우리나라 다문화가정 청소년의 구강질환증상과의 관련요인 (Related factors of oral symptoms in adolescents from Korean multicultural families)

  • Objectives: The purpose of this study was to identify the related factors of dental caries and periodontal disease in adolescents from Korean multicultural families, thereby helping to reduce the prevalence rate of oral disease. Methods: The subjects were 710 multicultural adolescents recruited using a web-based survey, National 2015 Korean Youth Risk Behavior, from the Korean Center for Disease Control. A multicultural family was defined in this study as one having an immigrant mother or father. Oral symptoms included dental caries and periodontal disease. Toothache was defined as a symptom of dental caries. Tender or bleeding gums were defined as symptom of periodontal disease. For statistical analysis, Statistical Package for Social Sciences (SPSS) Version 21.0 for Windows was used. Descriptive analysis and a Chi-square test were conducted to determine the factors associated with general characteristics, health behavior, and oral health behavior. Finally, to investigate the associations among oral disease symptoms, logistic regression analysis was performed. Results: Toothache was significantly higher in female 1.52 (95% CI; 1.45-1.60), high school 1.23 (95% CI; 1.18-1.28), women school 1.10 (95% CI; 1.05-1.16), individuals with poor economic status 1.45 (95% CI; 1.30-1.52), and participants who consumed alcohol 1.32 (95% CI; 1.27-1.37). Toothache related to perceived health status was significantly lower in the healthy group 0.69 (95% CI; 0.64-0.75), and was higher in usual stress group 1.65 (95% CI; 1.57-1.74). Gum bleeding was significantly higher in female 1.32 (95% CI; 1.27-1.37), high school 1.15 (95% CI; 1.10-1.19), and individuals with poor economic status 1.38 (95% CI; 1.27-1.50). Gum bleeding related to perceived health status was significantly lower in the healthy group 0.68 (95% CI; 0.63-0.74), and was higher in usual stress group 1.54 (95% CI; 1.46-1.62). Conclusions: Taking into account of social and economic levels, and dietary habits in the multicultural families adolescents, further education and support will be needed for oral disease prevention and early treatment.

Sensory change after implant surgery: related factors for recovery

  • Objectives: This retrospective study aimed to analyze data on nerve damage in patients who complained of sensory changes after dental implant surgery, the clinical results according to proximity of the implant fixture to the inferior alveolar nerve (IAN) canal, and the factors affecting recovery of sensation. Materials and Methods: The electronic medical records of 64 patients who had experienced sensory change after implant surgery were reviewed. Patients were classified by sex, age, implant installation sites, recovery rate and the distance between the implant fixture and IAN canal on computed tomography (CT). The distance was classified into Group I (D>2 mm), Group II (2 mm≥D>0 mm), and Group III (D≤0 mm). Results: The 64 patients were included and the mean age was 57.3±7.3 years. Among the 36 patients who visited our clinic more than two times, 21 patients (58.3%) reported improvement in sensation, 13 patients (36.1%) had no change in sensation, and 2 patients (5.6%) reported worsening sensation. In Group II, symptom improvement was achieved in all patients regardless of the removal of the implant fixture. In Group III, 8 patients (40.0%) had reported symptom improvement with removal of the implant fixture, and 2 patients (33.3%) of recovered patients showed improvement without removal. Removal of the implant fixture in Group III did not result in any significant difference in recovery (P=0.337), although there was a higher possibility of improvement in sensation in removal cases. Conclusion: Clinicians first should consider removing the fixture when it directly invades the IAN canal. However, in cases of sensory change after dental implant surgery where the drill or implant fixture did not invade the IAN canal, other indirect factors such as flap elevation and damage due to anesthesia should be considered as causes of sensory change. Removal of the implant should be considered with caution in these situations.

노인 대상 입체조 프로그램의 구강건조증 개선 효과 (The effect of dry mouth improvement by oral exercise program in elderly people)

  • Objectives : The purpose of this study was to confirm effects of oral exercise program intended to improve the condition of salivary hypofunction and to provide basic data for development of oral health program for the elderly Methods : The subjects were 125 elderly women who resided in Gyeong-In area and were aged 65 years and older. During a 2-month period, an oral exercise was conducted twice a week. A survey was conducted by interviewing the selected elders to determine the state of their subjective dry mouth. Unstimulated whole saliva was measured before and after the oral exercise program. Results : There was a significant improvement in subjective dry mouth symptoms, severity scores of dry mouth after the oral exercise, and there were affected by age and income level. There was a significant increase in unstimulated whole-saliva after the oral exercise program. There was positive interrelationship between dry mouth Symptom improvement level and inconvenience improvement level, and between inconvenience improvement level and the increase of unstimulated whole-saliva. Conclusions : This study showed the effects of the oral exercise program. It is suggested that this oral exercise program has positive effects on the condition of dry mouth. For the explicit improvement effect on the condition of dry mouth, the oral exercise program needs to be carried out in consideration of the elderly's general characteristics.

