폐성심을 시사하는 심전도 소견 유.무에 따른 만성폐쇄성 폐질환 환자의 예후 (The Prognostic role of Electrocardiographic Signs of Cor Pulmonale in Chronic Obstructive Pulmonary Disease)

  • 연구배경 : 만성폐성심을 시사하는 심전도 소견 유무에 따른 만성 폐쇄성 폐질환 환자의 예후를 평가하고 이 심전도 소견이 폐기능검사 성적 및 동맥혈가스분석 소견등과 연관되어 예후에 어떤 영향을 미치는 가에 대해서 알아 보고자 하였다. 방법 : 만성폐쇄성 폐질환의 급성악화로 입원한 61명의 환자를 대상으로 심전도, 동맥혈가스분석 및 폐기능검사 성적을 비교하였다. 폐성심을 시사하는 심전도 소견은 우심방부하, 우각차단, 우심실비대 low-voltage QRS로 하였으며 심전도상 이상소견이 없는 1군은 36명이었고, 이상소견이 하나 이상인 2군은 25명 이었다. 결과 : 다변량분석 결과 나이 (risk ratio=1.13, 95% confidence interval 1.05-1.23), DLco % pred. (risk ratio=0.97, 95% confidence interval 0.94-0.99), $PaO_2$ (risk ratio=0.95, 95% confidence interval 0.90-0.99), 심전도상 우심방부하(risk ratio=5.27, 95% confidence interval 1.40-19.85) 가 만성폐쇄성 폐질환 환자에서 각각 독립적으로 생존율에 영향을 미치는 인자로 나타났다. 심전도상 우심방부하의 소견이 없는 경우에서는 1년, 2년, 5년 생존율이 각각 95.4%, 81.4%, 50.0% 인데 비해 우심방부하 소견이 있는 경우는 각각 82.4%, 70.6%, 27.5% 로 유의한 차이가 있었다. 결론 : 이상의 결과로 만성폐쇄성 폐질환 환자에서 폐성심을 시사하는 심전도 소견, 특히 우심방부하를 나타내는 심전도 소견은 중요한 예후예측 인자일 것으로 생각된다.

한국인 IgA 신병증 환아에서 MHC Class II유전자형과 예후와의 관계 분석 (MHC Class II Allele Association in Korean Children With IgA Nephropathy and its Role as a Prognostic Factor)

  • 목 적 : IgA 신병증은 혈뇨와 단백뇨를 보이는 사구체 질환으로 세계적으로 가장 빈도가 높은 질환중의 하나 이며 우리 나라를 포함한 아시아 지역에서도 높은 발생율을 보이고 있으나 아직 정확한 발생기전이나 원인은 불분명한 상태이다. IgA 신병증은 발병 10년 후 $20-30\%$가 말기 신부전증으로 진행됨이 보고되고 있으나 신부전으로 진행속도는 환자에 따라 매우 다양하기 때문에 특이적인 조직적합항원을 찾아냄으로서 치료 및 예후 결정에 도움을 주기 위하여 본 연구를 시행하였다. 대상 및 방법 : 세브란스 병원에 입원하여 IgA 신병증으로 진단 받은 환자를 대상으로 정상 신기능을 유지하고 있었던 소아환아 69명(Group A)과 이미 신부전으로 진행하여 신이식을 시행 받은 70명(Group B)을 포함한 신기능 저하군으로 분류하여 HLA대립유전자의 결정은 SSOP (sequence specific oligonucleotide probe)법으로 분석하여 HLA-class II(DR, DQ)대립유전자의 발현빈도와 IgA 신병증의 예후와의 관계를 분석하였다. 결 과 : 대상 환자의 진단 당시의 평균나이는 A군은 $8.8{\pm}2.9$세, B군은 $35.0{\pm}15.5$세였고, 남녀비는 각각 2.8:1과 2.5:1이었다. HLA-DQB1 유전자중 139명중 $47.5\%$$03^{**}$의 발현빈도가 가장 증가되어 있었고 $06^{**}(32.4\%),\;04^{**}(28.7\%),\;05^{**}(20.1\%),\;02^{**}(8.6\%)$순이 었다. DQ7의 HLA-DQB1*0301 이 전체 환자의 20.1$\%$(28/139)로 가장 흔한 대립유전자였으나 두군간에 유의한 차이는 없었다. 대립유전자중 HㄴA-$DQBI^*03^{**}과\;DQB1^*05^{**}$가 이미 말기신부전으로 진행된 Group B에서 유의하게 증가되어 있었으며(P<0.05), $03^{**}$ 아형중 $DQB1^{*}0302와\;05^{**}중\;DQB1^{*}05031$이 신기능이 정상인 A군에 비해 유의하게 증가되어 있었다(P<0.05). DR 유전자형의 low-frequency type상 HLA-$DRB1^*03과\;HLA-DRB1^*15$가 Group B에서 유의 있게 증가되어 있었으나, high-frequency type상에서는 양군 사이에 유의한 차이가 없었다. A군의 경우 신조직 검사의 소견에 따라 4등급으로 분류하여 대립유전자의 발현빈도를 분석하였으나 의미 있는 관계가 없었다. 곁 론 : IgA신병증의 예후를 예측하기 위한 여러 조사에서 발병 초기의 심한 단백뇨, 고혈압, 조직 병리학상의 심한 변화 등이 있을 경우 예후가 나쁘다고 하였으며 이런 위험 인자외에 유전적 요인에 대해 알아보고자 하였다. 본 연구에서 신부전으로 진행하여 신대체요법을 받은 IgA신병증환자군에서 HLA-$DQB1^*0302와\;^*05031$ 대립유전자의 발현빈도가 유의하게 증가되어 있어 이 결과를 바탕으로 치료후의 경과 및 예후를 진단초기에 예측하여 환자의 관리에 적용할수 있는 중요한 자료로 이용될 수 있을 것으로 사료된다.

