암 환자의 한의학 치료 이용 현황 (Use of Korean Medicine among Cancer Patients)

  • Objectives: Many cancer patients have been using Korean medicine with conventional medical care. This study aimed to look into current status of Korean medicine in cancer patients in Korea. Methods: We reviewed utilization of Korean medicine for cancer by searching DB (KoreaMed, KMbase, KISS, NDSL, KISTI, National Assembly Library, OASIS, Korean Traditional Knowledge Portal). The first search keyword used was "cancer" and the second keyword used was either "oriental", "alternative" or "complementary". Results: The utilization rate of Korean medicine was 31.0%. In terms of patient's disease characteristics, most of them were suffering from stomach cancer, lung cancer and colon cancer. Most of them were diagnosed as fourth stage of cancer. Ginseng (14.8%) was a favorite Korean medicine, followed by medicinal treatment (8.1%), acupuncture (3.1%), moxibustion (3.0%), and cupping (2.1%). Conclusions: In Korea, one third of cancer patients are using Korean medicine. We need to know the expectations and demand for Korean medicine for treating cancer and establish our strategy to attract public attention.

국내 뇌졸중 편마비 환자를 대상으로 한 건측억제-환측유도 치료효과의 메타분석 (Meta-Analysis of Constraint-Induced Movement Therapy in Hemiplegic Stroke Patient in Korea)

  • This meta-analysis investigated the effects on arm motor impairment, arm motor function and disability, and psychological aspects of constraint-induced movement therapy (CIMT) for upper extremity hemiparesis following stroke, based on Korean studies. A comprehensive search of the complete Korean studies information service system (KISS), Research Information Sharing Service (RISS), Korea National Library, and the Korean Medical Database to September 2011 was conducted. Eleven eligible controlled clinical trials compared CIMT to a control group or an alternative treatment. All outcome measures of arm motor impairment, arm motor function and disability, and psychological aspects were pooled for calculating effect size. The overall effect size of CIMT was .700 (95% confidence interval=.482~.918). The CIMT programs showed large effect on the aspect of arm motor function and disability (the effect size is .920) and the psychological aspect (the effect size is .946). The effect of CIMT on arm motor impairment was moderate (the effect size is .588). These results show that CIMT may improve upper extremity motor impairment, function and disability, and psychological aspects following stroke. However, these results were based on a small number of studies, and not all of them were randomized control trials. Additional research is needed to include larger well-designed trials to resolve these uncertainties.

성인 환자의 산림치유 프로그램에 대한 체계적 문헌 고찰 (Systematic Review of Forest Therapy Program for Adult Patients with Diseases)

  • Purpose: The purpose of this study was to analyze the characteristics and effects of the forest therapy programs for adult patients. Methods: Six databases (PubMed, Cochrane library, CiNii, PsychoInFO, AGRICOL, EMBASE) and six domestic databases were systematically searched with language restrictions of Korean, English, and Japanese. The systematic review was based on the Preferred Reporting Items Systematic Reviews and Meta-Analysis (PRISMA) flow diagram. Results: Twenty five studies met the inclusion criteria. A systematic review identified adult patients classified as 1) disease of the circulatory system, 2) mental and behavioral disorders, 3) malignant neoplasms, 4) etc. (diabetes mellitus, chronic pain). This integrative review showed a positive effect of the forest therapy program for adult patients on physiological indices such as blood pressure, heart rate, heart rate variability, level of stress hormone, and natural killer cells as well as social·psychological indices such as the Profile of Mood States (POMS), anxiety, depression, and quality of life. Conclusion: The results of this study could be useful key principles in developing and applying the forest therapy programs in nursing intervention. Further research should be conducted to develop the forest therapy programs suitable for each patient group and measure its effectiveness.

