Correlation between clinical symptoms and magnetic resonance imaging findings in patients with temporomandibular joint internal derangement

  • Objectives: The purpose of this study was to clarify which findings in magnetic resonance imaging (MRI) are good predicators of pain and mouth opening limitation in patients with temporomandibular joint (TMJ) internal derangement (ID). Materials and Methods: Clinical examinations for pain and mouth opening limitation were conducted for suspected TMJ ID. MRI scans were taken within a week of clinical examinations. On the oblique-sagittal plane image, readings were obtained in terms of the functional aspect of disc position, degree of displacement, disc deformity, joint effusion, and osteoarthrosis. Multiple logistic regression analyses were conducted to identify the predictors of pain and mouth opening limitation. Results: A total of 48 patients (96 TMJs) were studied, including 39 female patients and 9 male patients whose ages ranged from 10 to 65 years. The resultant data showed significant correlations between pain and the MR imaging of the degree of disc displacement (P<0.05). The probability of there being pain in moderate to significant cases was 9.69 times higher than in normal cases. No significant correlation was found between mouth opening limitation and MRI findings. Conclusion: We identified a significant correlation between clinical symptoms and MRI findings of ID. The degree of anterior disc displacement may be useful for predicting pain in patients with TMJ ID.

New J Testing Method Using Load-COD Curve for Circumferential Through-Wall Cracked Pipes under Bending (하중-균열열림변위를 이용한 굽힘하중이 작용하는 원주방향 관통균열 배관의 새로운 J 실험법)

  • The present paper provides experimental J estimation equation for the circumferential through-wall cracked pipe under four-point bending, based on the load-crack opening displacement (COD) record. Based on the limit analysis and the kinematically admissible rigid-body rotation field, the plastic ${\eta}$-factor for the load-COD record is derived and is compared with that for the load-load line displacement record. Comparison with the J results from detailed elastic-plastic finite element (FE) analysis shows that the proposed method based on the load-COD record provides reliable J estimates even for shallow cracks, whereas the conventional approach based on the load-load line displacement record gives erroneous results for shallow cracks. Thus, the proposed J estimation method could be recommended for testing the circumferential through-wall cracked pipe, particularly with shallow cracks.

Analysis of Fatigue Crack Opening: Belhlavioll Using Direct Measuring Method (직접측정법에 의한 피로크랙 개구거동의 해석)

  • The purpose of this study is to identify the effects of loading variables on fatigue crack opening behavior, using structural steel SS41. To use various stress ratios, three kinds of the methods varying stress ratio were used; .DELTA.P const., P$_{max}$ const., and P$_{min}$ const.. To measure the opening load, the direct measuring method which measures the distance between two micro indentations 20 .mu.m behind crack tip and the elastic compliance method were used. As the results of present study, the following conclusions are obtained. First, using the direct measuring method, it was possible to measure the COD at any location behind crack tip. Second, as measuring point becomes farther from crack tip, opening load becomes smaller. Third, the acceleration of da/dN near notch is due to crack opening behavior. Finally, opening ratio is a function of not only R, but .DELTA.K.K.K.

  • This paper presents the model for evaluating restraint effect of pressure induced bending (PIB) on the circumferential through-wall crack opening displacement (COD), which considers plastic behavior of crack. This study performed three-dimensional elastic-plastic finite element (FE) analyses for different crack angle, restraint length, pipe geometry, stress level, and material conditions, and evaluated the influence of each parameter on the PIB restraint effect on COD. Based on these evaluations and additional perfectly-plastic FE analyses, a closed-form model to evaluate the restraint effect of PIB on the plastic crack opening of circumferential through-wall crack, was proposed as functions of crack angle, restraint length, radius to thickness ratio, axial stress corresponding to an internal pressure, and normalized COD evaluated from linear-elastic crack opening condition.

Opening Exercise Therapy with Locking-free Appliance(LA) : Preliminary Study (Locking-free Appliance(LA)를 이용한 개구운동치료 : 예비실험)

  • Objective: All patients who have the disc displacement with reduction (DDwR) are always not progressive but some of them with significant catching or locking progress to disc displacement without reduction (DDw/oR). In this study, we suggest opening exercise therapy using Locking-free Appliance (LA) to be helpful to prevent progressive derangement. Methods: Retrospectively, a total 37 patients who received exercise treatment using LA was analyzed from January 2010 to December 2011. Patients had been instructed in proper exercise technique to practise for opening and closing of mouth wearing the appliance 30 times a day in locking-free position. Results: None of the patients took a turn for worse to DDw/oR (37 patients). The 16 patients (43.2%, locking-free group) of them could open their mouth without intermittent condylar locking. Measurement of data was analyzed using and Mann-Whitney test. There was no statistically significant difference for age, gender, presence of palpational pain, locking period, VAS quantitative pain score between locking-free group and locking group. Conclusions: In this study, some patients could open their mouth without intermittent locking and did not take a turn for worse to DDw/oR after opening exercise therapy with LA. Furthermore, LA had advantages which were short wearing time, low costs and minimizing patients' uncomfortable compared to conventional treatment method. This therapy can be more appropriate new treatment to prevent progress to DDw/oR.