만성 외측 발목 불안정 (Chronic Lateral Ankle Instability)

  • Chronic lateral ankle instability is a major complication of acute ankle sprains, which can cause discomfort in both daily and sports activity. In addition, it may result in degenerative changes to the ankle joint in the long term. An accurate diagnostic approach and successful treatment plan can be established based on a comprehensive understanding of the concept of functional and mechanical instability. The patients' history and correct physical examination would be the first and most important step. The hindfoot alignment, competence of the lateral ligaments, and proprioceptive function should be evaluated. Additional information can be gathered using standard and stress radiographs. In addition, concomitant pathologic conditions can be investigated by magnetic resonance imaging. Conservative rehabilitation composed of the range of motion, muscle strengthening, and proprioceptive exercise is the main treatment for functional instability and mechanical instability. Regarding the mechanical instability, surgical treatment can be considered for irresponsible patients after a sufficient period of rehabilitation. Anatomic repair (modified $Brostr{\ddot{o}}m$ operation) is regarded as the gold standard procedure. In cases with poor prognostic factors, an anatomical reconstruction or additional procedures can be chosen. For combined intra-articular pathologies, arthroscopic procedures should be conducted, and arthroscopic lateral ligament repair has recently been introduced. Regarding the postoperative management, early functional rehabilitation with short term immobilization is recommended.

Cellular Protrusions - Lamellipodia, Filopodia, Invadopodia and Podosomes - and their Roles in Progression of Orofacial Tumours: Current Understanding

  • Background: Protrusive structures formed by migrating and invading cells are termed lamellipodia, filopodia, invadopodia and podosomes. Lamellipodia and filopodia appear on the leading edges of migrating cells and function to command the direction of the migrating cells. Invadopodia and podosomes are special F-actin-rich matrix-degrading structures that arise on the ventral surface of the cell membrane. Invadopodia are found in a variety of carcinomatous cells including squamous cell carcinoma of head and neck region whereas podosomes are found in normal highly motile cells of mesenchymal and myelomonocytic lineage. Invadopodia-associated protein markers consisted of 129 proteins belonging to different functional classes including WASP, NWASP, cortactin, Src kinase, Arp 2/3 complex, MT1-MMP and F-actin. To date, our current understanding on the role(s) of these regulators of actin dynamics in tumors of the orofacial region indicates that upregulation of these proteins promotes invasion and metastasis in oral squamous cell carcinoma, is associated with poor/worst prognostic outcome in laryngeal cancers, contributes to the persistent growth and metastasis characteristics of salivary gland adenoid cystic carcinoma, is a significant predictor of increased cancer risk in oral mucosal premalignant lesions and enhances local invasiveness in jawbone ameloblastomas.

Cognitive Outcome of Pediatric Moyamoya Disease

  • Quality of life is the current trend and issue for the most of human diseases. In moyamoya disease (MMD), surgical revascularization has been recognized as the possible assistance to reduce the neurological insult. However, the progressive nature of the disease has been invincible so far. To improve the quality of life of MMD patients not only the protection from the neurological insult but also the maintenance or improvement of cognitive function is inevitable. For pediatric MMD patients, younger age or longer duration of disease is the key factor among the prognostic factors for bad neurological outcomes. Hence, 'the earlier, the better' is the most precious rule for treatment. Protection from neurological insult is very critical and foremost important to improve cognitive outcome. Clinicians need to know the neuropsychological profile of MMD patients for the care of whole person and make an effort to protect the patients from neurological insults to maintain or improve it.

청장년층 뇌졸중환자의 회복 경험 (The Recovery Experience of Young Adults and Middle Aged Stroke Patients)

  • Purpose: The purpose of this study was to determine an increase in the number of younger and middle-aged people who have a stroke and the differences in their recovery experience compared with older people. Methods: The research question for this study was "What is the recovery experience of young adults and middle-aged people who suffer a stroke?". In order to answer the question, the grounded theory method was utilized. The data was collected through individual in-depth interviews of six participants, their age ranged from 36 to 45 years old. Results: After comparative analysis, the core category was "standing up for oneself with limit". The experience process were categorized into four stages: 'Facing Reality stage', 'Motivation stage', 'Desire Recovery stage', 'Self-Overcoming stage'. Conclusion: Most of the research for the recovery process was prognostic in nature and the results. This study was demonstrated certain indicators which can be useful in further research.

