Spontaneous Cerebellar Hemorrhage with the Fourth Ventricular Hemorrhage : Risk Factors Associated with Ventriculoperitoneal Shunt

  • Objective : The purposes of this study are to investigate the factors that may be related to ventriculoperitoneal (VP) shunt in patients with cerebellar hematoma and the effect of severe fourth ventricular hemorrhage, causing obstructive hydrocephalus on subsequent VP shunt performance. Methods : This study included 31 patients with spontaneous cerebellar hematoma and concomitant fourth ventricular hemorrhage, who did not undergo a surgical evacuation of hematoma. We divided this population into two groups; the VP shunt group, and the non-VP shunt group. The demographic data, radiologic findings, and clinical factors were compared in each group. The location of the hematoma (whether occupying the cerebellar hemisphere or the vermis) and the degree of the fourth ventricular obstruction were graded respectively. The intraventricular hemorrhage (IVH) score was used to assess the IVH severity. Results : Ten out of 31 patients underwent VP shunt operations. The midline location of cerebellar hematoma, the grade of fourth ventricle obstruction, and IVH severity were significantly correlated with that of VP shunt operation (p=0.015, p=0.013, p=0.028). The significant variables into a logistic regression multivariate model resulted in statistical significance for the location of cerebellar hemorrhage [p=0.05; odds ratio (OR), 8.18; 95% confidence interval (CI), 1.00 to 67.0], the grade of fourth ventricle obstruction (p=0.044; OR, 19.26; 95% CI, 1.07 to 346.6). Conclusion : The location of the cerebellar hematoma on CT scans and the degree of fourth ventricle obstruction by IVH were useful signs for the selection of VP shunt operation in patients with spontaneous cerebellar hematoma and concomitant acute hydrocephalus.

Modified Orbitozygomatic Approach without Orbital Roof Removal for Middle Fossa Lesions

  • Objective : The purpose of the present study was to describe an OrBitoZygomatic (OBZ) surgical variant that implies the drilling of the orbital roof and lateral wall of the orbit without orbitotomy. Methods : Design : cross-sectional study. Between January 2010 and December 2014, 18 patients with middle fossa lesions underwent the previously mentioned OBZ surgical variant. Gender, age, histopathological diagnosis, complications, and percentage of resection were registered. The detailed surgical technique is described. Results : Of the 18 cases listed in the study, nine were males and nine females. Seventeen cases (94.5%) were diagnosed as primary tumoral lesions, one case (5.5%) presented with metastasis of a carcinoma, and an additional one had a fibrous dysplasia. Age ranged between 27 and 73 years. Early complications were developed in four cases, but all of these were completely resolved. None developed enophthalmos. Conclusion : The present study illustrates a novel surgical OBZ approach that allows for the performance of a simpler and faster procedure with fewer complications, and without increasing surgical time or cerebral manipulation, for reaching lesions of the middle fossa. Thorough knowledge of the anatomy and surgical technique is essential for successful completion of the procedure.

Efficacy of Gamma Knife Radiosurgery for Recurrent High-Grade Gliomas with Limited Tumor Volume

  • Objective : This study aims to determine whether gamma knife radiosurgery (GKR) improves survival in patients with recurrent high-grade gliomas. Methods : Twenty nine patients with recurrent high-grade glioma underwent 38 GKR. The male-to-female ratio was 10 : 19, and the median age was 53.8 years (range, 20-75). GKR was performed in 11 cases of recurrent anaplastic oligodendrogliomas, five anaplastic astrocytomas, and 22 glioblastomas. The median prescription dose was 16 Gy (range, 10-24), and the median target volume was 7.0 mL (range, 1.1-15.7). Of the 29 patients, 13 (44.8%) received concurrent chemotherapy. We retrospectively analyzed the progression-free survival (PFS) and overall survival (OS) after GKR depending on the Eastern Cooperative Oncology Group (ECOG) performance status (PS), pathology, concurrent chemotherapy, radiation dose, and target tumor volume. Results : Starting from when the patients underwent GKR, the median PFS and OS were 5.0 months (range, 1.1-28.1) and 13.0 months (range, 1.1-75.1), respectively. On univariate analysis, the median PFS was significantly long in patients with anaplastic oligodendroglioma, ECOG PS 1, and target tumor volume less than 10 mL (p<0.05). Meanwhile, on multivariate analysis, patients with ECOG PS 1 and target tumor volume less than 10 mL showed improved PFS (p=0.043 and p=0.007, respectively). The median OS was significantly increased in patients with ECOG PS 1 and tumor volume less than 10 mL on univariate and multivariate analyses (p<0.05). Conclusion : GKR could be an additional treatment option in recurrent high-grade glioma, particularly in patients with good PS and limited tumor volume.

