SVC Syndrome Caused by Leiomyosarcoma of SVC - A Case Report - (상대정맥에 발생한 평활근육종의 수술 - 1례 보고 -)

  • Among the causes of SVC syndrome, intraluminal tumor, especially the leiomyosarcoma is very rare. We report a 39 year old female patient who had been suffering from headache and facial edema for 6 weeks before admission. On physical examination, facial edema and venous engorgement on upper extermities and upper chast wall were showed. The chest CT scan and SVC cavogram showed a long intraluminal mass lesion resulting in a near total obstruction of the SVC Surgery was performed through median sternotomy. For complete resection of the tumor and thrombus, we used partial and total CPB. The follow up SVC cavogram revealed no abnormality 14 months after the operation.

  • Kartagener's sydrome is an autosomaly inherited recessive condition characterized by situs inversus, bronchiectasis, and chronic sinusitis. And recently it was recognized as a subclass of dyskinetic cilia syndrome which caused by a defect in mucociliary transport owing to immotile or dyskinetic beating of cilia. Electron microsopy of cilia from sperm tails, nasal and bronchial epithelium of patients reveals the partial or complete absence of dynein arms. Our four patients were diagnosed as a Kartagener's syndrome by classic triad. We carried out electron microscopy of cilia of the nasal mucosa. And many other tests were done. One patient had squamous cell carcinoma of the lung, and another one patient revealed features of adult respiratory distress syndrome at admission. All patients improved with conservative therapy such as physiotherapy, bronchodilater, antibiotics except one patient who mechanical ventilation was required. A brief review of literature was made.

Optimal Design of Fuel-Rich Gas Generator for Liquid Rocket Engine (액체로켓의 농후 가스발생기 최적설계)

  • An optimal design of the gas generator for Liquid Rocket Engine (LRE) was conducted. A fuel-rich gas generator in open cycle turbopump system was designed for 10ton in thrust with RP-1/LOx propellant. The optimal design was done for maximizing specific impulse of thrust chamber with constraints of combustion temperature and for matching the power requirement of turbopump system. Design variables are total mass flow rate to gas generator, O/F ratio in gas generator, turbine injection angle, partial admission ratio, and turbine rotational speed. Results of optimal design provide length, diameter, and contraction ratio of gas generator. And the operational condition predicted by design code with resulting configuration was found to maximize the objective function and to meet the design constraints. The results of optimal design will be tested and verified with combustion experiments.

  • As the partial admission turbine has a intrinsically unsteady and three dimensional flow region, numerical calculation time of these study has been too long time. The numerical analysis for gas temperature of turbine blade surface has been performed to investigate development of temperature with various pyro start pressure. Computations have been carried out several turbine rotational speeds in the range from 0 to 16000 rpm and inlet conditions with 1423K, 7.2MPa. As a result, the more rotational speed and pyro starter pressure of turbine increased, the more turbine blade's temperature decreased. It is also found that flow field of turbine blade inlet area at pyro starter pressure of 5.75MPa and rotational speed of 12100 rpm formed surface temperature uniformly.

Vaccination Status and In-hospital Mortality Among Adults With COVID-19 in Jakarta, Indonesia: A Retrospective Hospital-based Cohort Study

  • Objectives: Prospective studies on vaccination status and mortality related to coronavirus disease 2019 (COVID-19) in low-resource settings are still limited. We assessed the association between vaccination status (full, partial, or none) and in-hospital mortality among COVID-19 patients at most hospitals in Jakarta, Indonesia during the Delta predomination wave. Methods: We conducted a retrospective cohort study among hospitalized COVID-19 patients who met the study criteria (>18 years old and admitted for inpatient treatment because of laboratory-confirmed severe acute respiratory syndrome coronavirus 2 infection). We linked individual-level data in the hospital admission database with vaccination records. Several socio-demographic and clinical characteristics were also analyzed. A Cox proportional hazards regression model was used to explore the association between vaccination status and in-hospital mortality in this patient group. Results: In total, 40 827 patients were included in this study. Of these, 70% were unvaccinated (n=28 543) and 19.3% (n=7882) died during hospitalization. The mean age of the patients was 49 years (range, 35-59), 53.2% were female, 22.0% had hypertension, and 14.2% were treated in the intensive care unit, and the median hospital length of stay across the group was 9 days. Our study showed that the risk of in-hospital mortality among fully and partially vaccinated patients was lower than among unvaccinated adults (adjusted hazard ratio [aHR], 0.43; 95% confidence interval [CI], 0.40 to 0.47 and aHR, 0.70; 95% CI, 0.64 to 0.77, respectively). Conclusions: Vaccinated patients had fewer severe outcomes among hospitalized adults during the Delta wave in Jakarta. These features should be carefully considered by healthcare professionals in treating adults within this patient group.

