Pharmaceutical Care for Medication Safety in Critically Ill Neonates (신생아중환자의 안전한 약물사용을 위한 약료서비스)

  • Objective: This study aimed to investigate pharmaceutical care for critically ill neonates and suggest targeted strategies compatible with the Korean health-system pharmacy. Methods: Articles that reported pharmacy practices for critically ill neonates were reviewed. Pharmaceutical care practices and roles of neonatal pharmacists were identified, and criteria were developed for neonates in need of specialized care by clinical pharmacists. Results: Neonatal pharmacists play many roles in the overall medication management pathway. For clinical decision support, multidisciplinary ward rounds, clinical pharmacokinetic services, and consultation for pharmacotherapy and nutrition support were conducted. Prevention and resolution of drug-related problems through review of medication charts contributed to medication safety. Pharmaceutical optimization of intravenous medication played an important role in safe and effective therapy. Information on the use of off-label medicine, recommended dosage and dosing schedules, and stability of intravenous medicine was provided to other health professionals. Most clinical practices for neonates in Korea included therapeutic drug monitoring and nutrition support services. Reduction in medication errors and adverse drug reactions, shortening the duration of weaning medicines, decreasing the use and cost of antimicrobials, and improvement in nutrition status were reported as the outcomes of pharmacist-led interventions. The essential criteria of pharmaceutical care, including for patients with potential high-risk factors for drug-related problems, was developed. Conclusion: Pharmaceutical care for critically ill neonates varies widely. Development and provision of standardized pharmaceutical care for Korean neonates and a stepwise strategy for the expansion of clinical pharmacy services are required.

  • Obesity is increasing worldwide and has become a major health burden in Western societies affecting every third American and every fifth European. Obesity makes a major contribution to morbidity and mortality, predisposing individuals to cardiovascular disease and diabetes. Many new substances are currently being investigated for their usefulness in the pharmacotherapy of obesity. Most anti-obesity drugs can be divided into four groups: those that reduce food intake; those that alter metabolism; those that increase thermogenesis; and those that regulate hormone involved in feeding behavior. In this article we review these and other agents available in various countries for the treatment of obesity. Perhaps more importantly, we have focussed on areas of potential productivity in the future. Over the last 5 or so years, this impetus in obesity research has provided us with exciting new drugs targets involved in the regulation of feeding behavior and cellular mechanism involved in energy expenditure. Recent development in the quest for control of human obesity include the discovery of hormones, neuropeptides, receptors and transcription factors involved in feeding behavior, metabolic rate and adipocyte development. For developing new, perhaps even more specific pharmacological agents, further research is needed to understand the individual different genetic and physiological basis of obesity. It remains the hope of research scientists that in the not too distant future we shall see a new class of anti-obesity drugs arising logically from the molecular biology revolutions.

Updates in the Management of Coronary Artery Disease (관동맥질환 치료의 최신 지견)

  • Coronary artery disease (CAD) has been increasing during the last decade and is the one of major causes of death. The management of patients with coronary artery disease has evolved considerably. There are two main strategies in the management of CAD, complementary, not competitive, each other; the pharmacologic therapy to prevent and treat CAD and the percutaneous coronary Intervention (PCI) to restore coronary flow. Antiplatelet drugs and cholesterol lowering drugs have central roles in pharmacotherapy. Drug eluting stent (DES) bring about revolutional changes in PCI. In the management of patients with 57 segment elevation acute myocardial infarction (AMI), there has been a debate on the better strategy for the restoration of coronary flow. Thrombolytic therapy is widely available and easy to administer, whereas primary PCI is less available and more complex, but mote complete. Recently published evidences in the pharmacologic therapy including antiplatelet and stalin, and PCI including DES and reperfusion therapy in patients with ST segment elevation AMI were reviewed.

Recent Studies about Conservative Treatment of Osteoarthritis in Knee Joint -Focusing on Domestic Theses for Degree and Journal Since 2000- (퇴행성 슬관절염의 보존적 치료의 최신 연구 -2000~2009년 발표된 임상논문을 중심으로-)

  • The purpose of this study is to know recent studies about conservative treatment of osteoarthritis in knee joint. We reviewed and analyzed 71 theses published by korean research institution since 2000. These theses were classified by year, field of study and subject. The results of this study are as follows. 1. We found 24 oriental medical theses about osteoarthritis in knee joint since 2000. 2. Classified by the major field of study, the journal of korea acupuncture and moxibustion society accounted for 17 pieces in oriental medical journals. 3. In oriental medicine, the most subject is acupuncture(19 pieces). 13 pieces in it is about pharmacopuncture. 4. 47 theses are published in medical journals after 2000. 5. 35 associations dealt with this theme, the korean society of physical medicine is major. 6. Most subject is kinesitherapy, followed by pharmacotherapy, massotherapy, taping therapy, physiotherapy and insole. We found several therapies were effective through oriental and western medical theses. Combined oriental medicine and western conservative therapy like kinesitherapy make better effect on osteroarthritis of knee joint.

