Clinical Practice Guideline for Soeumin Disease of Sasang Constitutional Medicine: Diagnosis and Algorithm (소음인체질병증 임상진료지침: 진단 및 알고리즘)

  • Objectives This research was proposed to present Clinical Practice Guideline(CPG) for Soeumin Disease of Sasang Constitutional Medicine(SCM): Diagnosis and Algorithm. This CPG was developed by the national-wide experts committee consisting of SCM professors. Methods We searched the literature and articles related to Soeumin Symptomatology diagnosis and algorithm. For developin diagnosis and algorithm, we searched the classification, ordinary symptom, present symptom of the Soeumin Symptomatology Results & Conclusions We classified the Soeumin Symptomatology by 4 steps: Exterior-Interior disease, favorable-unfavorable pattern, mild-moderate-severe-critical pattern, initial-intermediate-advanced pattern. And at the unfavorable pattern, ordinary symptom is very important. So Doctors focuss on the symptom of unfavorable's ordinary symptom such as temperament inclined symptom, excessive sweating, diarrhea, and vexation.

A study on stress factors in dental hygienists caused by students in clinical practice by pattern of hospital (병원형태별 치과위생사가 임상실습 과정 중 실습생으로부터 경험하게 되는 스트레스 요인에 관한 연구)

  • Objectives : The research was performed to be used as basic data for dental hygienists' job satisfaction during clinical practice, and planning and operating effective clinical practice by analyzing stress factors in dental hygienists and the extent of stress caused by students in clinical practice. Methods : The research was aimed at dental hygienists working at college dental hospitals, dental clinic of general hospitals, dental hospitals and dental clinics located in Seoul and Gyeonggi province where clinical practices were conducted for dental hygiene students. For analysis, collected data was analyzed by using SPSS 20.0. Results : Results derived from analysis on characteristic difference by pattern of hospital were observed that there was a significant difference in job satisfaction by pattern of hospital. Results from analysis on correlation among stress factors were observed that there was correlation between behavior and attitude, interpersonal relationships, role and activity, ideal and value, and treatment fields. Results derived from analysis on the effect of stress factors on the job satisfaction were observed that role and activity (p=.002<.01), ideal and value (p=.019<.05), and treatment fields (p=.032<.05) affected the job satisfaction, while behavior and attitude (p=.386>.05), and interpersonal relationships ((p=.421>.05) did not affect the job satisfaction. A stress factor, role and activity (${\beta}$=.350) affected the job satisfaction the most, followed by ideal and value (${\beta}$=.242), and treatment fields (${\beta}$=-.185). For the explanation ability, the stress factors accounted for 20.5% of the job satisfaction. Conclusions : In conclusion, most of the dental hygienists are satisfied with students' clinical practice by pattern of hospital. Positive correlation was shown between stress factors. Behavior and attitude and ideal and value are high in stress in college dental hospitals, role and activity and treatment fields are high in stress in dental hospital, and interpersonal relationships are high in stress in dental clinics.

A Study on the Relationship between Knowledge of Breast feeding and Practice of feeding pattern (모유수유지식에 따른 수유형태에 관한 연구)

  • The purpose of this study was to investigate the relationship between knowledge of breast feeding and practice of feeding pattern. A sample of 117 mothers who have the babies between age from 1 month to 12 months were selected from one university hospital and one public heath care center in J city. The data were collected with the questionnaire during the period from the January. 5, 2002 through January. 30, 2002. Analysis was done by using frequency, percentage, t-test, analysis of variance, and scheffe test. The results of this study can be summarized as follows. Comparision of the method of feeding by general characters. Income state, rearing (p=.019), rearing supporters(p=.026), infant age(p=.03) have shown significant difference as the variables affecting Breast practice rate. Breastfeeding rate were 24.8%, mixedfeeding type, 14.5%, milkfeeding type, 60.7%. The mean of knowledge on breastfeeding by feeding patterns were the highest in the group of breastfeeding. There was statistical significance between the group of breast feeding and milkfeeding(p=.000). In the conclusion, this study revealed that knowledge of breastfeeding promoted practice of breastfeeding. Therefore, to promote practice of breastfeeding, it is necessary that nurses provide adequate information about knowledge of breastfeeding.

