Objectives: To evaluate the pulmonary toxicity of Korean Chrysotile(KC), the lung function and the number of fibers in the bronchoalveolar lavage fluid of Sprague-Dawley rats instilled with KC were estimated. Materials: Rats were administered 1 mg KC by a single intratracheal instillation. At each time point(5 days, 5 weeks, 10 weeks), the lung function of the rats was analyzed by pressure transducer and the amount of asbestos in the bronchoalveolar lavage fluid of the rats was calculated by transmission electron microscope equipped with energy dispersive X-ray spectrometer. Results: The lung function of the rats at 5 days after instillation of KC was confirmed to be degraded. However, after 5 weeks the test groups showed a tendency to recover lung function. Still, after 10 weeks the lung function of the test groups had not recovered completely. The number of fibers in the bronchoalveolar lavage fluid of the rats instilled with KC rapidly decreased. At 5 weeks the number of fibers had reduced to approximately 1/2 of that found at 5 days. Over time, at 10 weeks it had rapidly decreased to 1/100 that found at 5 days. Conclusions: Korean chrysotile fibers rapidly decreased in the lungs of rats, but the lung function of rats instilled KC does had not completely recovered by 10 weeks.
Lung granulomas are uncommon in Thailand. The disease typically develops from an occupational environment and is mostly caused by infection. Herein is a case report of a female patient, aged 48, working as a nurse in an Accident and Emergency Department at a hospital. Eighteen years prior to admission the patient was diagnosed with myasthenia gravis and pulmonary tuberculosis. The chest X-ray and CT scans showed a solitary pulmonary nodule in the lower left lung. The patient received an open thoracotomy with a left lobectomy. Granulomatous and nonseptate hyphae were found in the pathology diagnosis. The patient was thus diagnosed as having a lung granuloma. The galactomannan antigen test was positive. The solitary pulmonary nodule-found from the use of a Polymerase Chain Reaction (PCR) test-was an Aspergillus spp. The fungus culture was collected from air samples. The air samples were collected by the impaction technique using a microbial air sampler. Three types of Aspergillus spp. were found as well as Penicillium spp. and Monilia sitophila. The Aspergillus spp. was a match for the patient's disease. The patient was diagnosed as having a lung granuloma possibly Aspergillus nodule which was caused by airborne Aspergillus spp. from the occupational environment.