Non-Surgical Management for Benign Vocal Fold Lesions (양성 성대 병변의 비수술적 치료)

  • Benign vocal fold lesions, such as vocal nodules, polyps and Reinke's edema, usually result from chronic voice overuse. Conservative management such as voice therapy and pharmacotherapy are used as the primary treatment techniques. The main purpose of voice therapy is to identify and reduce voice misuse to achieve the optimal voice. But complete resolution may not be possible in all patients after voice therapy. Furthermore, some patients with voice-related occupations, voice rest and voice therapy are sometimes difficult, which makes it hard to carry out the treatment. When conservative therapy is ineffective, laryngeal microsurgery can be performed under general anesthesia. However, potential complications following laryngeal suspension and violation of the layered structure of the vocal fold during surgery should be considered before surgery. In recent decades, emerging literatures have demonstrated the potential usefulness of vocal fold steroid injection as an alternative treatment option for benign vocal fold lesions. The most advantageous feature of vocal fold steroid injection is the maintenance of regional anti-inflammatory effects while preventing the potential systemic adverse effects of the steroid. Many non-surgical treatment methods can be conducted using different approaches in the office setting. It can be applied as an alternative treatment modality for the management of various benign vocal fold lesions.

  • In the treatment of hemifacial spasm, the operative methods, such as neurovascular decompression, rhizotomy etc. are the most efficacious therapy on its control. But, due to some side effects and complication, pharmacotherapy has been the first attempted by first intention of hemifacial spasm. The carbamazepine is the most commonly used agent, however it is often inefficacious in some cases, and attenuates as the time passes, engages in some symptoms, such as dizziness, nausea, vomiting, and causes an aplastic anemia, it has many deficiencies in becoming the first intention. Acupuncture is a simple, effective and conservative spasm control modality. It is so simple to use in hemifacial spasm. We had observed 3 cases of hemifacial spasm which were treated by acupuncture in Oriental clinic recently. We reported these cases and reviewed related literatures briefly. The results of treatment are as follows; The clinical conditions of spasm intensity was improved after the therapy. These results suggest that acupuncture and negative therapy be effective as the first intention in healing hemifacial spasm.

Analysis of Drug Compliance Status for the Hormone Replacement Therapy (HRT) Treatment Patients (호르몬대체요법 환자의 복약실태분석)

  • This study is intended to provide information to menopausal women so they can control their symptoms. The basic data was gathered by interviewed 200 postmenopausal women on a HRT program. The women were questioned about their perception of menopause and compliance to the therapy with the fellowing result $71.7\%$ of the women experianced hot flushes, and $64.1\%$ experienced short term memory loss and some psychological symptoms. The women reported that these symptoms reduced their quality of life. $53\%$ of the women responded that menopause is a natural state and $59.1\%$ agreed to HRT based on their doctor's recommendation. $76.2\%$ of the women reported that HRT was effective, with $74.7\%$ reporting reduced hot flush symptoms $67.3\%$ of the women reported compliance with the medication schedule. and $43.9\%$ reported periodic non-compliance. Although HRT has been shown to be an effective treatment, the study found that less than $10\%$ of postmenopausal women are currently being treated and these treated women are generally negligent in continuing with HRT, The study recommends that a program be developed that explains the benefits and risks of HRT to be distributed to patients being treated by doctors and pharmacists. In conclusions, It is very important for hormone replacement therapy on menopausal women to take more intensive medication consultation to increase medication compliance and effectiveness of pharmacotherapy.

The obesity is the aggregation of symptoms accompanied with the metabolic disorder and the increase of fatty layer accumulation by the enlargement or increase of fatty cells, which is the condition that has 20% overweight than the standard. 2. The final metabolic of fat depends on the defensive energy. The shape of fatty layer is changed by the point where is the defensive energy is replenish or not. So Goin, Bein and Yuckin is made a discriminated and there is different between Besudaeso and Herlgidaso. 3. In case of liver overacting spleen by seven emotions, we can use reinforcing the spleen and don't use depressing energy. 4. Damp, weakness of Qi(氣), weakness of spleen came from seven emotion, and social stress can occur obesity. Obesity is reason of C.V.A, dizziness, dementia, schizophrenia, etc. 5. The obesity which can occurs various adult diseases came from eating disorder and various stresses at the standpoint of western medical. This is cured by diet, kinesiatrics, operation, pharmacotherapy, etc.

  • Though infanticide, killing the baby after birth of the neonate and denial of pregnancy, are very rare psychiatric disorder, they have been receiving a lot of social concerns. We report and review infanticide and denial of pregnancy administration in a 19 year-old adolescent with bipolar disorder and borderline personality disorder. Patients with a young age, cognitive immaturity, an unwanted child, hid the pregnancy facts were consistent with the results of previous studies. In addition, the patient's impulsivity and emotional instability is affecting infanticide. After inpatient care with pharmacotherapy (escitalopram 20mg, alprazolam 1.5 mg, clonazepam 0.5 mg, valproate sodium 1,100-1,300 mg, and quetiapine 100-400 mg) and supportive psychotherapy, and there were significant improvement of clinical symptoms.

  • Recently Iontophoresis is tried to physical therapy field.. It is method for direct penetrate to skin with ionization pharmacon because it is decreased side effect of pharmacon treatment such as gastrophathy and aphylaris of disease-causing germs. We designed WIT-I that is trial manufacture for clinical experiment This system is has been elrectrical paramete with 0-140V out range, 1-7mA current 1-15mim time set. Invite experiment, WIT-I kept designed performance which is a constant current in variable load resistance such as skin and electrode contact. It is estimated to have a practical use for physical therapy by iontophcresis.