The Effect of Doctor's Payment Method on Practice Behavior (지불보상체계가 의사의 진료행태에 미치는 영향 : 미국사례 분석)

  • Considering the existence of asymmetric information between doctor and patient, the doctor's reimbursement method has been considered as a desirable policy device of improving efficiency of patient's use of medical care in terms of its affecting doctor's practice pattern by determining doctor's practice revenue. By using the Community Tracking Study (CTS) physician data set, which includes not only various information on doctors practice arrangements and sources of practice revenue, but also vignettes of various clinical presentations, this paper investigates doctor's reaction to the financial incentive under the control of patient's specific medical situation. Under the econometric model for exploring the effect of doctor's reimbursement method on his/her practice patterns; referring patients, recommend doctor-visit or medical tests, the Hausman's specification test was used for checking out the possibility of the doctor's reimbursement method being endogeneized explanatory variable. In the case where the endogeneity problem of doctor's reimbursement method exists, the 2SLS method was used for correcting that problem, and the multiple regression method was used in the case where the problem is found to be nonexistent. Based on the empirical results, this paper finds that doctors do appear to respond to financial incentive. The empirical results show that the doctor's reimbursement method statistically significantly affects doctor's practice pattern and are coincident with the theoretical result proposed by previous researches, This results suggest, as doctor's practice revenues are mainly determined by prospective method, such as capitation, doctors would more refer their patients to specialists, and hesitate in recommending doctor-visit or medical exam.

  • Objectives The purpose of this study is analyzing internal research trends of oriental obesity pattern identification in korean literature based on obesity pattern identification questionnaires, development and application of Pattern Identification System to help solve problems of future researches. Methods 6 Korean databases were searched for articles of oriental obesity pattern identification, irrespective of publication year and 13 studies were reviewed. An analytical method such as descriptive statistics and an actual number and percentage was used. Results We collected 13 studies. 4 studies were published in 2012 and 2008, the highest number of studies. 7 Clinical studies were the major research method. The Studies were classified according to the characteristics and design. 5 studies were about research of analysis and improvement of oriental obesity pattern identification questionnaire, the most number of studies. 4 studies were about research of observational studies in clinical on obesity pattern identification. 3 studies were about research of intervention studies in clinical on obesity pattern identification and 1 study was about clinical practice recommendation. Conclusions Establishment of obesity pattern identification system and its clinical application could lead to standardizing obesity pattern identification and clinical practice guideline. Applied on other diseases, obesity pattern identification system could also lead to improving treatment rate, contributing to the development of clinical practice guidelines and academic field of research.

Development of a Family Pattern Appraisal to Guide a Rogerian Nursing Practice (Rogers 이론에 근거한 가족양상 사정지침개발)