치성 각화낭종의 임상적 연구 (CLINICAL STUDY OF ODONTOGENIC KERATOCYST)

  • Purpose: The purpose of this study is to investigate the clinical and histological features of odontogenic keratocyst Patients and Methods: A retrosective review of 100 patients who were diagnosed as odontogenic keratocyst by hitological findings during the period of January 2000 and December 2005 in the Dept. of Oral and Maxillofacial surgery Pusan National University was consecuted. For each patient, age, sex, location of lesion, initial diagnosis by radiographic features, treatment procedure, hitologic findings and recurrance rate were evaluated. Results: In this study, OKC has male prevalance to female by 1.38:1, and most likely occurs during third decade. The most common site of lesion was mandibular ramus region(34.6%) and the most common symptom was swelling(50%). The most common initial diagnosis by radiographic findings was OKC and cyst enucleation was the most common treatment method. The recurrance rate was 28% and existence of daugther cyst is thought to be most convincing factor for prediction of recurrence. Conclusion: In this study, total recurrence rate was 28% and existence of daugther cyst is thought to be most convincing factor for prediction of recurrence. But, since 97% of patients were treated by enucleation and adjuntive excision, further styudy is need about concordance of recurrence rate with surgical method.

자활 참여 근로자들의 구강건강 상태와 구강건강 삶의 질의 관계에서 불안의 매개효과 (The Mediating Effect of Anxiety in the Relationship between the Oral Health Status and Oral Health Quality of Life of Workers Participating in Self-sufficiency)

  • 본 연구는 대전, 대구 4곳의 지역자활센터 프로그램 참여 근로자 415명을 대상으로 2020년 9월 5일~30일까지 설문조사를 실시하였다. 연구목적은 자활 참여 근로자들의 구강 증상을 살펴보고 심리적 불안의 매개변수가 구강건강 삶의 질에 미치는 영향에 대하여 살펴보고자 한다. 연구결과 구강건조증, 악관절, 치주 질환 증상 모두 구강건강 삶의 질에 유의한 영향을 미치는 것으로 나타났으며, 심리적 불안 요인은 부분 매개효과를 나타냈다. 주관적 구강 증상 경험은 부정적인 불안 상태로 이어져 구강건강 삶의 질을 더욱 저하시키는 결과를 보였다. 추후 자활 근로자들을 위한 심리 상담 프로그램 확대 및 정기적인 구강보건교육을 위한 구강보건정책 개선에 기초자료로 활용될 것이다.

Study on Changes in Endogenous Stem Cells in the Salivary Gland of Streptozotocin-induced Diabetic Rats

  • Type1 diabetes mellitus (DM) is generally known to be caused by destruction of insulin-producing pancreatic ${\beta}$ cells or an immune-related problem. Polydipsia is a representative symptom of DM, and it has been reported that this condition is closely related to xerostomia and is considered that hyposalivation from the salivary gland results in this phenomenon. Although various studies have reported that induction of diabetes reduces endogenous stem cells in other organs (heart, brain etc.), diabetes-related changes in endogenous stem cells in the salivary gland have not yet been well established. Therefore, in this study, to verify the change in salivary gland stem cells after diabetes, salivary gland tissues in the control and diabetes-induced groups were processed by histochemistry (Masson's trichrome staining) for morphological analysis, TUNEL assay for cell death, and immunohistochemistry (Ki-67 and c-Kit) for cell proliferation and maturation. Diabetes induced by STZ leads to vacuolization, apoptosis, and reduction in proliferating cells/salivary gland stem cells in salivary glands of rats. This result suggests that diabetes may be associated with reduction in salivary gland function such as degeneration and inhibition of regeneration in the salivary gland.

The Therapeutic Value of Ultrasound-Guided Irrigation in Sialodochitis: A Case Report

  • In the present case study, we reported a female patient with sialodochitis fibrinosa in which a conservative ultrasound (US)-guided irrigation method was used to relieve her symptoms. A 30-year-old female patient visited Kyung Hee University Dental Hospital with chief complaints of persistent swelling and discomfort in the left facial area. Her primary symptom was facial swelling that was accompanied by neuropathological symptoms, such as itching, tingling, and warmth that usually continued for 3 to 4 days. During clinical examination of orofacial area, mild swelling and fever were noted in the left face including parotid region, and mild induration could be felt at the corresponding site; sialadenitis of the left parotid gland was tentatively diagnosed. Herein, we performed intraductal irrigation of the left parotid gland in three times per month and prescribed some medication. Thus, her signs and symptoms have been improved, and she did not experience a recurrence for 12-month follow-up to date. This study can support the usefulness of the US-guided irrigation method to treat the sialodochitis fibrinosa without remarkable side effects.

