New Chords Formation -6 cases reports- (새로운 건삭형성술 -6례 보고-)

  • New chords formation of mitral valve using expended-polytetrafluoroethylene sutures was performed in six patients with mitral regurgitation (MR) from April, 1994 to March, 1995. There were 3 men and 3 women whose mean age was 65 years, ranged from 55 to 75. A double-armed pledget suture was passed through the lateral portion of papillary muscle head and tied. Each arm of the suture was brought up to the free margin of the leaflet and passed through the area where the native chorda was attached. At leaflet surface, one more weaving was made. fter the lengths of the two arms were adjusted, the ends were tied. There were no operative deaths. Patients have been followed up from 2 to 13 months, mean 8. Echocardiographic sutudies were performed at 10 days after operation and revealed normal mitral valve function in all patients.

Surgical treatment for ventricular septal defect associated with aortic insufficiency (대동맥판맥 폐쇄 부전증이 동반된 심실중격 결손증의 수술성적)

  • Between January 1983 and December 1992, we had experienced 79 patients of ventricular septal defect [ VSD ] associated with aortic insufficiency [AI] which constitute 4.6 % of total numbers of VSD. The mean age of the patients was 10.2 years with a range of 1 to 35 years and the average degree of aortic insufficiency classified by Sellers was 2.1. The type of VSD was subpulmonic in 57 patients and perimembranous in 22. Most common pathologic finding causing AI was prolapse of right coronary cusp [ 54 cases ; 71.4% ] ,followed by prolapse of both right and non-coronary cusp[ 12 cases ; 7.9% ]. VSD closure alone was performed in 51 patients and their mean age was 7.7 years [ ranged 1 to 13 years ]. VSD closure and aortic valve reconstruction was performed in 22 patients, VSD closure and aortic valve replacement in 6 patients, and the mean age of the patients was 14.5 years [ ranged 2 to 28 years ], 20.4 years [ ranged 18 to 35 years ] respectively. There was no hospital mortality. All patients were followed up from 1 month to 9 year 4 months [average; 21.4 months ] and there was one late death. Our data suggests that, early closure of VSD without any manipulation on the valve may be sufficient procedure to improve or at least withhold progression of AI in children and furthermore patients with VSD associated AI should be corrected promptly after diagnosis.

Imprinting of a foal and large intestine prolapse associated with uterine rupture in a Thoroughbred broodmare (Thoroughbred 씨암말에서 자궁파열과 연관된 대장탈출증 및 망아지의 각인 1례)

  • In this report, we described a case of acute colic with uterine rupture in a Thoroughbred broodmare during parturition. A 14-year-old broodmare showed acute colic characterized by rolling just after delivering of a healthy foal. Large intestine was prolapsed through vulva, contaminated with soil and its serosa was heavily damaged. We performed a euthanasia according to the owner's request. Post mortem examination revealed about 15 cm rupture on ventral side of the uterus. In addition, the orphan foal follow his owner after necropsy on his dam.

A Case Report of A Mixed Hemorrhoid Patient Treated by Herbal medicine Treatment (한약 치료로 개선된 혼합 치핵 환자에 관한 증례 보고)

  • Objectives: The purpose of this study is to report the clinical course of a patient with mixed hemorrhoids treated with Korean medicine alone. Methods: A patient visited for an mixed hemorrhoids was treated with Korean herbal medicine. The effects of the treatment were evaluated via photography of hemorrhoids and symptoms of pain, bleeding, prolapse, itching, discharge and Total Symptom Score(TSS). Results: After treatment, in photography before and after, the size of hemorrhoids was reduced and lost. Scores of Pain (4→0), bleeding (3→0), prolapse (3→2), itching (0→0), discharge (1→0) and TSS (11.5->3) were observed. Conclusion: In light of the reduced size of hemorrhoids and scores of symptoms, herbal medicine might be a possible treatment method on mixed hemorrhoids.

A Case of a Holstein Cow with Eyelid Carcinomatosis (Holstein 유우의 안검암종 증레)

  • A ten-year-old holstein cow was presented because of prolapse of the third eyelid and apparent hyperplasia of the right lower eyelid. Historical findings included increased appetite as well as polyuria and polydipsia for about two weeks. The most remarkable fadings on physical examination were a large periocular proliferative tissue and bleeding. Surgical incision was used both as a biopsy and therapeutic tool in holstein cow with mass. Histopathological examination of the mass revealed squamous cell carcinoma. Blood and milk tests of patient with squamous cell carcinomatosis were peformed, but normal values.

Study on the Medicinal Parts and Expected Efficacys of the Hibiscus syriacus by Literature Review on the Classics of Oriental Medicine (한의학 고문헌 분석을 통한 무궁화의 약용부위와 기대효능 연구)

  • Hibiscus syriacus is rarely used herb. For finding up new applications of this herb, the medicinal parts and expected efficacys that recorded in 24 classics of oriental medicine were inquired. As a result of the literature review, medicinal parts and expected efficacys of Hibiscus syriacus were as follows: The medicinal parts were mainly flower and bark. But the root, branch, leaf and fruit were mentioned too. Some unidentified medicinal parts seemed to be branch or bark. The mentioned symptoms that prescribed Hibiscus syriacus are intestinal 'wind', red-white dysentery, thirst with fever after dysentery, insomnia, 'wind' syndrom, leukorrhea, regurgitation, etc. And this herb was mentioned that applied externally for scabies, hemorrhoids, prolapse of the rectum, headache, extracted tooth, etc. Therefore, bleeding intestinal diseases, insomnia, vaginal or uterine hemorrhage(for internal use), scabies or mycosis, hemorrhoids. anal fistula, prolapse of the rectum, headache(for external use) were presented as expected efficacys of Hibiscus syriacus. In future, studies for verifying the efficacys are needed.