A Study of Operating Forces on a Partially Admitted Turbine Blade (부분분사에 의한 터빈익형에서의 작동력 변화에 관한 연구)

  • An experimental study has been conducted to analyze the operating forces on a partially admitted turbine blade using a linear cascade apparatus. Axial-type blades were used and the blade chord was 200mm. The rectangular nozzle was applied and its size was $200mm{\times}200mm$. The experiment was done at $3{\times}10^5$ of Reynolds number based on the chord. The rotational force and axial force on the blade were measured at steady state by moving the blade to the rotational direction. The operating forces were measured at three different nozzle install angles of $58^{\circ}$, $65^{\circ}$ and $72^{\circ}$ for off-design performance test. In addition, three different solidities of 1.25, 1.38 and 1.67 were applied. From the results, the maximum rotational force was increased when the solidity was decreased and the nozzle install angle was decreased. The axial force was increased by decreasing the nozzle install angle. The reverse axial force was obtained in the partially admitted region when the nozzle install angle was increased to $72^{\circ}$.

Traumatic Disc Injuries and the Iatrogenic Spinal Disability (외상성 추간판 손상과 의원성 척추장애인 만들기)

  • Objective : Traumatic disc lesion is a lesion with tremendous controversies. The causal relationships of this lesion are not established along with pathogenesis, diagnostic criteria, methods of treatment and the outcome. However, a significant number of patients with back pain after a trauma underwent spinal operations under the diagnosis of traumatic disc lesions. Such an ill-directed operation eventually produce a person with an iatrogenic disability. We present two illustrative cases, and tried to make a preventive method. Methods : We examined the path from mild trauma after a road traffic accident into the iatrogenic disability in two illustrative cases, who requested disability assessment and medical appraisement. We evaluated the reason and background for such an unwanted outcome and tried to find a method to reduce or prevent it by a literature review. Results : These two patients were admitted to the hospital with the diagnosis of lumbar sprain after a road traffic accidents. They eventually underwent spinal surgery under another diagnosis such as traumatic disc herniation or internal disc disruption. They stayed at the hospital for more than six months and finally lost their jobs. They became the disabled at last. Although they complained back pain, they never insisted by themselves that their symptoms were due to the traumatic disc lesion. To prevent such an iatrogenic disability, the doctors should assist them to go-back to the workplace promptly instead of a reckless extension of the treatment period. It may be necessary to evaluate the certificates to extend the treatment period with an additional diagnosis by a medical expert. To reduce the unnecessary long-term admission, a new billing system such as a partial share for the high cost of the treatment by the patients may be needed. Conclusion : It is not the patient but the doctors, who has the responsibility to avoid the unnecessary operations. All treating doctors should try to reduce or prevent such an iatrogenic complication caused by ourselves before we are forced to do so.

