IPL 처리를 통한 고분자 나노구조의 기계적 특성 향상 연구 (A Study of Mechanical Property Enhancement of Polymer Nanostructure using IPL Treatment)

  • 논문에서는 고분자 나노구조 필름의 기계적 물성을 향상하기 위하여 광열효과를 이용한 열처리 공정을 응용하여 나노임프린트로 제작된 고분자 나노구조 필름의 기계적 물성에 미치는 영향을 규명하였다. Hybrid resin과 UV 나노임프린트을 이용하여 저반사 나노구조를 성형하고 IPL (intense pulsed light)를 이용하여 열처리를 수행한 뒤, 제작된 나노구조 필름의 투과율과 내스크래치성을 평가하여 나노구조의 성형성과 기계적 물성의 변화를 관찰하였다. 나노패턴의 특성에 의해서 나노구조의 투과율은 550 nm 파장에서 97.6%로 고투과율의 기능을 확인하였으며, IPL을 이용한 열처리를 진행한 경우 Hotplate를 이용한 열처리보다 경도는 0.51 GPa로, 0.27 GPa로 열처리한 시편에 비해 1.8배 향상하였으며, 동일 실험 조건에서 탄성율은 Hotplate 이용 시 4 GPa에서 IPL 이용 시 5.9 GPa로 1.4배 증가하였다.

영상 인식 및 생체 신호를 이용한 운전자 졸음 감지 시스템 (Driver Drowsiness Detection System using Image Recognition and Bio-signals)

  • 매년 교통사고의 가장 큰 원인으로 손꼽히는 졸음운전은 운전자의 수면 부족, 산소 부족, 긴장감의 저하, 신체의 피로 등과 같은 다양한 요인을 동반한다. 졸음 유무를 확인하는 일반적인 방법으로 운전자의 표정과 주행패턴을 파악하는 방법, 심전도, 산소포화도, 뇌파와 같은 생체신호를 분석하는 방법들이 연구되고 있다. 본 논문은 영상을 검출하는 딥러닝 모델과 생체 신호 측정 기술을 이용한 운전자 피로 감지 시스템을 제안한다. 제안 방법은 일차적으로 딥러닝을 이용하여 운전자의 눈 모양과 하품 유무, 졸음으로 예상되는 신체 동작을 파악하여 졸음 상태를 감지한다. 이차적으로 맥파 신호와 체온을 이용하여 운전자의 피로 상태를 파악하여 시스템의 정확도를 높이도록 설계하였다. 실험 결과, 실시간 영상에서 운전자의 졸음 유무 판별이 안정적으로 가능하였으며 각성상태와 졸음 상태에서의 분당 심박수와 체온을 비교하여 본 연구의 타당성을 확인할 수 있었다.

조선 의원 이원풍(李元豊)의 마진 의서, 『마진휘성(麻疹彙成)』연구 (A Study on the Majinhwiseong (麻疹彙成), a Medical Text on Measles Written by Joseon physician Lee Wonpung)

  • Objectives : In this paper, the outline and overall content of the Majinhwiseong, a specialized medical text on measles written by Lee Wonpung was introduced, along with its academic historical meaning. Methods : The entire Majinhwiseong was analyzed according to content and form. In terms of form, organization, construction, cited literature, etc., were studied, while in terms of content, diagnosis of disease pattern and treatment formulas were studied. Later, based on cited medical texts and the author's social position, the academic historical meaning of this book was discussed. Results : Through the Majinhwiseong, Lee Wonpung strengthened the credibility of the text by not only providing medical knowledge on measles but listing their sources and comparing and analyzing related contents. In the diagnosis part, Lee focused on the changes in symptom, shape, color, and pulse of measles, discussing in detail its differential diagnostic methods. In the treatment part, while listing numerous formulas suggested by Ming (明) masters, Lee did not leave out treatment experiences of Joseon physicians. Meanwhile, the Majinhwiseong is indicative of measles medicine in 18th century Joseon having been progressed in the private sector rather than the official, and how the results of private sector medicine were being absorbed into the official realm through the Uiyakdongcham (議藥同參) system. Conclusions : The Majinhwiseong is a practical treatment manual written by clinician Lee Wonpung to deal measles which was widely spread at the time. The author organized existing medical knowledge on measles for clinicians while reflecting outcomes and medical situation of Joseon physicians in this book. Based on these findings, we could verify that medicine in 18th century Joseon had been progressing actively around the private medical sector.