Anesthetic efficacy of Gow-Gates versus inferior alveolar nerve block for irreversible pulpitis: a systematic quantitative review

  • This review aimed to assess and compare the outcomes of the anesthetic efficacy of inferior alveolar nerve block (IANB) and Gow-Gates mandibular nerve block (GGMNB) in patients with symptomatic irreversible pulpitis. A descriptive systematic review of quantitative research was conducted wherein the "Preferred Reporting Items for Systematic Reviews (PRISMA)" was adopted, and the Problem/Patient/Population, Intervention/Indicator, Comparison, Outcome (PICO) criteria were used to structure the research question. A literature search was performed using PubMed/Medline, Cochrane Library, Google Scholar, and Ovid. Selection criteria were applied for populations over nine years of age, of either sex, with irreversible pulpitis, and articles published in English regarding conventional IANB or IANB and Gow-Gates techniques between 2009 and 2019. Prospective randomized clinical trials or randomized controlled trials were included in the review, in which anesthetic efficacy or success was measured. After screening, four articles were included. Three studies were randomized clinical trials, and two were randomized controlled trials. The validity and reliability of the individual studies were examined. There was evidence of the higher efficacy of the GGMNB technique than that of the IANB technique. However, both techniques can be mastered through training.

응급환자 간호를 위한 시뮬레이션 교육효과: 메타분석 (Effects of Simulation-Based Education for Emergency Patient Nursing Care in Korea: A MetaAnalysis)

  • Purpose: The purpose of this review was to evaluate the effects of emergency nursing simulation program on nursing students and nurses. Methods: This systematic review was performed as per the guidelines of the Preferred Reporting Items for Systematic Reviews and Meta-Analyses and using the R program meta package (version 4.9-2). RISS, KISS, and DBpia Library databases were searched for studies published between June 2000 and August 2018 using the following key words: ($Emerge^*$ OR nursing) AND ($nurs^*$ OR simulation). Selected studies were assessed for methodological quality using Risk of Bias for Non randomized Studies. Results: 7 studies were identified and all of them met the inclusion criteria. The outcome variables were significant clinical performance, self-efficacy except knowledge, and problem-solving ability. Conclusion: This review provides updated evidence of the simulation-based education program in emergency nursing. Further studies are required to increase generalizability using randomized population, research design and controlled trials with sufficient sample size. Moreover, valid measurements are needed to assess the main outcomes.

Meta-analysis of the Diagnostic Test Accuracy of Pediatric Inpatient Fall Risk Assessment Scales

  • Purpose: This study was conducted to obtain data for the development of an effective fall risk assessment tool for pediatric inpatients through a systematic review and meta-analysis of the diagnostic test accuracy of existing scales. Methods: A literature search using Medline, Science Direct, CINAHL, EMBASE, and the Cochrane Library was performed between March 1 and 31, 2018. Of 890 identified papers, 10 were selected for review. Nine were used in the meta-analysis. Stata version 14.0 was used to create forest plots of sensitivity and specificity. A summary receiver operating characteristic curve was used to compare all diagnostic test accuracies. Results: Four studies used the Humpty Dumpty Falls Scale. The most common items included the patient's diagnoses, use of sedative medications, and mobility. The pooled sensitivity and specificity of the nine studies were .79 and .36, respectively. Conclusion: Considering the low specificity of the pediatric fall risk assessment scales currently available, there is a need to subdivide scoring categories and to minimize items that are evaluated using nurses' subjective judgment alone. Fall risk assessment scales should be incorporated into the electronic medical record system and an automated scoring system should be developed.

동의사상진료의전(東醫四象診療醫典)에 대한 서지학적 고찰 (Bibliographic Study on the Book Dongeuisasangjinryoeuijeon)

  • Objective The purpose of this study was to examine the composition, bibliographic characteristics, and contents of the book, Dongeuisasangjinryoeuijeon(東醫四象診療醫典). Method The images of this book published in 1941 at Haenglim publishing company, stored in the National Library of Korea were acquired and used as basic data, and books related to Sasang constitutional medicine such as Dongeuisasangshinpyun (東醫四象新編), Dongmuyugo(東武遺稿), and Sasanggeumgyebibang(四象金匱秘方) were compared. Result Many of the data were based on the Dongeuisasangshinpyun(東醫四象新編), and were made with reference to Dongmuyugo(東武遺稿) and Sasanggeumgyebibang(四象金匱秘方) partly. This book is thought to be made to be used conveniently for clinicians at the time when data on Sasang Constitutional Medicine was insufficient. In particular, it can be said that the mention of well-established diagnostic method, drugs that cause adverse reactions when a patient of a different constitution takes them and the quatrains of prescriptions using seven Chinese characters in each line are very unique. Conclusion Although most of the books, including Dongeuisasangshinpyun(東醫四象新編), were cited, it is thought that the book was well-organized in order to provide good information to the clinicians practicing Sasang Constitutional Medicine at the time. For the original part of this book, further research should be conducted.

