Analysis of Causes for Primary Treatment Failure of Pulmonary Tuberculosis (폐결핵환자에서 초치료실패에 대한 요인 분석)

  • Background : Nowadays drug resistant tuberculosis is making problems in the treatment of pulmonary tuberculosis and its number is increasing. Several reasons for this are considered including irregular medication, poor drug compliance and wrong regimens. But there are treatment failure cases in spite of regular medication with short-term standard regimens. We reviewed clinical data of 50 patients to find out possible causes of this. Method : Subject of this study was 50 patients who failed in the primary treatment of pulmonary tuberculosis in spite of regular medication with short-term standard regimens. All of them were under treatment with secondary regimens in National Masan Tuberculosis Hospital on Oct 1996. The patient's records were analyzed retrospectively and direct interviews with patients were done. Results : There were relatively more patients in the age of 20th. Male overwhelmed in number. There were smoking in 22 patients and drinking in 24 patients during medication. 17(34%) patients had family history of tuberculosis. Public health center was the most common site for the initial diagnosis among medical institutes. 42 patients had subjective symptoms for pulmonary tuberculosis. 38 patients got sufficient explanation from medical institute about tuberculosis and medication courses. 24 patients had bilateral lesions on chest X-ray film and 43 patients had cavitary lesions. 29 patients had past history for pulmonary tuberculosis with regular medication. The results of drug sensitivity test showed resistance in 41 patients of whom we could get the results. Conclusion : Main cause of treatment failure of pulmonary tuberculosis in spite of regular medication with short-term standard regimens was drug resistance. Several factors were considered to be related to high prevalence of drug resistance, including age of 20th, male, family history for tuberculosis, bilateral lesions or remaining cavitary lesion on chest X-ray film.

  • Bisphosphonate-related osteonecrosis of the jaw (BRONJ) is a side effect of Bisphosphonates (BPs) use. These days, oral bisphosphonates are normally prescribed to treat osteoporosis. Intravenous BPs are used extensively to treat osteolytic bone lesions related to multiplemyeloma and bone metastasis of solid cancers, breast cancer or prostate cancer. As the prescription of BPs is universalized and the number of people treated with BPs is increasing, an accurate understanding and proper management of BRONJ are required. The aim of this study was to improve the clinicians' understanding of BRONJ by reviewing the literature. To achieve this, this paper introduces case reports as well as the current concept of BRONJ based on the 2009 updates by American Association of Oral and Maxillofacial (AAOMS) including the definition, epidemiology, etiology, diagnosis, treatment and prevention of BRONJ.

  • Osteoblastoma is a relatively rare benign bone tumor representing less than 1% of all bone tumors. The tumor usually involves the spine and sacrum of young individuals, less than 10% being localized to the skull, and nearly half of these affect the mandible, especially the posterior segments. In clinical finding, osteoblastoma present mainly with pain, swelling, and expansion of bone cortex. Radiographic appearances are variable, but frequently a well-delineated radiolucent lesion containing varying amounts of mineral deposits is seen. Histologically, ostoeblastoma is consists of irregular trabeculeae of osteoid and immature bone present within highly vascular connective tissue matrix. Osteoblastoma must be differentiated from a number of bone-producing lesions, including osteoid osteoma, fibrous dysplasia, ossifying fibroma, fibrous dysplasia, and osteosarcoma. If diagnosis may be mistaken for osteosarcoma, there are risks of more aggressive and irreversible treatment. Differential diagnosis of osteoblastoma is important. The preferred treatment of osteoblastoma is conservative approach and surgical excision. Recurrence following surgical intervention is rare. We treated osteoblastoma located in premolar area of mandible by excision with preservation of vital structure, such as nerves and teeth. So we report our clinical treatment with literature review

  • A number of studies have shown the value of fine needle aspiration cytology for the diagnosis of breast lesions, but reports on the comparison of cytologic and histologic variations of breast cancers are few. We reviewed histologically proven 106 breast aspirates with an emphasis on the cytology of ductal carcinomas and of an area of uncertain atypia. Positive and negative predictive values of breast aspiration cytology were 100% On review of the cases originally diagnosed as atypia of uncertain nature (19 %), features more suggestive of malignancy or benignancy were recognizable in most underdiagnosed cases and a half of overdiagnosed cases, respectively. However, indistinguishable atypism also remained. Thirty seven cases out of 53 infiltrating duct carcinomas revealed malignant cytology, with variations in background, cellularity, morphology of cell groups, and extent of isolated tumor cells. When classifying the infiltrating duct carcinomas into scirrhous, solid-tubular, and papillotubular types, scirrhous type frequently appeared as irregularly shaped tight or loose cell clusters, and solid-tubular type as round-margined cell balls.

  • Coarctation of the aorta is an important congenital cardiovascular defect,. which occurs in a significant number of persons. The basic anatomic defect is a localized deformity of the media, manifested by two types of strictures in the aorta: "true" coarctation and tubular hypoplasia. The zone of coarctation is characteristically located distal to the origin of left subclavian artery at or just beyond the insertion of the ligamentum arteriosum. It shortens life if untreated, but it can be corrected to render the patient functionally normal. Here we have a 2-year operative experiences with 3 cases of the aortic coarctation, two of hypoplastic type and one of postductal type, at age of 17 to 19 year old Korean, operated in 1965 and l968 at the National Medical Center. The purposes of this report are to describe the immediate and late effect of surgery, histopathologic bases and the rarity of this lesions in Korea.