  • 2D TOF혈관조영술에서 presaturation RF펄스를 사용하여 정맥피에서 나오는 MR신호를 제거하고 얻은 동맥 angiogram에는 band형태로 동맥이 끊어져 나타나는 현상이 자주 보인다. 일부 동맥에서 피의 흐름은 한 심장 주기 동안에 3번의 펄스를 갖는데, 이 중 가운데 펄스는 짧은 기간 동안 역류를 한다. 이 역류하는 동맥피는 정맥피와 같은 방향이기 때문에 presaturation RF펄스에 의해 정맥피 처럼 saturation이 되어서 imaging slice로 흘러 들어가게 될 수 있다. 특히 이러한 경우가 phase encoding step의 dc 부근에서 발생하게 되면 그 때의 단면 영상에서 동맥이 강조가 될 수 없게 되어, 결과적으로 angiogram에는 그 단면을 지나는 동맥의 신호가 없어서 band 형태로 핏줄이 끊어져 나타나게 된다. Imaging slice와 saturation band와의 간격을 변화 시켜 가면서 angiogram을 얻어 봄으로서 이러한 현상을 실험적으로 확인하였다 나아가 saturation band를 rectangle형태에서 ramp 형태로 변경함으로써 이러한 artifacts를 제거 할 수 있는 방법을 제시하였다

Effectiveness of thaumaturgic distraction in alleviation of anxiety in 4-6-year-old children during inferior alveolar nerve block administration: a randomized controlled trial

  • Background: Dental anxiety is a matter of serious concern to pediatric dentists as it may impede the efficient delivery of dental care. If not adequately resolved, a persistent negative response pattern may emerge. Thaumaturgy, commonly known as magic trick, has become popular recently. It is a tool that distracts and relaxes the child by using magic trick while the dentist performs necessary treatment. Hence, the aim of this study was to evaluate the effectiveness of Thaumaturgic aid in alleviation of anxiety in 4-6 - year-old children during administration of local anesthesia using the inferior alveolar nerve block (IANB) technique. Methods: Thirty children aged between 4-6 years with dental anxiety requiring IANB were included in this study. Patients were divided equally into two groups: Group I, thaumaturgic aid group and Group II, conventional non-pharmacological group using randomization. Anxiety was measured before and after using the intervention with Raghavendra Madhuri Sujata-Pictorial scale (RMS-PS), Venham's anxiety rating scale, and pulse rate. All the data were tabulated and compared using statistical analysis. Results: Children in thaumaturgy group (Group- I) exhibited significantly lower anxiety during IANB in comparison with children in the conventional group (Group- II) and the difference was statistically significant. Conclusion: Magic tricks are effective in reducing anxiety among young children during IANB; Moreover, it expands the arsenal of behavior guidance techniques for treatment of children with anxiety and plays an important role in shaping the behavior of a child in pediatric dentistry.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500, 550, 570 and 610nm) for blood glucose strip. Mice from both groups were irradialed for approximately 15 minutes at 100, 300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. Under our experimental conditions, the presence of Photolfrin II increased surface temperature by at least 40% and the temperature rise due to 300 mW/cm2 irradiation exceeded values for hyperthermia. Light and temperature distributions with depth were estimated by a computer model. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The results suggested that the femoral component suitable for Asians should be different from the currently-used components designed and manufactured mostly by European or American companies. 30537 T00401030537 ^x It is well known that nonlinear propagation characteristics of the wave in the tissue may give very useful information for the medical diagnoisis. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. Our department has developed an information system for routine hematology using down-sized computer system. The operating system and database management system were SCO UNIX and SCO foxbase, respectively. For program development, we used Xbase language provided by SCO foxbase. The C language was used for interface purpose. To make the system use friendly, pull-down menu was used. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer. Our system interfaced with fwd complete blood count analyzers(Sysmex NE-8000 and Coulter STKS) for unidirectional data tansmission from analyzer to computer. The frame and leaflet of the polymer valve were made from polyurethane, To evaluate the hemodynamic performance of the polymer valve a comparative study of flow dynamics past a polymer valve and a St. Jude Medical prosthetic valve under physiological pulsatile flow conditions in vitro was made. Because the point of maximun temperature of laser-irradiated gelatin exists below the surface due to surface cooling, evaporation at the boiling temperature is made explosively from below the surface. The important parameters of this process are the conduction loss to laser power absorption (defined as the conduction-to-laser power parameter, Nk), the convection heat transfer at the surface to conduction loss (defined as Bi), dimensionless extinction coefficient (defined as Br.), and dimensionless irradiation time (defined as Fo). Dependence of Fo on Nk and Bi has been observed by experiment, and the results have been compared with the numerical results obtained by solving a 2-dimensional conduction equation. Fo and explosion depth (from the surface to the point of maximun temperature) are increased when Nk and Bi are increased.To find out the minimum laser power for explosive evaporative removal process, steady state analysis has been also made. We obtained a two-dimensional current density distribution within a phantom by processing the real and imaginary MR images from a 4.77 NMR machine. We implemented a resistivity mage reconstruction algorithm using the finite element method and sensitivity matrix. Optimal stimulus parameters, which can minimize muscle fatigue and can achieve effective muscle contraction were determined in slow and fast muscles in Sprague-Dawley rats. Stimulus patterns of our stimulator were designed to simulate electromyographic activity monitored during locomotion of normal subjects. Muscle types of the lower extremity were classified according to their mechanical property of contraction, which are slow muscle (msoleus m.) and fast muscle (medial gastrocneminus m., rectus femoris m., vastus lateralis m.). Optimal parameters of electrical stimulation for slow muscles were 20 Hz, 0.2 ms square pulse. For fast muscle, 40 Hz, 0.3 ms square pulse was optimal to produce repeated contraction. Higher stimulus intensity was required when synergistic muscles were stimulated simultaneously than when they were stimulated individually. Electrical stimulation for each muscle was designed to generate bipedal locomotion, so that individual muscles alternate contraction and relaxation to simulate stance and swing phases. Portable electrical stimulator with 16 channels built in microprocessor was constructed and applied to paraplegic patients due to lumbar cord injury.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. Muscle types of the lower extremity were classified according to their mechanical property of contraction, which are slow muscle (msoleus m.) and fast muscle (medial gastrocneminus m., rectus femoris m., vastus lateralis m.). Optimal parameters of electrical stimulation for slow muscles were 20 Hz, 0.2 ms square pulse. For fast muscle, 40 Hz, 0.3 ms square pulse was optimal to produce repeated contraction. Electrical stimulation for each muscle was designed to generate bipedal locomotion, so that individual muscles alternate contraction and relaxation to simulate stance and swing phases. CAVI를 이용한 급성기 중풍환자의 Arteriosclerosis와 한방변증의 관련성 연구 (The Relationship between Oriental Medical Diagnosis and Arteriosclerosis by Carotid-Ankle Vascular Index(CAVI) in Acute Cerebral Infarction Patients)

