유치 치수치료가 치아 탈락에 미치는 영향에 관한 후향적 연구 (A Retrospective Study on the Effect of Pulp Treatment on the Exfoliation of Primary Teeth)

  • 이 연구의 목적은 유치 치수치료가 치아의 상실에 미친 영향과 관련 요인에 대해 후향적으로 분석하는 것이다. 연구에는 단국대학교 부속 치과병원 소아치과에 내원하여 유치 치수치료를 받은 6개월 - 12세의 환자 97명의 167개 치아가 포함되었고, 치수치료와 치아상실에 관련된 정보가 수집되었다. 연구에 포함된 환자는 남자 56명(57.7%), 여자 41명(42.3%), 치아는 유전치 72개(43.1%), 유구치 95개(56.9%)였다. 평균 관찰기간은 106.1 ± 38.7개월이고, 치료 시 연령은 평균 유전치 34.8 ± 15.4개월, 유구치 69.1 ± 25.1개월이었다. 한 악궁에서 한쪽 치아만 치수치료를 시행한 환자의 좌우측 동명치 탈락 또는 발치 시기를 비교하였을 때 치수절제술된 치아가 유의하게 빨리 상실되었다(p < 0.05). 또, 치료 전에 치근단 염증을 보이는 경우, 치수치료 후에도 염증으로 인해 발치할 확률이 유전치에서 증가했고(p < 0.05, Odds Ratio = 11.04), 유구치에서는 그렇지 않았다(p > 0.05). 치수절제술이 시행된 유치는 유전치에서 평균 7.8개월, 유구치에서 평균 8.5개월 조기 탈락하였다. 유치의 조기 탈락은 공간상실과 계승 영구치의 조기 맹출로 이어질 수 있고, 이는 유치의 치수치료 및 치료계획 수립 시 판단기준으로 활용될 수 있을 것으로 보인다.

X선에 의한 유합치와 계승치에 관한 통계학적 고찰 (The Statistical Study on Primary Fused Teeth and The Corresponding Permanent Teeth by Orthopantomograph)

  • 970 children visited attached hospital, S.N.U, were taken orthopantomograph. Of those children, there were 14 patients who have primary fused teeth. The author observed those cases and got following results. 1) Incidence of primary fused teeth was higher in female. 2) In the order of frequency, fused teeth of mandibular right primary lateral incisor and canine were 9 and that of mandibular left primary lateral incisor and canine were 4, and there was only one case in maxillary. 3) 13 cases showed the congenital absence of lateral incisor of the corresponding permanent teeth. 4) Incidence of fused teeth was 1.44%.

치아별 우식경험도로 본 유치열의 우식패턴에 관한 연구 (CARIES PATTERNS IN PRIMARY DENTITION BY CARIES EXPERIENCE OF INDIVIDUAL TEETH)

  • To study the caries patterns in primary dentition, 719 preschool children, 4-6 years old, were examined for their caries activity(salivary reductase activity) and caries experience of individual teeth. Teeth groups were made by cluster analysis using dft indexes of individual teeth as criteria. The six major teeth groups in the order of dft index from high to low were (1) lower primary molars, (2) upper primary molars, (3) upper central incisor, (4) upper lateral incisor, (5) canines, and (6) lower incisors. There were significant differences in dft index between teeth groups except upper lateral incisor and canines. Upper and lower primary molars showed the highest correlation in dft index, and the next couples were upper central incisors and upper lateral incisors, upper lateral incisor and canines, upper central incisor and canines, upper lateral incisor and upper primary molars, and canines and upper primary molars in descending order. Upper first primary molar showed the greatest differences in dft index between caries activity levels.

유치 발거 원인에 대한 후향적 연구 (Reasons for primary teeth extraction in children : Retrospective study)

  • The purpose of this study was to investigate the principal reasons for primary teeth extractions and the tooth type extracted in children. 1159 patients were selected in this study. Dental records and radiographs were reviewed and age, gender, medical history, type of tooth extracted and the reasons for extraction were collected. The data were statistically analyzed using Chi-square test. Total 2078 primary teeth were extracted. Central incisors(34.1%) were most frequently extracted. Extractions due to physiological mobility(77.5%) were the most frequent followed by caries(13.8%), orthodontic(3.9%), trauma(1.7%). Reason for the extraction was different according to age (p = 0.000), but there was no difference according to gender (p = 0.109). While extractions due to physiological mobility predominated overall, reason for extraction was different according to the tooth type. There was no previous treatment in 54.6% of primary teeth extracted due to caries. Excluded physiological mobility, caries are the most common reason for extraction of primary teeth. The importance of preventive care should be emphasized in order to preserve primary teeth and improve children's oral health.