  • When a electric power cable is installed in the air for newly or exchanged working, it is necessary at all time to hold a constant tension for an overhead power cable. And also a pendanted power cable is an extreme job to have work in a high sky. For this reason, the objective of this paper developments working robot for preventing disaster that tension of cable installed automatically power cable to hold a constant. So the working robot works at all the time two tasks for mobil and tension it come into a inference between two tasks, control is difficult. Control methode needs to suppress inference of two tasks. In this paper, for installation of overhead power cable, the scheme and control methode of working robot is presented. the robot work at a same time tow tasks that have hold a constant tension of the power cable and move a constant place while unfasten a winding cable at a drum on a chassis. Working robot consist of three parts with mobile system, tension system and control part. As it is applied the feedback/feedforward control, methode of hybrid control is established to suppress that interference come into between two tasks. The simulation programs is made out using models of mobil and tension system, and a proposed controllers. In accordance with simulation, the model of each systems is discussed to make out proper. And also parameters of controllers is selected a suitable value and the driving performance of robot is evaluated.

One Case Study of a Non Small Cell Lung Cancer Patient Experiencing Gefitinib Adverse Effects Managed by Traditional Korean Medicine (한방치료 후 Gefitinib(Iresa)복용으로 인한 부작용 호전된 비소세포폐암 환자 1례)

  • Objective : The aim of this study is to evaluate the synergistic effects of Traditional Korean Medicine with Gefitinib chemotherapy on a non small cell lung cancer. Methods : A 61 year-old male patient diagnosed with left non small cell lung cancer stage IIIb (T2aN0M1a) was admitted to East-West Cancer Center (EWCC) on Apr. 2012. He received Gefitinib chemotherapy since 20th June. 2011. He suffered from many complication like as skin toxicities, peripheral neuropathy, lassitude, diarrhea and so on. He was treated with Traditional Korean Medicine consisted of herbal medicine, acupuncture, and moxibustion. The symptoms were measured by Common Terminology Criteria for Adverse Events (CTCAE version 3.0) and visual analogue scale (VAS). Performance status was measured by Eastern Cooperative Oncology Group (ECOG). Results : TKM consisting of acupuncture, moxibusion, herbal medicine significantly alleviated Gefitinib induced complication. Quality of life was also significantly improved. Conclusion : This case study suggests that TKM would beneficial to adverse effects such as skin toxicities, peripheral neuropathy, lassitude from gefitinib.

Evaluation of Applicability of Heavy Oil Upgrading By-Product (Pitch) as A Pavement Paving Material (중질유 고도정제 부산물의 도로포장용 역청재료로서의 적용성 평가)

  • PURPOSES : The objective of this study is to evaluate the applicability of the pitch, which is produced during SDA petroleum upgrading process, as a pavement paving material. In order for the purpose, the physical and chemical properties of the pitch are analyzed, and then the various plasticizers are applied in the pitch. METHODS : Two types of pitch are selected from oil refinery companies, which are owned the SDA petroleum upgrading process. Also, two types of asphalt binders, PG 64-22 and PG 58-22, are employed to compare with the pitch because these two types of asphalt binders are currently used as paving materials. For the chemical property of the pitch, the composition of SARA (Saturate, Aromatic, Resin, Asphaltene), the elementary composition, and the functional group are analyzed. For the physical property of the pitch, the basic material property tests, such as penetration test, softening point test, flash point test, ductility test, and rotational viscometer test, are performed. Also, the DSR (Dynamic Shear Rheometer) test and the BBR (Bending Beam Rheometer) test are conducted using asphalt binder specimens obtained by both short term aging (Rolling Thin Film Oven, RTFO) and long term aging (Pressure Aging Vessel, PAV) processes. The rheological property of each pitch type is evaluated as a function of temperatures and loading cycles. PG 64-22 asphalt binder is used as a control material. RESULTS AND CONCLUSIONS : The Pitch may not be suitable for the pavement paving material without modifications, but the pitch can be used as alternatives of modified addictive or asphalt. If low molecular component, such as saturate and aromatic components, are added in the pitch based on the development of various plasticizers, it has a strong possibility for the pitch to be used as a alternative. However, in order to verify the performance property of the pitch, further research is needed.

