  • 직업상 피폭에 대한 현행 방사선 위험성 평가는 종사자의 피폭선량 평가 및 건강진단에 중점을 두고 있다. 이러한 개인 중심의 위험성 평가는 선량계 미착용 및 개개인의 기호로 인한 건강영향 문제 등 정확한 데이터 확보의 어려움으로 인한 오류의 가능성이 있다. 또한 평가의 기준이 되는 선량한도는 법적 최대 상한값으로 방사선 방호에 최적화된 값을 의미하지는 않는다. 이에 선원적, 환경적 및 인적 측면을 복합적으로 고려할 수 있고 방사선방호의 최적화를 이행할 수 있는 국가적 차원의 새로운 위험성 평가 모델이 요구되고 있다. 본 연구에서는 고용노동부의 위험성 평가에 기반하여 개인이 아닌 작업장 중심의 위험성 평가 모델을 연구하였다. 이를 위해 여러 분야의 위험성 추정 방법을 분석하여 방사선 분야에 적용하기 적합한 모델을 도출하고, 모델에 적용하기 위한 데이터 획득 방법 및 절차에 대해 기술하였다. 본 연구에서 도출한 작업장 중심의 다차원적 위험성 평가 모델은 위험성을 점수화하고 Rader Plot을 이용하여 표현함으로서 보다 정확한 방사선 위험성 평가를 가능하게 하며, 결론적으로 효율적인 종사자 관리, 선제적 종사자 보호 및 방사선 방호의 최적화 이행에 기여할 것으로 판단된다.

환경 바이오모니터링에서의 혈중 중금속 분석 숙련도 시험을 위한 표준물질 제조 및 적용성 평가 (Development and Validation of Reference Material for Proficiency Testing of Blood Heavy Metals in Environmental Biomonitoring)

  • Objectives: This study developed and validated reference materials (RMs) to analyze metal compounds in blood. Methods: We referred to KoNEHS (Korea National Environmental Health Survey) to estimate concentrations of blood metals (cadmium, Cd; lead, Pb; mercury, Hg) and applied analytical methods (inductively coupled plasma - mass spectroscopy, ICP-MS, for Cd and Pb; graphite furnace - atomic absorption spectrometry, GF-AAS, for Cd and Pb; and direct mercury analyzer, DMA, for Hg). Homogeneity and stability tests were carried out. In addition, certified values and uncertainties of RMs were calculated through internal and external experiments. All RMs were developed and assessed in various forms according to element, analytical method, and two types of concentration levels high concentration for occupational exposure and low concentration for environmental exposure. Results: All samples showed acceptable homogeneity, except for low concentration of Cd in the GF-AAS method. Short- and long-term stabilities were satisfied by ANOVA testing. In the inter-laboratory comparison, robust medians were lower than the certified values of all RMs (robust median/reference value; $1.301/1.327{\mu}g/L$ for Cd, ICP-MS, low concentration; $3.152/3.388{\mu}g/L$ for Cd, ICP-MS, high concentration; $1.219/1.301{\mu}g/L$ for Cd, GF-AAS, low concentration; $3.074/3.321{\mu}g/L$ for Cd, GF-AAS, high concentration; $14.473/14.516{\mu}g/L$ for Pb, ICP-MS, low concentration; $50.069/50.114{\mu}g/L$ for Pb, ICP-MS, high concentration; $12.881/14.147{\mu}g/L$ for Pb, GF-AAS, low concentration; $47.015/47.591{\mu}g/L$ for Pb, GF-AAS, high concentration; $4.059/4.218{\mu}g/L$ for Hg, DMA, low concentration; $11.474/11.181{\mu}g/L$ for Hg, DMA, high concentration). Conclusion: This study demonstrates procedures for developing and validating RMs for biomonitoring in the field of the environmental health.