Clinical Characteristics and Treatment Effects of Shoulder Pain Patients Admitted to a Korean Medicine Hospital Based on MRI Findings (한방병원에 입원한 견통 환자들의 MRI소견에 따른 임상적 특징, 한방치료 효과에 대한 비교고찰)

  • Objectives : The aim of this study is to observe clinical characteristics and treatment outcomes of inpatients admitted to a Korean medicine hospital for shoulder pain based on magnetic resonance imaging(MRI) findings. Methods : 28 patients with MRI data were included. Data on MRI findings, duration of pain, and treatment effects were collected. To measure treatment outcomes, verbal numerical rating scale(VNRS), range of motion(ROM), and a clinical condition grading system were used. Results : 1. The patient population was 75.0 % female and 39.3 %(n=11) belonged to the 51~60-year-old age group. In the under 40 population, 1 or less MRI findings were found. 2. As for the type of disorder, 82.1 % of the patients were diagnosed with 'Supraspinatus lesion', 39.3 % with 'subacromial-subdeltoid(SA-SA) bursitis', and 28.6 % with 'Adhesive capsulitis' 3. Numerous correlations could be made between duration of pain and MRI findings. 'rotator cuff full thickness tear' was more prevalent in acute shoulder pain patients, and 'Adhesive capsulitis' in chronic patients. 4. VNRS and ROM at the time of discharge had significantly improved as compared to VNRS and ROM at the time of admission. 5. As for correlation between MRI findings and clinical condition grade, patients diagnosed with 'rotator cuff partial tear' reported to be in 'good' condition or better whereas those with 'rotator cuff full thickness tear' reported to be in 'fair' condition or worse. Conclusions : Patients admitted to a Korean Medicine hospital for shoulder pain had varying degrees of severity as shown on MRI. Almost all lesions responded well to Korean medicine treatment.

  • Median cleft of the lower lip and/or mandible is a rare congenital anomaly, first mentioned by Couronne in 1819. Monroe(1966), Fujino(1970), Ranta(1984) and Oostrom(1996) conducted comprehensive reviews and list cases in literature. Median cleft varies greatly, from a simple vermilion notch to a complete cleft of the lip involving the tongue, the chin, the mandible, the supporting structures of the median of the neck, and the manubrium sterni. The associated anomalies include ankyloglossia, cleft tongue, neck contraction, heart lesion, absence of hyoid bone, and so on. The etiology of median cleft is unknown. Various possibilities, such as failure of mesodermal penetration into the midline, failure of fusion of mandibular processes, external factors apart from the embryogenic pattern such as pressure, position in utero, circulatory failure caused placental adhesion, diseases in pregnancy, and so on, have been discussed. A 8-year-old girl was referred to the Dept. of Oral & Maxillofacial Surgery, Kyungpook National University Hospital and had been aware of the fact that at birth "she had something wrong with her mouth." Shortly after birth she had been examined by a plastic surgeon and at that time surgical procedure had been performed to release the tongue from the lower jaw and lip at local hospital. On admission, she had a slight notching of lower lip and two fibrous frenum ran from the lip along the ventral surface of the tongue, diastema between her mandibular central incisors, and slightly constricted bifid mandible associated independent movement of the two halves of mandible. The patient had autogenous iliac bone graft to reconstruct the mandibular midline defect. The postoperative result was uneventful. In future, the correction of the soft tissue deformities such as notching of the lower lip and partial ankyloglossia will be required for the esthetic and functional improvement.

The Clinical Study on 77 Traffic Accident Patients (교통사고 환자 77례에 대한 임상적 고찰)

  • Objectives : The purpose of this study was to find out tendency of traffic accident patients treated in korean medicine hospital. Methods : We investigated 77 patients who were treated in Semyung University Korean Medicine Hospital from 1 st, January, 2007 to the 30st September, 2009. Results and Conclusions : 1. In distribution of gender and age, the rate of male was 36.4%, that of female was 63.6% and majority of patients were twenties, thirties and forties. 2. In the traffic accident patterns, the most was car crash from behind(49.4%) and after traffic accident, 42 patients(54.6%) visited our hospital in less than 5 days. 3. 22 patients(28.6%) choose the oriental medical treatment for primary care. And 35 patients(45.4%) visited our hospital due to preference of oriental medicine. 4. In partial pain, most was the neck(76.6%), followed by low back(61.0%). 5. In radiation test(excluded sprain and straightening), the most was HIVD of L-SPINE(14.0%). 6. In the periods of admission, less than 7 days was the 1st(36.4%), 14 days was the 2nd(27.3%). 7. The treatment was effective to 67 patients out of 77 patients who had been hospitalized.