Complications of olecranon osteotomy in the treatment of distal humerus fracture

  • Distal humerus fractures (DHFs) are challenging to treat due to the locally complex osseous and soft tissue anatomy. Adequate exposure of the articular surface of the distal humerus is crucial when performing an anatomical reconstruction of the elbow. Even though "triceps-on" approaches are gaining popularity, one of the most commonly used surgical treatments for DHF is olecranon osteotomy. The incidence of complications related to this approach is unclear. This review was performed to assess the type and frequency of complications that occur with the olecranon osteotomy approach in the treatment of DHF. A literature search was conducted in the PubMed/Medline, Embase, and Cochrane Library digital databases up to February 2020. Only English articles describing complications of olecranon osteotomy in the treatment of DHF were included. Data on patient and surgical characteristics and complications were extracted. Statistical analysis was performed using SPSS. A total of 41 articles describing 1,700 osteotomies were included, and a total of 447 complications were reported. Of these 447 complications, wound infections occurred in 4.2% of osteotomies, of which 1.4% were deep infections and 2.8% were superficial. Problems related with union occurred in 3.7% of osteotomies, 2% of which represented non-union and 1.7% delayed union. The high risk of complications in olecranon osteotomy must be considered in the decision to perform this procedure in the treatment of DHF.

Complementary and Alternative Medicine for treating Low Back Pain with Teaching Exercise: A narrative review

  • Objectives: In modern society, many people have low back pain (LBP) and spinal diseases. About 80% of them experience severe LBP more than once in their lifetime. We can find studies on many Korean medicine-based treatments, such as acupuncture treatment for LBP and exercise therapy, which are effective in reducing the symptoms. This study focuses on the combined effect of both Korean medicine and exercise therapy for treating LBP. Method: For this review, we searched for articles focusing on pain and disability recovery in pre-clinical and clinical studies of extension and flexion exercise therapy related to LBP. The search databases were as follows: PubMed, Google Scholar, and seven Korean electronic databases (Korea Citation Index (KCI), Korean studies Information Service System (KISS), Research Information Service System (RISS), Oriental Medicine Advanced Searching Integrated System (OASIS), DBPIA, National Digital Science Library (NDSL), and KOREAMED). The keywords were as follows: Korean Medicine, back pain, flexion exercise, extension exercise, McKenzie method, McKenzie exercise, Williams' flexion exercise, and Mechanical Diagnosis and Therapy. Results & Conclusions: This review shows the usefulness of flexion and extension exercises for LBP treatment and effective patient education, but further studies are necessary.

수면 장애가 있는 환자에게 적용한 디지털 치료기기의 임상적 유효성에 대한 체계적 문헌고찰: 대조군분류에 따른 하위 분석 (A Systematic Review on the Clinical Efficacy of Digital Therapeutics for Sleep Disorders: Subgroup Analysis by Control Groups)

  • Background: The purpose of this systematic review was to investigate the effects of digital therapeutics for insomnia on sleep disorders and mental health improvement compared to the control group. Methods: Following the guidelines on systematic review(PRISMA, NECA), a literature search was conducted through PubMed, Cochrane Library, EMBASE, RISS, KISS, and KoreaMed using keywords. The Cochrane Risk of Bias Tool and Review Manager version 5.3 were used for risk of bias and effect size assessment. Results: Thirty eight RCT met criteria for inclusion. When compared against three control conditions, the digital therapeutics for insomnia was an effective intervention for improvement sleep disorders and mental health in comparison to waiting list and Patient-directed care with some intervention by medical staff. However, digital therapeutics for insomnia were no more effective than face-to-face CBT-I control group. Conclusion: The efficacy of digital therapeutics for insomnia was evaluated differently depending on the control group. Therefore, in phase 3 clinical trials for efficacy evaluation, it is necessary to review whether the control group has been properly established.
