Comprehensive Orthodontic Treatment in a Middle-Aged Patient with Missing Maxillary Left First Premolar: A Case Report

  • As the adult population continues to increase, orthodontic treatment for adult patients is becoming more common. This case report describes comprehensive orthodontic treatment of a middle-aged patient with closure of the extraction space without prosthetic restoration. A 55-year-old woman with her maxillary left first premolar extracted because of a periodontal problem, wanted to close the space with orthodontic treatment. Since she had generalized crowding and mild skeletal discrepancy, we planned comprehensive orthodontic treatment, including closure of the extraction space by protraction of the left maxillary molars using miniscrews and aesthetic alignment of anterior teeth. The total treatment period was 28 months. As a result of these treatments, the extraction space was successfully closed, good tooth alignment and satisfactory occlusion were achieved.

AN INITIAL HISTOLOGIC STUDY ON MOLAR INTRUSION OF CAT WITH THE USAGE OF PERMANENT MAGNET (영구자석을 이용한 고양이 구치부 intrusion에 관한 초기 조직학적 연구)

  • Utilizing the repelling force of permanent magnet of find out weather it shows the findings of molar intrusion histologically and compares the result with the resin bite plane. As for the experimental animals, 10 cats of completion of permanent dentition with mean weight of 2.2kg which have flat occlusal plane of molar areas were used. I raised the cats under the condition of their being attached with manufactured appliance by direct bonding system and of their wearing vertical chin strap anesthetized for 12-14 hours per day through 5 days. Then, observing the root apex areas with lightmicroscope. The results were as follows; 1. In the group with resin bite plane, osteoblasts and osteoclasts could not find out but could find out periodontal ligment fiber obliquely angulated in the apical and lingual direction. 2. In the group with the permanent magnet, not only the obliquely angulated periodontal ligament fiber but also the osteoclasts at the apical areas were observed.

ROOT RESORPTION AND BONE RESORPTION BY JIGGLING FORCE IN CAT PREMOLARS (교대성 교정력이 고양이의 치근 흡수 및 치조골 흡수에 미치는 영향)

  • The purpose of this study was to evaluate root resorption and alveolar bone resorption pattern by jiggling movement. 16 adult cats were divided into 4 groups(6, 12, 18, 24 days). In test side, mesio-distal jiggling force was applied in right maxillary 1st premolar in 3 days cycle In control side, mesial force was applied in left maxillary 1st premolar. Radiographic and histologic observation were performed in 6, 12, 18, 24 days after force application. The results were as follow: 1. Alveolar bone resorption was more severe by jiggling force than by unidirectional force. 2. Root resorption pattern was not different between jiggling force and unidirectional force. 3. Combined pattern of bone resorption and new bone formation appeared in jiggling group. 4. New bone formation began to appear at periapical area of jiggling group after 24 days, because alveolar bone resorption was severe and extrusion resulted.

THE EFFECTS OF HIGH PULL HEADGEAR IN MIXED DENTITION WITH CLASS II MALOCCLUSION (혼합치열기 II급 부정교합에 대한 high pull Headgear의 효과)

  • The purpose of this study is to observe the effect of high pull headgear on the craniofacial structures of mixed dentition with Class II malocclusion. The cephalometric headplates of 16 children treated by high pull headgeaar during 6 months and 18 children during 12 months were traced, digitized and statistically analyzed. The results were as follows. 1. Inhibition of foreward growth of maxilla was observed in both group. 2. Clockwise rotation of maxilla was observed in both group. 3. There were distal movement of maxillary 1st molar and inhibition of alveolar bone growth of maxilla. 4. There was compensatory extrusion on mandibular 1st molar. 5. The ratio of anterior facial height to posterior facial height was almostly not changed. In the treatment plan of C II malocclusion by high pull headgear, ire must prevent the mandibulasr 1st molar from extruding and for orthopedic effect, at least 6 months is needed.

  • This research was carried out in order to study the effects of Rapid Maxillary Expansion on maxilla and it's surrounding skeletal structures. The sample for this study consists of 14 patients who were in retention period after rapid maxillary expansion. Following results were obtained after performing comparative analysis of cephalograms taken before and after rapid maxillary expansion. 1. In almost every cases, inferior displacement of palatal planes with concurrent changes in their inclination were observed. 2. Changes in the inclination of palatal plane can be categorized into inferiorty inclined group, superiorly inclined group and constant group in relation to the states before rapid masillary expansion. 3. Decrease in < Se-FMN/PMV and increase in < Se-FMN-A were evident in the superiorly inclined group and vice versa for the inferiorly inclined group.

