Diet and Health-Related Factors of the Middle-Aged and the Elderly in Korea

  • This study was designed to assess diet and health-related factors of older adults in Korea. Subjects females were 2,660 adults aged 50 and over living in Korea. Males were 847 persons and were 1813 persons. The mean weight and height for males and females were 63.8 $\pm$ 0.3kg / 164.0 $\pm$ 0.2cm and 57.0 $\pm$ 0.2kg /150.6 $\pm$ 0.1cm respectively. BMI (body mass index), body fat, and percent fat were significantly greater in females than in males. The muscle mass and body water were significantly greater in males than in females. Twenty-one percent of total subjects lived alone and 26% with spouse only. Most of the subject's self-reported income was in middle level (65%) or low level (24%). Proportion of subjects who answered 'very poor' or 'poor' on perceived health status was higher in older group. The 50-64 years old group was facing more stress than 65yr and over group. Among male subjects,38.4% were current-smokers and 22.0% were ex-smokers. But only 6.5% of female subjects were current-smokers. Males turned out to have better dietary habits-meal frequency per day, mealtime regularity, regular meal size and balanced eating-than females (p < 0.001). This study revealed that the diet and health-related factors affect nutritional status and chronic diseases of the elderly. For better management and evaluation of health status of the elderly, more effective nutritional assessment tools should be developed.

A Gender analysis of the association between Self-rated Health and Disability of daily living among Korean Elderly (주관적 건강 수준은 노인의 일상생활 수행 능력 정도를 예측할 수 있는가: 성별 비교를 중심으로)

  • Objective : This study aimed to examine the association Self-Rated Health(hereafter SRH) and Activities of Daily Living(hereafter ADL), Independent Activities of Daily Living(hereafter IADL) among Korean elderly. Methods : Using nationally representative cross sectional studies called 2014 Survey of living conditions and welfare needs of Korean elder persons. We used SRH as an independent variable, disabilities of ADL and IADL as dependent variables. Logistic regression was implemented and potential confounders such as socio-economic status, demographic characteristics and others were adjusted. Results : It was revealed that people who were female(than male) and 75 or more years old(than less than 75 years old) were shown to have poor SRH and disabilities of both ADL and IADL. In logistic regression model, poor SRH was associated with ADL and IADL disabilities. Especially, their association was more remarkable among male than among female. It can be understood that socio-economic contexts affect quality of life among elderly and SRH can be differently perceived by gender. Conclusions : Our notable findings showed that gender perspective is needed to understand SRH and daily living among elderly.

Comparison of Sleep Pattern, Factors of Sleep Disturbance and Sleep Enhancement Behaviors between Sleep Disturbance and Non-Sleep Disturbance Aged Patients (수면장애 노인 환자와 일반 노인 환자의 수면양상, 수면장애요인 및 수면증진행위)

  • Purpose: This study was done to compare sleep patterns, factors of sleep disturbance and sleep enhancement behaviors between sleep disturbance and non-sleep disturbance elderly patients. Method: The participants were 117 patients over 65 years old who were hospitalized at 5 general hospitals in Seoul. There were 83 patients in the good sleeper group and 34 in the poor sleeper group. The survey questionnaires included questions on general characteristics, sleep patterns, sleep disturbing factors, and sleep enhancement behaviors of the patients. Results: For sleep patterns and environmental factors of sleep disturbance, the poor sleepers was significantly higher than that of the good sleepers. Significant differences between good sleepers and poor sleepers were also found on the following variables: Environmental factors of sleep disturbance -'Sound of other patient's or care-giver's', 'Discomfort of bed, linen, pillow and patient's gown', 'Light in the room', 'Emergency situations', 'Temperature of patients rooms too hot or too cold'. The significant physical factor of sleep disturbance was 'Hard to breath even without moving' and for sleep enhancement behaviors: 'Use of ear plugs or eye covers' Conclusion: Consequently it is necessary to develop positive and differentiated programs for sleep enhancement for the inactive and poor sleeper among the elderly patients.