정상인에서 혈중 연과 zinc protoporphyrin과의 상관관계 및 HPLC 와 Hematofluorometer로 측정한 zinc protoporphyrin량간의 비교 (The Relationship between Zinc protoporphyrin and Lead levels in Normal Adults' Blood And Comparison of Zinc protoporphyrin values by High Performance Liquid Chromatograph and Hematofluorometer)

  • Blood samples obtained from 200 adults who had visited the "S" general hospital were analyzed to compare the zinc protoporphyrin (ZPP) levels quantified by high performance liquid chromatograph (HPLC) and by hematofluorometer (HF) to investigate the methodological difference if any and the relationship between the levels of blood lead and ZPP among no-lead exposed adults. Also investigated were the distribution of ZPP and protoporphyrin IX (PPIX) concentrations, the establishment of normal levels of blood ZPP and blood lead, and the contribution of age and sex factors to these values. These subjects had no previous occupational exposure to lead. The results obtained were as follows : 1. The mean values of blood lead for male and female subjects were $9.46{\pm}2.44{\mu}g/dl$ and $8.09{\pm}2.17{\mu}g/dl$, respectively. The difference observed in the mean concentrations between male and female subjects was statistically very significant. 2. The mean values of blood ZPP by HPLC for male and female subjects were $15.94{\pm}4.55{\mu}g/dl$ and $22.26{\pm}6.61{\mu}g/dl$, respectively. The difference observed in the mean concentrations between male and female subjects was statistically not significant. The mean values of blood PPIX by HPLC for male and female subjects were $2.51{\pm}1.78{\mu}g/dl$ and $2.81{\pm}1.56{\mu}g/dl$, respectively. The difference observed in the mean concentrations between male and female subjects was statistically not significant. 3. The mean values of blood ZPP by HF for male and female subjects were $28.44{\pm}7.11{\mu}g/dl$ and $37.77{\pm}8.04{\mu}g/dl$, respectively. The difference observed in the mean concentrations between male and female subjects was statistically very significant. 4. No statistically significant correlation was found between the levels of blood ZPP and blood lead. 5. The ratio of ZPP and protoporphyrin IX (PPIX) concentration to erythrocyte protoporphyrin (EP, EP=ZPP+PPIX) concentration was 87.4% and 12.6%, respectively. 6. A statistically very significant correlation was found between the ZPP concentrations determined by HPLC and the values by HF (r=0.7565). The ZPP concentraitons quantified by HF were 1.75 times as high as the values obtained by HPLC. 7. The blood ZPP concentrations quantified by HPLC, HF, and spectrofluorometer (SF) from the blood samples obtained from 14 lead-exposed workers and from 16 no-lead exposed adults showed wide variations. The ZPP concentrations by HF were the highest followed by the levels obtained by SF and by HPLC. In the exposed group, no statistically significant difference was found among three methods of quantifying blood ZPP levels. In the no-lead exposed group, however, statistically significant difference was observed among these methods. The ZPP concentrations by HF were about twice as high as those of by HPLC or by SF. Among three methods of quantifying blood ZPP (HPLC, SF and HF), the results revealed significant difference. Therefore it is suggested that objective methods of quantifying blood ZPP and a system of correcting different ZPP levels be developed by the ministry of Labor.

퇴직한 납 취급 근로자들에서 골밀도 저하와 경골납량이 혈중납량에 미치는 영향 (Effect of Bone Demineralization and Tibia Lead on Blood Lead in Retired Lead Workers)

  • This study was designed to investigate the effect of bone demineralization and tibia lead on blood lead in retired lead workers. Two hundred thirty five(126 females and 109 males) retired lead workers who worked in 4 different lead factories and 101 non-occupationally lead exposed subjects(51 females and 51 males) were recruited from March 2004 to October 2004. Bone mineral density(BMD) was measured at left calcaneous bone area by broadband ultrasound attenuation(BUA) method with QUS-2(Metra Biosystems Inc, USA). The BUA value transformed into T-score by WHO standard conversion criteria. Tibia bone lead was measured for skeletal bone lead with K-xray fluorescence(K-XRF) and blood lead was analyzed with flameless atomic spectrophotometer. Hemoglobin, hematocrit, serum calcium and iron were also analyzed. In addition, information for smoking and drinking status and basic personal data such as age, gender and lead exposure were also collected using questionnaire inquiry. Blood lead was correlated with tibia lead (r=0.664) and these two variables were negatively correlated with BMD in bivariate analysis. BMD showed significant main effect on the change of blood lead independent to tibia lead without any effect modification of age or gender; the one T-score unit decrease of mineral bone density made $0.43{\mu}g/dl$ increase of blood lead. On the other hand, tibia lead showed effect modification with gender on blood lead; the slope of tibia lead on blood lead in male was steeper than in female and crossed at around zero of tibia lead. In the multiple regression analysis of blood lead and tibia lead on BMD after adjustment of related covariates, only blood lead showed statistically significant effect on BMD. This study confirmed that BMD and blood lead were significantly associated. To verify the causal association of BMD on blood lead and vice versa, further longitudinal studies are needed.

