1,512 geriatric hospitals using the Health Insurance Review and Assessment Service dataset. The data collected were processed using the SAS Enterprise Guide 4.3 for descriptive statistics, the chi-square test, and the binary logistic regression analysis. Findings: As a result of the study, in the facility characteristics of geriatric hospitals, the long-stay hospitalization of the aged with dementia were found to be related to the type of facility establishment, the number of hospital beds, the number of medical specialists, the number of nursing personnel, and the number of geriatric hospitals by region and province. In the personal features of patients, the long-stay hospitalization was found to be associated with the gender, age, insurance, and the patient classification groups. Practical Implication: Considering the results of this study, it seems that securing the sufficient medical personnel in a geriatric facility, providing the good quality medical services, and preparing the appropriate discharge plan can reduce the unnecessary long-stay hospitalization and spend the medical expenses for the older patients.

Study of the Drugs Prescribed on Alzheimer's Disease: from the Insurance Claims Data of Korea National Health Insurance Service (우리나라 건강보험 청구자료를 이용한 알츠하이머성 치매 치료제의 사용현황 분석)

  • Objective: The aims of this study are to investigate the total volume of prescribed medicines against Alzheimer's disease (AD) and the trends of usage by analyzing the claims-data from the Korea National Health Insurance Service. Method: The demographic and claims-data were included the major AD treating medicines such as donepezil, galantamine, rivastigmine and memantine, and analyzed during the period of 2010~2012. The assessing criteria were gender, age, habitation, types of medical institution, code of ingredients, outcomes of treatment, volume and amount of claims, and the numbers of patients with dementias. After trimming the data, it were analyzed by the market size, demographic traits, characteristics of medical service, characteristics of each anti-AD medicine, etc. Results: Among the chosen 4 medicines, donepezil had the top prescription volumes. Most prevalent prescribing preparations of donepezil were conventional types. However, among the non-conventional types, oro-dispersible formulation is the fast increasing one in both volume and growth rate. This specialized preparations to improve both toleration and adherence, tend to being prescribed generally at the tertiary medical institutions. While the younger patients with mild-to-moderate AD mostly treated by expensive medicines in resident at the tertiary hospitals, the rest older patients with severe AD have been treated non-expensive one at long-term care facilities. Conclusion: AD is a chronic illness therefore, long-term use of therapeutic medications are highly important. If an anti-AD treatment was applied steadily in the earlier stages, it would be achieved not only improving the quality of life of patient but also reducing the expenses in the medical and nursing cares. As the socioeconomical impacts of AD is expanding, healthcare professionals need to aware the importance of pharmacotherapy and to improve sociopolitical fundamentals.
