  • Chronic periodontitis is one of the most common diseases in clinical dentistry, which requires various surgical interventions to treat the moderately to severely destructed supporting periodontium. Most patients have anxiety and fear to these surgical procedures and dentists often have problems dealing with these patients. By applying the conscious sedation technique in outpatient units, periodontists have become able to manage their patients successfully with less anxiety or fear. Also, we have experienced the increased level of patients' satisfaction. Generally, periodontal treatments are time consuming procedures and patients are usually reluctant to the periodontal instruments. This study is focused on the sedation procedure with intravenous midazolam infusion performed in Department of Periodontology of Ewha Womans University Hospital. Using questionnaires, we evaluated 80 randomly selected patients for the anxiolytic effect of intravenous midazolam. Anxiety control using IV sedation was very helpful in performing various periodontal reconstructive and advanced surgical procedures in implant dentistry.

  • Objectives : This study aimed to analyze the structure and the trend of utilization and expenditure for Korean Medicine (KM) in Korea. Methods : Data were drawn from the 2008-2013 annual Korea Health Panel (Version 1.2.2), a national representative sample. We combined the data of household members with the data of outpatient KM service use. The volume of KM use was estimated based on the frequency of use and co-payment. Results : The KM utilization rate slightly increased in recent years, and it is presumed to be resulted from the increase of elderly population. Most KM outpatient visits were being concentrated in treating musculoskeletal diseases, and the procedures used frequently were acupuncture, moxibustion, cupping, and physical therapy. The imbalance of KM use between lower income group and higher income group was deepening. Conclusions : To expand restricted disease areas KM covered, the more herbal prescriptions should be insured and the insured form of herbal medicines need to be diversified.

  • Objectives: Drug-induced Parkinsonism has similar symptoms to Parkinson's disease, but each has different causes. Drug-induced Parkinsonism accounts for the largest proportion of secondary Parkinsonism We report a outpatient case of drug-induced Parkinsonism after taking Risperidone, an atypical antipsychotic. Method: With discontinuing of antipsychotic drug, modified Yigan-san extract was administered for 12 weeks, and acupuncture and electroacupuncture procedures were performed 20 times. Results: Abnormal Involuntary Movement Scale (AIMS) score decreased from 23 to 3 during 59 days of treatment period without adverse events and worsening of depression. The Patient was highly satisfied. Conclusion: Modified Yigan-san and electroacupuncture (GB34) can be used as an treatment option in patients with drug-induced Parkinsonism.

  • Finances of health insurance can be explained by factors determining benefit expense and premium collection. This study was conducted to analyze factors contributing to the financial status of rural health Insurance. Nationwide 134 health insurance associations except the six pilot project counties were analyzed and obtained the followings. 1. In univariate analysis, statistically significant variables that explain 1) outpatient benefit expenditures include public health center utilization, proportion of pregnant women. premium and collection rate of premium 2) inpatient benefit expenditures include public health center utilization, Proportion of old age, proportion of pregnant women, premium and collection rate of premium 3) profits include public health center utilization, proportion of old age, proportion of pregnant women and collection rate of premium. 2. In multiple regression analysis, statistically significant determinants in 1) outpatient benefit include premium and public health utilization 2) inpatient benefit include premium 3) profit include public health center utilization, premium and collection rate of premium.

Effects of Guided imagery on Stress and Anxiety of Women Receiving in Vitro Fertilization (지시적 심상요법이 체외 수정을 받는 여성의 스트레스와 불안에 미치는 효과)

  • Purpose: The purpose of this study was to identify effects of guided imagery on stress including cognitive, affective, marital and social, and anxiety among women receiving in vitro fertilization (IVF). Methods: Data were collected between April, 21 and June, 17, 2008. The participants in this study were 57 women (26 for the experimental group, 31 for the control group) receiving IVF for primary or secondary infertility in one of the outpatient infertility centers in Seoul. The guided imagery (Suk, 2001) was provided through audio CD to the experimental group by themselves 8 minutes per day for 2 weeks. Data were analyzed by SPSS 12.0 windows program. Results: After guided imagery, the experimental group showed significantly lower affective stress and total stress scores. Anxiety scores increased significantly in the control group, but not in the experimental group after treatment. Conclusion: The findings suggest that guided imagery is an effective nursing intervention for reducing stress especially affective stress and anxiety among infertile women receiving IVF in outpatient infertility center.