The Development of a Retirement Home Model with Emphasis on Clothing, Nutrition, Housing, and Psychological Dimension (노인의 의.식.주생활 및 심리적 적응을 위한 이상적 모델개발 연구 -양로원을 중심으로-)

  • The present study was conducted to devlop and appropriate retirement home model for the elderly in terms of clothing and textiles; nutrition, health, and foodservice; housing and environment; and psychological adaptation. Specifically, the purposes of the study were: 1) to provide basic guidelines for clothing by comparing the clothing behavior of the elderly living in the retirement home and those living in their own homes with family, 2) to provide basic guidelines for balanced diet and effective foodservice, 3) to develop an ideal life space and facilities, and 4) to assess the psychological characteristics of the elderly. Questionnaires, observation, experimental method, and survey of literature were used for the study. Clothing behavior showed that the elderly were much concerned about clothing, and they preferred comfortable as well as fashionable designs. The elderly in the retirement homes complained of a lack of quantity and variety in clothing. They preferred natural fiber rather than blended fabrics. Flame resistance, thermal insulation, and flexibility of textile fabrics were found to be prime considerations in manufacturing and selecting clothing materials for the elderly. The health status of the surveyed elderly was generally good, but some poor eating habits were observed. Dietary nutrients intakes were generally sufficient, but several nutrients intakes were insufficient. The level of equipment in the kitchens of the institutions was low. Furthermore, the employment rate of dietitians in institutional settings was extremely low. This resulted in a lack of systematic foodservice management. Residents in the institution were generally satisfied with present life space and facilities but this was mainly because of abandonment, adaptation, and past experience. Optimal allocation of residents per bedroom and an adequate design for storage, bathroom, utility room, and dining room were recommended. The comparison of psychological status of the elderly living at home was more stable than those living in an institution. The emotional state of the elderly living in the institution was characterized by loneliness; they did not have close interpersonal relationships or future plans. An appropriate model for the elderly was developed on the basis of these findings.

  • As the elderly population increases rapidly, existing houses many elderly reside are under poor conditions that they work as a factor to impede on the independent living of elderly people. Thus, improving existing housing environment to suit the needs of elderly life has been required. Academics has been continuously researched on the field considering the importance of elderly housing modification and the importance on the research of this field is projected to be greater with the coming of aging society. Hence, this study has a purpose on analyzing trends of previous studies related to housing modification for the elderly published in Korean academic journals in depth. This study is a content analysis study and analyzed the trend of previous studies with academic journal field, purpose of study, research subject, research area, research method, research content as analysis categories. As a result, previous studies was published the most in natural science academic journals and main stream of research was to suggest direction of housing modification by conducting surveys on actual conditions and needs of single elderly families and to suggest improving the directions of policies related to modification. On the other hand, empirical studies needed to execute housing modification such as standard and method of housing modification and supporting tools are still at minimal level that researches on this field should be conducted additionally. This study is expected to be used as basis to examine the current state of previous studies on housing modification for the elderly and to seek further research directions.

Comparison of Health Promoting Behavior in Rural Elderly by Complementary Alternative Medicine Utilization (농촌노인의 보완대체요법 사용에 따른 건강증진행위 비교)

  • Purpose: To compare the health promoting behavior in rural elderly people by complementary alternative medicine(CAM) utilization. Methods: The data were collected from March 06th to May 26th, 2006. The participants were 207 rural elderly persons in Korea. Data were collected using structured questionnaires and analyzed with the SAS win 8.0 program. Results: The rate of using CAM was 60.9%. Non-religious, perceived poor health and high concerns about health group used it more. The most common type was oriental medicine(35.7%), the most common motive was body protection(21.2%) and major source of information was other patients(35.9%). Almost all the subjects(90%) were satisfied with using CAM. The average score for the health promoting behavior was $2.67{\pm}.32$(range 1.65-3.71). The average scores for each subscale were, personal communication 2.98, self-actualization 2.79, nutrition 2.78, health responsibility 2.58, exercise 2.40, stress management 2.38. Significant differences in health promoting behavior were found according to CAM utilization: personal communication, self actualization. Conclusion: There were differences health promoting behavior between the elderly CAM utilization: Systemic guidelines of CAM use are needed.