Associations of Dietary Calcium Intake and Serum Calcium Level with Blood Lead Levels in Korean Male Lead Workers

  • A cross-sectional study was performed to estimate the nutritional status of Korean male lead workers and to assess the relationship between calcium nutritional status and blood lead levels. A flood consumption survey was conducted by the 24-hr recall method with 118 lead workers and 63 non-lead exposed controls. Blood lead levels were analyzed from whole blood and serum calcium concentrations were also assessed. Results of dietary analysis showed Korean lead workers consumed relatively sufficient nutrients (more than 75% of RDA) except, calcium. Mean dietary calcium intake of lead workers was 502.2mg(72% of RDA) while that of the non-lead workers was estimated as 600.8mg(86% of RDA). Intakes of protein, iron, niacin and vitamin C of lead workers were significantly lower than those of non-lead workers. There was a wide range of blood lead levels(5.5 to 73.5$\mu\textrm{g}$/㎗) observed while mean blood lead level of lead workers was 30.9$\mu\textrm{g}$/㎗. However, 98% of lead workers showed normal serum ca1cium concentrations (range ; 8.9 to 10.7mg/㎗, mean ; 9.77mg/㎗) while 66% of lead workers were estimated to intake a dietary calcium lower than 75% of RDA. Mean blood lead levels of non-1ead workers were significantly lower(mean ; 5.1$\mu\textrm{g}$/㎗, p < 0.001) and the serum calcium concentration was significantly higher(mean ; 10.20mg/㎗, p < 0.001) than lead workers. Results of unadjusted correlation showed that serum calcium level and dietary calcium intake were negatively correlated with blood lead concentration. In a multiple regression of blood lead levels with variables known as affecting blood lead 1eve1 such as age, body mass index and occupational lead exposure, serum calcium was insignificant while dietary calcium intake showed statistically significant(p < 0.05) relation. Since calcium is a very important nutrient to reduce hazardous effects of lead, it should be strongly recommended that lead workers need to increase dietary calcium intake.