미취학 아동의 치아우식증과 식습관 및 간식섭취와의 관계 (A study on the correlation between the dental caries and dietary habits and snack intake of pre-school children)

  • Objectives : The purpose of this study was to correlation between the index of dental caries in primary teeth and the habits of diet and snack which are expected to influence on the index. The subjects of this study were 165 parents of the children who visited a pediatric dental hospital located in Gyeong-gi do from Jun. 15th to Sep. 26th, 2009, and the self-report questionnaire survey was performed. Methods : A questionnaire used in a previous study was revised and completed for this study and it consisted of 15 items on general characteristic, nine items on dietary habit, and five items on snack intake habit. The collected data was analyzed using SPSS program and findings are as follows Results : 1. The index of dental caries in primary teeth by ages was that children of four years old has 4~5 with 27.5%, children of five years old has 2~3 and 4~5 with 33.3% and children of six years old has 1 and 8~9 with 25.6%, showing a statistically significant(${\chi}^2$=37.72, p<.01). 2. The frequency of brushing teeth was found to have statistically significant negative correlation with the number of primary teeth with dental caries (${\chi}^2$=61.20, p<.001). 3. The frequency of taking snack was found to have statistically significant positive correlation with the number of primary teeth with dental caries (${\chi}^2$=51.42, p<.001). 4. The desirability of dietary habit was found to have statistically significant negative correlation with the number of primary teeth with dental caries (${\chi}^2$=36.51, p<.001). 5. The frequency of taking biscuit was found to have statistically significant positive correlation with the number of primary teeth with dental caries (${\chi}^2$=67.74, p<.001). 6. The frequency of taking bread was found to have statistically significant positive correlation with the number of primary teeth with dental caries (${\chi}^2$=29.63, p<.01). 7. The frequency of taking caramel and candy was found to have statistically significant positive correlation with the number of primary teeth with dental caries (${\chi}^2$=38.85, p<.001). 8. The frequency of taking soft drinks was found to have statistically significant positive correlation with the number of primary teeth with dental caries (${\chi}^2$=52.92, p<.001). 9. The frequency of taking ice-creams was found to have statistically significant positive correlation with the number of primary teeth with dental caries (${\chi}^2$=75.07, p<.001). Conclusions : Those findings show that the children with undesirable dietary habit and higher frequency of taking snack have higher index of dental caries in primary teeth, therefore, it is considered that the regular brushing teeth and establishment of desirable dietary habit are very important for keeping health oral cavity. It is recommended that the continuous concern and repeated learning are needed in house and group of rearing infants so that the children take foods of fruits which have function of self-purification than the food of carbohydrates which has high adhesion and level of sweetness.

한국인아동의 유합유치에 관한 통계학적 연구 (STATISTICAL STUDY OF THE FUSED TEETH IN THE PRIMARY DENTITION IN KOREAN CHILDREN)

  • This study was designed to find out the prevalence d the fused teeth in the primary dentition and their successors. The clinical and panoramic radiographic examination was undertaken of 5226 children (male 2812, female 2414) attended department of Pedodontics, College of Dentistry, Yon Sei University, Seoul, Korea. The results were as follows; 1. The prevalence of the fused teeth in the primary dentition was 1.03 percent and female (1.2%) affected more frequently than male (0.89%). 2. Compare with the maxilla and mandible, the prevalence of the fused teeth were higher in the mandible than in the maxilla. 3. The fusion of the primary teeth were occured between central with lateral incisor in the maxilla, and central with lateral and lateral with canine in the mandible. 4. There were 33 cases (61%) of succeding teeth missing among 54 case of the rused teeth.

한국 어린이에서 하악 유절치와 그 계승 영구치의 선천적 결손 간의 관계 (THE RELATIONSHIP BETWEEN THE CONGENITALLY MISSING PRIMARY LOWER ANTERIOR TEETH AND THEIR SUCCEDANEOUS PERMANENT TEETH IN KOREAN CHILDREN)