  • We, clinical nurse specialists practising and guiding student practice in a Community health nursing clinic, wanted to develop a family pattern appraisal consistent with Rogers' conceptual system, the nursing model guiding our practice. We use Rogers' model because it is harmonious with the traditional Korean view of the one human, natural and cosmic world. The purpose of our research was to contribute to science - based nursing practice, not only, one helpful model, but also a model of how to use, in guiding practice, a conceptual system which reflects nurse practitioners' philosophy of nursing, is intellectually satisfying, and enriches meaning in daily nursing life. The research objectives were to review the literature on Rogers' model and analyse it according to Kim's five - level analytical framework, to explore Rogers' definition of family, to review appraisals based on Rogers' model, and to develop a family appraisal which is culturally appropriate for use in our community. This work including the use of the appraisal and its refinement with families in our practice which was done during 1994 and 1995, in Seoul, in the Capital of the Republic of Korea. At the highest level of analysis, Rogers conceptual system emphasizes acausality and multidimentional meaning ; the world view is characterized by process, movement and wholeness. The epistemology Is one of holism and the knowledge base includes all forms of experience, from sensory to mystical, objective, and subjective. At the metaparadigm level, nursing focuses on the unitary human being and the environment. At the level of nursing philosophy, the model identifies human being, nursing, nurse, and illness and health. At the paradigm level the model assumes the irriducibility of the human to parts, noncausality and continual change. Rogers' practice methodology consists of pattern manifestation appraisal and deliberative mutual patterning. Under-standing patterns and patterning of people is the key to helping them achieve their potential. At the theory level, the basic assumptions, key concepts, and homeodynamic principles were identified. Rogers states the family energy field is an undividable, four-dimensional negentropic energy field which is in a larger envircinmental field show-ing such characteristics as cannot be predicted by knowledge of individual family members. Based on the word of Rogers scholars, we chose Rogers' correlates of patterning to understand the family unit as a whole-frequency, rhythms, motion, time perception, sleeping-waking beyond waking, pragmatic -imaginative-visionary to develop the appraisal. We, also used some of Barrel's (1988) criteria including interpersonal network and professional health care access and use, and Cordon's (1982) criteria including self perception - self concept modified to fit the family. Our family Pattern appraisal included 1. Influencirg data, 2. Professional health care access and use, 3. Family self perception-self concept, 4. Family interpersonal network, 5. Sleep-wake-be-yond waking, 6. Pragmatic-imaginary-visionary, 7. Family frequency and rhythm, 8. Family motion, 9. Family time perception. The appraisal was used with four families and modified to eliminate overlap and to make it possible for the family member to express themselves more easily. We plan to gain more experience with the appraisal toward further development of the tool.

Research about Relationship between the Quality of life, Oral Health and Total Health of adults (성인들의 구강보건과 건강.삶의 질의 관련성 연구 - 영남지역 일부 주민들을 대상으로 -)

  • The purpose of this research is to increase adults' quality of life by improving oral health. Total 572 people participated in this study in Young-Nam area. Survey research method was used to measure various tools of participants' general index, subjective view of oral health status, oral health knowledge, oral health practice, dietary pattern, OHIP-14, THI, QOL for analyzing the effect on quality of life. The result of the study is summarized as follows; 1. Oral health knowledge has positive effects on oral health practice (0.442), dietary pattern(0.343). Diet has positive effects on OHIP-14(0.187) and OHIP-14 has significant positive effects on THI(0.564). THI also has positive effects on QOL(0.380). But oral health practice index has slight effects on OHIP-14(0.056). 2. Indirect effect indexes on QOL are OHIP-14(0.214), diet(0.040), oral health knowledge(0.019), oral health practice(0.012) listed in order of effect size, and the biggest direct effect on QOL is from THI(0.308). The result of this study shows oral health and total health are important to improve the quality of life. And the knowledge about oral health dietary pattern should be important to improve oral health. But, even with high level of oral health knowledge, the oral health practice is at low level. Health education developing program should be needed. It is required to present basic data which represent adults with national view by extracting groups using sample probability methods as of next task of this study.

An Exploratory Analysis of Constructivist Teaching Practices and Science Teaching Interactions in Earth Science Classes

