A Case for Chest Wall Panetrating Injury by Nail Gun (못총에 의한 흉부관통상에 대한 치험 1예)

  • We describe the case of a 56-year-old man who had been shot by a pneumatic nail gun in the chest during work. He had removed the nail by himself immediately at the accident field. He visited to the emergency department of a local hospital and, after a simple dressing and simple history had been taken, he was referred to our emergency department for penetrating thoracic injury. Immediately, Transthoracic echocardiography were done and showed moderate hemopericardium. Patient had been hydrated and transported to the operating room. After cardiac wound repaired by midsternotomy, the patient was discharged on the 13th postoperative day without complications except mild mitral valve regurgitation.

A Highly Effective and Long-Lasting Inhibition of miRNAs with PNA-Based Antisense Oligonucleotides

  • MiRNAs are non-coding RNAs that play a role in the regulation of major processes. The inhibition of miRNAs using antisense oligonucleotides (ASOs) is a unique and effective technique for the characterization and subsequent therapeutic targeting of miRNA function. Recent advances in ASO chemistry have been used to increase both the resistance to nucleases and the target affinity and specificity of these ASOs. Peptide nucleic acids (PNAs) are artificial oligonucleotides constructed on a peptide-like backbone. PNAs have a stronger affinity and greater specificity to DNA or RNA than natural nucleic acids and are resistant to nucleases, which is an essential characteristic for a miRNA inhibitor that will be exposed to serum and cellular nucleases. For increasing cell penetration, PNAs were conjugated with cell penetrating peptides (CPPs) at N-terminal. Among the tested CPPs, Tat-modified peptide-conjugated PNAs have most effective function for miRNA inhibition. PNA-based ASO was more effective miRNA inhibitor than other DNA-based ASOs and did not show cytotoxicity at concentration up to 1,000 nM. The effects of PNA-based ASOs were shown to persist for 9 days. Also, PNA-based ASOs showed considerable stability at storage temperature. These results suggest that PNA-based ASOs are more effective ASOs of miRNA than DNA-based ASOs and PNA-based ASO technology, compared with other technologies used to inhibit miRNA activity can be an effective tool for investigating miRNA functions.

Penetrating Injury to the Left Ventricle from a Fractured Rib Following Blunt Chest Trauma (흉부 둔상으로 골절된 늑골로 인해 발생한 좌심실 천공)

  • The perforation of a cardiac chamber by a fractured rib after blunt trauma is a rare event. Here, we report the case of patient who was referred for multiple rib fractures after a fall from a height. The patient was found to have a penetrating cardiac injury which was detected on a computed tomography chest scan. Computed tomography is a useful screening tool for victims of blunt chest trauma. Once cardiac perforation has been confirmed or is highly suspected, it is important to preserve the patient's vital signs until reaching the operating room by minimally manuplating the chest wall and permitting hypotension, which also prevents exsanguinating hemorrhage. For the same reasons, early cardiac tamponade may also improve the patient's survival.

  • 현재 다양한 분야에서 널리 사용되고 있는 지하투과레이더(Ground Penetrating Radar, GPR)를 이용하여 지하수면 및 함수량을 추정하였다. 비피압대수층 내에서의 얕은 포화대(saturated zone) 깊이을 산정하는 연구(livari and Doolittle, 1994, van Overmeeren, 1994)와 포화대 상부 습윤대(wetting fronts)의 거동를 조사한 연구(Vellidis et al, 1990) 등에 활용된 바 있는 GPR 기숙을 바탕으로 비피압대수층의 통기대와 포화대 내의 함수량 및 지하수면 추정을 위한 기초 실험을 수행하였다. 지하수면 및 함수량의 현장 적용성을 검증하기 위해서는 시간과 경제적인 면에서 비효율적인 점을 고려하여 사질토로 구성된 실험용 토조를 제작하여 건조시 획득된 GPR 자료, 지하수면의 변화에 따른 GPR 이미지를 비교하여 그 적용성을 검토하고 시${\cdot}$공간적 지하수면의 정확한 추정을 위해서 삼차원으로 비교${\cdot}$검토할 수 있도록 하였으며, GPR 자료의 정확성을 검증하기 위해서 토조 하부에 액주계(piezometer)를 설치하였다. 본 연구에서 적용된 GPR 실험은 획득된 이미지의 해석에 다소 어려움이 있지만 토양을 교란시키지 않고 비교적 간편하게 함수랑 및 지하수면의 위치를 파악하는데 매우 효과적이며, 추가적으로 GPR을 이용한 다양한 실험이 수행된다면 GPR 기술은 향후 기존 방법에서 쉽게 판단하기 어려운 시${\cdot}$공간적인 함수량 및 지하수의 분포 특성을 효율적으로 파악하는데 매우 큰 도움을 줄 수 있을 것이다.