Lead Pollution and Lead Poisoning among Children in China

  • Lead is ubiquitous in the human environment as a result of industrialization. China's rapid industrialization and traffic growth have increased the potential for lead emissions. Lead poisoning in children is one of the most common public health problems today, and it is entirely preventable. Children are more vulnerable to lead pollution and lead in their bodies can affect their nervous, circulatory, and digestive systems. Children are exposed to lead from different sources (such as paint, gasoline, and solder) and through different pathways (such as air, food, water, dust, and soil). Although all children are exposed to some lead from food, air, dust, and soil, some children are exposed to high dose sources of lead. Significant sources of lead for China's children include industrial emissions (often close to housing and schools), leaded gasoline, and occupational exposure that occurs when parents wear lead-contaminated clothing home from work, burning of coal for home heat and cooking, contaminated food, and some traditional medicines. To assess the blood lead level in children in China, a large-scale study was conducted in 19 cities among 9 provinces during 1997 to 2000. There were 6502 children, aged 3-5 years, were recruited in the study The result indicates that the mean blood lead level was 8.83ug/dl 3-5 year old living in city area. The mean blood lead level of boys was higher than that of girls (9.1l ug/dl vs 8.73ug/dl). Almost 30 percent childrens blood lead level exceeded 10ug/dl. The average blood lead level was higher than that of in 1985 (8.83ug/dl vs 8.lug/dl). An epidemiological study was carried on the children living around the cottage industries recycling the lead from battery. Nine hundreds fifty nine children, aged 5-12 years, living in lead polluted villages where the lead smelters located near the residential area and 207 control children live in unpolluted area were recruited in the study. The lead levels in air, soil, drinking water and crops were measured. The blood lead and ZnPP level were tested for all subjects. The results show that the local environment was polluted. The lead levels both in the air and crops were much higher than that of in control area. In the polluted area, the average blood level was 49.6ug/dl (rang 19.5-89.3ug/dl). Whereas, in the unpolluted area, the average blood level was 12.4ug/dl (rang 4.6-24.8ug/dl). This study indicates that in some countryside area, some cottage industries induce seriously lead pollution and cause children health problem. For the introducing of unleaded gasoline in some large cities, such as Beijing and Shanghai, the blood lead level showed a declined trend since 1997. By 2000, the use of leaded gasoline in motor vehicles has been prohibited in China. The most recent data available show that levels of lead in blood among children in Shanghai decreased from 8.3ug/dl in 1997 to 7.6ug/dl in 1999. The prevalence rate of children lead poisoning (blood lead >10ug/dl) was also decreased from 37.8% to 24.8%. In children living in downtown area, the blood lead level reduced dramatically. To explore the relationship between gene polymorphisms and individual susceptibility of lead poisoning, a molecular epidemiological study was conducted among children living in lead polluted environment. The result showed that the subjects with ALAD2 allele has higher ZPP level, and the subjects with VDR B allele has larger head circumference than only with b allele. In the present study, we demonstrated that ALAD genotypes modify lead effects on heme metabolism and VDR gene variants influence the skull development in highly exposed children. The polymorphism of ALAD and VDR genes might be the molecular inherited factor modifying the susceptibility of lead poisoning. Recently, Chinese government pays more attention to lead pollution and lead poisoning in children problem. The leaded gasoline was prohibited used in motor vehicles since 2000. The government has decided to have a clampdown on the high-polluted lead smelters for recycling the lead from battery in countryside. Lung Function Profiles among Individuals with Nonmalignant Asbestos-related Disorders

  • Background: Inhalation of asbestos fibers can lead to adverse health effects on the lungs. This study describes lung function profiles among individuals with nonmalignant asbestos-related disorders (ARDs). Methods: The study population was from the Workers' Compensation (Dust Diseases) Board of New South Wales, Sydney, Australia. Lung function measurements were conducted in males with asbestosis (n = 26), diffuse pleural thickening (DPT; n = 129), asbestosis and DPT (n = 14), pleural plaques only (n = 160) and also apparently healthy individuals with a history of asbestos exposure (n = 248). Standardized spirometric and single-breath diffusing capacity for carbon monoxide ($DL_{CO}$) measurements were used. Results: Mean age [standard deviation (SD)] was 66.7 (10.3) years for all participants. Current and ex-smokers among all participants comprised about 9.0% and 54.8%, respectively. Median pack-years (SD) of smoking for ex- and current-smokers were 22.7 (19.9). Overall 222 participants (38.6%) and 139 participants (24.2%) had forced expiratory volume in 1 second ($FEV_1$) and forced vital capacity (FVC) measurements < 80% predicted, and 217 participants (37.7%) had $FEV_1/FVC$ results < 70%. A total of 249 individuals (43.8%) had DLCO values < 80% predicted and only 75 (13.2%) had DLCO/VA results < 80% predicted. A total of 147 participants (25.6%) had peak expiratory flow (PEF) measurements < 80% predicted. The presence of ARDs lowered the lung function measurements compared to those of healthy individuals exposed to asbestos. Conclusion: Lung function measurement differs in individuals with different ARDs. Monitoring of lung function among asbestos-exposed populations is a simple means of facilitating earlier interventions.