The relationship between subjective oral health and dental fear in Korean adolescents

  • Background: This study is aimed to evaluate the level of fear and to reduce the overall fear, thereby enabling patients to receive treatment via timely visits. Methods: In a survey conducted by 460 South Korean middle school students, we used 453 data that faithfully responded to the survey. Multiple regression analysis was conducted to investigate the factors influencing subjective oral health and dental fear. The significance level used for statistical significance was ${\alpha}=0.05$. Results: The level of fear was higher for upper grade, female students. The factors affecting dental fear were higher for gingival bleeding and dental pain. Regarding factors for dental fear affecting subjective oral health, lower fear of puncture needle and tooth removal tool resulted in higher subjective oral health. Conclusions: The study found that adolescents had higher fear of dental care when they had gingival bleeding and tooth pain. Gingival bleeding is a symptom of early gingival disease and dental pain is likely due to advanced dental caries. These results suggested that it is necessary to have a program to reduce dental fear and anxiety as well as a program to prevent dental diseases through regular periodic screening and education.

Comparative Analysis of Salivary Cortisol in Young Adult Patients with Temporomandibular Disorders

  • Purpose: The goal of this study was to investigate the causative factors related to the stress of temporomandibular disorders (TMDs) by evaluating salivary cortisol concentration in young adult TMD patients and control groups. Methods: Saliva was obtained from 32 young adult TMDs patients and 34 control patients without a history of TMDs who visited Chosun University Dental Hospital between June 1 and August 31, 2021. Enzyme-linked immunosorbent assay was conducted to measure the salivary cortisol concentration. Results: The salivary cortisol concentration in the TMD patient group and the control group differed significant significantly (p<0.05). The salivary cortisol concentration according to the duration of the clinical symptom of TMD differed significantly difference between the two groups in the male. The salivary cortisol concentration according to perceived stress level differed significantly in the mild and moderate groups (p<0.05). There was no significant difference in salivary cortisol concentration between the two groups related to bruxism or clenching (p>0.05). Conclusions: The salivary cortisol concentration in the TMD patient group and the control group showed statistical relevance, indicating that stress was a causative factor.

단층촬영을 이용한 악관절 기능장애 환자의 과두위에 관한 연구 (A TOMOGRAPHIC STUDY OF THE CONDYLE POSITION IN TEMPOROMANDIBULAR DISORDERS)

  • The aim of this study was to determine whether T.M.J. tomographic examination yielded significant differences in condyle positions among asymptomatic, myalgia, derangement, and arthrosis groups of T.M.J. disorders. The author obtained sagittal linear tomograms of right and left T.M.Js. of 36 asymptomatic, 22 myalgia, 54 derangement, and 31 arthrosis patients taken at serial lateral, central, and medial sections in the intercuspal position after submentovertex radiographs analyzed. With the dual linear measurements of the posterior and anterior interarticular space, condyle positions were mathematically expressed as proportion. All data from these analysis was recorded and processed statistically. The results were obtained as follows. 1. In asymptomatic group, radiographically concentric condyle position was found in 50.0% to 65.4% of subjects, with a substantial range of variability. No significant differences existed between men and women and also between right and left T.M.Js. for condyle position. 2. In women, significant difference for mean condyle position of left lateral section of each diagnostic category existed between derangement and myalgia groups (P<.05). Also that of left central section existed between derangement and myalgia groups, and that of left medial section existed between derangement and myalgia groups (P<.05). 3. In main-symptom side, condyle position in myalgia group was more concentric, and condyle position in derangement group was more posterior. This showed significant differences between derangement and myalgia groups in lateral, central, and medial sections of main- symptom sides, and only between derangement and myalgia groups in central section of contra-lateral sides (P<.05). Condyle position in arthrosis group was broadly distributed among all positions. 4. In contra-lateral side, significant difference for mean condyle position of central section of each symptomatic group existed between derangement and myalgia groups (P<.05). Condyle position in derangement group was more posterior. The distribution of the condyle position of contra-lateral side in patients with unilateral symptoms was similar to that of main-symptom side in each symptomatic group. No significant difference existed between main-symptom and contra-lateral sides. 5. For internal derangement subgroups, condyle position in reducible disc displacement group was more posterior than non-reciprocal and locking groups, but there was no significant difference. 6. From 16 to 25 years, significant difference for mean condyle position of medial section of main-symptom side of each symptomatic group existed between myalgia and derangement groups (P<.05).