Technique for the Measurement of Crack Widths at Notched / Unnotched Regions and Local Strains (콘크리트의 노치 및 비노치 구역에서의 균열폭 및 국부 변형률 정밀 측정기법)

  • Crack widths play an important role in the serviceability limit state. When crack widths are controlled sufficiently, the reinforcement corrosion can be reduced using only existing concrete cover thickness due to low permeability in the region of finely distributed hair-cracks. Thus, the knowledge about the tensile crack opening is essential in designing more durable concrete structures. Therefore, numerous researches related to the topic have been performed. Nevertheless accurate measurement of a crack width is not a simple task due to several reasons such as unknown potential crack formation location and crack opening damaging strain gages. In order to overcome these difficulties and measure precise crack widths, a displacement measurement system was developed using digital image correlation. Accuracy calibration tests gave an average measurement error of 0.069 pixels and a standard deviation of 0.050 pixels. Direct tensile test was performed using ultra high performance concrete specimens. Crack widths at both notched and unnotched locations were measured and compared with clip-in gages at various loading steps to obtain crack opening profile. Tensile deformation characteristics of concrete were well visualized using displacement vectors and full-field displacement contour maps. The proposed technique made it possible to measure crack widths at arbitrary locations, which is difficult with conventional gages such as clip-in gages or displacement transducers.

THE EFFECT OF INTRA-ARTICULAR INJECTION OF HYALURONIC ACID AFTER ARTHROCENTESIS IN TREATMENT OF INTERNAL DERANGEMENTS OF THE TMJ (악관절 내장증 치료를 위한 악관절 세정술 후 관절강내 Hyaluronic Acid 주입 효과)

  • This study was designed to investigate the effect of intra-articular injection of hyaluronic acid for the treatment of internal derangements of the temporomandibular joint(TMJ). Sixteen patients with internal derangements of TMJ in 1 male and 15 females aged 68 to 18 years comprised the study material. The patients' complaint was limited mouth opening and pain during function. Patients were divided into 3 groups(articular disc displacement with reduction, articular disc displacement without reduction, osteoarthritis group). The preauricular area was disinfected anesthetized locally with 2% lidocaine hydrochloride. Arthrocentesis was performed. Hyaluronic acid(1.5 ㏄) was then injected into the superior compartment of the TMJ. Active range of motion exercises were instituted at approximately 24 hours postoperatively. Antibiotics and NSAID, three times daily by mouth, was prescribed for 3 days. Clinical evaluation of the patients was done before the procedure, immediately after the procedure, final follow-up visit postoperatively. Pain during function was assessed using visual analog scales(VAS). Maximal mouth opening was recorded as a distance between upper incisal edge and lower incisal edge. Intra-articular injection of hyaluronic acid caused significant reduction of pain during function, significant increase of maximal opening range. These findings suggest that intra-articular injection of hyaluronic acid is useful for decreasing patient reports of pain while increasing functional mobility of the mandible in internal derangements of the TMJ.

A Study on the Interaction of Segmented Hydraulic Fractures (다중으로 분할된 수압파쇄균열의 상호작용에 관한 연구)

  • Recent observations based on geological evidence and laboratory tests confirm that complex segmentation of hydraulic fractures is common phenomena. It is expected that the segmentation causes mechanical interaction between the fractures and affects fracture opening and measured net pressure. In this study, therefore, the opening of the fractures is computed using boundary collocation method to evaluate the mechanical interaction quantitatively. Also, improved boundary collocation method is suggested to evaluate the displacement of the fracture wall accurately and the reliability of this method is confirmed by comparing with that of the finite element method.

  • The stress conditions acting on the practical structure are complex, and thus most cracks existing in the practical structures are under mixed-mode loading conditions. The effect of shear load component of mixed-mode loading acts more greatly in the stage of crack initiation and initial propagation than crack propagation stage. Hence, research on the behavior in the stage of crack initiation and initial propagation need to be examined in order to evaluate behavior of mixed-mode fatigue cracks. In this study, the crack tip displacement(CTD) was measured by using the direct measuring method(DMM). We examined the behavior at crack tip by determining crack opening load$(P_{op})$. From the test results, the propagation behavior of mixed-mode fatigue cracks was evaluated by considering mixed-mode crack closure. Also, we examined the characteristic of crack propagation under mixed-mode loading with crack propagation direction.

A simple prediction procedure of strain-softening surrounding rock for a circular opening

  • A simple prediction procedure was investigated for calculating the stresses and displacements of a circular opening. Unlike existed approaches, the proposed approach starts each step with a radius increment. The stress for each annulus could be obtained analytically, while strain increments for each step can be determinate numerically from the compatility equation by finite difference approximation, flow rule and Hooke's law. In the successive manner, the distributions of stresses and displacements could be found. It should be noted that the finial radial stress and displacement were equal to the internal supporting pressure and deformation at the tunnel wall, respectively. By assuming different plastic radii, GRC and the evolution curve of plastic radii and internal supporting pressures could be obtained conveniently. Then the real plastic radius can be calculated by using linear interpolation in the evolution curve. Some numerical and engineering examples were performed to demonstrate the accuracy and validity for the proposed procedure. The comparisons results show that the proposed procedure was faster than that in Lee and Pietrucszczak (2008). The influence of annulus number and dilation on the accuracy of solutions was also investigated. Results show that the larger the annulus number was, the more accurate the solutions were. Solutions in Park et al. (2008) were significantly influenced by dilation.