  • This study aimed to explore how to characterize the earth science inquiry in schools in terms of science teaching interaction and constructivist teaching practice. The constructivist teaching practices were analyzed with Reformed Teaching Observation Protocol (RTOP) in three aspects including (1) student oriented class implementation, (2) subject knowledge and representation, and (3) classroom communication. Fourteen earth science classes were observed and scored with RTOP. The class was evaluated to be transitional stage in terms of constructivist teaching, e.g., moving toward student-centered teaching practice. Especially, Korean teachers tend to lean their classes more on propositional knowledge than procedural knowledge. To interpret science teaching interactions, an earth science teacher with a RTOP top rank was selected. Her class was then videotaped for detailed analysis. I adopted the analytical framework of communicative approaches and discourse patterns among the five aspects of interactions presented by Mortimer and Scott (2003). It was found that this earth science teacher used more authoritative patterns than the dialogic. In addition, she used IRE discourse pattern more frequently. Interestingly, teachers interacted with their students more frequently in the form of repeated (or IRE chain pattern), that is IRFRF (teacher initiation-student response-teacher feedback-student response-teacher feedback) in the context of dialogic communicative approaches, while simple IRE occurred in an authoritative approach. In earth science classrooms, typical interaction may well be constructed in the form of IRFRF chains to allow students free conjectures and abduction.

E-mail Survey for Developing Clinical Guideline Protocol on Acupuncture Treatment for Low Back Pain (요통에 관한 침구임상 진료지침 프로토콜 개발을 위한 전자우편 설문조사)

  • Objectives: This survey was done in order to find out how Korean medical doctors derive pattern identification for acupuncture prescriptions in treating low back pain in real clinical practice. Methods : The survey questionnaire was developed by the committee of experts who major in acupuncture & moxibustion or statistics for acupuncture clinical trial protocol development. The questionnaires were distributed via e-mail to 75 members of Korean Acupuncture & moxibustion society from March 26th to April 14th in 2009. 57 members completed answers, and the computerized data were analyzed by SPSS 17.0 statistical program. Results : 1.57 Korean medical doctors selected meridian pattern identification based on the course of the meridians(44.6%), visceral pattern identification(32.1%), pattern identification based on cause of disease(14.3%) as the most commonly used pattern identification methods for acupuncture prescription when treating low back pain patients in real clinical practice. 2. In meridian pattern identification based on the course of the meridians, gallbladder meridian of hypochondriac region(13.0%), bladder meridian of lateral low back region(11.2%), governer vessel of central low back region(11.7%) were selected 3. In visceral pattern identification, yang deficiency of kidney(20.2%), deficiency of kidney(19.3%), liver(16.7%), yin deficiency of kidney(14.0%), violence qi of kidney(8.8%), small intestine(7.9%) were selected. Conclusions : In our e-mail survey, Korean medical doctors answered that Meridian Pattern Identification based on the course of the meridians is the most often used diagnosis method. Visceral pattern identification, pattern identification based on cause of disease, meridian pattern identification based on symptom and pattern identification based on qi-blood-yin-yang theory in order of frequency used, were selected for low back pain diagnosis in real clinical practice.

Results : 1.54 Korean medical doctors selected meridian pattern identification based on the course of the meridians(52.5%), visceral pattern identification(27.1%), pattern identification based on cause of disease(8.5%) as the most commonly used pattern identification methods for acupuncture prescription when treating knee pain patients in real clinical practice. 2. In meridian pattern identification based on the course of the meridians, liver meridian of the medial knee region(13.2%), bladder meridian of the posterior knee region(12.0%), spleen meridian of the lateral knee region(11.7%), stomach meridian of the anterior knee region(9.8%) and kidney meridian of the medial knee region(8.6%) were selected. 3. In visceral pattern identification, blood stasis of sinews due to liver and kidney deficiency(5.3%), damp joint with yang deficiency of liver and kidney(4.9%), kidney qi deficiency with congealing cold(4.5%), yin deficiency of liver and kidney(4.1%) were selected. Conclusions : In our e-mail survey, Korean medical doctors answered that Meridian Pattern Identification based on the course of the meridians is the most often used diagnosis method. Visceral pattern identification, pattern identification based on cause of disease, pattern identification based on symptom and pattern identification based on qi-blood-yin-yang theory in order of frequency used, were selected for knee pain diagnosis in real clinical practice.