  • In this article, we introduce a new method of tooth positioner fabrication using modified T.A.R.G. to measure the inclination and angulation of individual teeth. In finishing stage of orthodontic treatment with a fixed appliance, we anticipate that tooth positioner fabricated using the described construction method provides the movement of individual tooth into a desired position; corrected inclination and angulation of teeth, extrusion, intrusion, rotation and so on.

Minimum Presurgical Orthodontic Treatment with Two Jaw Surgery Combined with Anterior Segmental Osteotmy in Skeletal Class II Malocclusion: A Case Report

  • This case report describes the treatment of a 23-year-old woman who had lip protrusion with gummy smile and mentalis muscle strain. Orthognathic surgery was performed in conjunction with orthodontics. Minimum dental decompensation was performed with presurgical orthodontics followed by an anterior segmental osteotomy for the majority of dental decompensation. Counterclockwise rotation of the maxillomandibular complex was applied by LeFort I osteotomy, and bilateral sagittal split ramus osteotomies with anterior segmental osteotomy to achieve overall facial balance. The active treatment period was 15 months. Stable occlusion and skeletal relationship were observed after a 10-month follow-up period.

A STUDY OF THE 'WITS' APPRAISAL OF THE KOREAN ADULTS (한국 성인에 있어서의 'Wits' 수치에 관한 연구)

  • The purpose of this study was giving some helpful datas in analysing the degree of anteroposterior jaw disharmony with the 'Wits' appraisal. The cephalograms of 105 Korean adults with normal occlusion ranging from 17 to 20 years of age and 76 Koreans with various malocclusion ranging from 12 to 24 years of age were studied and analyzed statistically. The results were as follows; 1. The mean value of 'Wits' appraisal in normal occlusion group was -0.82mm in males and -0.17mm in females. 2. There was no significant difference between the males and females in normal occlusion group. 3, The mean value of 'Wits' appraisal in Angles class II malocclusion group was 2.67mm and higher than normal group. 4. The mean value of 'Wits' appraisal in Angles class III malocclusion group was -8.11 mm and lower than normal group.

  • The purpose of this study was to investigate the interrelationship of the experimental group and control group by analyzing case histories, intraoral radiographs, orthopantomographs, intraoral slide films and dental casts. The data for this study were complied from 654 outpatients of the Department of Orthodontics, Seoul National University Hospital. The following conclusions were obtained. 1. When one or more thins molar teeth were congenitally missing, the incidence of the other congenitally missing teeth was high. 2. The frequency of congenitally missing teeth was comparatively higher in male, maxilla, class II and class III. 3. The congenitally missing srea of the third molar by Angle's classification was not significant. 4. The order of frequency of congenitally missing teeth was the third molar, the second premolar, the lateral incisor, the first premolar, the central incisor, the canine, the first molar, the second molar.

A ROENTGENOCEPHALOMETRIC STUDY ON THE DEPTH OF THE ANTEGONIAL NOTCH AND THE CRANIOFACIAL MORPHOLOGY IN CLASS III MALOCCLUSION (제3급 부정교합자의 악각전절흔 심도와 두안면골격 형태에 관한 두부방사선 계측학적 연구)

  • This study was undertaken to investigate the relationship between the depth of antegonial notch and the craniofacial morphology, and to predict the mandibular growth direction & potential in class III malocclusion. The computerized analyses were carried out on longitudinal lateral cephalometric radiographs of 50 children with class III malocclusion, divided into two groups ; 30 deep notch subjects (more than 2.6mm) and 20 shallow notch subjects (less than 1.5mm). The conclusions were as follows: 1. The mandibular growth direction in deep notch group was more vertically directed than in shallow notch group. 2. Deep notch group had shorter anterior & posterior cranial base than shallow notch group. 3. There was not significant difference between deep & shallow notch groups in the amount of mandibular growth during treatment period. 4. Notch depth increased in both deep & shallow notch groups during treatment period.