  • 많은 연구에서, 유치와 그 계승 영구치의 선천적 결손 간에는 상당한 연관관계가 있는 것을 보고하였다. 그러나 하악 전치부에서, 유치의 선천적 결손에도 불구하고, 그 계승 영구치는 정상적으로 존재하는 증례가 임상적으로 종종 관찰되기도 한다. 본 연구의 목적은 한국 어린이에서 하악 유절치와 그 계승 영구치의 선천적 결손 간의 상호 연관관계를 평가하는 데에 있다. 2005년 1월 1일부터 2008년 9월 5일까지 서울대학교치과병원에 내원하여 파노라마 방사선사진을 촬영한 6세 이하의 남녀 어린이 총 14,307명을 대상으로 하악 전치부의 치아양상을 관찰하여 유치와 영구치의 선천적 결손 관계를 평가하여 다음과 같은 결과를 얻었다. 1. 하악 유절치의 결손률은 0.24%였으며, 하악 유절치가 존재 시 후속 영구치가 존재할 승산(odds)은 7163.5였으며, 유절치의 결손시 후속 영구절치가 존재할 승산(odds) 0.79였다. 2. 하악 유절치 결손의 증례 중 후속 영구치가 모두 존재하는 경우는 44.12%였다. 유절치의 양측 결손 시 후속 영구치가 하나라도 존재할 승산(odds)은 유절치 편측 결손 시에 비하여 1.57 배였다. 3. 유절치 결손이 있을 때 유절치의 편측 결손일 승산(odds)은 여자가 남자에 비해 2.2 배였다. 또한, 유절치 결손이 있을 때 후속 영구절치가 존재할 승산(odds)은 남자가 여자의 경우에 비하여 2.22 배였다.

치면열구 전쇄재의 미세누출에 관한 자가방사법적 연구 (AN AUTORADIOGRAPHIC STUDY OF THE MICROLEAKAGE AROUND A PIT AND FIGURE SEALANT)

  • The purpose of this study was to observe the microleakage of a pit and fissure sealant at the tooth-restoration interface. One hundred and sixty-seven extracted permanent and deciduous teeth were used and some simple mechanical pretreatment were done before etching. The degree of microleakage was assessed by $Ca^{45}$ after 24 hours, and 2 months. The results were as follows: 1. Among total 167 teeth, 53 teeth (32%) were demonstrated the microleakage. 2. After 2 months the leakage was increased from 22 percent to 35 percent in permanent teeth, and from 32 percent to 43 percent in primary teeth. 3. The group treated by fluoride paste was demonstrated the largest leakage, 50 percent in primary teeth and 33 percent in permanent teeth. There are more leakage in primary teeth (38%) than permanent teeth (28%).

상악 우측 유전치부에 발생한 삼중치 : 증례 보고 (Triplication of upper right primary anterior teeth : a case report)

  • The fusion is an anomalous union of two or more tooth germ at some stage in the development process resulting in a decreased number of teeth in the dental arch. Fusion is common odontogenic anomaly but triplication of primary teeth is a rare phenomenon. We report a case of a 3 - year - old girl who visited our clinic for the pain on maxillary anterior tooth. The patient was diagnosed by the fusion of a the maxillary primary right central and lateral incisor with a supernumerary tooth and caries of dentine. And then, Endodontic treatment and composite resin restoration were performed on the triple teeth. After follow-up of 6 months period, there was no marked complications.

Open-face Stainless Steel Crown을 이용한 유전치의 치료에 대한 증례보고 (A CASE REPORT ON PRIMARY INCISOR TREATMENT USING OPEN-FACE STAINLESS STEEL CROWN)

  • In spite of the improvements of the techniques in the field of preventive dentistry, many children still present with extensive destruction of primary anterior teeth. Not only the practioner must consider the pulp state of the primary incisor, but also restore the form, function and esthetics of the tooth. Restorative treatment of primary incisor tooth requires durability, retention and esthetics. Stainless steel crowns used in restoring primary anterior teeth is retentive and durable in comparison with the composite resin, celluloid crown. But they are not esthetic. To enhance the esthetics of the anterior stainless steel crown without reducing its superior retention, an open-face stainless steel crown has been suggested. Several authors have suggested cutting away the labial portion of the stainless steel crown and placing the composite resin in that area. By following this technique, the practioner can prepare a retentive, durable, and esthetic restoration for primary teeth which have suffered from extensive loss of teeth structure. In addition, the single missing primary anterior teeth can be successfully restored by soldering the stainless steel crown together. Open-face stainless steel crown is indicated in the areas of large interproximal lesions involving incisal edge, crown fracture with pulp exposure and congenital malformation of the teeth. By this technique, the practioner can restore primary anterior teeth successfully regardless of the amount of remaining tooth structure, bruxism habit and presence of attrition. In this case, rampant caries with extensive loss of tooth structure and single missing of primary anterior tooth hart been successfully treated with open-face stainless steel crown.