Study on Association of DSOM Items for Uterine Myoma in Oriental Medicine -Control Group: Outpatient and Clinical Trials Data - (자궁근종 여부에 대한 DSOM 항목의 연관성분석 - 대조군 : 한방부인과 외래환자와 임상시험 피시험자 -)

  • Uterine myoma is a benign tumor of smooth muscle in the uterine wall. Recently, in Oriental medicine, concerns about uterine myoma patients have increased. We analyzed the medical records for 944 patients, including 257 uterine myoma patients, who visited Dongeui University Oriental Medical Center from May 2001 to June 2006. We investigated the DSOM (Diagnosis System of Oriental Medicine) symptom scores which effect uterine myoma patients using stepwise logistic regression model. Logistic regression analysis indicated as follows: In the control group composed of 558 outpatients, 18 items of DSOM were associated with myoma, 9 positively and 9 negatively, and the results showed that the correct rate was equal to 81.1%, sensitivity 72.8%, and specificity 84.9%. In 129 clinical trials data, 33 items of DSOM were associated with myoma, 18 positively and 15 negatively, and the results showed that the correct rate was equal to 85.8%, sensitivity 84.8%, and specificity 87.6%. In 687 outpatient and clinical trials data, 18 items of DSOM were associated with myoma, 10 positively and 8 negatively, and the results showed that the correct rate was equal to 82.8%, sensitivity 70.8%, and specificity 87.3%.

Influencing Factors on Depression in Stomach Cancer Patients Receiving Chemotherapy (항암화학요법을 받는 위암 환자의 우울과 영향요인)

  • Purpose: The purpose of this study was to identify factors that influence depression in stomach cancer patients receiving chemotherapy at outpatient clinic. Methods: A cross-sectional design was used. Ninety two subjects diagnosed with stomach cancer and receiving chemotherapy agreed to participated in the study. Subjects completed a survey which included questions about depression, symptom experience, social support and self-efficacy. Results: The level of depression was $13.45{\pm}6.92$. Reported depression differed according to patient's perceived health status. Depression was significantly correlated with symptom experience, social support and self-efficacy. The most important factor related to depression was symptom experience, followed by social support and self-efficacy. These three factors could explain about 47% of depression in stomach cancer patients receiving chemotherapy. Conclusion: Although the level of depression in stomach cancer patients receiving chemotherapy was relatively low, 34.8% of patients reported to have depression. It is important to apply nursing intervention including screening of depression level in stomach cancer patients receiving chemotherapy that focused on symptom experience and to consider about social support and self-efficacy at outpatient clinic.

Trend on the Curtailments of Medical and Drug Expenditure Before and After the Separation between Prescription and Dispensing in General Hospitals (의약분업 전후 일부 종합병원 진료비 및 약제진료비 삭감추이)

  • Fiscal crisis in the medical insurance system has put the pressure upon hospitals by increasing the rate of curtailment, since the implementation of the separation of prescription and dispensing of medicine. The purpose of this study is to analyse the curtailment of mdical and drug expenditure before and after the system of separation between prescribing and dispensing and to suggest the problems about current inspection system. Data were obtained from 13 general hospitals and used for analysis of trends on medical & drug expenditure, and curtailment in 1999-2000 at three months intervals. The results were as follows; The scale of curtailment for drug expenditure has been increased on outpatient and inpatient since 2000. For the curtailed drug cost with outpatient, the ratio of curtailed drug expenditure has been increased in the case of prescription within the hospital. These results suggest that review system in social insurance were over-focused to control the cost and it might to impede the validity of review function in insurance system. Therefore, it' s needed to develope the scientific and reasonable criteria for Inspection and evaluation of durg expenditure.

  • Objective: To assess the effectiveness of emergency single-dose levonorgestrel contraception in preventing unintended pregnancies among woman who visited the emergency department (ED) due to sexual assault (SA). Methods: We conducted a retrospective chart review in a university hospital in South Korea. Cases from November 10, 2006 to November 9, 2009 were enrolled. Information from the initial visit to the ED and subsequent follow-up visits to the gynecology outpatient clinic was collected. Results: In total, 1,179 women visited the ED due to SA. Among them, 416 patients had a gynecological examination and 302 patients who received emergency contraception (EC) (1.5 mg single-dose levonorgestrel) at the ED due to SA were enrolled. Ten patients did not return for follow-up examinations. In follow-up visits at the outpatient clinic, two pregnancies were confirmed, which showed the failure rate of the EC to be 0.68%. Conclusion: Single-dose levonorgestrel EC is extremely effective at preventing pregnancy among victims of SA.

  • Musculoskeletal ultrasound has unique advantages that may be free from exposure to radiation, low price compared to MRI, outpatient procedure that can be easily accessible, and better accuracy combined with physical examination. Dynamic ultrasound performed with stress tests are known to be useful for detecting the hidden lesions in the tendons, ligaments, nerves. Ultrasound in the elbow can be used easily in the outpatient for evaluation of the joint surface and synovial space; diagnosis for tendon diseases such as lateral epicondylitis, medial epicondylitis and morbidity of peripheral nerves; guide for anterior-posterior bursal and intra-articular injections.