Application of Bimodal Histogram Method to Oil Spill Detection from a Satellite Synthetic Aperture Radar Image

  • As one of segmentation techniques for Synthetic Aperture Radar (SAR) image with oil spill, we applied a bimodal histogram method to discriminate oil pixels from non-oil pixels. The threshold of each moving window was objectively determined using the two peaks in the histogram distribution of backscattering coefficients from ENVISAT ASAR image. To reduce the effect of wind speed on oil spill detection, we selected ASAR image which satisfied a limit of wind speeds for successful detection. Overall, a commonly used adaptive threshold method has been applied with a subjectively-determined single threshold. In contrast, the bimodal histogram method utilized herein produces a variety of thresholds objectively for each moving window by considering the characteristics of statistical distribution of backscattering coefficients. Comparison between the two methods revealed that the bimodal histogram method exhibited no significant difference in terms of performance when compared to the adaptive threshold method, except for around the edges of dark oil spots. Thus, we anticipate that the objective method based on the bimodality of oil slicks may also be applicable to the detection of oil spills from other SAR imagery.

  • In today's world economy, technology and knowledge have become the key words for value creation. Since 1998, various research institutions and universities in Korea, with support from Small and Medium Business Administration, have buckled to establish and operate incubating centers in order to foster such industries. Even though these centers have been operated in accordance with the purpose of the establishment, it has been over six years now, and it is necessary to evaluate its overall performance before moving on. Incubating a venture company results in not only technology innovation, but also jobs creation and economic growth. Knowing the importance of that, a government, at all levels, has supported the incubating centers as the most effective way to support venture companies in Korea. Incheon now plays a great role as a hub of logistics and trade in North East Asia. In order to nurture the city as the center of Korean economy, the city of Incheon has financially supported various incubating centers in the city, in an effort to incubate potential venture companies with lack of business experiences. It is imperative to find out what these companies need and how they evaluate the system at this point to develop a guiding principle of its operation as well as a careful selection process of a company in the future. In section II of this research, a concept of an incubating center is defined. An objective, a policy of its operation as well as its structure are explained in this chapter with case studies of Incheon College. 'Resources' are divided into human resources and material resources. And the survey that 71 different companies (out of 164 companies in 10 different incubating centers) have participated in is included in this section as well.

PERFORMANCE ASSESSMENT OF THE RANS TURBULENCE MODELS IN PREDICTION OF AERODYNAMIC NOISE FOR AIR-CONDITIONER INDOOR UNIT (에어컨 실내기의 공력소음 예측을 위한 RANS 난류모델의 성능 평가)

  • The objective of the present study is to investigate the effects of various turbulence models on the aerodynamic noise of an air-conditioner (AC) indoor unit. The results from URANS (unsteady Reynolds-averaged Navier-Stokes) simulations with the standard k-$\varepsilon$, k-$\omega$ shear stress transport (SST) and Spalart-Allmaras (S-A) turbulence models were analyzed and compared with the noise data from the experiments. The frequency spectra of the far-field acoustic pressure were computed using the Farrasat equation derived from the Ffowcs Williams-Hawkings (FW-H) equation based on the acoustic analogy model. Two fixed fan casings and the rotating cross-flow fan were used as the source surfaces of the dipole noise in the Farrasat equation. The result with the standard k-$\epsilon$ model showed a much better agreement with the experimental data compared to the k-w SST and S-A models. The differences in the pressure spectra from the different turbulence models were discussed based on the instantaneous vorticity fields. It was found that the over-estimated power spectra with the k-w SST and S-A models are related to the emphasized small-scale vortices produced with these models.

  • Today, the use of composite materials become an essential part in the design and manufacturing process of the flight vehicles to reduce the structural weight. Since the structural properties can be varied largely due to the stacking sequence of ply angles, it is very important problem to determine the optimized ply angles under a design objective. Thus, in this study, the analysis of static aeroelastic optimization of a composite wing has been performed. An analytical system to calculate and optimize tile aero-structural equilibrium position has been developed and incorporated with the genetic algorithm. The effects of stacking sequence on the structural deformation and aerodynamic distribution have been studied and calculated with the condition of minimum structural deformation for a swept-back composite wing. For the set of practical stacking angles, the design results to maximize the performance of static aeroelasticity are also presented.
