A Case of Acrodermatitis Enteropathica with Normal Serum Zinc Level in a Breastfed Preterm Infant (혈중 아연 농도가 정상인 모유수유를 한 미숙아에서 보인 장병성 선단 피부염 1례)

  • Acrodermatitis enteropathica due to zinc deficiency is characterized by periorificial dermatitis, alopecia, and intractable diarrhea. Here we report a case of transient acrodermatitis enteropathica in a 2-month-old preterm infant, whose skin is erythematous, scaly, eczematous, with an eruption starting from the periorificial area. While her serum zinc level was normal at 118.2 ug/dL, the serum alkaline phosphatase level was low at 48 IU/L and the serum alkaline phosphatase level get increased concordant with clinical improvement after zinc supplementation. Hence, this case shows that the diagnosis of zinc deficiency can be aided with a low level of serum alkaline phoaphatase although serum zinc level is normal.

  • Hemophilia B is a rare blood coagulation disorder. Complications such as bleeding and hematoma can cause necrosis of flaps, wound disruption, and the disturbance of wound healing. In particular, guidelines for flap operations in hemophilia B patients have still not been defined, and case reports are rare. We reconstructed the heel of a 41-year-old male hemophilia B patient using a reverse sural artery flap operation. The patient presented with mild hemophilia, having 27% of the normal value of coagulation factor IX. Coagulation and the changing value of the coagulation factor were regularly measured, and 70% of the normal value of coagulation factor IX was maintained through the injection of recombinant coagulation factors and antihemorrhagics. Hematoma developed twice (postoperative day [POD] 5 and POD 7) and in each case the hematoma was removed. Injections of recombinant coagulation factors and antihemorrhagics were continuously administered until postoperative week 2. When the coagulation factors were within normal ranges. In this article, a hemophilia B patient underwent reverse sural artery flap surgery and the healing progress was analyzed. We conclude that higher than baseline levels of coagulation factors are needed for successful healing in reverse sural artery flap surgery.

  • The recurrence of an anterior mediastinal mass was discovered incidentally on the chest film for an asymptomatic 72-year-old female. She underwent the mass resection at the same site in 1989 and was diagnosed as a paraganglioma of the anterior mediastinum. She had poorly controlled hypertension which was converted into normal blood pressure after the first operation. During the follow up, the catecholamine levels were within normal limits, and there were no evidence of recurrence of the tumor. However, 6.3 years later, the mass recurred at the same anterior mediastinum. The patient had no hypertension and catecholamine levels were still within normal limits. The recurring mass was successfully removed and had the same histological findings - recurring paraganglioma.

Analysis of Voltage, Current and Temperature Signals for Poor Connections at Electrical Connector (커넥터에서 접촉불량 발생시의 전압, 전류 및 온도 신호 특성 분석)

  • This paper is aimed to analyze the characteristics of simultaneous voltage, current and temperature signals for poor connection on electrical connector. In order to attain this purpose, detected were the current and voltage signals on electric wire with series arc, named arc signals, and also monitored were the changes of RMS, instantaneous value of waveform in time domain and temperature value with video. Two states are made normal state over $5kgf{\cdot}cm$ and poor connections state below $0.5kgf{\cdot}cm$ by screw gage. In the voltage signal case, the voltage drop was increased with which the current was increased. In the current signal case, poor connections at the time interval 1~4A all showed "shoulder", as distinct difference from the normal state shown waveform pattern. In the temperature signal case, poor connections are twice at 1A and five times at 4A in the normal state. The temperature continues insulation of electrical wiring and connector can be carbonized. The results of this study will be effectively used in developing the preventive devices and system for electric fire by poor connections.

  • We present a case of tacrolimus-induced encephalopathy after successful kidney transplantation. An 11-year-old girl presented with sudden onset of neurologic symptoms, hypertension, and psychiatric symptoms, with normal kidney function, after kidney transplantation. The symptoms improved after cessation of tacrolimus. Magnetic resonance imaging (MRI) showed acute infarction of the middle cerebral artery (MCA) territory in the right frontal lobe. Three days later, she had normal mental function and maintained normal blood pressure with left hemiparesis. Follow-up MRI was performed on D19, showing new infarct lesions at both cerebral hemispheres. Ten days later, MRI showed further improvement, but brain single photon emission computed tomography (SPECT) showed mild reduction of uptake in both the anterior cingulate gyrus and the left thalamus. One month after onset of symptoms, angiography showed complete resolution of stenosis. However, presenting as a mild fine motor disability of both hands and mild dysarthria, what had been atrophy at both centrum semiovale at 4 months now showed progression to encephalomalacia. There are two points of interest in this case. First, encephalopathy occurred after administration of tacrolimus and improved after discontinuation of the drug. Second, the development of right-side hemiplegia could not be explained by conventional MRI; but through diffusion tensor imaging (DTI) and diffusion tensor tractography (DTT) of white matter tract, visualization was possible.

  • By using the high tensile steel plate (SPFH590) 1 Primary normal shear process clearance was studied through an experiment for the effect of the second shaving process in this study. Experiment result, Shaving process in the case of processed 15% of the Primary normal shear process clearance was some residual surface does not remove it completely. and Shaving process of shearing products by the general shearing process clearance of 10% it was found that the wider the size of shear surface. The shear surface of 93% occurred in case of the clearance of general shearing process is 10% and the Shaving clearance is 2%, The shear surface of 87% were to occur in case of the Shaving clearance is 3%.

  • We developed the projected l-axial skew-normal(LASN) family of distributions for I-axial data. The LASN family of distributions contains the semicircular skew-normal(SCSN) and the circular skew-normal(CSN) families of distributions as special cases. The LASN densities are similar to the wrapped skew-normal densities for the small values of the scale parameter. However CSN densities have more heavy tails than those of the wrapped skew-normal densities on the circle. Furthermore the CSN densities have two modes as the scale parameter increases. The LASN distribution has very convenient mathematical features. We extend the LASN family of distributions to a bivariate case.

  • We introduce the notion of Lie invariant normal Jacobi operators for real hypersurfaces in the complex hyperbolic quadric Qm∗ = SOom,2/SOmSO2. The invariant normal Jacobi operator implies that the unit normal vector field N becomes 𝕬-principal or 𝕬-isotropic. Then in each case, we give a complete classification of real hypersurfaces in Qm∗ = SOom,2/SOmSO2 with Lie invariant normal Jacobi operators.

  • This paper is devoted to the geometry of vector fields and timelike flows in terms of anholonomic coordinates in three dimensional Lorentzian space. We discuss eight parameters which are related by three partial differential equations. Then, it is seen that the curl of tangent vector field does not include any component in the direction of principal normal vector field. This implies the existence of a surface which contains both s - lines and b - lines. Moreover, we examine a normal congruence of timelike surfaces containing the s - lines and b - lines. Considering the compatibility conditions, we obtain the Gauss-Mainardi-Codazzi equations for this normal congruence of timelike surfaces in the case of the abnormality of normal vector field is zero. Intrinsic geometric properties of these normal congruence of timelike surfaces are obtained. We have dealt with important results on these geometric properties.

  • Quality characteristics on the properties of process capability indices (PCIs) are often required to be normally distributed. But, if a characteristic is not normally distributed, serious errors can result from normal-based techniques. In this case, we may well consider the use of new PCIs specially designed to be robust for non-normality. In this paper, a newly proposed measure of process capability is introduced and compared with existing PCIs using the simulated non-normal data.