The Beneficial and Adverse Effects of Raloxifene in Menopausal Women: A Mini Review

  • Objectives: The present mini review aimed to summarize the existing knowledge regarding the beneficial and adverse effects of raloxifene in menopausal women. Methods: This study is a review of relevant publications about the effects of raloxifene on sleep disorder, depression, venous thromboembolism, the plasma concentration of lipoprotein, breast cancer, and cognitive function among menopausal women. Results: Raloxifene showed no significant effect on depression and sleep disorder. Verbal memory improved with administration of 60 mg/day of raloxifene while a mild cognitive impairment risk reduction by 33% was observed with administration of 120 mg/day of raloxifene. Raloxifene was associated with a 50% decrease in the need for prolapse surgery. The result of a meta-analysis showed a significant decline in the plasma concentration of lipoprotein in the raloxifene group compared to placebo (standardized mean difference, -0.43; 10 trials). A network meta-analysis showed that raloxifene significantly decreased the risk of breast cancer (relative risk, 0.572; 95% confidence interval, 0.327-0.881; P = 0.01). In terms of adverse effects of raloxifene, the odds ratio (OR) was observed to be 1.54 (P = 0.006), indicating 54% increase in the risk of deep vein thrombosis (DVT) while the OR for pulmonary embolism (PE) was 1.05, suggesting a 91% increase in the risk of PE alone (P = 0.03). Conclusions: Raloxifene had no significant effect on depression and sleep disorder but decreased the concentration of lipoprotein. Raloxifene administration was associated with an increased risk of DVT and PE and a decreased risk of breast cancer and pelvic organ prolapse in postmenopausal women.

Korean Medicine Treatment for Patients with Grade I-III Hemorrhoids (Goligher Classification): A Case Series (Goligher 분류상 1기-3기에 해당하는 항문질환 환자의 한의치료 증례군보고)

  • Background: Hemorrhoid surgery is a frequently performed surgery in Korea, second only to cataract surgery. However, it has serious complications, including bleeding, infection, anal stenosis, and urinary retention, so active conservative therapies are in need at an early stage. Case summary: Thirteen patients with anal disease complained of main symptoms of pain, bleeding, and prolapse, with additional complaints of itching and mucus discharge. An herbal medicine mixture of Eulja-tang and Hwangryeonhaedok-tang was administered twice daily, and dry cupping therapy, acupuncture, and hot pack placements were administered on pelvic and sacral regions once weekly for four weeks. The symptoms were evaluated before and each week after the treatments. The treatments had statistically significant therapeutic effects. Conclusion: Korean medicine treatment-effectively alleviating the symptoms of anal disease, such as pain, bleeding, and prolapse-may represent an alternative therapy for conservative treatments in the early stages of anal disease.

  • Aortic dissection refers to the separation of the aorta into a true and false lumen as the medial membrane of the aorta is torn along the long axis due to a high aortic pressure when a minute rupture occurs in the aortic lining. The mortality rate is very high, and aortic dissection occurs 2~5 times more in men than women. The prevalent age range is 50~70 years old. In this case, the authors experienced a diagnosis of aortic dissection that occurred in a young woman in her 30s, which does not occur frequently. In the process of tracking severe aortic regurgitation, aortic valve prolapse was initially suspected. We report this case because aortic dissection could be diagnosed by observing the intimal flap of the ascending aorta in the process of confirming this suspected part.

Surgical Management of Aortic Insufficiency in Behcet`s Syndrome - An Experience of 8 Cases - (Behcet 씨 증후군에 의한 대동맥판 폐쇄부전의 수술치험 -3례 보고-)

  • In Behcet syndrome, cardiac involvements are rare and have been reported pericarditis, myocarditis, right heart endocardial fibrosis, right ventricle mural thrombus with pulmonary embolism, active endocarditis, granulomatous endocarditis, conduction disturbance, acute aortic insufficiency, mitral valve prolapse. Our three patients underwent AVR because of aortic insufficiency and ascending aorta enlargement combined with Behcet syndrome. Two patients had mitral regurgitation too. So one underwent MAP and the other underwent MVR concomitantly. One who underwent AVR have been well for 50 months. Another who underwent AVR+MAP and redo AVR due to aortic paravalvular leakage was died of congestive heart failure. The other who underwent AVR+MVR and repeated AVR three times because of aortic paravalvular leakage is in condition of aortic paravalvular leakage. Paravalvular leakage is considered to recur due to progressive dilatation and fragility of aortic root that is the result of pathologic change of Behcet syndrome in it. If Open heart surgery is needed in Behcet`s syndrome during inflammatory reaction is active, postoperative complications such as paravalvular leakage or suture line rupture may be prevented with pre- and postoperative anti-inflammatory management.
