A Case of Rifampin-induced Acute Renal Failure (리팜핀에 의한 급성 신부전)

  • Rifampin is common drug to treat tuberculosis. Rifampin induced acute renal failure, hemolytic anemia and thrombocytopenia is rare and severe complication. We have experienced a case of rifampin induced acute renal failure, hemolytic anemia and thrombocytopenia. Forty-six years old male was suffered from reactivation of pulmonary tuberculosis, and had to medicate antituberculosis drugs including rifampin(600mg/day). Seven years ago, antituberdulosis medication were successfully administered to treat pulmonary tuberculosis without any side effects of drugs. But eight days after readministration of rifampin, fever, abdominal pain, vomiting, oliguria, elevated BUN and creatinine were developed. And thrombocytopenia was also identified after administration of rifampin. The patient was recovered slowly after discontinuation of rifampin & intensive medical care. The renal function was normalized at 55 days after cessation of rifampin. The renal pathologic findings were interstitial nephritis and acute tubular necrosis. And, the rifampin dependent antibodies were identified by indirect antiglobulin test in the presence of rifampin. So we report this case with a brief review of literature.

  • Adrenocorticotropic hormone (ACTH)-producing pheochromocytoma has been rarely reported, whereas only a few cases of Cushing syndrome accompanied by opportunistic infections have been reported. We experienced a patient with pheochromocytoma with ectopic Cushing syndrome complicated by invasive aspergillosis. A 35-year-old woman presented with typical Cushingoid features. Her basal plasma cortisol, ACTH, and 24-hour urine free cortisol levels were significantly high, and 24-hour urine metanephrine and catecholamine levels were slightly elevated. The endogeneous cortisol secretion was not suppressed by either low- or high-dose dexamethasone. Abdominal computed tomography (CT) revealed a heterogeneous enhancing mass measuring approximately 2.5 cm in size in the left adrenal gland. No definitive mass lesion was observed on sellar magnetic resonance imaging. On fluorine-18 fluorodeoxyglucose positron emission tomography/CT, a hypermetabolic nodule was observed in the left upper lung. Thus, we performed a percutaneous needle biopsy, which revealed inflammation, not malignancy. Thereafter, we performed a laparoscopic left adrenalectomy, and its pathologic finding was a pheochromocytoma with positive immunohistostaining for ACTH. After surgery, the biochemistry was normalized, but the clinical course was fatal despite intensive care because of the invasive aspergillosis that included the lungs, retina, and central nervous system.

  • This study is a clinical report for hepatitic C patient and alcoholic hepatitis patient who were improved by treatment of a herbal viscous extracts(Chunggan plus). We checked up Aspartate-aminotransferase(AST), Alamine-aminotransferase(ALT) and ${\gamma}$-Glutamyl transpeptidase (${\gamma}$-GTP) and compared the level of AST, ALT, ${\gamma}$-GTP after treatment. After medication the level of AST, ALT, ${\gamma}$-GTP was significantly normalized. So we suggested that herbal viscous extracts(Chunggan plus) has effects on hepatitis.

  • This study was performed to identify and analyze the specifically expressed plasma proteins during early pregnancy in both pregnant and non-pregnant Hanwoo. Blood samples were collected at 0 (the day of AI), 2, 3, 4, 7, and 11 weeks after AI from pregnant (n=3) and non-pregnant (n=4) Hanwoo, respectively. The hematological parameters were measured. After 2-dimensional electrophoresis using serum, normalized protein spots were selected for the significant expression variation deviated over two fold in its expression level between two groups. Among 17 spots selected, 15 were identified as albumin, IgG1 heavy chain constant region, haptoglobin, ferrochelatase, fibrinogen, hemopexin. 5 spots were expressed only in non-pregnant specific. The spot identification of 1105 and 6106 was decreased after 3 weeks from AI. However, 2/17 spots were still unidentified. Further studies are needed to analyze the function of the proteins associated with early pregnancy.

A Study on the Strain Rate and Temperature Dependence of Yield Stress of Al-Li Alloy (Al-Li합금의 항복응력에 대한 변형속도 및 온도의존성에 관한 연구)

  • The effect of strain rate on the yield stress of an Al-Li alloy has been investigated at temperatures between 77 and 523 K and over the strain rate range from $1.77{\times}10^{-4}s^{-1}$ to $1.77{\times}10^{-2}s^{-1}$. At testing temperatures below 373 K, the yield stress is almost independent of strain rate at any aging stage. At testing temperatures above 373 K, the yield stress increases linearly with the logarithm of strain rate, and the strain rate dependence increases with increasing testing temperature. The yield stresses of under-aged alloy at temperatures between 373 and 473 K at high strain rates are greater than the yield stress at 77 K. For the alloy under-aged or aged nearly to its peak strength, the temperature range within which the positive temperature dependence of yield stress appears expands to the higher temperature side with increasing strain rate. The strain rate dependence of the yield stress is slightly negative at this aging stage. The yield stress of the over-aged alloy decreases monotonically with decreasing strain rate and with increasing testing temperature above 373 K. The modulus normalized yield stress is nearly constant at testing temperatures below 373 K at any strain rate investigated. And, strength depends largely both on the aging conditions and on the testing temperature. The peak positions in strength vs. aging time curves shift to the side of shorter aging time with increasing testing temperature. For the specimens aged nearly to the peak strength, the positive temperature dependence of yield stress is observed in the temperature range. The shift of peak positions in the aging curves are explained in terms of the positive temperature dependence of cutting stress and the negative temperature dependence of by-passing stress.