An Outbreak of Porcine Pleuropneumonia in Korea (돼지 흉막폐렴(胸膜肺炎)의 발생(發生))

  • Authors observed an outbreak of porcine pleuropneumonia at a piggery in Gyeongbug, Korea, which were diagnosed on the ground of clinical and pathological findings with the following results. 1. One hundred and forty weanlings out of 200 ones died suddenly without noticiable signs during the period of 15 days. All of the dead pigs showed formy, blood-tinged discharge from the nose and mouth. 2. Main clinical findings observed from 60 alive pigs from group consisted of fever, cyanosis severe degree of dyspnea and pleuritic friction rub. 3. On hematological observation for typical 5 cases leukocytes showed marked degenerative left shift with leukopenia (10500-13070/cmm), neutropenia (7-13%) and increase in the number of band neutrophils (6-27%). 4. The most prominent and constant pathological changes observed at necropsy for dead or alive pigs consisted of fibrinous pleurisy with partial adhesion, overlying of the pulmonary surface with fibrinous menbrane and necrotic pneumonic lesions with marked edema and congestion of lung parenchyma. 5. From these results the outbreak was considered to be typical porcine pleuropneumonia, although no bacteriological isolation was attempted.

An Etiology Report for Burns Caused by Korean Folk Remedies

  • Background In this modern era of science-based medicine, some people still accept folk remedies as an alternative form of medicine. However, misinformation and misuse of folk medicines can result in dangerous complications. Among the possible complications of folk remedy use, this study focused on the clinical characteristics of burns caused by folk remedies. Methods We retrospectively reviewed the clinical records of patients who had been treated for burns caused by folk remedies from May 2015 to April 2022. Data were collected on patients' age and gender, type of folk remedy, reason for using the folk remedy, the severity of the burn, the number of wounds, lesion type, and type of treatment. Results We found 59 patients with burns due to folk remedies. Most were female (76.3%) and ≥ 60 years old (72.9%). The most common type of folk remedy was moxibustion (74.6%), followed by the use of glacial acetic acid (20.3%). The reasons for using folk remedies were arthralgia relief (39%), health improvement (18.6%), and treatment of tinea pedis (11.9%). Most patients had multiple wound sites and had burns that were considered severe, requiring surgical treatment (72.9%). The majority of lesions were on the lower extremity, including the foot. Conclusion This study described the risk of burns caused by folk remedies and the clinical characteristics of the wounds. The results emphasize the need for greater public awareness of the risk of burn injuries when using folk remedies.

  • Authors have made a study on the prevalence of the dental caries of distal surface of the lower 2nd molar adjacent to the lower wisdom tooth turned anteriorly. The total number examined was 893 including 542 cases of male and 351 cases of female from the 10s to the 60s. This was taken from the diagnostic charts and intraoral standard films in the infirmary of the college of dentistry, Seoul National University from Jan. 1974 to Aug. 1976. The results are as follows: 1. The caries incidence rate in male was higher than that of female (p<0.01), but the difference between right side and left side was not significant statistically(p>0.1). 3. The Incidence rate of the periapical lesions considered by dental caries of the distal surface of the lower 2nd molar was 3.9%.

  • Throughout the years, various classifications have evolved for the diagnosis of vascular anomalies. However, it remains difficult to classify a number of such lesions. Because all hemangiomas were previously considered to involute, if a lesion with imaging and clinical characteristics of hemangioma does not involute, then there is no subclass in which to classify such a lesion, as reported in one of our cases. The recent classification proposed by the International Society for the Study of Vascular Anomalies (ISSVA, 2014) has solved this problem by including non-involuting and partially involuting hemangioma in the classification. We present here five cases of vascular anomalies and discuss their diagnosis in accordance with the ISSVA (2014) classification. A non-involuting lesion should not always be diagnosed as a vascular malformation. A non-involuting lesion can be either a hemangioma or a vascular malformation depending upon its clinicopathologic and imaging characteristics.

CENTRAL MUCOEPIDERMOID CARCINOMA ARISING IN MANDIBLE WITH MULTIPLE METASTASES (다수의 전이병소를 수반한 하악골의 중심성 점액표피양암종)

  • The authors diagnosed a 54-year-old male as central mucoepidennoid carcinoma after undergoing clinical, radiological and histopathological examinations. The characteristics were as followed : 1. Clinically, the patient complained of the painless unilateral swelling of the left mandibular molar region and had a pus discharge through the fistula. Painful nodule was palpated on the scalp of the left frontal area and it was regarded as a metastatic lesion. 2. Plain radiographs showed the ill-defined permeative radiolucent lesion. The osteolytic lesions were also detected in the cranial bone, number 9 and 11 ribs, scapula, and vertebral bodies. 3. The mandibular CT and PNS MRI showed the swelling of the left mandible and the enlargement of the several lymph nodes of 1.5 cm in size. 4. Histopathologically, many solid epidermoid type cells were mixed with mucus-secreting cells and they were arranged in duct-like structure. Most of them were epidermoid type, which indicates a high grade tumor. Mucins could be found in mucicamrine staining.
