Study on the Power Consumption Characteristics of Korean Domestic Species in Peripheral Milling with Image Analysis Technique

  • Peripheral milling is one of the most important wood machining processes in wood industry. Power consumption characteristics of twelve Korean domestic species in peripheral milling were investigated in this study. Image analysis technique was applied to extract proper data from the power consumption profiles. Average power consumption increased as cutting depth increased and specific cutting power decreased as cutting depth increased. However, no significant relationship could be found between density and power consumption and between cutting depth and surface roughness.

  • The complicated helical flow formed in the regenerative turbomachines is usually decomposed into a peripheral component and a circulatory component. On the basis of the momentum exchange theory, the circulatory flow plays a critical role of imparting angular momentum to the peripheral flow. Therefore, the accuracy of performance prediction is dominated by the circulatory flow modeling. Until now the circulatory flow has been accounted of a standstill flow normal to the peripheral flow. However, the circulatory path from the impeller exit to the re-entrance inlet is exposed to the adverse pressure gradient, so it would be more realistic to describe that the circulatory flow is formed on the skewed plane not perpendicular to the peripheral flow. Present study suggests new circulatory flow loss model including the effect of adverse pressure gradient and modifies the effective circulatory flow rate and circulatory pivot which were previously published.

신경차단 시 국소마취제와 함께 사용되는 보조 약물들 (Adjuvant medications to local anesthetics in nerve blockade)

  • Peripheral nerve blocks are commonly used for surgical anesthesia, postoperative analgesia, and to reduce opioid requirements. Although these blocks have traditionally been carried out using local anesthetics, single-injection techniques can be short-lived and limited by the relatively brief duration of action of currently available local anesthetics. Increasing the dose or concentration of local anesthetics may prolong the duration of analgesia, but may also increase the risk such as unwanted motor weakness or systemic toxicity of local anesthetics. Numerous adjuvant medications have been added to local anesthetics to prolong the durations of anesthesia and analgesia achieved by peripheral nerve blocks, and currently, a number of different adjuvants are used to improve quality of the block. This article will review the several nerve block adjuvants used in combination with local anesthetics to provide blockade of peripheral nerves in clinical practice, describing the rationale for their use in peripheral nerve blocks, and the evidence for their effectiveness.

감각이상을 주소로 내원한 당뇨병 환자 8례에 대한 임상보고 (Clinical Study of 8 Diabetic Patients with Paresthesia)

  • Objective : The purpose of this study was to report the effect of Bogijetong-tang on diabetic peripheral neuropathy. Methods : Nine patients who were diagnosed or suspected with diabetic peripheral neuropathy enrolled in this study. They took Bogijetong-tang 2 or 3 times a day and were given acupuncture therapy. To evaluate the therapeutic effect, Visual Analog Scale (VAS) or Total Symptom Score (TSS) were examined at intervals of about 7 days. Result : TSS score decreased an average of 2.74 points and VAS scores decreased an average of 3.67 points. As the pain decreased, accompanied gait disturbance also improved in some patients. Conclusion : Bogijetong-tang is effective in treating diabetic peripheral neuropathy, but study of the concrete mechanism and efficacy is needed.

주변성 법랑모세포종의 증례 (Peripheral ameloblastoma: A case report)

  • Peripheral ameloblastoma is an extremely rare odontogenic soft tissue tumor with histologic characteristics similar to those of the intraosseous ameloblastoma. It appears in the gingiva and oral mucosa. And it usually does not show any bone involvement on radiographs, except for saucer shaped erosion of underlying alveolar bone. Recurrence is considered uncommon. We report a case of peripheral ameloblastoma with bone involvement. Histologically it presented with follicles and nest of tumor cells with palisading pattern. And radiographs showed the typical saucer shaped alveolar bone erosion at the distal area of right mandibular third molar. At 6-month follow-up after operation, no local recurrence was noted.

개의 말초성 거대세포 육아종(peripheral giant cell granuloma) 증례 보고 (Peripheral Giant Cell Granuloma in a Dog)

  • A gingival mass was detected from a 1-year-old female Great Dane dog. After surgical removal, the lesions recurred in 2 weeks and died of septicemia. Characteristic histologic features were large numbers of multinucleated giant cells which were connected with capillary vessels. Neovascularization was prominent with mononuclear and polynuclear cell infiltration. Overall features of these lesions except for giant cell infiltration were similar to granuloma. From these results, a gingival mass excised from a dog was diagnosed to be a peripheral giant cell granuloma (PGCG). This is the first report of canine subcutaneous PGCG in Korea.

  • The paraproteinemia is a disorder in which a single clone of plasma cells (monoclonal gammopathy) is responsible for the proliferation of monoclonal proteins (M-proteins). Approximately 10% of patients with idiopathic peripheral neuropathy have monoclonal gammopathy. Some M-proteins have the properties of an antibody to the components of peripheral nerve myelin, but the pathophysiological relationship between the neuropathy and the M-protein is often obscure. The relationship between peripheral neuropathy and monoclonal gammopathy requires the appropriate neurological and hematological investigations for precise diagnosis and treatment. In this review, we provide an update on the causal associations between peripheral neuropathy and monoclonal gammopathy as well as characteristics of clinical and electrophysiologic features.

  • Ischemic monomelic neuropathy and myopathy are rare complications of peripheral arterial occlusive disease. We report a case of ischemic monomelic neuropathy of the right sural, common peroneal and posterior tibial nerves and ischemic myopathy of the right tibialis anterior resulting from the occlusion of the right common femoral arteries despite successful revascularization. Ischemic monomelic neuropathy and myopathy can occur as a result of occlusion of the specific peripheral artery.

Identification of Genetic Causes of Inherited Peripheral Neuropathies by Targeted Gene Panel Sequencing

  • Inherited peripheral neuropathies (IPN), which are a group of clinically and genetically heterogeneous peripheral nerve disorders including Charcot-Marie-Tooth disease (CMT), exhibit progressive degeneration of muscles in the extremities and loss of sensory function. Over 70 genes have been reported as genetic causatives and the number is still growing. We prepared a targeted gene panel for IPN diagnosis based on next generation sequencing (NGS). The gene panel was designed to detect mutations in 73 genes reported to be genetic causes of IPN or related peripheral neuropathies, and to detect duplication of the chromosome 17p12 region, the major genetic cause of CMT1A. We applied the gene panel to 115 samples from 63 non-CMT1A families, and isolated 15 pathogenic or likelypathogenic mutations in eight genes from 25 patients (17 families). Of them, eight mutations were unreported variants. Of particular interest, this study revealed several very rare mutations in the SPTLC2, DCTN1, and MARS genes. In addition, the effectiveness of the detection of CMT1A was confirmed by comparing five 17p12-nonduplicated controls and 15 CMT1A cases. In conclusion, we developed a gene panel for one step genetic diagnosis of IPN. It seems that its time- and cost-effectiveness are superior to previous tiered-genetic diagnosis algorithms, and it could be applied as a genetic diagnostic system for inherited peripheral neuropathies.