Background: Welders are exposed to many known and suspected carcinogens. An excess lung cancer risk among welders is well established, but whether this is attributable to welding fumes is unclear. Excess risks of other cancers have been suggested, but not established. We investigated welding cancer risks in the population-based Canadian Census Health and Environmental Cohort. Methods: Among 1.1 million male workers, 12,845 welders were identified using Standard Occupational Classification codes and followed through retrospective linkage of 1991 Canadian Long Form Census and Canadian Cancer Registry (1992-2010) records. Hazard ratios (HRs) were calculated using Cox proportional hazards models based on estimated risks of lung cancer, mesothelioma, and nasal, brain, stomach, kidney, and bladder cancers, and ocular melanoma. Lung cancer histological subtypes and risks by industry group and for occasional welders were examined. Some analyses restricted comparisons to blue-collar workers to minimize effects of potential confounders. Results: Among welders, elevated risks were observed for lung cancer [HR: 1.16, 95% confidence interval (CI): 1.03-1.31], mesothelioma (HR: 1.78, 95% CI: 1.01-3.18), bladder cancer (HR: 1.40, 95% CI: 1.15-1.70), and kidney cancer (HR: 1.30, 95% CI: 1.01-1.67). When restricted to blue-collar workers, lung cancer and mesothelioma risks were attenuated, while bladder and kidney cancer risks increased. Conclusion: Excess risks of lung cancer and mesothelioma may be partly attributable to factors including smoking and asbestos. Welding-specific exposures may increase bladder and kidney cancer risks, and particular sources of exposure should be investigated. Studies that are able to disentangle welding effects from smoking and asbestos exposure are needed.
Background: Building demolition can lead to emission of dust into the environment. Exposure to silica dust may be considered as an important hazard in these sites. The objectives of this research were to determine the amount of workers' exposure to crystalline silica dust and assess the relative risk of silicosis and the excess lifetime risk of mortality from lung cancer in demolition workers. Methods: Four sites in the Tehran megacity region were selected. Silica dust was collected using the National Institute for Occupational Safety and Health method 7601 and determined spectrophotometrically. The Mannetje et al and Rice et al models were chosen to examine the rate of silicosis-related mortality and the excess lifetime risk of mortality from lung cancer, respectively. Results: The amount of demolition workers' exposure was in the range of $0.085-0.185mg/m^3$. The range of relative risk of silicosis related mortality was increased from 1 in the workers with the lowest exposure level to 22.64/1,000 in the employees with high exposure level. The range of the excess lifetime risk of mortality from lung cancer was in the range of 32-60/1,000 exposed workers. Conclusion: Geometric and arithmetic mean of exposure was higher than threshold limit value for silica dust in all demolition sites. The risk of silicosis mortality for many demolition workers was higher than 1/1,000 (unacceptable level of risk). Estimating the lifetime lung cancer mortality showed a higher risk of mortality from lung cancer in building demolition workers.
Objectives: Although several cases of lung diseases caused by indium have been reported in Japan, the United States and China, South Korea, which is estimated to have been the world's largest consumer of indium, has not yet established a criteria for the diagnosis of lung diseases caused by indium exposure. In this study, we tried to determine the applicability of the Krebs von den lungen-6, which has been widely recognized for its use with interstitial lung disease in Japan, as a biological exposure index for indium. Methods: Methods: The analysis of indium in serum was conducted by inductively coupled plasma mass spectrometry and the analysis of KL-6 in serum was carried out using enzyme-linked immunosorbent assay kit. Results: The indium levels in serum were distributed from below the detection limit to a peak of $125.78{\mu}g/L$, and the values of the KL-6 were distributed from 104.5 U/mL to 2162.2 U/mL. The serum indium and KL-6 showed good correlation ($R^2$=0.389,pfortrend=0.000) and smoking did not affect the KL-6. Conclusions: The usefulness of KL-6 as a specific biomarker for interstitial lung disease has been recognized. In addition, it is expected that effective prevention of health problems can be achieved by determining the lung-damage progress at an early stage according to individual susceptibility.
Objectives: To evaluate the pulmonary toxicity of 2 Korea asbestos(chrysotile, anthophyllite), Sprague-Dawely rats were exposed to 2 mg domestic asbestos by intratracheal instillation(IT). Methods: Lung function of rats was analyzed by pressure transducer(MAX1320, Buxco Electronics, USA). The effects of 2 mg asbestos(chrysotile ; $8,814,244{\times}10^{6}$ fibers/mg, average diameter 0.08 ${\mu}m$, average length 4.39 ${\mu}m$, anthophyllite ; $5,182{\times}10^{6}$ fibers/mg, average diameter 0.95 ${\mu}m$, average length 7.29 ${\mu}m$) on pulmonary function and pathological changes were evaluated at after a single IT. Lung function and histopathological evaluation were assessed in 5 animals from each group at each time point. Results: Due to differences in fiber numbers, chrysotile induce marked lung pathology and lung function change than anthophyllite at the same mass dose. Chrysotile showed notable thickening of interstitial areas surrounding the alveolar ducts and terminal bronchioles. Conclusions: On a mass dose basis, chrysotile that have 1,700 times numbers of fibers per unit weight than anthophyllite produced a greater persistent lung injury than anthophllite for at least 4 weeks after exposure.
Occupational lung disease (OLD) is a group of lung diseases caused and/or aggravated by organic and inorganic inhaled dust, fumes, and mist. OLD can develop under various occupational situations. Therefore, occupational history should be considered when evaluating respiratory symptoms. Once OLD is developed, it may not be treated and may even progress after exposure to the causative agents has stopped. The best ways to treat OLD are prevention and early detection by controlling the working environment and conducting regular surveillance of workers. Common OLDs in Korea are coal worker's pneumoconiosis, asbestos-related diseases, and occupational asthma. Recent aspects of these common OLDs in Korea will be described based on recently published studies.

Methods: Rats were administered 1 mg KC(low group) or 2 mg KC(high group) by a single intratracheal instillation. At each time point(5 days, 5 weeks, 10 weeks), the numbers of KC fibers and the changes of element composition(atomic %) of KC fibers from the lung of the rats were analyzed with transmission electron microscope equipped with energy dispersive X-ray spectrometer. Results: Over time, the number of fibers within the lungs of animals were reduced. The average length of the low and high group is significantly reduced from 5 days after administration. Over time, the fiber ratio of at least $5{\mu}m$ remaining in the lung tissue of the low concentration group was up but the high group was reduced. From day 5 after administration, the composition ratio(Mg) was significantly decreased in all groups. Conclusions: Size and composition of Korean Chrysotile in the lung tissue of rats was changed from 5 days.

Occupational long-term exposure to inorganic dusts may cause a variety of lung diseases such as pneumoconiosis and chronic obstructive pulmonary disease (COPD). Diagnosis of pneumoconiosis and COPD, however, is currently dependent on radiological findings and pulmonary test, which are both late diagnostic tools. Therefore, there is a need to identify novel biomarkers in pneumoconiosis and COPD. Hence, in this current study we investigated the serum concentrations of YKL-40, interleukin-6 (IL-6) and tumor necrosis factor-alpha (TNF-${\alpha}$) as biomarkers for pneumoconiosis and COPD in 161 retired male workers exposed to inorganic dusts. The serum concentration of YKL-40 was significantly increased with age, pneumoconiosis, and airflow limitation. The serum concentration of IL-6 was significantly higher in airflow limitation. These results suggest that serum concentration of YKL-40 is associated with age, pneumoconiosis, and airflow limitation. Also, serum concentration of IL-6 is associated with airflow limitation.