치매 환자에서 기능 영상법의 역할 (The Role of Functional Imaging Techniques in the Dementia)

  • Evaluation of dementia in patients with early symptoms of cognitive decline is clinically challenging, but the need for early, accurate diagnosis has become more crucial, since several medication for the treatment of mild to moderate Alzheimer' disease are available. Many neurodegenerative diseases produce significant brain function alteration even when structural imaging (CT or MRI) reveal no specific abnormalities. The role of PET and SPECT brain imaging in the initial assessment and differential diagnosis of dementia is beginning to evolve vapidly and growing evidence indicates that appropriate incorporation of PET into the clinical work up can improve diagnostic and prognostic accuracy with respect to Alzheimer's disease, the most common cause of dementia in the geriatric population. in the fast few years, studios comparing neuropathologic examination with PET have established reliable and consistent accuracy for diagnostic evaluations using PET - accuracies substantially exceeding those of comparable studies of diagnostic value of SPECT or of both modalities assessed side by side, or of clinical evaluations done without nuclear imaging. This review deals the role of functional brain imaging techniques in the evaluation of dementias and the role of nuclear neuroimaging in the early detection and diagnosis of Alzheimer's disease.

Variation of Blood T Lymphocyte Subgroups in Patients with Non- small Cell Lung Cancer

  • Objectives: To study variation in T lymphocyte subgoups and its clinical significance in non-small cell lung cancer (NSCLC). Methods: Levels of CD3+, CD4+, CD8+, CD4+/CD8+, NK and Treg cells in peripheral blood of NSCLC cases and healthy adults were determined by flow cytometry. Results: CD3+, CD4+ and CD4+/CD8+ ratio and NK cells in NSCLCs were decreased significantly in comparison with the control group (P < 0.01), and decreased with increase in the clinical stage of NSCLC, while CD8+ cells demonstrated no significant change (P > 0.05). Treg cells were significantly more frequent than in the control group (P < 0.01), and increased with the clinical stage of NSCLC. Conclusion: The cellular immune function of the NSCLC patients is lowered. It is important to detect change of T lymphocyte subgroups by flow cytometry for the diagnosis, treatment and prognostic assessment of NSCLC patients.

Regulatory Network of ARF in Cancer Development

  • ARF is a tumor suppressor protein that has a pivotal role in the prevention of cancer development through regulating cell proliferation, senescence, and apoptosis. As a factor that induces senescence, the role of ARF as a tumor suppressor is closely linked to the p53-MDM2 axis, which is a key process that restrains tumor formation. Thus, many cancer cells either lack a functional ARF or p53, which enables them to evade cell oncogenic stress-mediated cycle arrest, senescence, or apoptosis. In particular, the ARF gene is a frequent target of genetic and epigenetic alterations including promoter hyper-methylation or gene deletion. However, as many cancer cells still express ARF, pathways that negatively modulate transcriptional or post-translational regulation of ARF could be potentially important means for cancer cells to induce cellular proliferation. These recent findings of regulators affecting ARF protein stability along with its low levels in numerous human cancers indicate the significance of an ARF post-translational mechanism in cancers. Novel findings of regulators stimulating or suppressing ARF function would provide new therapeutic targets to manage cancer- and senescence-related diseases. In this review, we present the current knowledge on the regulation and alterations of ARF expression in human cancers, and indicate the importance of regulators of ARF as a prognostic marker and in potential therapeutic strategies.

TSPAN12 Precedes Tumor Proliferation by Cell Cycle Control in Ovarian Cancer

  • TSPAN12, a member of the tetraspanin family, has been highly connected with the pathogenesis of cancer. Its biological function, however, especially in ovarian cancer (OC), has not been well elucidated. In this study, The Cancer Genome Atlas (TCGA) dataset analysis revealed that upregulation of TSPAN12 gene expression was significantly correlated with patient survival, suggesting that TSPAN12 might be a potential prognostic marker for OC. Further exploration showed that TSPAN12 overexpression accelerated proliferation and colony formation of OVCAR3 and SKOV3 OC cells. Knockdown of TSPAN12 expression in A2780 and SKOV3 cells decreased both proliferation and colony formation. Western blot analysis showed that several cyclins and cyclin-dependent kinases (CDK) (e.g., Cyclin A2, Cyclin D1, Cyclin E2, CDK2, and CDK4) were significantly involved in the regulation of cell cycle downstream of TSPAN12. Moreover, TSPAN12 accelerated mitotic progression by controlling cell cycle. Thus, our data demonstrated that TSPAN12 could be a novel molecular target for the treatment of OC.