  • Objectives : This study aimed to clarify the relationship between the Oriental medical diagnosis and arteriosclerosis by measuring carotid-ankle vascular index(CAVI) in acute cerebral infarction patients. Method : One hundred thirty-one subjects were recruited from the patients admitted to the Internal Medical Department at Kyunghee University Oriental Medical Center from April 2007 to August 2008. We sorted cerebral infarction patients and assessed one hundred fourteen patients' CAVI data. We diagnosed dampness-phlegm by Oriental medical diagnosis and evaluated stroke type by single or multiple infarctions. then, we analyzed their characteristics with type of stroke, risk factor, lifestyle, metabolic syndrome and dampness-phlegm diagnosis. Result : 1. On the demographic variables of the patients, age, hypertension, hyperlipidemia, multiple infarction group and metabolic syndrome and dampness-phlegm group were significantly higher in the high CAVI score group than in the control. 2. According to the significant difference in the dampness-phlegm group, we analyzed dampness-phlegm related index for pattern identifications by CAVI score. As a result, dark circles, insomnia, headache, white coating tongue. slippery pulse, and rough pulse were significantly higher in the high CAVI score group then in the control. 3. In multivariate analysis, age, hypertension, multiple infarction and dampness-phlegm groups showed a close relationship with the high CAVI score group. Conclusions : According to the analysis, significance between dampness-phlegm diagnosed patients group and high CAVI score were clarified. Moreover, multiple location infarctions also have a relationship with high CAVI score in cerebral infarction patients. These results can be utilized in the future as a basis material.

  • 잡견의 간흡충증에서 조영기법의 역동적 CT 영상과 MIP 기법을 적용한 자기공명 영상의 질을 비교하여 임상적 적용 가능성을 알아보고자 하였다. 20마리의 잡견(평균 20kg)에 실험적으로 간흡충증을 유발한 후 13주 동안에 간을 중심으로 CT 영상과 MR 영상을 얻었다. CT는 이중나선식방법으로 영상을 획득하였고, bolustracking 방법으로 조영제를 주입하여 arterial phase는 5초 후에 정맥기는 동맥기 scan후 15초에 시작하여 single dynamic scan을 하고, 데이터를 다 단면 영상을 얻기 위해 MnP로 재구성을 하였다. MR 영상은 circularly polarized phased away body coil을 사용하였고, HASTE, FLASH, TSE 기법을 이용하여 모두 2차원 영상을 얻었다. HASTE와 TSE는 해상력을 증가시키기 위하여 interpolation과 지방소거기법을 적용하였다. 데이터를 획득하는 방식으로는 담관을 중심으로 3방향으로 수집한 다음 최대강도투사법으로 재구성하였다. CT와 MR 영상의 평가 방법으로서는 간내담관의 확장, 동맥기에 담관벽의 조영증강, 담관 말단부의 확장, 간의 실질조직묘사, 배경유무를 기점으로 병리학과 의사 1명과 방사선과 의사 3명, 방사선사 5명이 시각적으로 평가를 하였다. MR 영상의 경우 sequence 비교를 위해 정량적분석방법인 CNR과 CR로 평가를 하였다. 감염 후 5마리의 잡견은 죽었으며, 15마리 잡견에서 CT와 MR 검사에서 간내 담관의 만성 확장을 보여 주었다. 조영증강기법인 CT 영상에서는 간 실질조직을 비롯하여 담관벽의 조영증강이 보였고, MR 영상에서는 간내담관 형태와 담관의 말단부의 확장이 MIP 기법을 이용하여 3차원적으로 관찰되었고, CNR과 CR 값이 HASTE($16{\pm}0.83$, 73.3%), TSE($7.06{\pm}3.0$, 62.3%), FLASH($1.19{\pm}0.2$, 6.4%) 순으로 나타났다. CT와 MR 영상은 짧은 검사기법과 다양한 검사기법을 포함하고 있어 간흡충증을 영상화하는데 있어서 진단적 가치가 있다. CT와 MR 영상 검사의 장 단점을 서로 보안하여 검사를 시행한다면 높은 임상적 적용 가능성이 있다.