Two-Stage Endovascular Repair for Concurrent Penetrating Atherosclerotic Ulcers of the Thoracic and Abdominal Aorta

  • We report a case of concurrent saccular aneurysms caused by a penetrating atherosclerotic ulcer of the thoracic and abdominal aorta that were successfully treated by staged endovascular repair. Even though surgical open repair or endovascular repair is the treatment option, use of endovascular repair is now accepted as an alternative treatment to surgery in selected patients. To prevent contrast medium-induced nephropathy and spinal cord ischemia caused by a simultaneous endovascular procedure, a saccular aneurysm of the descending thoracic aorta was excluded by stent graft, followed by the placement of a bifurcated stent graft in the infrarenal abdominal aorta one month later.

  • It is very important to understand the electromagnetic characteristics of underground media in GPR (Ground Penetrating Radar) survey. Depending on the electrical characteristics of the underground medium, the energy of the electromagnetic wave becomes relatively small, and reflection from the interface may become difficult. In this study, electrical characteristics of sandy soils under various (loose and dense) conditions were analyzed. As a result, In dry sand is the dielectric constant increased as the relative density increased, and the dielectric constant and electrical conductivity increased as the moisture content of the sand increased.

  • A new ground penetrating radar imaging method for the estimation of buried artificial structures location and their approximate shapes in dispersive lossy ground is investigated. Fundamental idea is based on estimating delayed time and amplitude retrieval coefficients from scattered signals by buried scatterers. Using absolute value integration of each scanning site not only improve the accuracy of measured scattered signal, but also offers convenient ways to extract the image of buried structures. Multi-term Debye model was employed to describe a dispersive and lossy ground medium. We used the finite difference time domain method to discretize the wave equation in continuous form into the machine suitable form. This imaging method uses a new wave path tracing technique in time domain, which is helpful to identify the exact position of buried structures against the ground surface fluctuations.

  • Ultra-wideband(UWB) radar is widely used in many penetration radar applications, such as ground-penetrating radar and foliage-penetrating radar, because it has many advantages in detecting concealed objects. One type of UWB radar system is random noise radar, which many be robust to jamming environment. However conventional random noise radar requires high-speed analog-to-digital convertor(ADC) for matched filtering. In this thesis, a pseudo-random noise radar system that maintains anti-jamming characteristics but does not require high-speed ADC is researched. and The UWB system is implemented in a low frequency system, and its performance has been demonstrated by experiment, which proves the concept of the proposed pseudo-random noise radar system.

  • With the aim of global environmental monitoring we carried out GPR (Ground Penetrating Radar) surveys at the Livingstion Island in Antarctica. Research area is near the Mt. Charra (340 m) in Livingston Island which is located 80 km to the southwest of the King Sejong Station. We have collected 5 lines of GPR data. Two kinds of survey, CMP (Common Midpoint) surveys and common offset profiles, were performed. We classified the glacier into the three layers using electromagnetic velocity of the ice and reflection characteristic. The depth of glacier reached about 80∼110 m. Some reflectors showed the evidence of the water filled englacial drainage and volcanic ash-layers.

  • Track substructure (ballast, subgrade) should have sufficient strength and uniform stiffness to fully support track superstructure (rail, fastener, sleeper). Vertical support stiffness of track is strongly influenced by the condition of ballast and subgrade layers. Therefore, the evaluation of the condition of track substructure is very important to evaluate the vertical support stiffness of track. This paper proposes the trackbed evaluation system, which is composed of Ground Penetrating Radar (GPR), Portable Ballast Sample. (PBS), and Light Falling Weight Deflectomete. (LFWD), to diagnose track substructure. The laboratory and field tests are performed to evaluate the applicability of the proposed trackbed evaluation system.