Cost Analysis of Using a Closed-System Transfer Device (CSTD) for Antineoplastic Drug preparation in a Malaysian Government-Funded Hospital

  • Background: Apart from reducing occupational exposure to cytotoxic hazards, the PhaSeal(R) closed-system transfer device (CSTD) can extend the beyond-use dates (BUDs) of unfinished vials of antineoplastic drugs for up to 168 hours (seven days). In this study, the total material cost incurred by its use in a Malaysian government-funded hospital was calculated. Methods: A list of vial stability following initial needle punctures of 29 commonly-used antineoplastic drugs was compiled. The amount of the materials used, including drugs, infusion bottles, the PhaSeal(R) CSTD and other consumables, was recorded on a daily basis for three months in 2015. The total cost was calculated based on the actual acquisition costs, and was compared with that of a hypothetical scenario, whereby conventional syringe-needle sets were used for the same amounts of preparations. Results: The use of the PhaSeal(R) CSTD incurred a cost of MYR 383,634.52 (USD 92,072.28) in three months, representing an average of MYR 170.5 (USD 40.92) per preparation or an estimated annual cost of MYR 1,534,538.08 (USD 368,289.14). Compared with conventional syringe-needle approach, it is estimated to lead to an additional spending of MYR 148,627.68 (USD 35,670.64) yearly. Conclusion: Although there was a reduction of drug wastage achieved by extending BUDs of unfinished vials using the PhaSeal(R) CSTD, cost saving was not observed, likely attributable to the wide use of lower-priced generic drugs in Malaysia. Future studies should further evaluate the possibility of cost saving, especially in health settings where branded and high-cost antineoplastic drugs are more commonly used.

연 취급 근로자들의 혈색소치의 변동 (Hemoglobin Concentration in Female Workers of Occupational Exposure to Lead)

  • 연 폭로에 의한 조혈기능의 장해정도를 파악하고 연중독 환자를 조기에 발견하기 위하여 1985년 8월부터 1년간 경상북도 구미공단내 5개 산업장의 연취급 여성근로자 284명과 대조군 123명을 대상으로 혈중 연량콰 혈색소량을 측정하여 얻은 성적을 분석한 결과 다음과 같다. 연 취급 근로자의 평균연령은 $20.3{\pm}2.9$세이었고 대조군의 평균연령은 $21.1{\pm}3.5$세이었으며, 연작업자의 평균 근속기간은 $26.46{\pm}19.26$개월이었다. 연 취급 근로자의 평한 혈중연량은 $30.11{\pm}6.61{\mu}g/100ml$이었고 대조군의 경우 $21.86{\pm}3.75{\mu}g/100ml$로 두군 모두 정상범외내였으나, 폭로군이 대조군에 비해 유의하게 (P<0.01) 높았다. 연 취급 근로자의 평균 혈색소량은 $14.00{\pm}0.57g/100ml$이었고 대조군의 경우 $14.03{\pm}0.64g/100ml$로 정상치를 나타내었다. 혈색소치 12g/100ml이하는 한사람도 없었으며, 12.0~13.0g/100ml경우는 연 취급근로자에서 4.5%로 나타났다. 혈중 연량과 빈혈과의 용량-반응관계, 연령 및 근속기간과 혈중연량 및 혈색소치와의 사이에 유의한 상관관계를 관찰할 수 없었다. 이상의 성적으로 보아 작업장의 연 허용기준치를 넘지 않으면 장기간 근무하여도 연 중독의 피해가 없는 것으로 판단되기 때문에 작업장의 위생공학적인 환경관리가 무엇보다도 중요하다고 본다.

일부 대학생들의 카드뮴과 납 섭취량 및 체내부하 (Dietary intake and body burden of cadmium and lead among Korean college students)

  • Objectives: This study aims to evaluate the dietary intake and blood concentrations for examination of exposure levels and exposure characteristics of Cd and Pb via food among college students aged 20 years living in Busan area. Methods: Food duplicate per a day and venous blood samples were accepted from healthy man and women college students. The samples were analyzed with graphite furnace atomic absorption spectrophotometer. Results: Dietary intake of Cd and Pb in the participants were $17.5{\mu}g\;Cd/day$, $21.7{\mu}g\;Pb/day$. Blood concentrations were $2.01{\mu}g\;Cd/L$, $4.32{\mu}g\;Pb/dL$, respectively. Conclusions: Smoking and drinking habits might be high influential factors in 20 years college students. Dietary intake of Cd and Pb, however, from food group of seaweed, or fish and shellfish in 20 years college students were lower than that in any other age group.