전국 병원을 대상으로 조사한 소아 특발성 신증후군의 임상적 고찰 - 다기관 공동연구 결과보고 - (Primary nephrotic syndrome in children : A nationwide survey in Korea)

  • 목 적 : 최근 소아신장질환에 대한 관심도가 높아지고 있어 소아신장질환 중 많은 비중을 차지하고 있는 신증후군에 대해 우리나라에서의 성별, 나이, 계절에 따른 발생빈도, 신장생검상 분류에 따른 형태학적 발생빈도, 치료방법의 종류, 치료에 대한 반응의 정도와 반응시기, 재발의 빈도 등에 대해 통계적으로 조사, 정리하고자 하였다. 방 법 : 전국 38개의 대학병원 및 종합병원을 대상으로 대한소아신장학회에서 작성한 설문조사지를 발송하여 각각의 병원에서 신증후군 진단기준에 따라 진단, 치료, 추적관찰하는 신증후군 환아 2193명들의 의무기록을 토대로 기록한 설문조사지를 수집, 정리, 통계내었다. 결 과 : 남아가 1655명($75.5\%$), 여아가 538명($24.5\%$)이며 남여의 비가 3:1로 남아가 많았다. 이 중 미세변화 신증후군(minimal change nephrotic syndrome, MCNS)의 경우 남자가 1338명, 여자가 414명으로 남녀비는 3.23:1 이었다. 연령별 발생빈도는 1-5세 사이가 1060명($48.3\%$)으로 가장 많았고, 다음으로 6-10세 사이가 679명($31.0\%$) 이었다. 계절별 발생빈도는 보통 140명에서 180명정도로 발생하여 계절별 발생빈도에 큰 차이는 보이지 않았다. 신장생검을 실시한 942명($43\%$) 중에서 MCNS가 646명($68.6\%$)으로 가장 많았고, 다음으로는 국소성분절성사구체 경화증(focal segmental glomerulosclerosis, FSGS)이 149명($15.8\%$)이었다. 치료방법으로는 prednisolone이 1191명으로 가장 많이 사용되었고, 부작용이 적은 Calcort는 192명으로 아직 prednisolone을 대치하지 못하였다. 그 외 치료로는 cyclophosphamide가 251명, cyclosporin A가 223명, methylprednisolone(MPD) pulse therapy가 120명이었다. 치료에 대한 효과로는 1944명 중에 완전완해(complete response)가 1597명($82.2\%$)으로 반응이 좋았으며, 그 중 prednisolone으로 치료한 경우도 $85.1\%$의 좋은 결과를 보였다. 신장생검상 형태학적 분류에 따르면 MCNS의 경우 완전완해가 $86.5\%$이나, FSGS의 경우는 $35.8\%$로 질환의 형태에 따라 커다란 차이를 보이고 있다. 치료반응 시기는 1주에서 4주사이가 879명($67.4\%$)으로 가장 많았고, 다음으로 1주이내가 313명($24.0\%$)이었다. (n=1305) 질병의 재발은 994명($73.1\%$)이었고, 재발이 발생한 경우는 치료후 2개월에서 6개월사이에 361명으로 가장 많았다. 결 론 : 소아 신증후군은 남아에서 더 많이 발생(남녀비 3.08:1)하였고 1-5세 사이가 가장 많았다. 신장생검상 형태학적 분류에 따른 치료반응은 많은 차이를 보였으며, 재발하는 경우가 많아 계속적인 추적관찰이 필요할 것으로 사료되는 바이다.