Estimating the Amount of Nitrogen in Hairy Vetch on Paddy Fields using Unmaned Aerial Vehicle Imagery

  • Remote sensing can be used to provide information about the monitoring of crop situation. This study was conducted to estimate the amount of nitrogen present in paddy fields by measuring the amount of nitrogen in hairy vetch using an UAV (Unmaned Aerial Vehicle). NDVIs (Normalized Difference Vegetation Index) were calculated using UAV images obtained from paddy fields in Seocheon on May $14^{th}$ 2015. There was strong relationship between UAV NDVI and the amount of nitrogen in hairy vetch ($R^2=0.79$). Spatial distribution maps of green manure nitrogen were generated on each paddy field using the nitrogen-vegetation index relations to help farmers determine the amount of N fertilizers added to their rice fields after the application of green manure such as hairy vetch.

A Case of Functionary Cystic Parathyroid Adenoma with Papillary Thyroid Carcinoma (유두상 갑상선암과 동반된 기능성 낭종성 부갑상선 선종 1예)

  • Cystic parathyroid adenoma is one of rare causes of hyperparathyroidism, and tends to cause increased serum level of parathyroid hormone, alkaline phosphate and serum calcium level similar to when compared to those of solid adenoma. The possibility of a coincidental appearance of primary hyperparathyrodism and nonmedullary thyroid cancer is rare, and often neglected. A 40-year-old female presented with constipation and weight loss for 3 months. The serum calcium, phosphate and alkaline phosphate were 16.6 mg/dl, 2.2 mg/dl and 505 IU/L respectively and serum parathyroid hormone level was 1556.2 pg/mL. Neck US showed mixed nodules at both thyroid lobes, and PET-CT showed a right thyroid nodule without FDG uptake. The patient was diagnosed of primary hyperparathyroidism and underwent parathyroidectomy and a total thyroidectomy. Histopathologic results revealed parathyroid adenoma with cystic change (2 cm greater diameter) in the right lower parathyroid gland and coexistent papillary carcinoma of the right lobe of thyroid gland (infiltrating, 0.3 cm in greater diameter). She remained well and serum calcium, phosphate, parathyroid hormone level were normalized within 24 hours.

Feasibility of Vegetation Temperature Condition Index for monitoring desertification in Bulgan, Mongolia

  • Desertification monitoring as a main portion for understand desertification, have been conducted by many scientists. However, the stage of research remains still in the level of comparison of the past and current situation. In other words, monitoring need to focus on finding methods of how to take precautions against desertification. In this study, Vegetation Temperature Condition Index (VTCI), derived from Normalized Difference Vegetation Index (NDVI) and Land Surface Temperature (LST), was utilized to observe the distribution change of vegetation. The index can be used to monitor drought occurrences at a regional level for a special period of a year, and it can also be used to study the spatial distribution of drought within the region. Techniques of remote sensing and Geographic Information System (GIS) were combined to detect the distribution change of vegetation with VTCI. As a result, assuming that the moisture condition is the only main factor that affects desertification, we found that the distribution of vegetation in Bulgan, Mongolia could be predicted in a certain degree, using VTCI. Although desertification is a complicated process and many factors could affect the result. This study is helpful to provide a strategic guidance for combating desertification and allocating the use of the labor force.

Effect of Relaxin Expressing Adenovirus on Scar Remodeling: A Preliminary Study

  • Background: Relaxin is a transforming growth factor ${\beta}1$ antagonist. To determine the effects of relaxin on scar reduction, we investigated the scar remodeling process by injecting relaxin-expressing adenoviruses using a pig scar model. Methods: Scars with full thickness were generated on the backs of Yorkshire pigs. Scars were divided into two groups (relaxin [RLX] and Control). Adenoviruses were injected into the RLX (expressing relaxin) and Control (not expressing relaxin) groups. Changes in the surface areas, color index and pliability of scars were compared. Results: Fifty days after treatment, the surface areas of scars decreased, the color of scars was normalized, and the pliability of scars increased in RLX group. Conclusion: Relaxin-expressing adenoviruses improved the surface area, color, and pliability of scars. The mechanism of therapeutic effects on scar formation should be further investigated.

Hepatic glycogenosis in type 1 diabetes mellitus mimicking Mauriac syndrome

  • Hepatic glycogenosis in type 1 diabetes mellitus (DM) can be caused by poor glycemic control due to insulin deficiency, excessive insulin treatment for diabetic ketoacidosis, or excessive glucose administration to control hypoglycemia. Mauriac syndrome, which is characterized by hepatomegaly due to hepatic glycogenosis, growth retardation, delayed puberty, and Cushingoid features, is a rare diabetic complication. We report a case of hepatic glycogenosis mimicking Mauriac syndrome. A 14-year-old girl with poorly controlled type 1 DM was admitted to The Catholic University of Korea, Seoul St. Mary's Hospital for abdominal pain and distension. Physical examination revealed hepatomegaly and a Cushingoid face. The growth rate of the patient had decreased, and she had not yet experienced menarche. Laboratory findings revealed elevated liver enzyme levels. A liver biopsy confirmed hepatic glycogenosis. Continuous glucose monitoring showed hyperglycemia after meals and frequent hypoglycemia before meals. To control hyperglycemia, we increased insulin dosage by using an insulin pump. In addition, we prescribed uncooked cornstarch to prevent hypoglycemia. After strict blood glucose control, the patient's liver functions and size normalized. The patient subsequently underwent menarche. Hepatic glycogenosis is a complication of type 1 DM that is reversible with appropriate glycemic control.