Factors Affecting the Self-Rated Health of Vulnerable Elderly (취약계층 노인의 주관적 건강상태에 영향을 미치는 요인)

  • Purpose: This study was to investigate the factors affecting the self-rated health of vulnerable elderly in community. Methods: The subjects were 2,328 elderly over 65 years who were enrolled in the Visiting Health Care Center in J-gu of S-city from Apr. 2007 to Sep. 2008. Data were collected using questionnaires including general characteristics, health related behavior and health status by nurse at the time of enrollment. The collected data were analyzed by descriptive statistics, test and multivariate logistic regression. Results: 47.2% of the male subjects and 57.2% of the female subjects rated their health "poor". Gender differences were observed in the factors affecting on Self-Rated Health. ADL, depression and the number of diseases played a major role for men, whereas depression, IADL, the number of diseases, ADL, regular exercise and education played a major role for women. These factors explained $17{\sim}29%$ of variance in Self-Rated Health. Conclusion: Gender-specific programs for vulnerable elderly may be developed based on this study. Both physical and psychological functions need to be integrated into the programs to improve self-rated health of vulnerable elderly.

Analysis of Health and Social Needs for the Establishment of Coordinated Health and Social Service (보건복지 통합사업을 위한 보건복지욕구 분석)

  • The study was performed to provide basic data for developing a coordinated health and social service model. Method: The subjects consisted of 1000 residents in Wonju city. The sampling process was randomized stratified into 26 geographical areas. The data was collected from Apr. 16nd, to May. 15nd, 2002 by using a structured questionnaire that included general characteristics, the patterns of disease, the family function and the community support systems. The collected data was analyzed by the SPSS. windows program via descriptive statistics, correlation testing and t-tests. Result: 61.7% of the elderly perceived their health status as being poor. There were correlations among age, income and the way clients perceived their health status. The prevalence rate of chronic disease was 70.8% in the elderly. The prevalence rate for arthritis was the highest (33.6%). The average number of diseases in the target populations was 2.31; it was 2.65 in the deprived elderly group and 2.47 in the elderly group. 35.4% of residents did not have any family care giver. 61.2% of the clients needed support by the health and social services. Conclusions; Coordinated health and social services need to be allocated by priority to the elderly and to the socially and economic deprived clients.

The Effect of Shoulder Stabilization Exercise through Visit Rehabilitation on Muscle Activity and Postural Alignment, Self-Efficacy in Rural Elderly People with Round Shoulders

  • Purpose: This study covered the effect of shoulder stabilization exercise on muscle activity, postural alignment and self-efficacy of the elderly people with round shoulders by visiting village hall in rural area. Methods: 40 elderly people with round shoulders were recruited for this study (experimental group: 20 subjects and control group: 20 subjects). The experimental group was asked to perform shoulder stabilization exercise 60 minutes per one time for 8 weeks and three times in a week and the control group was asked to receive education related to pain management for the initial one time. Before and after the experiment, we measured the subjects' muscle activity of upper trapezius, serratus anterior, infraspinatus, pectoralis major and the lower trapezius, then postural alignment of craniovertebral angle and round shoulder posture. In addition, self-efficacy is measured through the questionnaire. Results: Compared to the control group, the experimental group showed significant differences in the muscle activity of all muscles, posture alignment of the craniovertebral angle. round shoulder posture, and improvement of self-efficacy. Conclusion: We found that shoulder stabilization exercise is effective in muscle activity, postural alignment and self-efficacy of the elderly people through visit-rehabilitation service. This study can be used for improving the physical and mental abilities through active visit-rehabilitation service for areas where have poor medical benefits. It is considered that systematic visit rehabilitation service should have institutional framework ultimately.

A Study of Proposal of Direction to Promote the Supply of Senior Welfare Housing in the Private Sector (민간부문의 노인복지주택 공급 활성화를 위한 방향성 제안)

  • Purpose: In our country, the aged population accounted for about 13% of the total population in 2018. In addition, it is expected to exceed 20% in 2026, entering an super-aged society. However, the senior welfare housing in Korea currently accommodates only 0.08% of the elderly population, and the increasing amount of the elderly population is very small. Accordingly, this study aims to analyze the causes of supply shortages and to propose directions for revitalizing the supply of welfare housing for the elderly in the private sector. Methods: First, the causes and problems of the poor supply of welfare housing for the elderly were analyzed. Next, the direction was proposed through institutional and case-by-case surveys and expert interviews. Results: It suggested directions such as legal and institutional improvement, inducing the use of REITs, preparing policies to support consumers, allocating a certain percentage of senior welfare housing for the elderly in the reconstruction project, improving awareness of monthly living expenses, and re-discussing sales permits. Implications: Experts said that the direction proposed in this study can have a positive effect on supply activation. However, there were many opinions that re-discussion on permission for sales was necessary due to the excessive impact of the inability to sell. In addition, further research is needed for specific application of the proposed direction.
