  • The persistent increase of aspartate aminotransferase (AST) due to the presence of a macro-AST can confuse diagnostic or therapeutic decisions in many clinical situations. In this study, we report a case of isolated and persistent AST-elevation without any clinical sign of dysfunction in organs such as the liver, skeletal muscle, cardiac muscle, etc. Despite various investigations, no definite cause for the elevation of AST could be found. With the help of polyethylene glycol (PEG) precipitation, we showed that macro-AST formation was responsible for the elevation of the AST titer in this case. Early recognition of macro-AST by PEG precipitation can prevent diagnostic confusion and unnecessary and even invasive tests.

  • Persistent organic pollutants (POPs) and emerging pollutants (EP) are characterized by their difficulty to be removed through biological oxidation processes (BOPs); they persist in the environment and could have adverse effects on the aquatic ecosystem and human health. The electro-oxidation (EO) process has been successfully used as an alternative technique to oxidize many kinds of the aforementioned pollutants in wastewater. However, the EO process has been criticized for its high energy consumption cost and its potential generation of by-products. In order to decrease these drawbacks, its combination with biological oxidation processes has been reported as a solution to reduce costs and to reach high rates of recalcitrant pollutants removal from wastewaters. Thus, the location of EO in the treatment line is an important decision to make, since this decision affects the formation of by-products and biodegradability enhancement. This paper reviews the advantages and disadvantages of EO as a pre and post-treatment in combination with BOPs. A perspective of the EO scale-up is also presented, where hydrodynamics and the relationship of A/V (area of the electrode/working volume of the electrochemical cell) experiments are examined and discussed.

  • Morbihan disease (MD) is a very rare condition characterized by rosaceous or erythematous lymphedema on the upper two-thirds of the face. A definitive management strategy for MD is lacking, and treatment is challenging. Herein, we present a case of persistent bilateral eyelid edema treated by lymphaticovenular anastomosis (LVA) and lymph node-vein bypass surgery. The patient experienced persistent bilateral eyelid edema. Indocyanine green lymphography was performed, and the diagnosis of bilateral facial lymphedema was confirmed. On the right side, a preauricular lymphatic vessel was anastomosed to a vein. On the left side, lymphostomy on the preauricular lymph node was done, with anastomosis to the transected proximal end of the concomitant vein of the transverse facial artery. Furthermore, a preauricular lymphatic vessel was anastomosed to a vein. Eyelid edema decreased and progressively improved on both sides. The outcome of this case suggests that LVA and lymph node-vein bypass surgery are appropriate for treating persistent eyelid edema related to MD.

The evaluation has been performed targeting 60 students who attending the subject of informational education in 3 different universities in 2012 by allowing them to select the available educational mobile App installed in their smart phone. After, the level of persistent learning-intension from each student and the efficacy of ARCS motivational strategy was measured at learner's perspective. The structural equation model(SEM) was established and analyzed with PLS method to understand the relationship between the ARCS motivational strategy and the persistent learning-intension. The results of the study could be summarized as followings. First, the educational mobile App in various the motivational strategies showed different results that is the highest attention as well as the lowest satisfactory level. Second, the relevance in motivation strategies had the statistically significant effect in attention, confidence, and satisfaction. On the other hand, the other factors of attention, relevance, and confidence showed statistically significant effect in satisfaction. Finally, result demonstrate that the relevance is the critical factor inducing the significant effect in persistent learning-intension among the motivational strategies.

  • Trauma to any nerve may lead to persistent paresthesia. Trauma to the nerve sheath can be produced by the needle. The patient frequently reports the sensation of an electric shock throughout the distribution of the nerve involved. It is difficult for the type of needle used in dental practice to actually sever a nerve trunk or even its fibers. Trauma to the nerve produced by contact with the needle is all that is needed to produce paresthesia. Hemorrhage into or around the neural sheath is another cause. Bleeding increases pressure on the nerve, leading to paresthesia. Injection of local anesthetic solutions contaminated by alcohol or sterilizing solution near a nerve produces irritation; the resulting edema increases pressure in the region of the nerve, leading to paresthesia. Persistent paresthesia can lead to injury to adjacent tissues. Biting or thermal or chemical insult can occur without a patient's awareness, until the process has progressed to a serious degree. Most paresthesias resolve in approximately 8 weeks without treatment. In most situations paresthesia is only minimal, with the patient retaining most sensory function to the affected area. In these cases there is only a very slight possibility of self injury. But, the patient complaints the discomfort symptoms of paresthesia, such as causalgia, neuralgiaform pain and anesthesia dolorosa. Most paresthesias involve the lingual nerve, with the inferior alveolar nerve a close second. This is the report of a case, that had the persistent paresthesia care on left lingual & buccal shelf regions after the lingual and long buccal nerve block anesthesia.

  • This study investigated a relationship among facilitating discourse, students' perceived challenge, and learning outcomes(persistent intention and learning achievement) in an online science gifted education program. Two hundreds and forty-two middle school students participated in the study. A survey questionnaire which was consisted of 6 items of facilitating discourse from teaching presence questionnaire(Shea, Swan, & Pickett, 2005) and 5 items of challenge from Student Perceptions of Classroom Quality(Gentry & Owen, 2004) was administered. First, the findings of this study showed that students' perceived facilitating discourse as a part of teaching presence was positively related to students' perceived challenge in an online course. Second, students' perceived facilitating discourse were positively related to persistent intention, but were negatively related to students' achievement. Third, students' perceived challenge was positively related to persistent intention and achievement. Finally, challenge mediated the relationship between students' perceived facilitating discourse and persistent intention, and the relationship between students' perceived facilitating discourse and students' achievement as well. This results suggested that online program should be designed to increase the levels of facilitating discourse.
